1.Preliminary application of histological evaluation of donor pancreas biopsy tissue in simultaneous pancreas-kidney transplantation

Objective To preliminarily investigate the safety and efficacy of donor pancreas needle biopsy in simultaneous pancreas-kidney transplantation. Methods Clinical data of 7 cases undergoing donor pancreas biopsy were collected retrospectively. All cases underwent donor pancreas biopsy before or during simultaneous pancreas-kidney transplantation. Frozen section or paraffin sectioning techniques were used for tissue preparation, and hematoxylin-eosin and Masson staining were performed to histologically evaluate the donor pancreas. The quality of donor pancreas was comprehensively assessed by combining histological findings with the donor's clinical data. Postoperative follow-up data of 5 simultaneous pancreas-kidney transplant recipients were collected to summarize the safety of donor pancreas biopsy and the prognosis of transplant recipients. Results The 7 pancreas donors were aged 28 to 62 years, with a body mass index ranging from 20.76 to 27.68 kg/m2. Liver ultrasound indicated fatty liver in 3 cases, while pancreatic ultrasound did not reveal any significant abnormalities. Among them, biopsy was performed on 2 donors after completion of pancreatic procurement and processing, and the frozen section histology showed moderate acute pancreatitis changes (edema of acinar cells, necrosis and inflammatory cell infiltration). Combined with a serum amylase level elevated more than 3 times the upper limit of normal value, these two donor pancreases were finally discarded. The remaining 5 cases underwent biopsy immediately after pancreatic vascular anastomosis during simultaneous pancreas-kidney transplantation, and histological evaluation was performed on paraffin-embedded sections. No biopsy-related complications (such as bleeding, pancreatic fistula, etc.) occurred after transplantation. One recipient died of severe infection 2 months after transplantation, while the other 4 recipients were followed up for more than 5 years, with well-functioning transplant kidneys and pancreases. Conclusions Donor pancreas biopsy is relatively safe, and the risk of biopsy-related complications after transplantation is controllable. Comprehensive assessment of donor pancreas quality by combining histological evaluation with the donor's clinical indicators is conducive to improving the accuracy of donor pancreas selection and organ utilization.

Objective To explore the effect of remote ischemic preconditioning (RIPC) on preoperative heart rate variability in patients with heart valves. Methods Patients scheduled to undergo on-pump cardiac valve surgery in the Department of Cardiovascular Surgery, General Hospital of Northern Theater Command, between January and July 2022 were initially enrolled. Eligible patients were randomly assigned at a 1 : 1 ratio to either the RIPC group or the control group. Relevant indicators of heart rate variability [standard deviation of NN interval (SDNN), standard deviation of mean value of NN interval in every five minutes (SDANN), mean square root of difference between consecutive NN intervals (RMSSD), percentage of adjacent RR interval>50 ms (PNN50), low frequency (LF) component, high frequency (HF) component and LF/HF] at 8 hours in the morning on the surgical day between two groups were compared. Results A total of 118 patients were initially assessed. After screening, 58 patients were excluded, and 60 patients provided written informed consent and were enrolled in the trial, with 30 allocated to the RIPC group and 30 to the control group. Seven patients in the control group and 5 patients in the RIPC group were subsequently excluded due to missing heart rate variability data resulting from cancelled operations. Finally, 23 patients in the control group and 25 patients in the RIPC group were included in the analysis. There was no statistical difference in baseline characteristics between the two groups, and there was no significant difference in heart rate variability 24 hours before intervention (P>0.05). After the intervention measures were taken, the comparison of the results of heart rate variability at 8 hours on the day of operation showed that SDNN and SDANN of patients in the RIPC group were higher than those in the control group, with statistical differences (P<0.05). Conclusion RIPC can stabilize the preoperative heart rate variability of patients undergoing cardiac valve surgery.
4.Expert consensus on the clinical application of parenteral direct thrombin inhibitors in special populations

