1.Protective Effect of MicroRNA-200a&c on Angiostrongylus cantonensis-induced Demylinating Optic Neuritis in Balb/c Mice

[Objective]To investigate the protective effect of overexpressed miRNA-200a&c on Angiostrongylus cantonensis-induced demyelinating optic neuritis in Balb/c mice.[Methods]SPF-grade Balb/c mice(2-3 weeks old)were divided into four groups:a normal group,and three A.cantonensis-infected groups at 7,14,and 21 days post-infection(dpi).Body weight,survival status,neurobehavioral scores,and visual function scores were recorded.Visual evoked potential(VEP)was used to detect visual damage,and transmission electron microscope(TEM)was applied to observe ocular structural changes.On day 7 post-infection,mice were stereotactically injected with exogenous miRNA-200a&c mimics into the lateral ventricle,and then divided into four groups:normal control,A.cantonensis-infected(AC-21 dpi),A.cantonensis-infected+negative control(AC+NC),and A.cantonensis-infected+overexpressed miRNA-200a&c(AC+miRNA-200a&c)mimics.VEP and TEM were repeated to assess visual damage and ocular structural changes.Immunofluorescence was performed to quantify retinal ganglion cells(RGCs)and oligodendrocytes(OLs)in the optic nerve.[Results]At 21 dpi,some mice exhibited complete eyelid closure(32.52±4.67)%or ocular atrophy(15.79±3.23)%,weight loss(P<0.05)and altered consciousness.Neurobehavioral scores significantly decreased(P<0.01),with a 68%decline in rotarod performance;some mice even displayed hemiplegia,slowed movement,ataxia,and directional deficits.Additionally,a subset of mice showed diminished sensory responses,unilateral vision loss(83%reduction in optokinetic threshold),and impaired visual function(P<0.05).VEP results revealed a mild prolongation of latency in infected mice at 21 dpi(P<0.05),predominantly affecting one eye.Following overexpression of miRNA-200a&c,compared with the 21 dpi group,VEP showed significantly shortened P1 latency(P<0.05);TEM showed alleviated cytoplasmic swelling of RGCs,and improved compactness and uniformity of myelin sheath(P<0.05);immunofluorescence showed increased numbers of RGCs and OLs with improved cell alignment(P<0.05).[Conclusions]A.cantonensis infection induces demyelinating optic neuritis in Balb/c mice.Overexpression of miRNA-200a&c alleviates the resulting damage and ameliorates ocular injury.
2.Mechanism of circ-0058063 on ferroptosis in esophageal cancer cells

Objective To investigate the mechanism of action of circ-0058063 regulating ERK/MAPK signaling pathway on ferroptosis in esophageal cancer cells.Methods EC9706 cells were randomly divided into the Control group,the si-NC group,the si-circ-0058063 group,and the si-circ-0058063+ISO group.qRT-PCR was used to detect the expression of circ-0058063 in esophageal cancer tissues and cells;CCK-8 and clone formation assay were used to detect the cell proliferation ability.The levels of Fe2+,malondialdehyde(MDA),glutathione(GSH),and reactive oxygen species(ROS)in cells were detected.Western blot was used to detect the ERK/MAPK signaling pathway and the expression of ferroptosis-related protein in cells.Results The expression level of circ-0058063 in esophageal cancer tissue was significantly higher than that in adjacent tissues(P<0.05);the expression level of circ-0058063 in human esophageal cancer cell EC9706 was significantly higher than that in human normal esophageal epithelial cell HEEC(P<0.05).Compared with the Control group,the expression level of circ-0058063 in cells,cell survival rate,number of cloned cells,GSH level,ratios of p-ERK1/2/ERK1/2 and p-p38MAPK/p38MAPK,as well as the expression levels of SLC7A11 and GPX4 protein in the si-circ-0058063 group were significantly reduced(P<0.05),while the levels of Fe2+,MDA,and ROS in cells were significantly increased(P<0.05).Compared with the si-circ-0058063 group,the expression level of circ-0058063 in cells,cell survival rate,number of cloned cells,GSH level,ratios of p-ERK1/2/ERK1/2 and p-p38MAPK/p38MAPK,as well as the expression levels of SLC7A11 and GPX4 protein in the si-circ-0058063+ISO group were significantly increased(P<0.05),while the levels of Fe2+,MDA,and ROS in cells were significantly decreased(P<0.05).Conclusion Knockdown of circ-0058063 can inhibit the proliferation of esophageal cancer cells and induce ferroptosis,and its mechanism of action may be related to the inhibition of ERK/MAPK signaling pathway activation.
3.Efficiency of MRI combined with TVCDS in diagnosing interstitial infiltration depth and pelvic lymph node metastasis of cervical cancer

