1.Current status of research on the mechanism of action of emodin in the prevention and treatment of chronic liver diseases

Chronic liver diseases are a group of diseases in which the liver is subjected to a variety of injuries over a long period of time, resulting in irreversible pathological changes that last longer than 6 months. Emodin (EMO) is a natural anthraquinone derivative derived from Rheum officinale, and its pharmacological effect has been extensively studied, exhibiting a variety of biological properties and involving multiple signaling molecules and pathways. Western medicine or surgical treatment is currently the main treatment regimen for chronic liver diseases, and the advance in treatment is limited by various reasons such as side effects and high costs. Due to its natural origin and efficacy, EMO has unique advantages in the treatment of chronic liver diseases and has now become a research hotspot. This article summarizes the therapeutic effect of EMO on chronic liver diseases and its mechanism, in order to provide a certain scientific basis for the traditional Chinese medicine treatment of chronic liver diseases and the development of drugs in clinical practice.

Ulcerative colitis (UC) is a refractory disease of the digestive system characterized by diverse etiologies, complex pathogenesis, a prolonged course, and frequent relapses. In recent years, the incidence of UC has been increasing annually, severely impairing patients' quality of life, posing a risk of malignant transformation that may threaten patients' lives, and resulting in a substantial medical burden. Traditional Chinese medicine (TCM) compound formulas, with their advantages of multi-component and multi-target actions, have become a new therapeutic option for UC. The NOD-like receptor pyrin domain-containing 3 (NLRP3) inflammasome is a core component of innate immunity, and its aberrant activation is closely associated with the onset and progression of UC, involving multiple processes such as inflammation and oxidative stress, and exhibiting crosstalk with pathways including nuclear factor-κB (NF-κB), nuclear factor erythroid 2-related factor 2 (Nrf2), and thioredoxin-interacting protein (TXNIP). At present, NLRP3 has become one of the most intensely studied hotspots in UC-related research. Although increasing studies have focused on the regulation of the NLRP3 inflammasome by TCM compound formulas for UC treatment, challenges remain due to the complex pathogenesis of UC and the compositional diversity of TCM, hindering the realization of precision therapy. In this context, by reviewing literature from the past decade, this paper summarizes the activation process of NLRP3 and its relationship with UC, and elucidates the roles and mechanisms by which TCM compound formulas regulate the NLRP3 inflammasome and related signaling pathways, with a view to providing a reference for further research into the pathogenesis of UC, TCM treatment strategies, and their mechanisms of action.

5.Related factors for dysphagia in patients with recent small subcortical infarction:an analysis of 188 cases

Objective To investigate the incidence and risk factors of dysphagia in patients after re-cent small subcortical infarction(RSSI).Methods A total of 188 RSSI patients admitted to our department from May 2018 to May 2024 were enrolled,and according to Gugging swallowing screen(GUSS),they were divided into dysphagia group(GUSS score≤19,n=51)and non-dysphagia group(the score=20,n=137).The clinical manifestations and imaging data were com-pared between the two groups.Results When compared with the non-dysphagia group,the dys-phagia group had significantly older age,larger proportion of dysarthria,higher NIHSS and mRS scores,larger lesion diameter,higher incidence of pontine infarction,and higher scores of periven-tricular and deep white matter hyperintensities,but lower scores of mini-mental state examination and Montreal cognitive assessment(P<0.05,P<0.01).Binary logistic regression analysis showed that age,dysarthria,NIHSS score,lesion diameter,and pontine infarction were risk factors for dysphagia in RSSI patients(OR=1.203,95%CI:1.070-1.352;OR=34.464,95%CI:5.013-236.942;OR=4.579,95%CI:2.180-9.617;OR=0.623,95%CI:0.463-0.838;OR=0.020,95%CI:0.002-0.191,P<0.01).Conclusion For RSSI patients,especially those with older age,larger lesion diameter,dysarthria,severe neurological deficits,and pontine infarction,clinicians should be alert to the occurrence of dysphagia in order to avoid serious complications.
6.Varicocele ligation with mobile phone microscope:Report of 5 cases and literature review

Objective:To investigate the feasibility of varicocele ligation with mobile phone microscope.Methods:The high-performance mobile phone and mobile phone stand were combined to act as a mobile phone microscope.And the varicocele ligation was performed under the mobile phone microscope.Results:All five patients successfully underwent varicocelectomy under the guidance of a mobile phone microscope.The average operation time was(112.8±52.2)with ranged from 74.0 to 195.0 minutes.Three pa-tients completed the follow-up after the operation with the proportion of improved sperm quality reaching 100.0%(3/3).Conclusion:High-performance mobile phone microscope can be used for varicocele ligation.
7.Effect of donepezil combined with hypoxia on CYP3A4 and its safety-evaluation

