1.Analysis of Risk Factors of Postpartum Mild Uterine Prolapse and the Effects of Buzhong Yiqi Decoction on Serum TGF-β1,MMP-2,and miRNA-30d

Objective To analyze the risk factors of postpartum mild uterine prolapse and to investigate the effects of Buzhong Yiqi Decoction on serum transforming growth factor β1(TGF-β1),matrix metalloproteinase 2(MMP-2),and microRNA-30d(miRNA-30d).Methods By blinded and controlled methods and through convenience sampling,80 postpartum patients with mild uterine prolapse were retrospectively collected from Tangshan Maternal and Children Health Hospital from January 2020 to January 2022,and they served as the uterine prolapse group.In addition,80 postpartum patients without uterine prolapse from the same department of the hospital in the same period served as the healthy group.Questionnaire investigation was performed for collecting the basic information such as delivery manner,age,parity,gestational weeks,neonatal weight and body mass index(BMI).The risk factors of postpartum uterine prolapse were investigated by univariate analysis and multivariate logistic regression analysis.The patients in the uterine prolapse group were then divided into the observation group and the control group according to the medication of Buzhong Yiqi Decoction or not,with 40 cases in each group.The control group was treated with electromyographic biofeedback therapy(EBT),while the observation group was treated with Buzhong Yiqi Decoction combined with EBT,and the course of treatment for the two groups covered one month.The changes of pelvic floor function and levels of serum TGF-β1,MMP-2,miRNA-30d in the patients of the two groups were observed before and after treatment.After treatment,the clinical efficacy of patients in the two groups was evaluated.Results(1)The results of univariate analysis showed that compared with the healthy group,the patients in the uterine prolapse group were elder,usually had given two childbirths or more by vaginal delivery,and accounted for a high percentage of overweight/obesity(BMI≥24.0 kg/m2)and perineal laceration,the difference being statistically significant(P<0.05 or P<0.01).(2)The results of multivariate logistic regression analysis showed that age,delivery manner,parity,laceration of perineum,and BMI were risk factors for postpartum uterine prolapse(P<0.01).(3)After one month of treatment,the total effective rate of Buzhong Yiqi Decoction in the observation group was 95.00%(38/40),while that in the control group was 77.50%(31/40),and the intergroup comparison(tested by chi-square test)showed that the efficacy of the observation group was significantly superior to that of the control group(P<0.05).(4)The observation of effects of Buzhong Yiqi Decoction on pelvic floor function showed that after treatment,the distance between the cervix and the hymenal edge,the maximum intravaginal pressure,the voltage of vaginal muscles,the contraction time of pelvic floor muscles of typeⅠand Ⅱmuscle fibers in the two groups of patients were significantly improved compared with those before treatment(P<0.05),and the improvement in the observation group was significantly superior to that in the control group(P<0.01).(5)The observation of effects of Buzhong Yiqi Decoction on serological indicators showed the post-treatment serum TGF-β1 levels in the two groups were increased compared with those before treatment(P<0.05),and the serum levels of MMP-2 and miRNA-30d were decreased compared with those before treatment(P<0.05),and the increase of serum TGF-β1 level and the decrease of serum MMP-2 and miRNA-30d levels in the observation group were both significantly superior to those in the control group(P<0.01).Conclusion The incidence of postpartum uterine prolapse is related to age,delivery manner,parity,laceration of perineum,BMI,etc.The treatment of Buzhong Yiqi Decoction on the basis of EBT can improve the pelvic floor function of the patients and regulate the expression of serum TGF-β1,MMP-2,and miRNA-30d,and the clinical efficacy is remarkable.
2.Pathogenic infection spectrum revealed by metagenomics high-throughput next-generation sequencing in patients with hematological diseases after allogeneic hematopoietic stem cell transplantation

