1.Clinical study on compound centella asiatica in treating diabetic kidney disease by regulating intestinal microflora metabolism

Objective To explore the effect of compound centella asiatica on diabetic kidney disease(DKD)by regulating intestinal microflora metabolism.Methods Sixty DKD patients who visited Hangzhou Ninth People's Hospital from January to December 2022 were randomly divided into experimental group and control group,with 30 patients in each group.Patients in control group received basic treatment guided by lifestyle and diet,while patients in experimental group received treatment with compound centella asiatica on the basis of control group.Both groups of patients were followed up for 6 months to measure gut microbiota metabolite levels,urinary protein,and other indicators.Results After treatment,the levels of short-chain fatty acid(SCFA)in experimental group increased,while the levels of trimethylamine oxide(TMAO),indophenol sulfate(IS),microalbuminuria/creatinine(ACR),and 24-hour urinary protein quantification(24hPro)decreased;24hPro and ACR of experimental group patients were lower than those of control group.ACR in DKD patients is positively correlated with TMAO and negatively correlated with SCFA(P<0.05).Conclusion ACR in DKD patients is positively correlated with TMAO and negatively correlated with SCFA;Compound centella asiatica can increase the levels of SCFA in DKD patients,reduce the levels of TMAO,IS,and ACR,indicating that compound centella asiatica can regulate the metabolic levels of gut microbiota in DKD patients and reduce proteinuria.

3.Virtual reality based early rehabilitation improves physical and cognitive function in patients with ischemic stroke

Objective To explore the effect of virtual reality based early rehabilitation on the physical and cognitive function rehabilitation of patients with ischemic stroke.Methods A total of 150 ischemic stroke patients were randomly divided into a control group and an observation group:the patients in control group were treated with conventional care and those in the intervention group with early rehabilitation training based on virtual reality.The two groups were compared in regard to the differences in cognitive function,motor function and ability of daily living(ABL),with and without the intervention.Result In comparison with the control group,patients in the observation group presented significantly higher scores in cognitive function,motor function and ABL and the difference before and after the intervention(all P<0.05).Conclusions The virtual reality based early rehabilitation for the patients with ischemic stroke can remarkably improve the recovery of their cognitive function and motor function as well as to enhance the ability of daily living of the patients.Further studies are required to confirm the effectiveness of virtual reality based early rehabilitation.
4.A pan-cancer analysis of TTC9A expression level and its correlation with prognosis and immune microenvironment

Objective To investigate the expression level of tetratricopeptide repeat protein 9A in tumors and its association with the patients'prognosis and immune infiltration.Methods TTC9A expression in different tumor tissues and its association with prognosis,DNA methylation,tumor mutation burden(TMB),and microsatellite instability(MSI)were analyzed based on data from TCGA and GTEx.TIMER and xCell were used to analyze the relationship between TTC9A expression and immune infiltration.Western blotting and RT-qPCR were used to detect the expression of TTC9A in 4 types of cancer cell lines.Results TTC9A expressions were significantly increased in many tumors and down-regulated in a few cancer types(P<0.05).Western blotting and RT-qPCR showed that TTC9A expressions were elevated in lung,colon and liver cancer cells but decreased in bladder cancer cells.In head and neck squamous cell carcinoma,renal clear cell carcinoma,renal papillary cell carcinoma,low-grade glioma,malignant mesothelioma,and endometrial carcinoma tumors,a high expression of TTC9A was strongly correlated with better overall survival(OS),disease-specific survival(DSS),and progression-free interval(PFI)(P<0.05),but was correlated with worse OS,DSS,and PFI in lung adenocarcinoma,pancreatic adenocarcinoma,adrenal carcinoma,and rectal adenocarcinoma(P<0.05).TTC9A hypermethylation was associated with a more favorable prognosis of glioblastoma multiforme,low-grade glioma,uveal melanoma,and ovarian plasmacytoid cystadenocarcinoma(P<0.05)but with poor prognosis of squamous cell carcinoma of the uterine cervix and intracervical adenocarcinoma,squamous cell carcinoma of head and neck,squamous cell carcinoma of the lungs,adrenal carcinoma,and endometrial carcinoma(P<0.05).In most of the cancer types,TTC9A was significantly correlated with the level of immune cell infiltration(P<0.05).Conclusion TTC9A can be used as a prognostic marker for a variety of cancers and is strongly associated with TBM,MSI and immune cell infiltration.

