1.A study on the inequality of information needs for cardiac rehabilitation in urban and rural patients with coronary heart disease

Objective:To analyze the current situation and influencing factors of information need for cardiac rehabilitation of urban and rural patients with coronary heart disease, and explore the inequality of information need for cardiac rehabilitation among urban and rural patients with coronary heart disease and its influencing factors, so as to provide a scientific reference for formulating targeted cardiac rehabilitation programs.Methods:From June to July 2024, hospitalized patients with coronary heart disease from a Class A tertiary hospital in Suzhou City were selected by convenience sampling as the study subjects. A General Information Questionnaire, Information Need in Cardiac Rehabilitation Hospital Anxiety and Depression Scale, Health Literacy Management Scale, and Coronary Artery Disease Self-Management Scale were used to conduct a questionnaire survey. Multiple linear regression was used to analyze the influencing factors of the cardiac rehabilitation information needs and the oaxaca-blinder model was used to analyze the causes of unequal information needs in urban and rural patients.Results:A total of 254 hospitalized patients with coronary heart disease who met the inclusion and exclusion criteria were surveyed, included 127 males and 127 females, 182 of them were aged 60 years or older. The total cardiac rehabilitation information needs score of rural and urban patients with coronary heart disease were 168.00 (115.50, 255.00) and 213.00 (132.00, 255.00), respectively, the difference was significant ( U = 5 389.50, P<0.05). Multiple regression analysis showed that health literacy ( β = 0.871, P<0.05) was the influencing factor of cardiac rehabilitation information needs of rural coronary heart disease patients, and depression ( β = 0.719, P<0.05) and living status ( β = -0.186, P<0.05) was the influencing factor of cardiac rehabilitation information needs of urban coronary heart disease patients. Average monthly household income per capita (C = 14.50%) and health literacy (C = 88.30%) were the main causes of the difference in cardiac rehabilitation information needs between urban and rural patients with coronary heart disease. Conclusions:The information demand for cardiac rehabilitation of patients with coronary heart disease in rural area is lower than that in urban areas in Suzhou, mainly due to health literacy and monthly income. It is recommended to narrow the urban-rural gap by improving rural health literacy, optimizing medical insurance policies, and promoting a multi-level rehabilitation service network.
2.Evidence-based practice for non-pharmacological prevention and management of venous thromboembolism in ischemic stroke patients

Objective:To explore non-pharmacological prevention and management methods for venous thromboembolism (VTE) in ischemic stroke (IS) patients based on the best evidence and evaluate the effectiveness of their application.Methods:Based on evidence acquisition, this study was guided by the Ottawa model of research use to dissect the barriers and facilitators of evidence application through clinical review, and to implement intervention strategies and effect evaluation. Convenience sampling method was used to select inpatients and nurses in the Department of Neurology of Affiliated Hospital of Jiangsu University before and after evidence application as study subjects. The baseline review period was from March 1 to May 31, 2023, and the review subjects were 100 patients hospitalised in the Department of Neurology and 32 nurses working in the Department of Neurology during this period, and the evidence application review period was from December 1, 2023 to February 29, 2024, and the review subjects were 100 patients hospitalised in the Department of Neurology and 32 nurses working in the Department of Neurology during this period. Nurses and patients knowledge of non-pharmacological prevention of VTE, implementation of the review indicators, and the incidence of VTE in patients were compared before and after the application of the evidence.Results:After evidence application, most of the evidence implementation rates were higher than those before evidence application, and the differences were statistically significant ( P<0.05). The VTE non-pharmacological prevention knowledge scores of IS patients and nurses were higher than those before evidence application, and the differences were statistically significant ( P<0.05) ; the VTE incidence rate of IS patients was lower than that before evidence application, and the differences were statistically significant ( P<0.05) . Conclusions:Evidence-based practice of non-pharmacological prevention and management of VTE in patients with IS, guided by the Ottawa model of research use, can effectively improve nurses evidence-based adherence to non-pharmacological prevention and management of VTE, as well as enhance patients knowledge of non-pharmacological prevention of VTE, thereby reducing the incidence of VTE.

