1.Effect of Yiqi Yangyin Huoxue Formula(益气养阴活血方)on Oxidative Stress and Inflammatory Injury in Membranous Nephropathy Model Rats:Based on the NOX4-mediated SDF-1α/CXCR4 Signaling Pathway

ObjectiveTo investigate the potential mechanism of Yiqi Yangyin Huoxue Formula (益气养阴活血方, YYHF) in the treatment of membranous nephropathy (MN) based on the NOX4-mediated stromal cell-derived factor-1α(SDF-1α)/CXC chemokine receptor 4 (CXCR4) signaling pathway. MethodsA total of 42 male SD rats were randomly divided into blank group (n=6) and modeling group (n=36). The modeling rats were given a single injection of sheep antirat Fx1A (6 ml/kg) serum into the tail vein to establish MN model. Thirty-six rats with successful modeling were further divided into model group (normal saline, 10 ml/kg), benazepril group (10 mg·kg-1·d-1), the medium-dose YYHF group (11.0 g·kg-1·d-1) and high-dose YYHF group (22.0 g·kg-1·d-1), with 9 rats in each group. All groups received daily intragastric administration for 6 consecutive weeks. During the experimental period, the general condition of rats in each group was observed. After gavage administration finished, a fully automated biochemical analyzer was used to measure the 24-hour urine total protein (24 h-UTP) and serum biochemical parameters, including albumin (ALB), aspartate aminotransferase (AST), blood urea nitrogen (BUN), serum creatinine (SCr), total cholesterol (TC), and triglyceride (TG). Hematoxylin-eosin (HE) staining and periodic acid-silver metheramine (PASM) staining were used to observe the histopathological changes of renal tissues in each group of rats. The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in renal tissues were detected by ELISA. The protein expression levels of NOX4, SDF-1α, CXCR4, Nrf2, nuclear factor kappa B subunit p65 (NF-κB p65) and podocyte slit diaphragm protein nephrin in rat renal tissues were detected by Western Blotting. The mRNA expressions of NOX4, SDF-1α, CXCR4, Nrf2, NF-κB p65 and Nephrin in renal tissues were detected by real-time fluorescence quantitative polymerase chain reaction (RT-PCR). ResultsCompared to the blank group, rats in the model group exhibited typical features of MN, including lethargy, reduced activity, dull and coarse fur, marked edema, and massive proteinuria. Compared to the model group, these abnormal conditions were improved to varying degrees in all treatment groups. The high-dose YYHF group showed a level of improvement comparable to that of the benazepril group, with overall recovery superior to that of the medium-dose YYHF group. Compared to the blank group, the model group showed increased levels of 24 h-UTP, BUN, SCr, TC and TG, decreased level of ALB, increased levels of MDA and protein and mRNA expressions of NOX4, SDF-1α, CXCR4 and NF-κB p65 in renal tissue, along with decreased levels of SOD and protein and mRNA expressions of Nrf2, Nephrin (P<0.01), accompanied by severe renal pathological damage. Compared to the model group, the benazepril and the medium- and high-dose YYHF groups showed significant improvements in the above indicators (P<0.01), along with alleviation of renal pathological injury. The 24 h-UTP level in the high-dose YYHF group was lower than that in the medium-dose group (P<0.01), while no statistically significant difference was observed among the other treatment groups (P>0.01). ConclusionYYHF may alleviate oxidative stress and inflammatory responses by down-regulating the expression of NOX4 and inhibiting the SDF-1α/CXCR4-Nrf2 signaling pathway, thereby improving podocyte damage and protecting renal function.

In March 2024, three male and two female specimens of the genus Lispe were intercepted on cargo ships inbound for Wan Zai Port under Gongbei Customs. Based on morphological characteristics and molecular analysis, the specimens were identified as Lispe pumila. In this study, we describe the morphological features, diagnostic characteristics, and geographical distribution of L. pumila, with the aim of providing a reference for the identification of this species when intercepted at ports.
3.Coronary Computed Tomographic Angiography-Derived Radiomics Combing CT-Fractional Flow Reserve for Detecting Hemodynamically Significant Coronary Artery Disease.