6.Evaluation of the reliability and validity of Nursing Humanistic Care Experience Scale for Chronic Disease Patients

Objective To cross-cultural adapt and test the reliability and validity of Humanistic Relationship Experience Scale (HRES) made by Boscart. Methods Cross-culture Adaptation was carried out by following American Academy of Orthopaedic Surgeons Evidence Based Medicine Committee (AAOS). A convenience sampling was conducted among 180 patients with chronic disease, and then assessed the reliability and validity of HRES. The internal consistency coefficient and the test-retest were used to assess the reliability of HRES, and the content validity index (CVI) and construct validity were used to assess the validity of HRES. Results In order to understand the purpose and content of the scale easily, the Chinese version title of HRES was revised to"Nursing Humanistic Care Experience Scale-Noninfectious Chronic Disease Patients Version (NHCES-NCDP)" after soliciting experts′ advice. The CVI of the scale was 1;factor analysis got three factors, which could explain 67.897%of the total variance. A 0.965 Cronbachαof total scale was obtained, and for each dimension range was 0.876-0.948;the test-retest reliability was 0.710 for the overall scale. Conclusion NHCES-NCDP is reliable and valid, which can be used to assess the experience of nursing humanistic care for patients with chronic diseases, and to explore nurses′implementation of nursing humanistic care.
7.Analysis of therapeutic effect of sodium hyaluronate intra-articular injection combined with oral Yanghe decoction in the treatment of knee osteoarthritis

Objective To investigate the curative effect of sodium hyaluronate intraperitoneal injection combined with oral Yanghe decoction in the treatment of knee osteoarthritis .Methods 80 patients with knee osteoar-thritis were randomly divided into two groups ,40 cases in each group .The control group was given intra -articular injection of sodium hyaluronate ,and the study group was treated with Yanghe decoction in addition .The TCM symptom score , knee joint index , overall treatment effect and quality of life were compared between the two groups .Results The total score of TCM symptoms after treatment in the study group was (7.08 ±1.18)points,which was significantly lower than (10.42 ±2.01)points in the control group(t=9.063,P<0.01).The VAS score and WOMAC score after treatment in the study group were (1.48 ±0.59)points,(19.10 ±3.08)points,respectively,which were significantly lower than (3.70 ±1.12)points and (23.40 ±3.56)points in the control group (t=11.091,5.777,all P<0.01). The HSS score after treatment in the study group was (83.04 ±10.49) points,which was significantly higher than (71.81 ±9.67)points in the control group(t =4.978,P<0.01).The physiological function,physical function, physical pain,mental health and energy scores after 6 months treatment in the study group were significantly higher than those in the control group (t=5.190,8.535,4.569,3.266,6.694,all P<0.01).The effective rate and total effective rate of the study group were 72.5%and 95.0%,respectively,which were significantly higher than those of the control group(50.0%,77.5%),the differences were statistically significant (χ2 =4.266,5.165,all P<0.05). Conclusion Sodium hyaluronate injection combined with oral Yanghe decoction is effective in the treatment of knee osteoarthritis.
8.Immunogenicity and safety of Sabin IPV vaccine in Banna mini pigs