OBJECTIVE To form an expert consensus addressing clinical issues regarding the use of parenteral direct thrombin inhibitors (DTIs) in special populations. METHODS Led by the Sichuan Academy of Medical Sciences & Sichuan Provincial People’s Hospital(the Affiliated Hospital of UESTC), a multidisciplinary working group was formed comprising experts from multiple fields, including clinical pharmacy, cardiac surgery, obstetrics, pediatrics and evidence-based medicine. Through literature review and the Delphi method, clinical questions regarding the efficacy and safety of parenteral DTIs used in special populations were identified. A structured design was adopted using the “Population-Intervention-Comparison-Outcome” (PICO) framework;systematic searches were conducted in CJFD, PubMed, Embase and other databases. Relevant evidence from randomized controlled trials,cohort studies and systematic reviews were included and synthesized. Evidence quality was assessed using the Grading of Recommendations Assessment,Development and Evaluation (GRADE) approach, and recommendations were formulated through three rounds of Delphi surveys and expert consensus meetings. RESULTS &CONCLUSIONS Seven clinical questions were ultimately selected (with a consensus rate exceeding 90%), resulting in the formulation of seven recommendations on the use of parenteral DTIs in special populations, including children, pregnant women, patients with hepatic or renal impairment, patients with mesenteric venous thrombosis, and individuals with thrombophilia. These recommendations clarify the preferred agents, dosing ranges, monitoring parameters, and safety management strategies for parenteral DTIs in these special populations. This expert consensus, which is formulated based on the best available evidence, provides evidence-based guidance for standardized and individualized use of parenteral DTIs in special populations.
5.Expert recommendations on vision friendly built environments for myopia prevention and control in children and adolescents

Abstract
The prevention and control of myopia in Chinese children and adolescents has become a major public health issue. While maintaining increased outdoor activity as a cornerstone intervention, there is an urgent need to explore new complementary approaches that can be effectively implemented in both indoor and outdoor settings. In recent years, environmental spatial frequency has gained increasing attention as one of the key environmental factors influencing the development and progression of myopia. Both animal studies and human research have confirmed that indoor environments lacking mid to high spatial frequency components, often characterized as "visually impoverished", can promote axial elongation and myopia through mechanisms such as disruption of retinal neural signaling, impaired accommodative function, and altered expression of related molecules. Based on the scientific consensus, it is recommended that "enriching of environmental spatial frequency" should be integrated into the myopia prevention and control framework. Following the principles of schoolled organization, family cooperation, community involvement, and student participation, specific measures are put forward in three areas:optimizing school visual settings, improving home spatial environments, and promoting healthy visual behavior. The aim is to create "visually friendly" indoor environments as an important supplement to outdoor activity, thereby providing a novel perspective and strategy for comprehensively advancing myopia prevention and control among children and adolescents.
6.Association of vegetables and fruits intake with depressive symptoms in Yunnan multi ethnic first year junior high school students

Objective:
To explore the association of vegetables and fruits intake with depressive symptoms among multi ethnic first year junior high school students in Yunnan Province, so as to provide data support for preventing and reducing depressive symptoms among first year junior high school students.
Methods:
From October to December 2022, a cluster random sampling method was used to select 8 500 first year junior high school students from 11 ethnic minority areas in Yunnan Province (Fugong County, Longling County, Longyang District, Luchun County, Mojiang County, Nanjian County, Qiaojia County, Shuangjiang County, Tengchong City, Yuanmou County, Zhenyuan County), to investigate with a questionnaire. The Dietary Frequency Questionnaire was used to collect dietary behavior datas, and the Chinese version of Depression Anxiety Stress Scale-21 (DASS-21) was used to assess depressive symptoms. The generalized linear model was used to analyze the association of vegetable and fruit intake with depressive symptoms in students, and stratified analysis was performed according to ethnicity.
Results:
The detection rate of depressive symptoms among first year junior high school students in Yunnan Province was 29.5%. The detection rates of depressive symptoms in Han and minority first year junior high school students were 26.9% and 31.6%. After controlling for demographic variables such as gender, age, family residence and other confounding factors, the generalized linear model analysis results showed that the intake of leafy vegetables ( β= -0.07 , 95%CI =-0.12 to -0.01), flat fruits ( β=-0.06, 95%CI =-0.12 to -0.00) and hot natured fruits ( β=0.11, 95%CI = 0.04- 0.17) were associated with depressive symptoms in Han first year junior high school students (all P <0.05). The intake of melon and fruit vegetables ( β=-0.06, 95%CI =-0.11 to -0.01) and hot natured fruits ( β=0.06, 95%CI =0.01-0.12) were associated with depressive symptoms in ethnic minority first year junior high school students (both P <0.05).
Conclusions
The intake of vegetables and fruits among multi ethnic first year junior high school students in Yunnan Province is related to the risk of depressive symptoms. It is suggested to strengthen the consumption guidance and education of vegetables and fruits to prevent depressive symptoms among first year junior high school students.
7.Beckwith–Wiedemann Syndrome Presenting as Persistent Non-Ketotic Hypoglycemia in a Preterm Infant

