Hypokalemia,a common clinical electrolyte disorder,can affect multiple systems and can be life-threatening in severe cases.Identifying the cause of hypokalemia is crucial for its prevention and treatment.However,the etiology of hypokalemia is complex and often requires detailed differential diagnosis.This article reports a rare clinical case of hypokalemia caused by long-term excessive consumption of strong tea and discusses its pathogenesis.The aim is to raise clinical awareness and understanding of the etiology of such cases of hypokalemia and reduce misdiagnosis and missed diagnosis.

3.Research progress on scientific connotations of decocting methods in traditional Chinese medicine decoction.

The therapeutic effects of traditional Chinese medicine(TCM) decoction is closely related to its decocting methods. A correct understanding of the scientific connotations of decocting methods in TCM is of great significance for guiding the application of decoctions and the development of modern TCM preparations based on decoctions. The decocting process is not only a hot water extraction process of chemical components but also accompanied by complex chemical and physical changes, forming a complex multiphase system and significantly affecting the absorption and therapeutic effect of TCM. This article reviews the research progress in scientific connotations of decocting methods in TCM from the perspectives of chemical composition changes, phase state differences,absorption behavior changes, and pharmacological and toxicological changes caused by decocting. This review is expected to provide implications for studying decocting methods and their scientific interpretation, boost the innovation and development of TCM decoctions,and promote the design and development of modern TCM preparations.

4.Under the regulation of"disease diagnosis-related group payment"the medical cost consciousness status of nurses working in interventional catheterization room and the analysis of its influencing factors

Objective To assess the medical cost consciousness status among catheterization lab nurses in grade-Ⅲ general hospitals where the diagnosis-related group(DRG)payment method has been implemented,and to explore its influencing factors.Methods By using the general data questionnaire form,the medical cost consciousness scale of medical staff and the Chinese version of the safety attitude scale,a total of 251 nurses working in the interventional catheterization room from 39 grade-Ⅲ general hospitals(being member unit of China Cardiovascular Health Alliance and being distributed in 13 provinces and cities)were surveyed.Results The total score of medical cost consciousness in nurses working in interventional catheterization room was(49.11±6.80)points.A positive correlation existed between the medical cost consciousness of nurses working in interventional catheterization room and the patient safety attitudes(P<0.01).Multivariate linear regression analysis showed that team collaboration,job satisfaction,stress perception,management mode of medical consumables,and cost-effectiveness training were the main factors influencing cost awareness.Conclusion The interventional catheterization room nurses have a good consciousness of medical costs,but their actual participation degree is insufficient.Under the regulation of DRG payment method the hospital administration and nursing care managers should take targeted measures to enhance the efficiency of catheter room cost management.
5.Effect of donepezil combined with hypoxia on CYP3A4 and its safety-evaluation

Aim To investigate the regulatory mecha-nisms of donepezil on the expression and enzymatic ac-tivity of cytochrome P450 3A4(CYP3A4),elucidate the synergistic impact of hypoxia on CYP3A4 function,and reveal its potential association with drug-induced cardiotoxicity,particularly QT interval prolongation.Methods Western blot,co-immunoprecipitation,and gene knockdown techniques were employed to evaluate the effects of donepezil and hypoxia on CYP3A4 pro-tein expression.CYP3A4 enzymatic activity was as-sessed using an in vitro incubation system with rat liver microsomes combined with high-performance liquid chromatography(HPLC),and the half-maximal inhib-itory concentration(IC50)was determined.Results Donepezil(10 μmol·L-1)and hypoxia reduced CYP3A4 protein expression to 31.75%and 45.90%of the control levels,respectively.Both interventions activated the gp78-mediated ubiquitin-proteasome path-way,significantly increasing CYP3A4 ubiquitination levels by 2.1-fold compared to the control group,thereby promoting proteasomal degradation.Donepezil inhibited CYP3A4 enzyme activity with an IC50 of 83.4μmol·L-1,and hypoxia synergistically enhanced this inhibitory effect,reducing the IC50 to 20.79 μmol·L-1.Conclusion Donepezil downregulates CYP3A4 function through dual mechanisms involving ubiquitin-mediated proteasomal degradation and direct enzymatic inhibition.Hypoxia potentiates this effect,leading to impaired metabolism of CYP3A4 substrate drugs,ele-vated plasma drug concentrations(1.6-2.3-fold in-crease compared to normal metabolic conditions),and an increased risk of QT interval prolongation and other forms of cardiotoxicity.
6.Application of Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion in patients after radical resection of colorectal cancer

Objective:To explore the therapeutic effects of Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion in patients after radical resection of colorectal cancer.Methods:A convenience sampling method was used to select 200 patients who underwent radical resection of colorectal cancer at Huzhou First People's Hospital from January 2022 to December 2023. Patients were randomly divided into three groups using a random number table: the control group ( n=62), the Zhuang medicine line acupoint moxibustion group ( n=66), and the combined group ( n=72). The control group received conventional interventions, the Zhuang medicine line acupoint moxibustion group received Zhuang medicine line acupoint moxibustion in addition to the conventional intervention, and the combined group received Jiugong abdominal meridian massage in addition to the Zhuang medicine line acupoint moxibustion. Gastrointestinal function recovery and pain scores were compared among the three groups after the intervention. Results:After intervention, there were statistically significant differences in the recovery time of bowel sounds, time to tolerate solid food by mouth, time to first bowel movement, gastric body motility frequency, total gastrointestinal output rate, gastric emptying rate, and pain scores among the three groups ( P<0.05) . Conclusions:Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion intervention can effectively improve the gastrointestinal function indicators and reduce postoperative pain in patients after radical resection of colorectal cancer.

10.Factors affecting fall incidence among the elderly in Ningbo City

Objective:
To investigate the incidence and influencing factors of falls among the elderly in Ningbo City, Zhejiang Province, so as to provide the basis for developing effective prevention strategies.
Methods:
The residents aged 60 years and above in Haishu District and Yuyao City of Ningbo City were selected by the multi-stage cluster random sampling method from June to October 2022. Demographic information, fall incidence in the past year, history of disease and self-rated health were collected through questionnaire surveys. Incidence of falls was descriptively analyzed, and factors affecting falls were identified using a multivariable logistic regression model stratified by gender and age.
Results:
A total of 1 275 elderly people were surveyed, including 635 men and 640 women. The median age was 72.00 (interquartile range, 13.00) years. In the past year, 158 residents fell, accounting for 12.39%. Additionally, 14 individuals experienced two or more falls, accounting for 8.86%. The incidence of falls was 14.69% in women, which was higher than the 10.08% in men (P<0.05). The incidence of falls was 14.86% in the elderly over 70 years, which was higher than the 9.39% in those aged 60 to 70 years (P<0.05). Multivariable logistic regression showed that the educational level (primary school and above, OR=0.501, 95%CI: 0.301-0.836), heart disease (present, OR=1.996, 95%CI: 1.076-3.703), and self-rated health status (good, OR=0.529, 95%CI: 0.319-0.875) were factors affecting falls in women; educational level (primary school and above, OR=0.514, 95%CI: 0.285-0.928) and self-rated health status (good, OR=0.456, 95%CI: 0.253-0.824) were factors affecting falls in residents aged 60 to 70 years.
Conclusion
Fall risk among the elderly is associated with gender, age, heart disease, educational level and self-rated health status, and the influencing factors for falls vary in different genders and ages.
