Objective To assess the incidence of contrast-induced nephropathy(CIN)in patients with chronic thromboembolic pulmonary hypertension(CTEPH)after receiving balloon pulmonary angioplasty(BPA),and to evaluate the effect of the contrast agents on renal function. Methods A total of 143 CTEPH patients, who received BPA at the China-Japan Friendship Hospital of China from December 2018 to May 2022,were enrolled in this study. The clinical data, hemodynamic indicators, and serum creatinine(SC)concentrations within one week before and 48-72 h after BPA were collected. The estimated glomerular filtration rate(eGFR)was calculated according to the Modification of Diet in Renal Disease(MDRD)formula. The SC concentration and eGFR changes before and after each BPA procedure were compared. The incidence of CIN and its risk factors were evaluated, and the changes in hemodynamics,SC and eGFR after the initial and last time of BPA treatment were analyzed. Results A total of 192 BPA procedures were performed in 115 CTEPH patients, including 88 BPA procedures in males and 103 BPA procedures in females. The mean amount of contrast agent used for each BPA was(145.58±47.26)mL.After BPA,12 patients developed 13 times of CIN,with an incidence of 6.8%.There was no significant differences(P>0.05)in the baseline characteristics and SC concentration before BPA between CIN patients and non-CIN patients. In terms of the hemodynamic indexes, the mixed venous oxygen saturation(SvO2)in CIN patients was significantly lower than that in non-CIN patients(58.58%±10.38%vs.66.15%±8.02%,P=0.002),and no statistically significant differences(P>0.05)in the other hemodynamic indexes existed between CIN group and non-CIN group. No statistically significant differences in SC concentration and eGFR existed before and after each BPA procedure. In patients who had received several BPA procedures, significant improvements in the SC[(78.09±18.760)μmol/L vs.(82.26±21.37)μmol/L,P<0.001]and eGFR[(86.08±21.22)mL/(min·1.73 m2)vs.(82.07±22.05)mL/(min·1.73 m2),P=0.007]was achieved when compared with their baseline values. Conclusion CTEPH patients may develop CIN after receiving BPA treatment. After receiving several BPA treatments the patient's clinical symptoms and hemodynamics can be improved, and the patient's renal function is also significantly improved.

Objective To promote the Institutional Review Board(IRB) construction and development via paying more attention to the detail management and providing better protection of subjects' rights and interests in ethical review of clinical trials and scientific research projects. Methods Assessment criteria were established according to the domestic and international laws and regulations on the management of ethical and scientific research project.Ethical site visit was performed on the 37 clinical trials/scientific research projects approved by IRB during the year 2012-2015.Results Clinical trials/scientific research projects were divided into tree types:the registered clinical trials, the post-marketing clinical trials and the researcher initiated scientific research project. All the types met with the following assessment criteria:satisfy the requirement of equipment, facilities and qualifications of drug preservation required for conducting clinical research;subjects were given compensation according to the protocol;informed consent were obtained.Among them, the registered clinical trials meet more criteria than other research types.Regarding to the standard of informed consent implementation, all types met with the following assessment criteria:all the subjects enrolled signed informed consent forms, informed consent forms are well documented;investigators and subjects/ the guardian signed the forms. However, during the site visit, some other criteria were also neglected more or less.Particularly, only few project recorded the consent process and time spots singed the forms. Conclusions Investigators should pay more attention to the activities and behaviors taken place during the research implementation process.Researchers and management staffs should pay attention to ethical detail management to guarantee better protection of subjects' rights and interests.

With the widely development of psychiatric clinical researches, the ethical issue has been concerned gradually.Although the ethical review has strict rules on informed consent, there are many problems and challenges on informed consent implementation because of the special mental illness population.According to the relevant laws and regulations, combined with the characteristics of clinical psychiatric researches, this paper discussed the protection of subjects' fights and interests during the implementation of informed consent in psychiatric clinical researches, from the following aspects:providing a quiet, comfortable, and relatively independent environment for the participants to ensure informed consent, guaranteeing full informed consent time, choosing qualified researchers for informed consent, ensuring the effective process of informed consent, and guiding the subjects to correctly sign their names and the date.

OBJECTIVETo detect the estrogenic activity of genistein and apigenin with ER-positive cell line MCF-7 human breast cancer cells.
METHODMTT method was adopted to study the impact of genistein and apigenin on MCF-7 proliferation in vitro. Real-time RT-PCR method was used to detect their impact on ERalpha, ERbeta, PR and PS2 mRNA expression levels.
RESULTGenistein and apigenin promoted the proliferation of MCF-7. Genistein 1 x 10(-10) mol x L(-1) group showed a significant increase in the expression of ERa mRNA levels or a 17. 76 times more than the control group and a 1.75 times more than the E2 group. Apigenin notably promoted the PR mRNA expression or a 4. 57 times more than the control group and a 1.11 times more than the E2 group. Both of them had different effect in promoting ERalpha, ERbeta, PR or PS2 mRNA.
CONCLUSIONBoth genistein and apigenin have a strong estrogen-like effect. Although they have different effect in promoting estrogenic response genes (such as ERa, ERbeta, PR and PS2 mRNA), genistein shows a stronger activity than apigenin. It also suggests that the signaling pathways of phytoestrogens showing estrogen-like effect are not completely identical with estrogen pathways. The B-cycle position of flavonoids is one of the key sites to estrogen-like activity, and isoflavones (cycle B on site 3) show stronger estrogen-like activity than flavones (B-cycle lies in site 2).