Introduction:
Beckwith–Wiedemann syndrome (BWS) is a congenital
overgrowth disorder associated with dysregulation of
genes on chromosome 11p15.5. Infants with BWS can
present with hyperinsulinemic hypoglycemia. There are
also distinctive clinical features including macrosomia,
macroglossia and visceromegaly that may raise suspicion
of this diagnosis.
Case:
A preterm female infant of 30 weeks gestation with a birth
weight of 1.87 kg (97th centile) had recurrent hypoglycemia
in the neonatal period, requiring escalation to a maximum
glucose infusion rate of 17.6 mg/kg/min. Hypoglycemia
only resolved after starting intravenous glucagon infusion.
Critical sampling during hypoglycemia revealed serum
insulin 3.9 µU/mL (3–25 µU/mL), serum ketone 0.2 mmol/L,
cortisol 3,053 nmol/L and growth hormone 16.2 ng/mL.
These findings supported the diagnosis of non-ketotic
hyperinsulinemic hypoglycemia. She was commenced
on oral diazoxide with resolution of hypoglycemia and
discontinuation of glucagon. Examination at birth revealed macroglossia, hepatomegaly
and ballotable kidneys. An initial US abdomen at birth
revealed bilateral enlarged kidneys but normal liver. An
initial chromosomal study revealed karyotype 46, XX. Over
time, additional clinical features became evident fulfilling
the diagnostic criteria for BWS, that is, polyhydramnios,
large for gestational age, transient hypoglycemia, hyperinsulinism, macroglossia, hemihypertrophy, facial nevi,
bilateral ear creases, hepatomegaly and ballotable kidney.
A repeat US abdomen surveillance at 4 months old revealed
a heterogeneous liver mass with marked vascularity,
prompting a diagnosis of hepatic hemangioma. She was
commenced on oral propranolol, with reduction in the size
and vascularity of the liver hemangioma and decline in
alpha-fetoprotein levels.
Conclusion
The clinical features of BWS may not be recognizable in
a preterm baby in early neonatal period. Careful clinical
examination should be done in a baby with persistent
hyperinsulinemic hypoglycemia for underlying syndromal
causes. US surveillance should also be carried out due to
increased risk of hepatoblastoma or liver hemangioma in
BWS, as was seen in this case.

8.Not Just Dehydration: A Case of Early Onset Persistent Hypernatremia in a Preterm Baby

Introduction:
Hypernatremia in preterm babies is often due to insensible
water loss, inadequate fluid intake or sodium imbalance
due to immature kidneys. Congenital nephrogenic diabetes
insipidus (CNDI) is an uncommon cause of hypernatremia
in neonates and presents shortly after birth. Early
recognition is important to prevent severe dehydration,
electrolytes imbalance and neurological complications.
Case:
We report a case of a preterm 32-week female infant, with a
birth weight of 1.14 kg with persistent hypernatremia (146–
156 mmol/L) from day 4 of life. She received total parenteral nutrition since birth and was started on breastmilk
since day 5 of life. The baby was mildly dehydrated with
weight loss and high urea (6.5 mmol/L) at day 7 of life. Total
fluids were increased to 160 mL/kg/day but serum sodium
remained elevated even though the urea had normalized.
The infant was also noted to have high urine output (5–6
mL/kg/hour) since day 3 of life and suboptimal weight gain.
Further evaluation at age 1 month revealed urine
osmolality 71 mOsm/kg, serum osmolality 308 mOsm/kg
and urine sodium <20 mmol/L when serum sodium was 150
mmol/L, suggestive of DI. A trial of desmopressin showed
unchanged serum sodium (Na), suggesting nephrogenic
DI. Administration of intravenous fluids of 1/5NSD10%
resulted in further increase of both sodium (159 mmol/L)
and serum osmolality (326 mOsm/kg) with low urine
osmolality (111 mOsm/kg). Intravenous fluids were
discontinued and she was started on hydrochlorothiazide
(1 mg/kg/dose bd), resulting in gradual normalization of
sodium 138 mmol/L. Post discharge, she had good weight
gain and normal developmental milestones at corrected
age of 1.5-month-old. Due to early onset hypernatremia,
the infant was referred for genetic testing to rule out CNDI.
Conclusion
Early onset persistent hypernatremia and high urine
output despite adequate fluid management should prompt
evaluation of DI. Early diagnosis is crucial to prevent a
chronic state of hypernatremia that can lead to growth and
developmental delay.