5.Curative Effect of Laparoscopic C-type Radical Hysterectomy Through Deep Uterine Vein Approach for Cervical Cancer

Objective To investigate the efficacy of laparoscopic C-type radical hysterectomy through deep uterine vein approach for the treatment of cervical cancer.Methods From January 2021 to December 2022,58 cases of cervical cancer were treated with deep uterine vein approach laparoscopic C-type radical hysterectomy in our hospital.After establishing the operation channel,the ureter was identified in the posterior lobe of the broad ligament,and the uterine artery was exposed after separating the ureter.The connective tissue was separated along the dorsal side of the uterine artery to gradually expose the deep uterine vein.The parametrial lymph nodes surrounding the deep uterine vein were resected and sent to pathology alone.The deep uterine vein was continuously tracked towards the bladder,and its branches were freed.The deep uterine vein and its tributaries were double clipped by vascular clamp.Results The operation time was(307.2±54.1)min,the median bleeding volume was 50(20,100)ml,the lymph node dissection number was(26.3±6.9),the indwelling catheter time was(20.6±4.7)d,the time to removal of abdominal drainage tube was(9.4±4.1)d,the anal exhaust time was(36.7±4.1)h,the antibiotics use time was(9.2±4.2)d,and the hospital stay was(13.4±2.6)d.The postoperative complication rate was 3.4%(2/58),and the postoperative pathological staging upgrade rate was 22.4%(13/58).The postoperative European Organization of Research and Treatment of Cancer(EORTC)Quality of Life Questionnaire-Core 30(QLQ-C30)score was significantly higher than before surgery[(78.6±10.7)points vs.(47.1±7.6)points,t = 17.177,P = 0.000].All the 58 cases were followed up for 4-25 months(mean,13.5±6.2 months),with no recurrence.Conclusion Laparoscopic C-type radical hysterectomy through deep uterine vein approach is effective,safe,and reliable.

Objective To observe the effect of Bushen Gutai mixture on uterine blood supply of rat model of abortion induced by hydroxyurea tablets combined with mifepristone through PKA-CREB signal pathway and its mechanism of calming fetus.Methods 60 pregnant rats of SPF grade SD rats were prepared by closing cages at 2∶1.According to the order of pregnancy,60 pregnant rats were randomly divided into 6 groups with 10 rats in each group:Bushen Gutai mixture group(low,middle and high dose),normal pregnancy group,model group and Di qu progesterone group.On the 1st to 9th day of pregnancy,except the normal group,the pregnant rats in each group were gavaged with hydroxyurea tablets at 5∶00 pm every day(450 mg·kg-1),and at 10∶00 am on the 10th day of pregnancy.Mifepristone tablets were given by intragastric administration(4.0 mg·kg-1).At 9∶00 am every day from the 1st to 9th day of pregnancy,Bushen Gutai mixture was given to the low,middle and high dose groups(0.5,1.0,2.0 g·kg-1),didrone group(3.02 mg·kg-1),model group and normal pregnancy group with 0.9%normal saline.24 hours after the last administration of pentobarbital sodium(50 mg kg-1),all pregnant rats were killed by intraperitoneal injection of pentobarbital sodium,and the uterine decidual tissue of pregnant rats was bluntly isolated in sterile environment.Hematoxylin-eosin(HE)staining was used to observe the lumen diameter and wall thickness of spiral artery in uterine decidual tissue.Immunohistochemical staining(IHC)was used to detect the expression of vascular endothelial growth factor(VEGF)in decidual tissues.Western blot was used to detect the protein expression of phosphorylated protein kinase A,protein kinase A,phosphorylated cyclic adenosine monophosphate response element binding protein,Cyclic Adenosine monophosphate response element binding protein and aquaporin 5 in the decidua of pregnant rats.The apoptosis of decidual cells was detected by in situ end labeling(TUNEL)of DNA fragmentation.Results Compared with the model group,the wall thickness of spiral artery was higher than that in other groups(P<0.05),the lumen diameter was lower than that in other groups(P<0.05)and the expression of VEGF protein was lower than that of other groups(P<0.01).Compared with the model group,the apoptosis level of decidual cells in uterine decidua of abortive rats in high,middle and low dose groups of Bushen Gutai mixture and diqu progesterone group decreased in varying degrees.Bushen Gutai mixture can up-regulate the levels of p-PKA/PKA and p-CREB/CREB in uterine decidua(P<0.01)and promote the expression of AQP5 protein in uterine decidua of abortion rats(P<0.01).Conclusion Bushen Gutai mixture can improve uterine blood supply of aborted rats by activating PKA and CREB phosphorylation,up-regulating AQP5 expression,promoting physiological recasting of spiral artery and high expression of Vascular endothelial growth factor.
8.Value of reflex testing in clinical practice of laboratory medicine

