Hepatocellular carcinoma (HCC) is a leading cause of cancer-associated death globally. Liver transplantation (LT) has emerged as a key treatment for patients with HCC, and the Milan criteria have been adopted as the cornerstone of the selection policy. To allow more patients to benefit from LT, a number of expanded criteria have been proposed, many of which use radiologic morphological characteristics with larger and more tumors as surrogates to predict outcomes. Other groups developed indices incorporating biological variables and dynamic markers of response to locoregional treatment. These expanded selection criteria achieved satisfactory results with limited liver supplies. In addition, a number of prognostic models have been developed using clinicopathological characteristics, imaging radiomics features, genetic data, and advanced techniques such as artificial intelligence. These models could improve prognostic estimation, establish surveillance strategies, and bolster long-term outcomes in patients with HCC. In this study, we reviewed the latest findings and achievements regarding the selection criteria and post-transplant prognostic models for LT in patients with HCC.

Aim To investigate the effect of chrysoeriol on pulmonary vascular remodeling in pulmonary hyper-tension by animal experiments combined with cell ex-periments,and to explore its potential therapeutic tar-gets by network pharmacology. Methods The target of chrysoeriol was collected in Targetnet,SEA and SwissTargetPrediction database.Pulmonary arterial hy-pertension(PAH)targets were collected in the Dis-GeNET and GeneCards databases, and PPI network map was drawn in the STRING database, and key tar-gets were screened. The GO and KEGG pathway en-richment analysis was carried out through DAVID data-base and Weishengxing platform.AutoDock software was used for molecular docking of key core targets. The PAH model of rats was constructed, and the pulmo-nary hemodynamics and vascular remodeling were de-tected by echocardiography,HE and Masson staining.Primary pulmonary smooth muscle cells were extracted, and the effects of drugs on pathway proteins were de-tected in vitro. Results The results of network phar-macology showed that chrysoeriol exerted therapeutic effects on pulmonary hypertension by affecting key tar-gets such as AKT1,SRC,EGFR,MMP9 and gsk3 β,and signaling pathways such as EGFR and PI3K-AKT.Molecular docking showed that chrysoeriol had good binding ability with 5 key target genes.Animal experi-ments showed that the pulmonary hemodynamic func-tion of PAH rats was significantly improved after ad-ministration of chrysoeriol. The remodeling of small pulmonary arteries was significantly reduced.Cell ex-periments showed that chrysoeriol could inhibit the ex-pression of proliferation, migration and phenotypic transformation genes. Conclusion Chrysoeriol may play a role in the treatment of pulmonary hypertension through multiple targets.

Objective To explore the early clinical manifestations, random cortisol levels, and management of late-onset circulatory collapse(LCC)in preterm infants. Methods Preterm infants with LCC from October to December 2023 at the Affiliated Suzhou Hospital of Nanjing Medical University were included.Maternal perinatal factors and infants'early clinical symptoms, signs, random serum cortisol levels, treatment, and outcomes were retrospectively analyzed. Results Seven preterm infants with LCC were included, with gestational ages of 25 weeks+2 days to 29 weeks and birth weights of 800-1 150 g.At 3 weeks of age, abnormal weight gain[gain rate:21-28.5 g/(kg·d)],generalized edema, low serum sodium(129.5-135.2 mmol/L),and decreased random serum cortisol concentrations(13.6-44.6 nmol/L)were observed. After 1-2 weeks of hydrocortisone treatment, edema subsided and serum sodium increased. Conclusions In clinically stable preterm infants, early manifestations of LCC include abnormal weight gain, generalized edema, recurrent hyponatremia, and decreased random serum cortisol concentrations.Hydrocortisone treatment effectively improves symptoms.

Progress in Modern Biomedicine 2025;25(19):3162-3171
Objective:To explore the predictive value of Nomogram prediction model for poor prognosis of severe pneumonia(SP)based on micrornas(miR)-221-3p,miR-155-5p and confusion, uremia, respiratory,BP,age 65years(CURB-65)score.Methods:439 SP patients who were admitted to Yichun People's Hospital from January 2021 to June 2024 were prospectively selected, they were randomly divided into modeling group(n=307)and validation group(n=132)according to the ratio of 7:3.The serum levels of miR-221-3p and miR-155-5p were detected before treatment, and the CURB-65 score was used for evaluation. The prognosis of SP patients within 28 days of hospitalization was observed, and the SP patients were divided into death group and survival group according to the prognosis within 28 days.Lasso regression was used to analyze the influencing factors of poor prognosis of SP patients, and multivariate Logistic regression was used to analyze the risk factors for poor prognosis of SP patients.A Nomogram prediction model for poor prognosis of SP patients was constructed, and the receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of the Nomogram model for poor prognosis of SP patients. Results:The mortality rates in the modeling group and the validation group were 29.32%(90/307)and 28.79%(38/132)respectively,and there was no significant difference in mortality rates and clinical data between the two groups(P>0.05).The age, proportion of underlying lung disease, pneumonia severity index(PSI)score,Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score,CURB-65 score, serum miR-221-3p,miR-155-5p,C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and procalcitonin(PCT)indexes in modeling and validation of the death group were higher than those of survival group(P<0.05).Logistic multivariate regression analysis showed that advanced age, high APACHE Ⅱ score, high expression of miR-221-3p,high expression of miR-155-5p and high CURB-65 score were risk factors for poor prognosis of SP(P<0.05).The area under the curve(AUC)of the constructed Nomogram prediction model for the poor prognosis of SP was 0.824,which had good prediction efficiency. Conclusion:High expression of miR-221-3p,high expression of miR-155-5p,high CURB-65 score, older age and high APACHE Ⅱ score are risk factors for poor prognosis of SP patients, and the Nomogram prediction model based on the above factors has a high predictive value for the poor prognosis of SP.
