1.The correlation between the test panel of serum CCL3,sPDPN,FABP5 and the severity of lung injury and clinical outcomes in children with severe pneumonia

Objective To explore the correlation between the test panel of serum CC chemokine ligand 3(CCL3),soluble podoplanin(sPDPN),fatty acid-binding protein 5(FABP5)and the severity of lung injury and clinical outcomes in children with severe pneumonia.Methods A total of 168 children with severe pneumonia who visited our hospital from January 2022 to January 2023 were included in this study as severe group.The patients were assigned to good outcome group(137 cases)or poor outcome group(31 cases)based on their prognosis.Additional 80 children with mild or moderate pneumonia were treated as non-severe group.In addition,80 children who underwent health checkup were included as control group.Baseline data such as body mass index and duration of disease were observed and recorded for all children.Enzyme linked immunosorbent assay(ELISA)was applied to detect the expression levels of C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),CCL3,sPDPN,and FABP5 in the serum of all children.The Acute Physiology and Chronic Health Evaluation Ⅱ(APACHEⅡ)score and Lung Injury Prediction(LIPS)score were rated to evaluate the degree of lung injury.Multivariate logistic regression was used to analyze the impact of various factors on the poor outcome of children with severe pneumonia.Spearman correlation was applied to analyze the correlation between the expression levels of CCL3,sPDPN,FABP5 and poor outcome,the severity of lung injury,APACHE Ⅱ score,and LIPS score in children with severe pneumoniae.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of CCL3,sPDPN,and FABP5 expression levels for poor outcome in children with severe pneumonia.Z-test was used to compare the differences in area under the curve(AUC).Results The baseline data such as duration of disease,body mass index,age,and sex did not show significant differences among the control group,non-severe group,and severe group(P>0.05).The levels of serum CRP,PCT,IL-6,TNF-α,CCL3,sPDPN increased,while the level of FABP5 decreased gradually from the control group to non-severe group,and severe group(P<0.05).The patients with poor outcomes showed higher serum levels of CRP,PCT,IL-6,TNF-α,CCL3,sPDPN,APACHE Ⅱ score,and LIPS score but lower FABP5 level compared to the patients with good outcomes(P<0.05).TNF-α,CCL3,sPDPN,APACHE Ⅱ,and LIPS scores were independent risk factors for poor outcomes in children with severe pneumonia,while FABP5 level was independent protective factor for poor outcomes in children with severe pneumonia(P<0.05).The levels of CCL3 and sPDPN were positively correlated with APACHE Ⅱ score and LIPS score,while FABP5 was negatively correlated with APACHE Ⅱ score and LIPS score(P<0.05).The AUC of CCL3,sPDPN,and FABP5 alone was 0.802,0.864,and 0.859 respectively for predicting poor prognosis in children with severe pneumonia.The test panel of CCL3+sPDPN+FABP5 was superior to CCL3,sPDPN,or FABP5 alone(Zcombination-CCL3=3.842,Zcombination-sPDPN=2.585,Zcombination-FABP5=2.957,P<0.05).Conclusions The serum levels of CCL3 and sPDPN are positively correlated with the severity of lung injury,while FABP5 is negatively correlated with the severity of lung injury in children with severe pneumonia.The test panel of CCL3+sPDPN+FABP5 is valuable for predicting poor outcomes in children with severe pneumonia.

3.Current situation of Investigator-Initiated Clinical Trial quality management: A scoping review

Objective:To systematically identify and summarizes the weaknesses of the key aspects of Investigator-Initiated Clinical Trial (IIT) quality management in China, and quantitatively assess these weaknesses with a synthesis of relevant evidence, thereby providing references for the subsequent establishment of a complete IIT quality management system in China.Methods:According to the Scoping review report checklist (PRISMA-ScR statement), we conducted a systematic literature retrieval and screening, data extraction, and result synthesis of IIT quality management issues after defining the research questions.Results:73 eligible studies were eventually included. It was found that the most frequently explored issues were a lack of guidance and support from methodological and statistical experts at the project initiation stage (60.9%), a lack of research funding or improper funding management at the project implementation stage (49.3%), mismanagement of archival materials at the project completion stage (70.0%). Meta-analysis results showed that after evidence synthesis, the incidence of irregular informed consent signing, untraceable raw data, delayed study progress, and protocol violation were all above 40%, but there was heterogeneity in the results.Conclusion:Some outstanding issues in IIT quality management need to be addressed. Future studies should conduct more practical research to obtain quantitative data, undertake demonstrative application of management protocols, further carry out pioneering exploration and research in the field of IIT quality management, and propose effective solutions and strategies to improve IIT quality.
4.Effect of circhipk3 on polarization of microglial cells in nerve injury caused by heat radiation