Aim To investigate the regulatory mecha-nisms of donepezil on the expression and enzymatic ac-tivity of cytochrome P450 3A4(CYP3A4),elucidate the synergistic impact of hypoxia on CYP3A4 function,and reveal its potential association with drug-induced cardiotoxicity,particularly QT interval prolongation.Methods Western blot,co-immunoprecipitation,and gene knockdown techniques were employed to evaluate the effects of donepezil and hypoxia on CYP3A4 pro-tein expression.CYP3A4 enzymatic activity was as-sessed using an in vitro incubation system with rat liver microsomes combined with high-performance liquid chromatography(HPLC),and the half-maximal inhib-itory concentration(IC50)was determined.Results Donepezil(10 μmol·L-1)and hypoxia reduced CYP3A4 protein expression to 31.75%and 45.90%of the control levels,respectively.Both interventions activated the gp78-mediated ubiquitin-proteasome path-way,significantly increasing CYP3A4 ubiquitination levels by 2.1-fold compared to the control group,thereby promoting proteasomal degradation.Donepezil inhibited CYP3A4 enzyme activity with an IC50 of 83.4μmol·L-1,and hypoxia synergistically enhanced this inhibitory effect,reducing the IC50 to 20.79 μmol·L-1.Conclusion Donepezil downregulates CYP3A4 function through dual mechanisms involving ubiquitin-mediated proteasomal degradation and direct enzymatic inhibition.Hypoxia potentiates this effect,leading to impaired metabolism of CYP3A4 substrate drugs,ele-vated plasma drug concentrations(1.6-2.3-fold in-crease compared to normal metabolic conditions),and an increased risk of QT interval prolongation and other forms of cardiotoxicity.

Clinical pharmacist teacher training is an important mean to improve the quality of clinical pharmacy talent cultivation and ensure the service ability and level of the clinical pharmacist team.The Pharmacy Administration and Pharmacy Practice in Healthcare Institutions-Part 4-8-2:Pharmacy Administration-Pharmacy Training Management-Clinical Pharmacist Teacher Training was based on the newly revised management document for clinical pharmacist teacher training of the Chinese Hospital Association.After sorting out relevant materials,such as standards,policies and regulations,technical specifications,liter-ature,documents of the Chinese Hospital Association,expert opinions,and the current situation of clinical pharmacist teacher training in China,the standard was formulated.In the standard,12 key elements,which can be divided into 3 parts of base manage-ment,training process and assessment,quality management and evaluation improvement,were standardized.This article aimed to introduce the construction method and content of the standard,to facilitate the understanding of the standard content for medical institutions which joined or willing to join the clinical pharmacist teacher training base,and to provide a reference for other medi-cal institutions to carry out related work.
9.Feasibility study of using clinical trial individual-level data sample bank as external control to support drug and device development:taking transcatheter aortic valve replacement device as an example

Objective To explore the feasibility and corresponding implementation methods of constructing a sample resource bank based on individual-level data of completed clinical trials and using it to construct external controls for drug/device clinical trials.Methods Taking the pre-marketing clinical trial of transcatheter active valve replacement(TAVR)for the treatment of aortic valve stenosis as an example,the individual-level databases of multiple trials were standardized to form a sample bank.The original data of any trial in the sample bank were selected as the experimental group,and the remaining samples were selected as the control group.The potential confounding was handled by using the propensity score matching and stratification methods to clarify the process of constructing external controls based on the sample bank of individual-level data of clinical trials.Results This study included individual-level data of single-group trials of 4 TAVR devices,with a total of 569 subjects(59.2%male).The number of subjects in Trials 1 to 4 was 120,120,163,and 166,respectively.Propensity score matching enabled the matching of 113,117,125,and 147 subjects with comparable or similar characteristics from individual-level data from other trials,respectively,demonstrating a high matching success rate.The PS score distribution plot after stratification showed that the proportions of subjects in the experimental and control groups in strata 1 to 5 in scheme 1 were 4/103,11/103,22/92,32/87,and 51/64,respectively.For all constructed external controlled trials,a certain number of control samples with similar baseline characteristics to the experimental groups were distributed within each propensity score stratum.The results of the simulation test also reflected the potential differences between different devices in the 12-month all-cause mortality rate.Conclusions The sample bank constructed with individual-level data from clinical trials,as a high-quality data source,can serve as a source of external control for single-arm trials in the same field,and as a useful supplement to the external control scenario of real-world evidence to support drug and device development.At the same time,targeted research on research methods and bias control measures in related fields is also needed.

Dark plum can be used to treat symptoms such as consumptive thirst due to deficiency-heat and chronic cough due to lung deficiency.Its active ingredients have auxiliary effects on lowering blood glucose,antibacterial and anti-inflammatory activities.Insulin resistance is mainly characterized by the weakening of the physiological effects of insulin in the body,with a relatively complex mechanism that can lead to various metabolic-related diseases and seriously affect health.The active ingredients of dark plum can improve insulin resistance by regulating insulin signaling pathways,endoplasmic reticulum stress,antioxidant stress,inflammatory signaling pathways,levels of related inflammatory mediators,and free fatty acid levels.By reviewing the relevant literature on the improvement of insulin resistance by the active ingredients of dark plum,this article summarizes and analyzes its mechanism of action,aiming to provide new ideas and scientific evidence for in-depth research on insulin resistance and the development and application of drugs.
