1.Optimization of Rh blood group antigen precision transfusion strategy across multiple hospital campuses by PDCA circle

[Objective] To explore the effectiveness of applying the PDCA (Plan-Do-Check-Act) cycle to enhance the compatibility rate of five Rh blood group antigen phenotypes between donors and recipients across multiple hospital campuses. [Methods] Clinical blood transfusion data from May to July 2022 were selected. Specific improvement measures were formulated based on the survey results, and the PDCA cycle management model was implemented from August 2022. The post-intervention phase spanned from August 2022 to October 2023. The Rh phenotype compatibility rate, the detection rate of Rh system antibodies, and the proportion of Rh system antibodies among unexpected antibodies were compared between the pre-intervention phase (May to July 2022) and the post-intervention phase. [Results] After the continuous improvement with the PDCA cycle, the compatibility rate for the five Rh blood group antigen phenotypes between donors and recipients from August to October 2023 reached 81.90%, significantly higher than the 70.54% recorded during the pre-intervention phase (May to July 2022, P<0.01), and displayed a quarterly upward trend (β=0.028, P<0.05). The detection rate of Rh blood group system antibodies (β=-9.839×10-5, P<0.05) and its proportion among all detected antibodies (β=-0.022, P<0.05) showed a quarterly decreasing trend, both demonstrating a negative correlation with the enhanced compatibility rate (r values of -0.981 and -0.911, respectively; P<0.05). [Conclusion] The implementation of targeted measures through the PDCA cycle can effectively increase the compatibility rate of five Rh blood group antigen phenotypes between donors and recipients, reduce the occurrence of unexpected Rh blood group antibodies, thereby lowering the risk of transfusion and enhancing the quality and safety of medical care.

OBJECTIVE To explore the potential mechanism of ketamine-induced cognitive impairment in mice based on metabolomics. METHODS Male C57BL/6 mice were randomly divided into control group and ketamine group (25 mg/kg), with 12 mice in each group. Each group of mice was intraperitoneally injected with normal saline or corresponding drugs, 4 times a day, for 10 consecutive days. On the last 2 days of drug administration, the cognitive behavior was evaluated by Y maze and novel object recognition test, and the histopathological changes in the prefrontal cortex (PFC) were observed. Ultra-high performance liquid chromatography-tandem mass spectrometry technology was used to analyze the changes of metabolites in PFC, screen for differential metabolites, and perform pathway enrichment analysis. RESULTS Compared with the control group, the morphology of PFC neurons in the ketamine group of mice was inconsistent. There were cavities around the nucleus, and the number of deeply stained cells increased. The mean optical density value of the Nissl staining positive area was significantly reduced, and the alternation rate and discrimination index were significantly reduced (P<0.05 or P<0.01). In the PFC tissue samples of mice of the two groups, there were a total of 114 differential metabolites, including 73 up-regulated and 41 down-regulated metabolites, including glutamine, succinic acid, ketoglutarate, and choline, etc. The differential metabolites mentioned above were mainly enriched in metabolism of alanine, aspartate and glutamate, metabolism of arginine and proline, γ aminobutyric acid synapses, pyrimidine metabolism, cholinergic synapses pathways, etc. CONCLUSIONS Ketamine can induce cognitive impairment in mice. Its neurotoxicity is related to abnormal synaptic transmission and energy metabolism, and neuroimmune regulation disorders.
5.Effect of hypoproteinemia on cefoperazone sulbactam plasma concentration