Objective To develop a diagnostic model combining the CT angiography(CCTA)-derived myocardial radiomics signatures with the CT-derived fractional flow reserve(CT-FFR)based on coronary CCTA and investigate the diagnostic accuracy of the hybrid model for hemodynamically significant coronary artery disease(CAD).Methods The patients presenting stable angina pectoris,diagnosed with CAD,and clinically referred for CCTA examination and invasive coronary angiography were prospectively recruited.Radiomics features of the left ventricular myocardium were extracted from the three main perfusion territories demarcated according to the coronary blood supply.The extracted features were first selected by the minimum redundancy maximum relevance feature ranking method.A least absolute shrinkage and selection operator Logistic regression algorithm with leave-one-out cross-validation was then employed to construct a radiomics model.The CT-FFR value was generated for each blood vessel.The area under the receiver operating characteristics curve(AUC_ROC),sensitivity,and specificity were adopted to evaluate the performance of each model against the reference standard invasive coronary angiography/FFR.Results A total of 70 patients[42 men and 28 women;(61±10) years old] were included in this study and complemented CCTA examination,with 175 vessels and the corresponding myocardial territories undergoing invasive coronary angiography/FFR.A total of 1 656 specific radiomics parameters were extracted,from which 14 features were selected to establish the radiomics model.The AUC_ROC,sensitivity,and specificity were 0.797(95%CI=0.732-0.861),77.1%,and 73.7%for the radiomics model,0.892(95%CI=0.841-0.943),81.4%,and 88.8%for the CT-FFR model,and 0.928(95%CI=0.890-0.965),83.3%,and 88.4%for the hybrid model,respectively.The hybrid model outperformed the radiomics model and CT-FFR alone(P=0.040).Conclusions The radiomics signatures of the vessel-related myocardium from CCTA could provide incremental value to the diagnostic performance of CT-FFR and improve vessel-specific ischemia detection.The hybrid model combining CT-FFR with radiomics signatures is potentially feasible for improving the diagnostic accuracy for hemodynamically significant CAD.

Most tumor cells and healthy neurons are at rest during G0 phase.Once the cell cycle is abnormally re-entered under certain conditions,the proliferation of tumor cells and the degenerative necrosis of neurons can be initiated.From the perspective of the cell cycle,cancer and central nervous system diseases,two seemingly different disease types,have a common pathogenesis.This type of diseases is named aberrant cell cycle diseases.As a newly discovered microRNA(miR),miR-1976 is closely related to the regulation of the cell cycle.This review summarizes the progress in the research on miR-1976 in cancer and central nervous system diseases,aiming to provide a reference for the clinical application of miR-1976 in aberrant cell cycle diseases in the future.

5.Diagnostic Value of Intestinal Ultrasound for Patients With Chronic Abdominal Pain or Diarrhea.

Objective To evaluate the diagnostic value of intestinal ultrasound (IUS) for organic lesions in the intestines of patients with chronic abdominal pain or diarrhea. Methods The IUS signs in 263 patients with chronic abdominal pain or diarrhea were retrospectively analyzed.With the endoscopic examination results as the gold standard,comparison was performed for the IUS signs between the groups with positive and negative endoscopic results,as well as between the inflammatory bowel disease group and the non-specific intestinal inflammation group of positive cases.Furthermore,the detection rates of IUS in different intestinal segments were analyzed to evaluate the accuracy of IUS in the diagnosis and localization of intestinal lesions. Results Among the 263 patients,194 (73.8%) and 69 (26.2%) patients were in the groups with positive and negative endoscopic results,respectively.The diagnosis sensitivity,specificity,and accuracy of IUS were 82.0%,71.0%,and 79.1%,respectively.The proportions of positive IUS signs in the group with positive endoscopic results were higher than that in the group with negative endoscopic results (all P<0.001).The proportions of positive IUS signs in the inflammatory bowel disease group were higher than those in the non-specific bowel inflammation group (all P<0.001).When the lesion was located in the ileum,ileocecal region,and colon,IUS demonstrated good consistency with endoscopic results in locating the lesion (kappa=0.642,0.686,and 0.601,respectively),with sensitivity and specificity of 82.7% (95%CI=75.4%-88.6%) and 81.5% (95%CI=73.5%-87.9%),73.7% (95%CI=62.3%-83.1%) and 93.0% (95%CI=88.4%-96.2%),and 68.9% (95%CI=58.3%-78.2%) and 89.6% (95%CI=84.1%-93.7%),respectively. Conclusions IUS can be used for screening the patients with chronic abdominal pain or diarrhea to detect organic lesions in the intestines. Moreover,it can effectively locate the affected intestinal segment,which is helpful for the monitoring and follow-up of intestinal diseases.