Objective:To investigate the expression of circhipk3 in microglial cells in heat-induced neurological injury, and to preliminary analyze the effect of circhipk3 on microglial polarization in heat-induced neurological injury.Methods:Mice were randomly (random number) divided into a control group and a heat radiation disease 0.8 h group (HS 0.8), a heat radiation disease 8h group (HS 8), and a heat radiation disease 24 h group (HS 24). By establishing a mouse model of heat shock (HS), heat-damaged brain tissue was obtained, microglia were isolated and RNA was extracted. Quantitative PCR method was used to detect M1 and M2 marker molecules in microglia, and to evaluate the polarization direction and type of microglia. The expression level of circhipk3 was detected in microglial cells in heat-induced neurological injury, and the effect of circhipk3 on microglial polarization was further elucidated by intervening the expression of circhipk3 in microglial cells.Results:The expression of CD45 and CD11-b in the HS 8 group was significantly higher than that in the control group [(4.41±0.18) vs. (1±0.15), P=0.000], [(3.47±0.19) vs (1±0.15), P=0.000] , and the CD45 and CD11-b of the HS 24 group was significantly lower than that of the HS 8 group [(1.34±0.15) vs. (4.41±0.18), P=0.000], [(1.38±0.21) vs. (3.47±0.19), P= 0.001]. At the same time, the expression of CD206, FIZZ and Arg1 in the HS 8 group started to increase compared with the control group [(1.59±0.16) vs. (1±0.12), P=0.014], [(1.62±0.15) vs. (1±0.15), P=0.002 ], [(2.23±0.28) vs. (1±0.19), P=0.004], and CD206, FIZZ, and Arg1 in the HS 24 group were significantly higher than those in the control group [(2.67±0.20) vs. (1±0.12), P=0.002], [(2.19±0.15) vs. (1±0.15), P=0.000], [(3.04±0.18) vs. (1±0.19), P=0.001]; circhipk3 mimicis significantly increased the expression of Arg1 [(7.26± 0.06) vs. (3.86±0.06), P=0.000]; at the same time, circhipk3 inhibitor promoted the expression of CD45 and HO-1 [(2.96±0.03) vs. (1.63±0.09), P=0.000], [(2.52±0.10) vs. ( 1.30±0.02), P=0.000]. Conclusions:Microglial cells are predominantly M1-type in early neurological injury of heat radiation disease. HO-1 may be one of the microglial M1-type markers. The high expression of circhipk3 in microglial cells mainly promotes its transformation to M2 type.
5.Maternal serum alpha fetoprotein and free β-hCG of second trimester for screening of fetal gastroschisis and omphalocele.

OBJECTIVETo assess the detection of maternal serum alpha fetoprotein (MSAFP) and free beta-HCG levels of second trimester for screening of fetal gastroschisis and omphalocele.
METHODSClinical data of 622 639 pregnant women from 5 prenatal screening centers in Hangzhou during October 2007 and September 2016 were analyzed retrospectively. Thirty cases of gastroschisis and 30 cases of omphalocele diagnosed by ultrasonography and postmortem findings were enrolled in the study and 116 cases of pregnant women with normal fetal development during the same period were selected as control group. The cut-off value and area under ROC curve (AUC) of MSAFP and free β-hCG for diagnosis of fetal gastroschisis and omphalocel were analyzed.
RESULTSMSAFP levels of women with fetal gastroschisis and omphalocele were 4.41 (0.88-11.69) MOM and 2.31 (0.72-23.20) MOM, which were significantly higher than that of control group[0.98 (0.41-2.26) MOM, all<0.01]. Free β-hCG level of women with fetal gastroschisis was 1.25 (0.35-19.94) MOM, which was significantly higher than that of control group[0.86 (0.17-6.11) MOM,<0.05). But there were no significant difference in free β-hCG between fetal omphalocele group[1.03(0.21-8.95)]and control group (>0.05). The AUCs of MSAFP for diagnosis of gastroschisis and omphalocele were 0.897 (95%:0.822-0.972) and 0.852(95%:0.762-0.942), respectively (all<0.01). Taking 1.655 MOM as the cut-off value of MSAFP for abdominal wall defects (gastroschisis and omphalocele), the sensitivity was 68.30%, specificity was 99.60% and Youden index was 0.649.
CONCLUSIONSMSAFP of second trimester is a better biomarker than free β-hCG in screening abdominal wall defects.