Objective:To explore application value of magnetic resonance imaging(MRI)combined with transvaginal color Doppler ultrasound(TVCDS)in diagnosing interstitial infiltration depth and pelvic lymph node metastasis of cervical cancer.Methods:A total of 86 patients with cervical cancer who admitted to Dalian The Fourth People's Hospital between January 2021 and April 2024 were selected as the study subjects,and all of patients received MRI and TVDCS examinations,and the result of pathological examination was used as"gold standard".The diagnostic value of MRI and TVCDS examinations for interstitial infiltration depth and pelvic lymph node metastasis were analyzed respectively.Results:Single MRI,TVCDS,and the combination of them have a certain consistency with postoperatively pathological result in diagnosing the depth of interstitial infiltration of cervical cancer(Kappa=0.735,0.712,P<0.05),while the consistency of the combined examination was the highest(Kappa=0.879,P<0.05).The sensitivity,specificity,and area under curve(AUC)of receiver operating characteristic(ROC)curve of the combined diagnosis of MRI and TVCDS in diagnosing the depth of interstitial infiltration of cervical cancer were respectively 87.46%,90.65%and 0.847(95%CI:0.683-0.957),which were higher respectively than the diagnostic value of each single diagnosis.Single MRI,TVCDS,and the combination of them have a certain consistency with postoperatively pathological result in diagnosing the pelvic lymph node metastasis of cervical cancer,while the highest consistency was observed in the combined examination(Kappa=0.823,P<0.05).The sensitivity,specificity and AUC value of the combination of MRI and VCDS were respectively 89.69%,92.25%,and 0.891(95%CI:0.714-0.968)in diagnosing pelvic lymph node metastasis of cervical cancer,which were higher respectively than the diagnostic value of each single diagnosis.Conclusion:MRI combined with TVCDS is helpful to diagnose the interstitial infiltration depth and pelvic lymph node metastasis of cervical cancer,which can provide important reference for formulating treatment plan of cervical cancer.

Jiegengtang is a basic formula for treating sore throat and cough. By means of bibliometrics, this study conducted a textual research and analysis on the key information such as formula origin, decocting methods, and clinical application of Jiegengtang. After the research, it can be seen that Jiegengtang is firstly contained in Treatise on Febrile and Miscellaneous Disease, which is also known as Ganjietang, and it has been inherited and innovated by medical practitioners of various dynasties in later times. The origins of Chinese medicines in this formula is basically clear, Jiegeng is the dried roots of Platycodon grandiflorum, Gancao is the dried roots and rhizomes of Glycyrrhiza uralensis, the two medicines are selected raw products. The dosage is 27.60 g of Glycyrrhizae Radix et Rhizoma and 13.80 g of Platycodonis Radix, decocted with 600 mL of water to 200 mL, taken warmly after meals, twice a day, 100 mL for each time. In ancient times, Jiegengtang was mainly used for treating Shaoyin-heat invasion syndrome, with cough and sore throat as its core symptoms. In modern clinical practice, Jiegengtang is mainly used for respiratory diseases such as pharyngitis, esophagitis, tonsillitis and lung abscess, especially for pharyngitis and lung abscess with remarkable efficacy. This paper can provide literature reference basis for the modern clinical application and new drug development of Jiegengtang.