Forensic medicine has been designated as a first-level discipline,presenting new opportunities and challenges for the development of forensic medicine.Since the 1980s,the establishment of foren-sic medicine discipline and the cultivation of high-level forensic talents have become hot topics in the development of forensic medicine in China.Since the 13th Five-Year Plan,the forensic team of Shanxi Medical University has been aiming at the forefront,proposing the development goals of"Five First-class"and the discipline development path"Six Major Achievements".It has selected benchmark disci-plines,identified gaps in disciplinary development,unified thoughts,formulated completion timelines,concentrated superior resources,assigned tasks to individuals,and created an"Organized Fill-in-the-Blank Format"forensic medicine discipline construction model with the characteristics of the new era.The construction model of forensic medicine has achieved good results in the goals,discipline frame-work,scientific research,talent cultivation,discipline team and platform construction,forming a rela-tively complete discipline construction and management system,and accumulating valuable experience for the construction of first-level discipline and high-level talent cultivation of forensic medicine.
7.Analysis of risk factors and construction of a prediction model for herpes zoster complicated by kidney dysfunction

Objective:To analyze the risk factors for herpes zoster complicated by kidney dysfunction and construct a prediction model.Methods:A retrospective analysis was conducted on the clinical data of 150 patients with herpes zoster who were admitted to Hangzhou Third People's Hospital from January 2022 to December 2023. The patients were divided into two groups based on the presence of kidney dysfunction: kidney dysfunction group ( n = 30, with kidney dysfunction) and control group ( n = 120, without kidney dysfunction). The risk factors for herpes zoster complicated by kidney dysfunction were analyzed. The logistic prediction model was constructed. The effectiveness of this prediction model in predicting herpes zoster complicated by kidney dysfunction was assessed using the receiver operating characteristic curve. Results:The differences in gender, body mass index, comorbidities (hypertension and hyperlipidemia), and sites of onset between the two patient groups were not statistically significant (all P > 0.05). The average age of the kidney dysfunction group was (67.74 ± 6.71) years, which was significantly older than that in the control group [(62.32 ± 5.58) years, t = 4.56, P < 0.001]. In the kidney dysfunction group, the proportions of patients with comorbid diabetes, severe pain, large lesion area, high-dose antiviral medication use, and hemoglobin levels < 120 g/L were 46.67% (14/30), 63.33% (19/30), 56.67% (17/30), 46.67% (14/30), and 40.00% (12/30), respectively. These values were all significantly greater than those in the control group [14.17% (17/120), 34.17% (41/120), 31.67% (36/120), 19.17% (23/120), and 10.00% (12/120), χ2 = 15.46, 2.60, 2.10, 9.76, 16.07, P < 0.001, 0.002, 0.035, 0.002, < 0.001). Logistic regression analysis revealed that older age ( OR = 3.023), comorbid diabetes ( OR = 4.315), severe pain ( OR = 3.623), large lesion area ( OR = 3.481), high-dose antiviral medication use ( OR = 4.030), and hemoglobin levels < 120 g/L ( OR = 0.151) were all significant risk factors for herpes zoster complicated by kidney dysfunction (all P < 0.05). Based on these risk factors, the following logistic prediction model was developed: Logit( P) = -8.753 + 1.106 × age + 1.462 × diabetes + 1.287 × pain severity + 1.247 × lesion area + 1.394 × high-dose antiviral medication use + (-1.889) × hemoglobin. The receiver operating characteristic curve analysis indicated that the area under the curve for predicting herpes zoster complicated by kidney dysfunction was 0.904 (95% CI: 0.859-0.937), with a sensitivity of 84.62% and a specificity of 89.04%. Conclusions:The risk factors for herpes zoster complicated by kidney dysfunction include older age, comorbid diabetes, severe pain, large lesion area, high-dose antiviral medication use, and hemoglobin levels < 120 g/L. Based on these factors, the construction of a logistic prediction model can provide a reliable basis for clinical prediction of the occurrence of kidney dysfunction.

