1.Four nutritional screening tools for evaluating the nutritional status of children with aplastic anemia

Objective:To evaluate the nutritional status of children with aplastic anemia(AA)using the children's nutritional risk index(CNRI),prognostic nutritional index(PNI),controlling nutritional status(CONUT),instant nutritional assessment(INA),and combined index(CI),in order to provide scientific evidence to support clinical diagnosis and treatment.Methods:A retrospective analysis was conducted on the clinical data of pediatric AA patients treated in the Department of Pediatrics,Ward 1,First Affiliated Hospital of Xinjiang Medical University from January 1st,2019 to April 30st,2024.Nutritional status was assessed using CNRI,PNI,CONUT,and INA,and integrated into CI.The agreement between different nutritional screening tools was evaluated using Kappa statistics. Results:A total of 68 pediatric AA patients were included,with an overall malnutrition prevalence of 44.1%.The malnutrition rates were 39.4% in the body mass index(BMI)normal group and 48.6% in the BMI abnormal group.Across disease severity groups,malnutrition was observed in 66.67% of very severe AA(VSAA)patients,54.17% of severe AA(SAA)patients,and 28.13% of non-severe AA(NSAA)patients,and the difference was statistically significant( χ2=6.778, P=0.034). Kappa analysis demonstrated the highest concordance between CONUT,INA,and CI,with Kappa values of 0.882( P<0.001). Conclusion:Pediatric AA patients have a high prevalence of malnutrition,and their nutritional status decline significantly with increasing disease severity.CONUT and INA are effective tools for assessing nutritional status in children with AA,providing reliable evidence for clinical evaluation.
2.Related factors of implementation status of cardiac rehabilitation by nurses in cardiovascular departments

Objective To investigate the factors influencing the implementation status of cardiac rehabilitation by cardiovascular nurses. Methods A self-designed questionnaire titled "Survey on Factors Influencing the Implementation of Cardiac Rehabilitation by Cardiovascular Nurses" was used to survey cardiovascular nurses from tertiary hospitals in Beijing, Anhui Province, and other regions through convenience sampling. Results A total of 655 questionnaires were distributed, with 548 valid responses, yielding an effective response rate of 83.66%. The main promoting factors for nurses in cardiology departments to implement cardiac rehabilitation were nurses' experience in guiding patients through cardiac rehabilitation, the establishment of dedicated cardiac rehabilitation centers in hospitals, and the inclusion score of cardiac rehabilitation in national medical insurance of (4.32±0.79) points; the main obstacles were patients' lack of understanding of cardiac rehabilitation [(3.03±1.04) points], the high turnover rate in the department [(4.21±0.79) points], and incomplete laws and regulations related to cardiac rehabilitation [(3.56±0.88) points]. Conclusion The implementation of cardiac rehabilitation by nurses in cardiology departments is related to factors such as the nurses, patients, hospitals, and policies. Clinical importance should be given to these factors, and corresponding measures should be taken to promote the implementation of cardiac rehabilitation.
3.Current status of knowledge-attitude-practice of cardiac rehabilitation and its influencing factors among patients undergoing coronaryartery bypass grafting

Objective To investigate the current status of knowledge-attitude-practice of cardiac rehabilitation in patients undergoing coronary artery bypass grafting(CABG) and analyze its influencing factors. Methods From July to November 2022, 288 patients with CABG in the coronary heart disease surgery center of Beijing Anzhen Hospital Affiliated to Capital Medical University were conveniently included, and general information questionnaire and cardiac rehabilitation knowledge and practice questionnaire were used to investigate the general situation and the level of knowledge-attitude-practice of cardiac rehabilitation in patients with coronary heart disease. Results A total of 290 questionnaires were sent out in this study, and 288 were effectively collected, with an effective recovery rate of 99.31%. The total score of the questionnaire was (106.81±13.35), and the total score rate was 62.83%, including the knowledge questionnaire scoring (39.34±9.13), belief questionnaire scoring (32.86±3.39) and behavior questionnaire scoring (34.61±3.54). The scoring rates of knowledge, attitude and practice questionnaire were 46.28%, 82.15% and 76.91%, respectively. Gathering together is an effectual way for improving quality and efficiency of the physical and chemical regenerative environmental control and life support system,as a regenerative gathering together technology,solid amine can simultaneously gather carbon dioxide and humidity with mild adsorption and desorption conditions,thus having the potential for miniaturization and low energy consumption,making it very suitable for manned deep space exploration missions.The current domestic and foreign systems cannot achieve perfect heat exchange between adsorption and desorption,resulting in low efficiency and less obvious comprehensive advantages.This study utilized multi-physics simulation methods to optimize system heat transfer,designed a combined control system for carbon dioxide and humidity,and verified various system characteristics such as flow rate and cycle time.The results show that:The designed porous thermal conductive medium can effectively reduce thermal resistance,and the obtained optimal filling ratio and structural size improve the heat transfer efficiency of the system.By adjusting the system characteristics,it is possible to dynamically satisfy the carbon dioxide and humidity control needs for 1~3 crews at different metabolic levels.This study verified the advantages of the regenerative combined carbon dioxide and humidity gathering system in terms of weight,power consumption,and volume,and obtained system characteristics,providing important technical support for manned deep space exploration missions.
5.Research progress of anterior cutaneous nerve injury and repair in knee arthroplasty.