Objective To evaluate the immunogenicity and safety of inactivated poliovirus vaccine derived from Sabin strain (sIPV) in Banna mini pigs, and to provide experimental evidence for the new animal model. Methods sIPV vaccines which are listed at Institute of Medical Biology at Chinese Academy of Medical Sciences were used in this study. The groups of intramuscular sIPV and the wild strain IPV injections (IPV derived from wild strain, wIPV) were designed, and the saline group was used as a negative control group. The Banna mini pigs in various groups were immunized at 0, 1 and 2 months. Blood samples were collected before immunization and on days 30 after each immunization. Levels of neutralizing antibodies were tested for evaluating immunogenicity. The safety was evaluated by observation of the status and weight of mini pigs. Results After the three dose immunization schedules in the Banna mini pigs, the seroconversion rates of type Ⅰ,Ⅱ and Ⅲ sIPV experimental groups and wIPV group were all up to 100%. The neutralizing antibody levels in all the three types were much higher than the protective titer 1: 8. The weight of mini pigs increased after vaccination. Conclusions The sIPV vaccine has good immunogenicity and safety in Banna mini pigs. Banna mini pigs could be a new animal model for evaluation of sIPV vaccine.
9.The effect of chronic intermittent hypoxia on serum high-density lipoprotein cholesterol and the expressions of peroxisome proliferators-activated receptors a and adenosine triphosphate binding cassette transporter A1 in rat liver tissues

Objective To investigate the effect of chronic intermittent hypoxia(CIH)on serum high-density lipoprotein cholesterol(HDL-C)and the changes of HDL-C metabolism-related indicators such as the expressions of peroxisome proliferators-activated receptor a (PPARα)and adenosine triphosphate binding cassette transporterA1(ABCA1)in liver in male SD rats.Methods Obstructive sleep apnea syndrome(OSAHS)-induced CIH rats were randomly allocated into 6 groups:10%CIH-3 weeks,5%CIH-3 weeks,5%CIH-3 weeks +RH(Removal of hypoxia-3 weeks),10%CIH-3 weeks + RH(Removal of hypoxia-3 weeks),control group-3 weeks,and control group-6 weeks.Serum lipids were measured and compared.To observe and compare the liver pathology,the expression levels of PPARα and ABCA1 in liver tissue of CIH rats were detected by immunohistochemical method.Results The levels of serum TC,TG and LDL-C was significantly higher in CIH rats than in control group.The levels of TC,TG and LDL-C were significantly lower in reoxygenation groups than in CIH groups.There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group,CIH rats had significantly lower levels of serum HDL-C;Compared with CIH groups,the levels of HDL-C were significantly higher in reoxygenation groups.There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group,the expression of PPARα and ABCA1 of CIH group was significantly lower;Compared with CIH groups,the expression of PPARα and ABCA1 was significantly higher in reoxygenation groups;There was no significant difference in the expression of PPARα and ABCA1 between reoxygenation groups and correspondent control groups,in which the oxygen level was recovered to normal.Conclusions The serum HDL-C level was obviously decreased in OSAHS-induced CIH rats,and the decreased serum HDL-C can be effectively improved by reoxygenation intervention.OSAHS-induced CIH may lead to the dyslipidemia through PPARα-ABCA1 pathway,and reoxygenation intervention for three weeks can effectively recover the expression levels of PPARα and ABCA1 to normal levels,which suggests that if CIH is effectively intervented,the change of PPARα and ABCA1 of liver can be reversed,thereby reversing dvslipidemia.
10.Difference in proprotein convertase subtilisin/kexin type 9 levels between premenopausal and postmenopausal women

Objective To compare proprotein convertase subtilisin/kexin type 9 (PCSK9) levels between premenopausal and postmenopausal women,and to investigate the relationship between serum PCSK9 and metabolic factors.Methods Totally 515 women were enrolled from the study on diabetes of prediction,prevention,and intervention in Nanjing in 2009.Survey,physical examinations,and determination of related metabolic indexes were performed.Serum PCSK9 level was measured by sandwich ELISA.Results Serum PCSK9 level was positively correlated with low density lipoprotein-cholesterol (LDL-C),total cholesterol (TC),triglyceride,fasting plasma glucose,body mass index,waist-hip ratio,and age in women (all P<0.01).PCSK9 level was significantly lower in premenopausal women than that in postmenopausal women [(58.18 ± 25.44 vs 80.91 ± 33.74) ng/ml,P <0.01].Conclusion Higher level of PCSK9 exists in postmenopausal women compared with premenopausal women.The level of PCSK9 is closely correlated with age,TC,and LDL-C.