9.Mid- and long-term efficacy of mitral valve plasty versus replacement in the treatment of functional mitral regurgitation: A 10-year single-center outcome

Objective To compare the mid- and long-term clinical results of mitral valve plasty (MVP) and mitral valve replacement (MVR) in the treatment of functional mitral regurgitation (FMR). Methods Patients with FMR who underwent surgical treatment in the Department of Cardiovascular Surgery of the General Hospital of Northern Theater Command from 2012 to 2021 were collected. The patients who underwent MVP were divided into a MVP group, and those who underwent MVR into a MVR group. The clinical data and mid-term follow-up efficacy of two groups were compared. Results Finally 236 patients were included. There were 100 patients in the MVP group, including 53 males and 47 females, with an average age of (61.80±8.03) years. There were 136 patients in the MVR group, including 72 males and 64 females, with an average age of (61.29±8.97) years. There was no statistical difference in baseline data between the two groups (P>0.05). There was no statistical difference between the two groups in the extracorporeal circulation time, aortic occlusion time, postoperative hospital and ICU stay, intraoperative blood loss, or hospitalization death (P>0.05), but the time of mechanical ventilation in the MVP group was significantly shorter than that in the MVR group (P=0.022). The total follow-up rate was 100.0%, the longest follow-up was 10 years, and the average follow-up time was (3.60±2.55) years. There were statistical differences in the left atrial diameter, left ventricular end-diastolic diameter, left ventricular end-systolic diameter and cardiac function between the two groups compared with those before surgery (P<0.05). The postoperative left ventricular ejection fraction in the MVP group was statistically higher than that before surgery (P=0.002), but there was no statistical difference in the MVR group before and after surgery (P=0.658). The left atrial diameter in the MVP group was reduced compared with the MVR group (P=0.026). The recurrence rate of mitral regurgitation in the MVP group was higher than that in the MVR group, and the difference was statistically significant (10.0% vs. 1.5%, P=0.003). There were 14 deaths in the MVP group and 19 in the MVR group. The cumulative survival rate (P=0.605) and cardiovascular events-free survival rate (P=0.875) were not statistically significant between the two groups by Kaplan-Meier survival analysis. Conclusion The safety, and mid- and long-term clinical efficacy of MVP in the treatment of FMR patients are better than MVR, and the left atrial and left ventricular diameters are statistically reduced, and cardiac function is statistically improved. However, the surgeon needs to be well aware of the indications for the MVP procedure to reduce the rate of mitral regurgitation recurrence.

The prescription Mahuang Shengmatang in the Treatise on Cold Damage (《伤寒论》) has sparked considerable debate among medical practitioners throughout history, with varying opinions on its indications, pathogenesis described in the text, principle of compatibility, and clinical applications. Both ancient and modern interpreters of Mahuang Shengmatang often focus on herbal compatibility as a primary approach to deduce the pathogenesis and treatment methods. Building upon this foundation, this paper utilizes herbal dosage as a clue to discern the primary and secondary herbs in the prescription. It further analyzes the principle of compatibility based on herbal indications, summarizing the indications and therapeutic principles of this prescription. Ultimately, it reveals the underlying pathogenesis reflected in the text. The internal closure of heat and toxin leads to the stagnation of Qi, preventing Yang Qi from reaching the extremities and causing cold hands and feet. When the pathogenic Qi finds no outlet, it floods both the upper and lower regions of the body, attacking the throat and causing cough with expectoration of pus and blood, and descending to the large intestine to consume Yin fluids, resulting in persistent diarrhea. Based on this pathogenesis, the paper expands the scope of symptoms and signs associated with the prescription, providing a more detailed portrayal of the applicable patient population and enhancing the basis for clinical prescription references. Additionally, the paper presents considerations on several controversial topics, suggesting that the "lower pulse" described in the text refers to the lower limb arterial pulsation, and the symptoms and signs resemble those of septic shock in modern medicine. Therefore, Mahuang Shengmatang should be categorized as a prescription for treating warm diseases and it is not developed by ZHANG Zhongjing. By employing a detailed discussion on the syndrome, pathogenesis, and clinical application in the texts of Mahuang Shengmatang from the dosage, principle of compatibility, and herbal indications, this paper not only enriches the theoretical foundation of Mahuang Shengmatang but also provides a comprehensive perspective and fresh ideas for understanding its clinical application.