OBJECTIVE:
To summarize the research progress of anterior cutaneous nerve injury and repair in knee arthroplasty.
METHODS:
The relevant literature at home and abroad in recent years was reviewed and summarized from the anatomy of anterior cutaneous nerve, nerve injury grade, clinical manifestations, prevention and treatment of anterior cutaneous nerve.
RESULTS:
The anterior cutaneous nerve injury is a common complication of knee arthroplasty. Because the anterior cutaneous nerve branches are many and thin, and mainly run between the first and second layers of fascia, this level is often ignored during surgical exposure. In addition, the knee arthroplasty does not routinely perform the exploration and repair of the cutaneous nerve. So the anterior cutaneous nerve injury is difficult to avoid, and can lead to postoperative skin numbness and knee pain. At present, studies have explored the feasibility of preventing its occurrence from the aspects of improved incision and intraoperative separation of protective nerve. There is no effective prevention and treatment measures for this complication. For patients with skin numbness after knee arthroplasty, the effectiveness of drug treatment is not clear. Local nerve block or nerve excision can be used to treat patients with painful symptoms after knee arthroplasty considering cutaneous pseudoneuroma.
CONCLUSION
Knee arthroplasty is widely used and anterior cutaneous nerve injury is common in clinic. In the future, more high-quality clinical studies are needed to further explore the prevention and treatment measures of this complication and evaluate the clinical benefits obtained.

6.Analysis of long-term trend changes in breast cancer incidence and death among females in Shanghai,1973-2017

Objective:To describe the long-term characteristics and trend changes in the incidence and mortality of female breast cancer in Shanghai from 1 973 to 2017,aiming to provide references for exploring the etiology of breast cancer and formulating strategies and measures for prevention,intervention and control. Methods:Joinpoint software was used to analyze the trend changes in the incidence and mortality of female breast cancer in Shanghai from 1 973 to 2017,and an age-period-cohort model was constructed to explore the effects of age,year of diagnosis,and birth cohort on long-term trend changes. Results:From 1 973 to 2017,there were 68 192 new cases of female breast cancer in Shanghai,with a diagnosed rate of 31.72/100 000.The incidence rate continued to rise,and the risk of the disease continued to rise from the age of 20 years,and the rise rate accelerated significantly after the age of 40 years.There were 21 535 female breast cancer deaths from 1 973 to 2017.The mortality rate was stable,with a death rate of 8.62/100 000,and the risk of death increased significantly from the age of 45 years.The effects of age,period and cohort had a significant impact on the incidence of breast cancer(P<0.01),while the increase in mortality rate was related to age and cohort effects(P<0.01). Conclusion:The incidence rate of female breast cancer in Shanghai is still rising rapidly,and the mortality trend is generally stable,suggesting that the treatment is effective and the quality of life is improved.However,breast cancer is still the main malignant tumor among females in Shanghai.It should be continued to implement prevention and control strategies such as lifestyle intervention and screening of high-risk individuals to further strengthen the prevention and control of breast cancer.
7.Colorectal cancer incidence and mortality trends in urban Shanghai,China from 1973 to 2017:a Joinpoint regression and age-period-cohort analysis

