An assay was established for detection of toxigenic Clostridioides difficile by combining microfluidic chip analysis with immunochromatography, and its performance was evaluated and compared with those of the Xpert C.difficile/Epi and VIDAS CD AB tests.Primer pairs were designed according to the tcdB and tpi genes in C.difficile.The specificity, limit of detection, reproducibility, and stability were evaluated.A total of 215 stool samples from patients with diarrhea were collected and tested in parallel with the Xpert C.difficile/Epi,VIDAS CDAB,and our assay.C.difficile was isolated from samples, and the tcdB gene was identified when discrepant results were obtained from the three above assays. Our assay showed no cross-reaction with other diarrhea-associated pathogens. Its reproducibility was 100%in testing of two standard plasmids containing tcdB and tpi genes at two concentrations(105 and 102 copies/μL).Two standard plasmids were detected after the PCR and immunochromatography reagents had been stored for 3,6,9,and 12 months, and all the results were posi-tive.The limit of detection was 10 copies/μL for toxigenic C.difficile.Testing of 33 samples positive for C.difficile with our assay(33/215,15.3%)yielded findings statistically coherent with those of the Xpert C.difficile/Epi test(kappa value=0.965).The sensitivity, specificity, positive predictive value, and negative predictive value of our assay, with respect to Xpert C.difficile/Epi as the standard, were 94.3%,100.0%,100.0%,and 98.9%;these values were significantly higher than those of VIDAS CDAB(60.0%,98.9%,91.3%,and 92.7%)(Kappa=0.714,OR=157.50,95%CI:62.03-847.28,P=0.013).In conclusion, our newly developed assay is specific, stable, and reproducible, and may be used for rapid and accu-rate detection of toxigenic C.difficile.The assay could be used for C.difficile infection screening in outpatient and emergen-cy,community medical service center, and epidemiological settings.

The pregnant woman was 30 years old,G2P0.This singleton pregnancy at 22 weeks of gestation was screened for second-trimester ultrasound malformations, suggesting fetal aortic valve atresia, aortic stenosis with reverse blood flow, mitral valve atresia, and markedly enlarged left ventricle, which was considered for the diagnosis of hypoplastic left heart syndrome(HLHS).The pregnancy was terminated at our hospital and subsequently underwent genetic testing with results of heterozygous variants in the NOTCH1 gene, which can cause aortic valve disease type 1.The findings of the fetal autopsy were aortic valve atresia, mitral valve widening and thickening, and left ventricular enlargement with myocardial infarction. This report focuses on the ultrasound characteristics of HLHS with left ventricular enlargement and its hemodynamic changes in order to improve clinicians'understanding of the progressive changes in the disease phenotype of HLHS.

Aim To investigate the effect of chrysoeriol on pulmonary vascular remodeling in pulmonary hyper-tension by animal experiments combined with cell ex-periments,and to explore its potential therapeutic tar-gets by network pharmacology. Methods The target of chrysoeriol was collected in Targetnet,SEA and SwissTargetPrediction database.Pulmonary arterial hy-pertension(PAH)targets were collected in the Dis-GeNET and GeneCards databases, and PPI network map was drawn in the STRING database, and key tar-gets were screened. The GO and KEGG pathway en-richment analysis was carried out through DAVID data-base and Weishengxing platform.AutoDock software was used for molecular docking of key core targets. The PAH model of rats was constructed, and the pulmo-nary hemodynamics and vascular remodeling were de-tected by echocardiography,HE and Masson staining.Primary pulmonary smooth muscle cells were extracted, and the effects of drugs on pathway proteins were de-tected in vitro. Results The results of network phar-macology showed that chrysoeriol exerted therapeutic effects on pulmonary hypertension by affecting key tar-gets such as AKT1,SRC,EGFR,MMP9 and gsk3 β,and signaling pathways such as EGFR and PI3K-AKT.Molecular docking showed that chrysoeriol had good binding ability with 5 key target genes.Animal experi-ments showed that the pulmonary hemodynamic func-tion of PAH rats was significantly improved after ad-ministration of chrysoeriol. The remodeling of small pulmonary arteries was significantly reduced.Cell ex-periments showed that chrysoeriol could inhibit the ex-pression of proliferation, migration and phenotypic transformation genes. Conclusion Chrysoeriol may play a role in the treatment of pulmonary hypertension through multiple targets.

Objective:To compare the current situation of research on workplace violence (WPV) among nurses at home and abroad, providing directions for further research. Methods:Research on WPV among nurses published in PubMed, Web of Science, EBSCO, China national knowledge infrastructure, Wanfang, and VIP databases from January 1, 2003 to October 14, 2023 were searched and screened, using visualization software CiteSpace and VOSviewer to analyze the distribution and correlation of countries, authors, institutions and keywords of the included studies. Results:A total of 1 082 Chinese articles and 2 770 English articles were included. From 2003 to 2023, the annual publication volume of research on nurse WPV showed a continuous upward trend in both Chinese and English literature. Among domestic institutions, Harbin Medical University published the most articles ( n=25). Among international research institutions, North South University ( n=9) and University of Malaya ( n=9) led in publication output. A total of 67 core authors were identified in Chinese literature and 194 in the English literature. Analysis of high-frequency keywords showed that the research topics could be summarized as research types, occurrence mechanisms, negative effects, high-risk precursors, intervention strategies, negative effects and population differences. Chinese Nursing Research ( n=40) and Journal of Nursing Management ( n=186) published the most Chinese and English articles. Conclusions:The amount of research on workplace violence among nurses has generally increased in recent years. The mechanisms of occurrence, high-risk precursors, and intervention strategies are important research directions. It is still necessary to further deepen the research content in the future.

Ischemic stroke and hemorrhagic stroke have different pathogenic mechanisms, but share similarities in metabolic dysregulation, inflammatory responses and oxidative stress. This paper summarized 28 metabolic markers shared between ischemic stroke and hemorrhagic stroke with consistent trends through literature review. It also provided an overview of their involvement in abnormal energy metabolism, inflammatory responses,blood-brain barrier disruption, and neural damage in relation to stroke. The aim is to provide a scientific basis for future prognosis, curative efficacy evaluation and future homotherapy of ischemic stroke and hemorrhagic stroke, and provide insights for the development of new therapies and new drugs.