Fear of disease progression(FoP)is a common psychological state among patients with coronary heart dis-ease(CHD),which exists throughout the course of disease and seriously affects the prognosis.High level of FoP not only harms the mental health of patients but also do not benefit their rehabilitation after discharge.This concept was first proposed by foreign researchers studying the psychological state of cancer patients.In recent years,there have been numerous studies on the influencing factors and qualitative aspects of FoP in CHD patients,while related inter-vention studies remain few.This paper reviews the concept,scales and related influencing factors of FoP,providing ideas for medical staff to provide targeted interventions for CHD patients.
9.Efficacy of arthroscopic ligament augmentation repair in the treatment of chronic lateral ankle instability

Objective:To investigate the efficacy of arthroscopic ligament augmentation repair in the treatment of chronic lateral ankle instability.Methods:The clinical data of 65 patients with chronic lateral ankle instability who received treatment at Shangluo Central Hospital from October 2019 to October 2022 were retrospectively analyzed. The clinical data were divided into three groups based on treatment methods: the mini-open group ( n = 15), in which patients underwent arthroscopy-assisted small incision lateral ligament repair; the arthroscopy group ( n = 15), in which patients received total ankle arthroscopic ligament augmentation repair; and the non-surgical group ( n = 35), in which patients were treated conservatively. The American Orthopedic Foot and Ankle Score (AOFAS) value, the Tegner Activity Scale (TAS) score, Visual Analog Scale (VAS) score, the time to achieve full weight-bearing, and the time to return to sports were compared among all groups. Results:After treatment, AOFAS value and the TAS score increased in all three groups compared with pre-treatment levels, while the VAS value decreased in all groups. All differences were statistically significant ( t = 6.53, 7.45, 6.58, 6.50, 7.62, 6.52, 6.80, 7.50, 6.49, all P < 0.01). There were no significant differences in AOFAS value, TAS score, and VAS score among the three groups (all P > 0.05). The time to achieve full weight-bearing in the arthroscopy group was 6.6 (5.5, 9.0) weeks, which was shorter than the 7.4 (6.0, 9.0) weeks in the mini-open group and the 9.4 (7.6, 12.0) weeks in the non-surgical group. These differences were statistically significant ( H = 7.98, P = 0.001). The proportion of patients returning to sports in the arthroscopy group was 80% (12/15), which was higher than the 66% (10/15) in the mini-open group and the 28% (10/35) in the non-surgical group. These differences were statistically significant ( χ2 = 30.78, P = 0.025). The time to return to sports in the arthroscopy group was 12.1 (9.5, 15.0) weeks, which was shorter than the 14.6 (12.0, 18.2) weeks in the mini-open group and the 16.1 (13.2, 19.0) weeks in the non-surgical group. These differences were statistically significant ( H = 8.90, P = 0.001). Conclusions:Ankle arthroscopic ligament augmentation repair for the treatment of chronic lateral ankle instability is safe and shows better clinical efficacy, making it a viable option for clinical practice.
10.Analysis of Predictive Factors for the Efficacy of Traditional Chinese Medicine in Treating Myasthenia Gravis and Development and Validation of the Nomogram Model

Objective To explore the relevant factors affecting the efficacy of traditional Chinese medicine in treating MG,and to develop and validate a nomogram model,in order to personalize the prediction of the probability of benefits for MG patients after traditional Chinese medicine treatment,further guide clinical physicians in targeted medication,and provide guidance for the traditional Chinese medicine treatment of MG.Method Retrospective analysis of MG patients who visited the Affiliated Hospital of Changchun University of Traditional Chinese Medicine from March 2018 to June 2022,13 clinical factors were selected to evaluate the results.To acquire independent factors,univariate and multivariate Logistic analyses were performed.The area under the receiver operating characteristic curve,Harrell's concordance index,calibration curve,and decision curve analyses were used to evaluate the predictive ability,accuracy,and clinical practicability of the prediction model.Results A total of 204 cases were included,divided into derivation cohort 139 cases and temporal validation cohort 65 cases.Multivariate Logistic regression showed 4 independent predictors affecting effectiveness of traditional Chinese medicine in treating MG,including age at onset,repetitive nerve stimulation positive,oral immunosuppressant and anxiety/depression.AUC values for the model group and validation group were 0.76(95%CI:0.68-0.84)and 0.83(95%CI:0.71-0.95),respectively,and based on calibration curve and decision curve analysis,we concluded that the nomogram showed excellent performance.Conclusion MG patients may experience a decrease in short-term efficacy with age,positive RNS,concomitant emotional abnormalities,or recent history of immunosuppressive therapy.This nomogram effectively predicts the possibility of short-term effective traditional Chinese medicine treatment in MG patients.