Objective:To describe the epidemiological features and temporal trends of colorectal cancer in urban Shanghai from 1973 to 2017. Methods:Data on colorectal cancer in urban Shanghai was obtained through Shanghai Cancer Registry and Vital Statistics System.Joinpoint analysis was used to describe the temporal trends and annual percent change(APC)and age-period-cohort analysis was used to estimate the association between age,period and birth cohort and colorectal cancer. Results:A total of 105 847 cases and 60 447 deaths of colorectal cancer were diagnosed in urban Shanghai over the 45-year study period.Both the number of new cases and the number of deaths showed an increasing trend.In the same period,the age-standardized incidence of colorectal cancer in urban areas of Shanghai increased significantly from 14.1/100 000 in 1973 to 27.7/100 000 in 2017,while the age-standardized mortality rate increased from 8.2/100 000 to 10.7/100 000.The overall average annual age-standardized incidence and mortality rates were 20.4/100 000 and 11.0/100 000,respectively.With the increase of age,the age-standardized morbidity and mortality of colorectal cancer showed an obvious upward trend.Taking 1993-1997 as reference,the risk of colorectal cancer in Shanghai reached the highest in 2013-2017,and the corresponding relative risk was 1.2(95%confidence interval:1.2-1.3),while the lowest was 0.9(95%confidence interval:0.8-1.0)during 1973-1977.Mortality risk,on the contrary,decreased with the increase of time.Before 1953-1957,the risk of colorectal cancer in urban Shanghai increased with the increase of birth cohort time,and then showed a downward trend.There was a corresponding decline in the risk of colorectal cancer death among people born after 1957. Conclusion:The incidence and mortality of colorectal cancer in Shanghai showed an increasing trend from 1973 to 2017,but the prevalence trend of colorectal cancer is still different among different populations.
8.Establishment of a topographic map assessment system for facial and cervical wounds and scars of burn patients based on the Delphi method

Objective:To construct a targeted and accurate evaluation system for facial and cervical wounds and scars of burn patients.Methods:The method combining literature analysis and survey research was adopted, and the basic principles of item system construction were followed. From June to August 2020, based on the aesthetic standards of facial and cervical plastic surgery, the topographic map assessment system for facial and cervical wounds and scars of burn patients was preliminarily formed, focusing on the assessment of wounds and scars in the necks and faces of patients after burns. In September 2020, 38 experts in the relevant fields were consulted in advance and the questionnaire was revised according to the experts' opinions. From December 2020 to March 2021, the Delphi method was applied to conduct inquiry by correspondence with 35 experts in relevant fields from Guangzhou, Shenzhen, Shanghai, Beijing, and other cities, who met the inclusion criteria, and the items were screened and established. The effective recovery rate of inquiry questionnaire was calculated to determine the level of enthusiasm of experts, the average authority coefficient of all items was calculated to determine the level of expert authority, the average importance expert score, the average coefficient of variation, and the average full score rate of all the third-level items were calculated to determine the concentration of expert opinions, the average coefficients of variation and Kendall's harmony coefficients of the importance, sensitivity, and operability expert scores of all the third-level items were calculated to determine the degree of coordination of expert opinions. The Kendall's harmony coefficients for the importance, sensitivity, and operability expert scores of all the third-level items were statistically analyzed with chi-square test.Results:Among the 35 experts consulted by Delphi method, mainly were male, aged (48±10) years, with 8-38 years of working experience, mainly with associate senior titles and above, all with a bachelor's degree or above education background, and of whom 11 were burn experts, 7 were wound repair experts, 4 were plastic surgery experts, and 13 were rehabilitation medicine experts. Finally, a topographic map assessment system for facial and cervical wounds and scars of burn patients was formed, including 4 first-level items, 21 second-level items, 40 third-level items, and 1 mask. The effective recovery rate of inquiry questionnaire was 100% (35/35). The average authority coefficient of all items was 0.89. The average importance expert score was 4.67, the average coefficient of variation of importance expert score was 0.01, and the average full score rate of all the third-level items was 86.3%. The average coefficients of variation of the importance, sensitivity, and operability expert scores of all the third-level items were 0.01, 0.01, and 0.02, respectively. The Kendall's harmony coefficients for the importance, sensitivity, and operability expert scores of all the third-level items were statistically significant (with χ2 values of 1 201.53, 745.67, and 707.07, respectively , P<0.05). Conclusions:The established topographic map assessment system for facial and cervical wounds and scars of burn patients has high scientificity and reliability, which can be used for the evaluation of facial and neck wounds or scars in burn patients.