Objective To analyze the effect of hypoproteinemia on the use of cefoperazone sulbactam in patients, and provide references for the rational use of cefoperazone sulbactam in clinical practice. Methods Clinical data of patients with cefoperazone blood concentration monitoring in our hospital from July 2021 to March 2022 were collected, and divided into low albumin group and normal albumin group according to different albumin concentration. Statistical analysis were conducted based on gender, age, laboratory examination, cefoperazone valley concentration and clinical efficacy of the two groups. Results Age, gender, inflammation and liver function of two groups had not shown statistical difference (P>0.05). Low protein group had significantly higher levels of creatinine than those in group with normal albumin (P<0.05). The trough concentration of cefoperazone in the low protein group was (46.29±36.94) mg/L, and that in the normal albumin group was (38.18±33.79) mg/L. Among the low protein patient group, the plasma concentration of cefoperazone in those treated with human albumin was significantly higher than that in the patients without human albumin treatment (P<0.05). Comparison of the clinical efficacy of the two groups revealed that the low protein group had a lower clinical response rate (P<0.05). Conclusion Supplementation of human serum albumin in patients with hypoproteinemia could increase the plasma concentration of cefoperazone, and correcting hypoproteinemia could be helpful for anti-infection treatment.

Background::Carotid intima-media thickness (IMT) and diameter, stiffness, and wave reflections, are independent and important clinical biomarkers and risk predictors for cardiovascular diseases. The purpose of the present study was to establish nationwide reference values of carotid properties for healthy Chinese adults and to explore potential clinical determinants.Methods::A total of 3053 healthy Han Chinese adults (1922 women) aged 18-79 years were enrolled at 28 collaborating tertiary centers throughout China between April 2021 and July 2022. The real-time tracking of common carotid artery walls was achieved by the radio frequency (RF) ultrasound system. The IMT, diameter, compliance coefficient, β stiffness, local pulse wave velocity (PWV), local systolic blood pressure, augmented pressure (AP), and augmentation index (AIx) were then automatically measured and reported. Data were stratified by age groups and sex. The relationships between age and carotid property parameters were analyzed by Jonckheere-Terpstra test and simple linear regressions. The major clinical determinants of carotid properties were identified by Pearson’s correlation, multiple linear regression, and analyses of covariance.Results::All the parameters of carotid properties demonstrated significantly age-related trajectories. Women showed thinner IMT, smaller carotid diameter, larger AP, and AIx than men. The β stiffness and PWV were significantly higher in men than women before forties, but the differences reversed after that. The increase rate of carotid IMT (5.5 μm/year in women and 5.8 μm/year in men) and diameter (0.03 mm/year in both men and women) were similar between men and women. For the stiffness and wave reflections, women showed significantly larger age-related variations than men as demonstrated by steeper regression slopes (all P for age by sex interaction <0.05). The blood pressures, body mass index (BMI), and triglyceride levels were identified as major clinical determinants of carotid properties with adjustment of age and sex. Conclusions::The age- and sex-specific reference values of carotid properties measured by RF ultrasound for healthy Chinese adults were established. The blood pressures, BMI, and triglyceride levels should be considered for clinical application of corresponding reference values.
7.Expression of ZIC5 protein in breast cancer and its relation-ship with clinicopathological characteristics and surgical prognosis

Objective:To explore the relationship between the expression of cerebellar zinc finger structure 5(ZIC5)protein in breast cancer and its clinicopathological characteristics,surgical prognosis.Methods:A total of 128 breast cancer patients,who were admitted to our hospital be-tween June 2016 and December 2017,were enrolled in this study.Immunohistochemistry was employed to assess the expression of ZIC5 protein.The positive expression rate of ZIC5 protein in breast cancer tissues was compared with that in normal surgical margin tissues.Additionally,an analysis was conducted to explore the association between the expression of ZIC5 protein and various clinical pathological characteristics of breast cancer patients,as well as its correlation with prognosis.Results:The positive expression rate of ZIC5 protein in breast cancer tissue was higher than that in normal tissue of resection margin.The postoperative survival rate of breast can-cer patients was 81.37%.TNM stage Ⅲ~Ⅳ,undifferentiated/poorly differentiated,tumor diam-eter ≥ 2 cm,invasion depth T3~T4,lymph node metastasis,ZIC5 protein positive expression were all risk factors for poor prognosis of breast cancer surgery.(RR=4.405,5.153,4.520,2.211,3.137,6.079,P<0.05).The survival rate of patients with negative ZIC5 protein expression in breast cancer tissue was higher than that of patients with positive ZIC5 protein expression(P<0.05).Conclu-sion:The positive expression rate of ZIC5 protein in breast cancer tissue is higher than that in nor-mal tissue of resection margin,and the expression of ZIC5 protein in cancer tissue is related to clinical stage,tumor differentiation,tumor size,invasion depth,and lymph node metastasis.The above indicators are risk factors for poor prognosis of breast cancer surgery.
8.Efficacy and Safety Analysis of the Interventional Treatment Through the Distal Transradial Access in Patients With Complex Coronary Lesions