Objective:To analyze the clinical features and prognosis of acute B lymphoblastic leukemia (B-ALL) children carrying a TCF3: : PBX1 fusion gene and to evaluate the prognostic value of this gene.Methods:Retrospective cohort study.A total of 2 164 B-ALL children aged 0-18 years diagnosed and treated at 19 pediatric centers from October 2016 to June 2022 were enrolled.They were divided into the positive group and the negative group according to whether they carried a TCF3: : PBX1 fusion gene.The clinical characteristics, treatment response, adverse reactions, and prognosis of the 2 groups of patients were analyzed.The rank sum and Kruskal-Wallis tests were used to compare two and more than two groups of numerical variables, respectively.Fisher′s exact test was used to compare categorical variables.Results:Among the 2 164 patients, 116 (5.4%) were TCF3: : PBX1 positive, of which 70 patients were female, accounting for 60.3%.There were 840 female patients in the TCF3: : PBX1-negative group, accounting for 41.0%.There was a significant difference in the ratio of females between the TCF3: : PBX1-positive and TCF3: : PBX1-negative groups ( P<0.001).No significant difference was observed in age of onset between the two groups( P>0.05).The proportion of bone marrow naive cells [54.00 (14.00, 76.50)% vs.29.00 (3.00, 68.00)%], white blood cell counts [25.30 (10.46, 60.94)×10 9/L vs.9.03 (4.38, 30.73)×10 9/L] and hemoglobin counts [82.00(63.00, 101.00) g/L vs.74.00(60.00, 90.00) g/L] in the TCF3: : PBX1-positive group were significantly higher than those in the negative group at the onset (all P<0.05).In terms of treatment response, the proportion of peripheral blood naive cells on Day 8 in the TCF3: : PBX1-positive group was significantly higher than that in the negative group [2.00 (0, 9.00)% vs.0 (0, 2.00)%, P<0.001].The proportion of minimal residual disease <0.1% on Day 15 in the TCF3: : PBX1-positive group was significantly higher than that in the negative group ( P=0.038).There were no significant differences in cumulative recurrence rate, treatment-related mortality (TRM), and overall survival (OS) between the TCF3: : PBX1-positive group and TCF3: : PBX1-negative group (all P>0.05).The cumulative recurrence risk of TCF3: : PBX1-positive patients was 9.646 times higher than that of ETV6: : RUNX1-positive patients with better prognosis( HR=9.646, 95% CI: 1.026-90.700, P=0.047).There were no significant differences in TRM and OS between TCF3: : PBX1-positive and ETV6: : RUNX1-positive patients (all P>0.05).A significant enrichment of PAX5 mutations was detected in TCF3: : PBX1-positive patients.Among the 7 high-risk TCF3: : PBX1-positive patients in a single center, 4 patients had PAX5 mutations, and this proportion was significantly higher than that in other patients ( P<0.001). Conclusions:B-ALL children carrying a TCF3: : PBX1 fusion gene have a high remission rate and good long-term prognosis after intensive chemotherapy.It is suggesting that TCF3: : PBX1-positive B-ALL patients should be rated at intermediate risk to receive intensive chemotherapy.
7.Early screening and diagnosis of prostate cancer based on the innovative care for chronic conditions framework

