Despite significant advances in the field of critical care medicine over the past three decades, veno-arterial extracorporeal membrane oxygenation (V-A ECMO) remains the primary temporary mechanical circulatory support modality for patients with acute severe circulatory failure. With the accumulation of clinical experience and the increasing maturity of operational techniques in V-A ECMO, its technical management—particularly hemodynamic management—has become a key factor influencing patient outcomes. To further improve patient survival, the Chinese Critical Care Ultrasound Study Group, in collaboration with the Hemodynamic Therapy of Critical Care Collaborative Group and the Critical Care Medicine Branch of the China International Exchange and Promotive Association for Medical and Health Care, organized experts in critical care medicine to develop the

As a distinct pathological factor and metastatic route of malignant tumors, perineural invasion(PNI)exhibits a close association with the prognosis of several cancer types. In recent years, the clinical significance and underlying mechanisms of PNI in gynecological malignant tumors have increasingly drawn research attention.Currently,it is recognized that PNI is clinicopathologically and prognostically linked to cervical cancer and vulvar cancer. The exploration of its mechanisms encompasses multiple gynecological tumors and numerous molecular pathways, including the CCL2/CCR2 axis,NGF/TrkA pathway,GluR2,among others.Nevertheless, the existing research models are fragmented, lacking systematicity and in-depth analysis, thus necessitating integration. This paper presents a comprehensive review of the occurrence of PNI in gynecologic malignancies, its correlations with clinicopathology and prognosis, its implications for surgical approaches and postoperative adjuvant therapies,as well as the potential molecular mechanisms.By synthesizing the advancements in both clinical and basic research, this review aims to offer a solid theoretical foundation for further exploration of the mechanisms underlying PNI in gynecologic malignancies and for the optimization of clinical therapeutic strategies.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.

Objective To conduct a cross-sectional study on the relationship between the serum uric acid(sUA)to high density lipoprotein-cholesterol(HDL-C)ratio(UHR)and female infertility. Methods We analyzed data from the National Health and Nutrition Examination Survey(NHANES)2013-2018 cycle, examining 2,963 women aged 18 to 45 years.A multivariable Logistic regression model was used to assess the association between UHR and infertility, adjusting for various confounding factors. The study also explored interactions and dose-response relationships through subgroup analyses and restricted cubic spline(RCS)models, and evaluated the predictive value of UHR for infertility by using receiver operating characteristic(ROC)curve analysis. Results The results showed that the prevalence of infertility was significantly higher among women with elevated UHR.Baseline characteristics revealed that the mean UHR in women with infertility(9.68±4.23)was significantly higher than in those without infertility(8.7±3.81,P<0.001).Multivariable Logistic regression analysis indicated that women in the highest quartile(Q4)of UHR had a significantly increased likelihood of infertility compared to those in the lowest quartile(Q1),with adjusted odds ratios(ORs)ranging from 1.61(95% CI:1.14-2.28)to 1.98(95% CI:1.42-2.78)across different models(P for trend<0.001).Subgroup analyses demonstrated that these associations were consistent across different age groups, body mass index(BMI)categories, and racial groups. The RCS model showed a significant linear dose-response relationship between UHR and infertility risk(nonlinear P=0.03,overall association P<0.001).Additionally,ROC analysis indicated that UHR had a moderate ability to discriminate infertility status, with an area under the curve(AUC)of 0.570.Conclusion A higher UHR is significantly associated with an increased risk of female infertility, and UHR has the potential to serve as a predictive biomarker for the risk of female infertility.

Most patients with differentiated thyroid cancer have a good prognosis after radioiodine-131 therapy, but a small number of patients are insensitive to radioiodine-131 therapy and even continue to develop disease. At present, some targeted drugs can improve progression-free survival in patients with radioactive iodine-refractory differentiated thyroid cancer(RAIR-DTC),such as sorafenib and levatinib, have been approved for the treatment of RAIR-DTC.However,due to the presence of primary and acquired drug resistance, drug efficacy in these patients is unsatisfactory. This review introduces the acquired drug resistance mechanism of sorafenib in the regulation of mitogen-activated protein kinase(MAPK)and phosphatidylinositol-3-kinase(PI3K)pathways and proposes related treatment strategies,in order to provide a reference for similar drug resistance mechanism of sorafenib and effective treatment of RAIR-DTC.