Reflex tests are ordered when a particular test result indicates that additional testing should be performed according to the guidelines or the feedback process formulated by clinical consultation. The application scope of the reflex tests involves various subspecialties of laboratory medicine. The clinical application needs the support of qualified laboratory doctors, comprehensive information and financial system, clinical guidelines, and so on. Active application of reflex tests can promote the standardization of evidence-based medicine in clinical practice, save medical resources, and shorten the diagnosis and treatment time of patients.
9.Relationship of CEUS quantitative parameters and enhance patterns with therapeutic effect of high-intensity focused ultrasound for hysteromyoma

Objective To observe the relationship of efficacy of high-intensity focused ultrasound (HIFU) in treatment of hysteromyoma and quantitative parameters and enhance manifestations of CEUS.Methods Forty patients with single hysteromyoma underwent CEUS before HIFU,immediately after and 6 months after treatment,respectively.Time to peak (TTP),peak intensity (PI) of time-intensity curve were analyzed,and the enhance manifestations were observed.The ablation ratio of hysteromyoma immediately after HIFU and the ratio of reduction volume 6 months after treatment were also analyzed.Results Among 40 hysteromyomas,21 evenly enhanced (even enhance group) and 19 unevenly enhanced (uneven enhance group).The ablation rate greater than or equal to 80 % were observed in 22 hysteromyomas (ablation rate ≥80% group),while ablation rate less than 80% were observed in 18 lesions (ablation rate <80% group).There were significant differences of TTP ([27.48 ± 7.89] s vs [21.78 ± 4.76] s;t =2.69,P =0.01),PI ([44.29 ± 3.74] dB vs [47.39±4.68]dB;t=-2.34,P=0.02) and the ratio of reduction volume 6 months after HIFU ([59.47±16.21]% vs [37.30±11.89]%;t=4.83,P<0.01) between ablation rate ≥80% group and ablation rate <80% group.The ablation rate ([65.03±18.39]% vs [41.64±18.78]%;t=-4.49,P<0.01) and the ratio of reduction volume 6 months after HIFU ([84.74±7.76]% vs [58.18±12.94]%;t=-3.21,P<0.01) were significantly different between even enhance group and uneven enhance group.TTP and the ratio of reduction volume 6 months after HIFU positively correlated with the ablation rate (r =0.448,0.660,both P< 0.01),while PI and enhance manifestations negatively correlated with ablation rate (r =-0.460,rs =-0.614,both P< 0.01).Conclusion TTP,PI of time-intensity curve and enhance patterns of hysteromyoma correlated with the ablation rate of HIFU,which having certain clinical value in evaluation of therapeutic efficacy.
10. Blocking effects of serum antibodies against HBGA binding to norovirus GII.4 of population in oyster culture area of Guangdong Province

Objective:
Evaluate GII.4 norovirus infection and blocking effects of serum antibodies against HBGAs binding to GII.4 norovirus of population in oyster culture area, provide references for screening of fully human monoclonal antibody.
Methods:
Using a random survey method to collect blood and saliva samples in oyster culture area, select serum samples from the inland region of Guangdong as control group. Identification of salivary HBGA receptor phenotype and detection of serum antibody levels between two areas by ELISA. A vitro neutralization model was to determine the efficiency of serum antibodies blocking GII.4 norovirus and HBGA receptors binding.
Results:
The age were (50.68 ± 15.17), (52.52 ± 15.90) and (51.37 ± 13.32) years old of 2015, 2016 in experimental group, and in control group, respectively. Males accounted for 5.9% (70/195), 36.6%(60/164), 40.8% (69/169) (χ2=0.93,