Objective:To construct an integrated management model for early screening and diagnosis of PCa based on the In-novative Care for Chronic Conditions Framework(ICCC)and the 1+1 contract-based tiered diagnosis and treatment system(TDTS)in China.Methods:Based on the 1+1 contract-based TDTS platform,we conducted PCa screening for the male residents aged 60 years and above during health check-ups in Pujin Community Health Center from January 1,2023 to December 31,2023.For those with abnormal total prostate-specific antigen(tPSA)≥4 μg/L,we promptly referred them to higher-level hospitals for further diagno-sis and treatment via the two-way referral green channel platform and information sharing service using the 1+1 contract model.We further analyzed the relevant data on screening and diagnosis.Results:A total of 4 080 males aged 71.39±5.059 years received PCa screening from January to December 2023.PSA screening was performed in 43.96%of the male residents,revealing 654 cases of PSA abnormality,with a PSA positivity rate of 16.03%,which was higher than that found in the previous large-scale PCa screenings in other regions of China.Among the males with PSA abnormality,292(44.65%)expressed their willingness for medical referral,while the others did not seek further medical attention for reasons of being asymptomatic,low awareness of the disease,no accompany for medical visits,and concerns about further costs of diagnosis and treatment.Prostate biopsy was recommended to 154 cases after further examinations,which was accepted by 92(59.74%).Fifty-eight cases were diagnosed with Pa,and thedetection rate reached 63.04%.Conclusion:The integrated management model for PSA examination-based early screening and diagnosis of PCa using the 1+1 contract-based TDTS platform is plays a significant role in enhancing peoples awareness and knowledge of PCa and improving the early detection rate of the malignancy.
8.Evaluate Value of Chest Vascular CT Parameters Combined with PLR and IL-17 for the Prognosis of Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Hypertension

Objective:To explore evaluate value of chest vascular computerized tomography(CT)parameters combined with platelet-to-lymphocyte ratio(PLR)and Interleukin(IL)-17 for the prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with pulmonary hypertension.Methods:92 patients with AECOPD complicated with pulmonary hypertension admitted to our hospital from February 2023 to June 2024 were prospectively selected as the research objects,and they were divided into poor prognosis group(n=41)and good prognosis group(n=51)according to the different prognosis.The chest vascular CT parameters,PLR and IL-17 indicators of the two groups were compared.Multivariate Logistic regression analysis were used to analyze the influencing factors of prognosis in patients with AECOPD complicated with pulmonary hypertension.The receiver operating characteristic(ROC)curve was used to analyze the evaluate value of chest vascular CT parameters,PLR and IL-17 for the prognosis of patients with AECOPD complicated with pulmonary hypertension.Results:Among 92 patients with AECOPD complicated with pulmonary hypertension,41 patients were readmitted for treatment,and the incidence of poor prognosis was 44.57%.The age and disease duration of patients in poor prognosis group were higher than those in good prognosis group(P<0.05).The ascending aortic diameter(AAD),main pulmonary artery diameter(MPAD),descending aortic diameter(DAD),PLR and IL-17 in poor prognosis group were higher than those in the good prognosis group(P<0.05).Multivariate logistic analysis showed that elevated MPAD,AAD,DAD,IL-17,PLR,advanced age and long disease course were risk factors for poor prognosis in patients with AECOPD complicated with pulmonary hypertension.ROC curve analysis showed that the area under the curve(AUC)of the chest vascular CT parameters MPAD,AAD,DAD combined with PLR and IL-17 for evaluating the prognosis of AECOPD complicated with pulmonary hypertension was 0.923,which was significantly higher than that evaluated by each index alone.Conclusion:Elevated MPAD,AAD,DAD,IL-17,PLR,advanced age and long disease course are risk factors for poor prognosis in patients with AECOPD complicated with pulmonary hypertension,the combined detection of chest vascular CT parameters and PLR,IL-17 has a high evaluating value for the prognosis of patients with AECOPD complicated with pulmonary hypertension.
9.Untargeted Metabolomics Analysis of Demyelination in the Brain of Balb/c Mice Infected by Angiostrongylus cantonensis