The number of people with type 2 diabetes mellitus (DM) is increasing every year,and the proportion of patients with digestive cancer is also increasing.At present,more and more studies have found that digestive system cancer is associated with type 2 diabetes.This paper reviewed the recent ten years of domestic and foreign researchers using meta-analysis in the study of type 2 diabetes and digestive system cancer outcomes.It suggested that type 2 DM has certain negative effects on the occurrence and prognosis of digestive system cancer.Its influence and mechanism still needs further study through a variety of methods.
7.Ensemble Partial Least Squares Algorithm Based on Variable Clustering for Quantitative Infrared Spectrometric Analysis

Due to the ability of overcoming both the dimensionality and the collinear problems of the spectral data, partial least squares ( PLS ) is in ever increasingly used for quantitative spectrometric analysis, especially for near-infrared spectrum, mid-infrared spectrum and Raman spectrum. In this work, an improved PLS algorithm is proposed for efficient information extraction and noise reduction. The spectral variables are clustering to several subsets, and several sub-models are built for each subset. Then, the sub-models are re-weighted and ensemble to the final model. Experiments on two near-infrared datasets ( octane number prediction in gasoline and nicotine prediction in tobacco leafs ) demonstrate that the new method provides superior prediction performance and outperformed the conventional PLS algorithm, and the root mean square error of prediction ( RMSEP) is reduced by 32% and 22%, respectively.
8.Posterior transpedicular-intervertebral disc wedge resection osteotomy for correction of thoracolumbar kyphotic deformity

Objective To explore a safe and effective posterior surgical operation for correction of the horacolumbar kyphotic deformity. Methods The study involved 16 patients with thoracolumbar kyphotic deformity treated with the modified posterior transpedicular-intervertebral disc wedge resection osteotomy and screws-rods internal fixation apparatus.There were 11 males and 5 females at an average age of 26.5 years(13-53 years).The kyphosis deformity was caused by ankylosing spondylit in four patients,old lumbothoracic fracture in nine,vertebral dysplasia in two and vertebral body in one.The preoperative kyphosis Cobb angle was 58.1(45°-85°),with the kyphosis deformity at T10 in two patients,at Thin two,at T12in six,at L1 in three and at L2 in three.The main clinical manifestations were different degrees of lower back pain and progressive aggravation of the deformity,influencing the work and living.The course of disease was 8.5 years(4-17 years).All patients underwent pesteriortotal vertebral osteotomy on the apex vertebra,trails-pedicular fixation combined with correction and fusion,after which the patients stayed in bed for four weeks and received orthosis fixation for three months after operation. Resuits The operation lasted for average 190 minutes(125-240 minutes),with average blood loss of 750 ml(450-1 900 m1).All patients were with single segment cut bone,with no spinal cord injury,neurological injury or hardware failure.The post-operative vertical plane facial deformity was corrected for average 55(44°-76°),wit average correction rate of 83%.The follow-up for 10-24 months showed firm internal fixation on the X-ray film and good fusion ofthe vertebral column,with no pseudoarticulation formation,loosening internal fixation or loss of correction.All the patients obtained obvious improvement in appearance of the deformity,with disappearance of the lower back pain and improvement of the quality of life. Conclusion One stage posterior transpedicular-interverte-bral disc wedge resection osteotomy is all effect and safe surgical technique for correction of horacolumbar kyphotic deformity.
9.Expressions of VEGF and TNF-? in interspinous ligament of patients with ankylosing spondylitis in active stage

Objective To detect the pathological expressions of TNF-? and VEGF in interspinous ligament of patients with ankylosing spondylitis(AS) in active stage. Methods The expressions of TNF-? mRNA and VEGF mRNA in the interspinous ligament of patient with AS group and control group were detected by hybridization in situ. Their different results were compared by image analysis system and the correlation between TNF-? mRNA, VEGF mRNA and both AS disease active index (DAI) and C reactive protein (CRP) was analysed respectively. Results The expressions of TNF-? mRNA and VEGF mRNA in the interspinous ligament of patients with active AS were positive,but those of the control group were negtive.