Objectives:Present study analyzed the efficacy and safety of percutaneous coronary intervention(PCI)using the distal transradial access(dTRA)for patients with complex coronary lesions. Methods:A total of 10 033 patients with complex coronary artery lesions(type B2 and type C lesions)who underwent percutaneous coronary intervention(PCI)via dTRA or conventional transradial access(TRA)at Fuwai Hospital between June 2021 and May 2022 were included(9 625 patients in the TRA group and 408 patients in the dTRA group).After propensity score matching,391 patients were included in each group.Baseline data,PCI intraoperative data(including lesion characteristics,intervention success rate,etc.),and incidence of major bleeding related to the access were compared between the two groups before and after propensity score matching. Results:Before propensity score matching,the proportions of patients with hypertension,hyperlipidemia,family history of coronary heart disease,history of myocardial infarction,and history of coronary artery bypass grafting were significantly higher in the dTRA group than in the TRA group(all P<0.05).After propensity score matching,the baseline data of the two groups were similar(all P>0.05).Before propensity score matching,compared with the TRA group,patients in the dTRA group had a higher proportion of patients with type B2 lesions,while the proportions of patients with type C lesions and those using intravascular ultrasound(IVUS)were lower(all P<0.05).The proportion of patients with chronic complete occlusion was similar between the two groups(P>0.05).After propensity score matching,compared with the TRA group,patients in the dTRA group had a lower proportion using IVUS and had a higher percent of stent implantation(both P<0.05).There was no statistically significant difference between the two groups in terms of SYNTAX score,guide catheter size,target lesion distribution,proportion of patients using intra-aortic balloon counterpulsation,success rate of intervention procedures,and incidence of major bleeding events related to the access(all P>0.05). Conclusions:Compared with the conventional TRA,interventional treatment of complex lesions through dTRA is equally safe and effective for patients with complex coronary lesions.
9.Role of Kv7.2 in dorsal root ganglion in reduction of paclitaxel-induced neuropathic pain by morphine in rats