Objective:To investigate the infection spectrum revealed by metagenomics high-throughput next-generation sequencing (mNGS), and to provide a reference for infection diagnosis after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:A total of 64 patients who developed systemic or local infection symptoms after allo-HSCT in Hebei Yanda Lu Daopei Hospital from January 2018 to November 2018 were enrolled. Gene sequences of pathogenic microorganisms in blood, cerebrospinal fluid and bronchoalveolar fluid specimens were detected by using mNGS. The pathogenic microorganisms or suspected pathogens were determined based on the clinical manifestations of patients.Results:There were 97 samples of mNGS detection for 64 patients who underwent allo-HSCT. The most common gram-positive bacteria were staphylococcus haemolyticus (19 times) and staphylococcus (14 times), and the most common gram-negative bacterium was acinetobacter baumannii (8 times). The most common viruses were cytomegalovirus, EB virus and Torque teno virus (35, 22 and 23 times, respectively), and the most common fungi were malassezia globus (14 times) and candida parapsilosis (8 times). There were 3 mycobacterium tuberculosis complexes detected in 3 patients with acute myeloid leukemia who received allo-HSCT. Mycoplasma orale was detected in one patient's sputum, and none parasite was detected.Conclusion:mNGS can comprehensively reveal the infection spectrum of hematologic diseases after allo-HSCT, especially for pathogenic microorganisms that are rare or difficult to cultivate, and it can effectively help the diagnosis of clinically infectious pathogens.
3.Expressions of growth factor receptor binding protein 7 and human epidermal growth factor receptor 2 in endometrial carcinoma and their significances

Objective To investigate the expressions of growth factor receptor binding protein 7 (GRB7) and human epidermal growth factor receptor 2 (HER2) in endometrial carcinoma and their clinical significances. Methods Immunohistochemistry was used to detect the expressions of GRB7 and HER2 in 52 endometrial carcinoma tissues, 38 endometrial atypical hyperplasia tissues and 30 normal endometrial tissues collected from patients in the Sixth People's Hospital of Shanghai Jiao Tong University from May 2015 to September 2017. The relationship between the expressions of GRB7 and HER2 and clinicopathological features of endometrial carcinoma patients was analyzed. Results The positive expression rates of GRB7 in endometrial carcinoma, endometrial atypical hyperplasia and normal endometrial tissues were 71.2% (37/52), 39.5%(15/38), and 13.3%(2/30), respectively, and the difference was statistically significant (χ2=35.14, P<0.05). The positive expression rates of HER2 in endometrial carcinoma, endometrial atypical hyperplasia and normal endometrial tissues were 67.3% (35/52), 34.2% (13/38), and 10.0% (3/30), respectively, and the difference was statistically significant (χ2= 29.360, P< 0.05). The positive expression rates of GRB7 in endometrial carcinoma patients with different clinical stages, histological grades and pathological types were statistically significant (all P<0.05). The positive expression rates of HER2 in endometrial carcinoma patients with different clinical stages, histological grades, lymph nodes metastasis and myometrial invasion were statistically significant (all P< 0.05). The expression of GRB7 in endometrial carcinoma was positively correlated with the expression of HER2 (r=0.375, P<0.05). Conclusion The high expressions of GRB7 and HER2 in endometrial carcinoma are closely related to the malignant degree of endometrial carcinoma.
4.Study of the reconstruction methods after vertical partial laryngectomy for glottic carcinoma of the larynx

To study the reconstruction methods after vertical partial laryngectomy for glottic carcinoma of the larynx. Method:58 cases with glottic carcinoma of the larynx were treated with vertical partial laryngectomy or extended vertical partial laryngetomy .The clinical materials of these cases were retrospectively studied.Result:The total decannulation rate was 93.1% and the 3-and 5-year survival rates were 87.2% and 80.5% respectively .The voice was socially acceptable in 93.1%(54/58) of the patients. Aspiration was only 3. All resumed mouth-food-taking.Conclusion:The reconstruction methods after vertical partial laryngectomy were mainly decided by the extent of the laryngeal defect. If the laryngeal defect of the framework was small, the pedicle muscular flap was the best reconstruction method in comparison with other reconstruction methods .If the laryngeal defect of the framework was too big ,allograft nasal septal cartilage or epiglottic laryngoplasty should be performed .