[Objective]To investigate the demyelination induced by Angiostrongylus cantonensis(AC)infection in the brain of Balb/c mice and analyze the untargeted metabolomic changes in the corpus callosum,aiming to elucidate the underlying mechanisms.[Methods]Balb/c mice were randomly assigned to a control group(n=6)and an infection group(n=6).The infection group was orally administered 30 third-stage larvae of AC,while the control group received an equal volume of saline.Body weight,visual function,and behavioral scores were measured on post-infection 3,6,9,12,15,18,and 21 days to assess neurological alterations.After 21 days,brain tissues were harvested for immunofluorescence staining,hematoxylin-eosin(HE)staining,and transmission electron microscopy to examine morphological changes in brain myelin and retina.Metabolomics analysis was performed,and differential metabolites were identified using volcano plots and heatmaps.The distribution of fold changes and bar charts were used to profile the key metabolites.These differential metabolites were then subjected to Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis and regulatory network analysis.[Results]On the 9th day after AC infection,Balb/c mice showed a decline in neurological behavioral scores(P<0.05).By day 15,visual scores decreased(P<0.05),and by day 21,significant weight loss(P<0.001)and mortality were observed.Concurrently,transmission electron microscopy and immunofluorescence staining revealed significant myelin damage in the corpus callosum and a marked reduction in oligodendrocytes(P<0.001).HE staining showed severe retinal ganglion cell damage.Metabolomic analysis revealed that glycerophospholipids were the most abundant differential metabolites,with steroids and sphingolipids being relatively less abundant.Cholesteryl ester CE(20:2)was significantly upregulated(P<0.001),while phosphatidylmethanol(18:0_18:1)was significantly downregulated(P<0.01).KEGG enrichment and regulatory network analyses demonstrated that the differential metabolites were mainly enriched in metabolic pathways like steroid biosynthesis,bile secretion,and cholesterol metabolism,and were involved in key metabolic pathways such as sphingolipid metabolism,neural signal regulation,and glycerophospholipid metabolism.[Conclusions]AC infection affects the metabolic state of mice via multiple pathways,modifying the levels of metabolites crucial for myelination and myelin stability.Demyelination may be closely linked to the disruption of these key metabolic pathways,particularly the dysregulation of cholesterol and sphingolipid metabolism,potentially playing a central role in demyelination onset.Furthermore,alterations in phospholipid metabolism and abnormal nerve signaling regulation may exacerbate myelin damage.
10.Integrated Multicomponent Pharmacokinetics and Metabolomics Study on the Pharmacokinetic Characteristics and Pharmacodynamic Markers of Bilberry(Vaccinium myrtillus)Anthocyanins in Rats

Objective To elucidate the pharmacokinetic characteristics and primary metabolic pathways of anthocyanins in vivo,as well as the endogenous metabolites exhibiting sensitive responses to bilberry extract intervention in rats by LC-MS/MS technology,following oral administration of bilberry extract.Methods Multi-component pharmacokinetic technology was employed to study the systemic exposure characteristics of anthocyanins in blood,the metabolic product profile,and the dynamic concentration changes of anthocyanins and their metabolites in plasma after oral administration of bilberry extract to rats.Metabolomics technology was used to analyze changes in endogenous metabolites at the time point when most anthocyanins reached their peak plasma concentration.Results After administration of European cranberry extract,a total of 18 anthocyanin parent compounds and 14 metabolites were detected in rat plasma.Glucuronidation was identified as the primary metabolic pathway.The time to reach peak concentration(Tmax)for most parent compounds ranged from 15 to 60 minutes.Moreover,30 minutes after oral dosing with the cranberry extract,the levels of various endogenous acyl-carnitines,fatty acids,and lysophosphatidylcholines in the blood decreased.The plasma exposure of most anthocyanin components and their metabolites showed a highly linear correlation with the administered dose(R2>0.9).Conclusion This study clarified the main pharmacokinetic characteristics and metabolic pathways of anthocyanins from bilberry extract in rats,as well as the potential impact on various endogenous metabolites.It provides data support for effectively exploring the pharmacodynamic material basis and mechanism of action of bilberry extract.