Objective:To evaluate the role of Kv7.2 in the dorsal root ganglion (DRG) in reduction of paclitaxel-induced neuropathic pain (NPP) by morphine in rats.Methods:SPF healthy male Sprague-Dawley rats, aged 5 weeks, weighing 140-160 g, were randomly selected. This experiment was performed in two parts. Experiment Ⅰ Seventy-two rats were divided into control group (group C), control + morphine group (group C+ M), NPP-1 group (group NPP1) and NPP-1 + morphine group (group NPP1+ M), with 18 animals in each group. Experiment Ⅱ Twenty-four rats were divided into NPP2 group, NPP2+ ML252 group, NPP2+ morphine group (NPP2+ M group), and NPP2 + morphine + ML252 group (NPP2+ M+ ML252 group), with 6 animals in each group. The model of NPP was developed by intraperitoneal injection of paclitaxel 2 mg/kg, once every 2 days for 4 times. After preparation of the model, morphine 5 mg/kg was subcutaneously injected in C+ M group, NPP1+ M group, NPP2+ M group and NPP2+ M+ ML252 group, and potassium channel inhibitor ML252 10 mg/kg was intraperitoneally injected for 7 consecutive days in NPP2+ ML252 group and NPP2+ M+ ML252 group. Six rats were randomly selected from each group on the 1st, 3rd and 7th days after the end of continuous administration in experiment Ⅰ and from each group on the 7th day after the end of continuous administration in experiment Ⅱ for measurement of the mechanical paw withdrawal threshold (MWT) and cold paw withdrawal latency (CWL). At the end of the behavioral assessment, the rats were sacrificed and the DRG was removed for determination of Kv7.2 expression by Western blot. On the 1st day after the end of continuous administration in experiment Ⅰ, the expression of Kv7.2 mRNA in DRG was detected using quantitative real-time polymerase chain reaction, and immunofluorescence was used to detect the co-expression of Kv7.2 with neurons and glial cells in group C.Results:Experiment Ⅰ Compared with C group, the MWT was significantly increased, the expression of Kv7.2 protein and mRNA was up-regulated at each time point ( P<0.05), and no significant change was found in the CWL in C+ M group ( P>0.05), and the MWT was significantly decreased, the CWL was shortened, and the expression of Kv7.2 protein and mRNA was down-regulated at each time point in NPP1 group ( P<0.05). Compared with NPP1 group, the MWT was significantly increased, the CWL was prolonged, and the expression of Kv7.2 protein and mRNA was up-regulated at each time point in NPP1+ M group ( P<0.05). Kv7.2 in DRG was expressed in peptideergic small and medium diameter neurons, non-peptideergic small and medium diameter neurons and large diameter neurons, but not in glial cells. Experiment Ⅱ Compared with NPP2 group, no significant change was found in the expression of MWT, CWL and Kv7.2 in NPP2+ ML252 group ( P>0.05), and the MWT was significantly increased, CWL was prolonged, and the expression of Kv7.2 in DRG was up-regulated in NPP2+ M group ( P<0.05). Compared with NPP2+ M group, the MWT was significantly decreased, CWL was shortened, and the expression of Kv7.2 in DRG was down-regulated in NPP2+ M+ ML252 group ( P<0.05). Conclusions:Down-regulation of Kv7.2 expression in DRG is involved in the reduction of paclitaxel-induced NPP by morphine in rats.
10.Clinical study of Jiaotai Shugan decoction in adjuvant treatment of type 2 diabetes mellitus complicated with depression with liver qi stagnation type

Objective To investigate the clinical effect of Jiaotai Shugan decoction in adjuvant treatment of type 2 diabetes mellitus(T2DM)complicated with depression with liver qi stagnation type.Methods From June 2022 to June 2023,90 patients with T2DM complicated with depression with liver qi stagnation type treated in the Second People's Hospital of Lishui City were selected,who were randomly divided into control group and observation group,with 45 cases in each group.Both groups were treated with basic treatment of diabetes,control group was given escitalopram to improve the emotion,while observation group was further given Jiaotai Shugan decoction.After 8 weeks,the clinical effective rate were evaluated by Hamilton depression scale-24(HAMD-24)and patient health questionnaire-9(PHQ-9)scores,the levels of 5-hydroxytrytamine(5-HT),noradrenaline(NE),brain-derived neurotrophic factor(BDNF),fasting blood glucose(FPG),2-hour postprandial blood glucose(2hPG)and haemoglobinA1c(HbA1c)were detected,and adverse reactions were recorded.Results The clinical total effective rates of observation group was higher than that of control group,and the difference was statistically significant(P<0.05).8 weeks after treatment,HAMD-24 and PHQ-9 scores of observation group were lower than those of control group(P<0.01).The levels of 5-HT,NE and BDNF of observation group were higher than those of control group(P<0.01).The levels of FPG,2hPG and HbA1c in observation group were lower than those in control group(P<0.01);There was no significant difference in adverse reactions between two groups(P>0.05).Conclusion Modified Jiaotai Shugan decoction is effective in treating T2DM complicated with depression with liver qi stagnation type,can reduce HAMD-24 and PHQ-9 scores,increase the levels of 5-HT,NE,BDNF,and reduce the levels of FPG,2hPG,HbA1c.There was no obvious adverse reactions.
