1.Surveillance results of causes of death among residents in Mengzi City, Yunnan Province, 2018-2021

Objective To analyze the mortality and its changing trend, and composition and order of causes of death of residents in Mengzi City, and to provide a basis for further disease prevention and control. Methods The death causes surveillance data from 2018 to 2021 were derived from the all-cause-of-death surveillance system in Mengzi City. A retrospective analysis was performed on the mortality rate, life expectancy, life expectancy eliminating causes of death, and life loss. The annual percentage change (APC) was analyzed by the Joinpoint regression model to describe changes in mortality trends. Results The overall crude mortality rate was 633.88/100,000. The age-adjusted mortality was 866.87/100,000. There was a significant downward trend in the crude and standardized mortality (APC=-1.73% , APC=-5.96% , P<0.05). Deaths due to chronic non-communicable diseases (NCDs) accounted for 76.44% of the total deaths. The top 5 causes of death of the residents were cerebrovascular diseases (140.38/100 000), heart diseases (104.24/100 000), malignant neoplasms (92.75/100 000), injuries (79.37/100 000), and respiratory diseases (63.17/100 000) in order, accounting for 75.71% of all causes of death. Life expectancy was 75.67 years, 72.32 years and 79.49 years in the whole-population, males and females, respectively. The potential life expectancy loss due to injury, malignant tumor and cerebrovascular disease accounted for 65.45% of all causes of death. Conclusion Chronic non-communicable diseases are the focus of prevention and control work in Mengzi City. Particular attention should be paid to the damage to health and loss of life caused by injuries, malignancies and cerebrovascular diseases.
2.Status quo of overweight, obesity and elevated blood pressure among children and adolescents aged 7 - 17 years in Mengzi City

【Objective】 To analyze the association of overweight and obesity with elevated blood pressure(BP) among children and adolescents in Mengzi City, in order to provide reference for making effective interventions. 【Methods】 A total of 30 classes of students in 14 schools were selected into this study by a stratified random cluster sampling method from October 2020 to September 2021. The paticipants completed a questionnaire survey, and had their height, weight and BP measured. Logistic regression analysis was used to evaluate the association of overweight and obesity with elevated BP. 【Results】 A total of 4 015 children and adolescents aged 7 - 17 years were involved in the study. The detection rate of overweight, obesity and elevated BP was 12.53%, 10.01% and 12.38%, respectively. The detection rate of elevated BP was 9.96%, 19.48% and 25.62% in normal weight, overweight and obese groups, respectively. The risk of elevated BP in the overweight and obese groups was 2.190 times(95%CI: 1.696 - 2.926, P<0.001) and 3.243 times(95%CI: 2.501 - 4.204, P<0.001) as high as that in normal weight groups. The dietary and exercise behaviors of 1 604 adolescents were analyzed, it was found that the detection rate of obesity was significntly higher in non-boarding students(15.09%) and those who consumed fresh fruits at least once a day(13.49%)(χ2=18.012, 7.225, P<0.05), the detection rate of elevated BP was significantly higher in among those who did not consume sugary beverages(16.88%), and performed moderate-to-high intensity physical exercise of 60 minutes or more ≤ 2 days per week(16.88%)(χ2=9.403, 14.921,P<0.01). 【Conclusions】 The risk of developing hypertension increases with the high prevalence of overweight and obesity and is strongly associated with multiple factors. Therefore, it is essential to conduct effective weight control and behavioral lifestyle interventions among children and adolescents.

Background:
and Purpose Recent studies suggested an increased incidence of cerebral venous thrombosis (CVT) during the coronavirus disease 2019 (COVID-19) pandemic. We evaluated the volume of CVT hospitalization and in-hospital mortality during the 1st year of the COVID-19 pandemic compared to the preceding year.
Methods:
We conducted a cross-sectional retrospective study of 171 stroke centers from 49 countries. We recorded COVID-19 admission volumes, CVT hospitalization, and CVT in-hospital mortality from January 1, 2019, to May 31, 2021. CVT diagnoses were identified by International Classification of Disease-10 (ICD-10) codes or stroke databases. We additionally sought to compare the same metrics in the first 5 months of 2021 compared to the corresponding months in 2019 and 2020 (ClinicalTrials.gov Identifier: NCT04934020).
Results:
There were 2,313 CVT admissions across the 1-year pre-pandemic (2019) and pandemic year (2020); no differences in CVT volume or CVT mortality were observed. During the first 5 months of 2021, there was an increase in CVT volumes compared to 2019 (27.5%; 95% confidence interval [CI], 24.2 to 32.0; P<0.0001) and 2020 (41.4%; 95% CI, 37.0 to 46.0; P<0.0001). A COVID-19 diagnosis was present in 7.6% (132/1,738) of CVT hospitalizations. CVT was present in 0.04% (103/292,080) of COVID-19 hospitalizations. During the first pandemic year, CVT mortality was higher in patients who were COVID positive compared to COVID negative patients (8/53 [15.0%] vs. 41/910 [4.5%], P=0.004). There was an increase in CVT mortality during the first 5 months of pandemic years 2020 and 2021 compared to the first 5 months of the pre-pandemic year 2019 (2019 vs. 2020: 2.26% vs. 4.74%, P=0.05; 2019 vs. 2021: 2.26% vs. 4.99%, P=0.03). In the first 5 months of 2021, there were 26 cases of vaccine-induced immune thrombotic thrombocytopenia (VITT), resulting in six deaths.
Conclusions
During the 1st year of the COVID-19 pandemic, CVT hospitalization volume and CVT in-hospital mortality did not change compared to the prior year. COVID-19 diagnosis was associated with higher CVT in-hospital mortality. During the first 5 months of 2021, there was an increase in CVT hospitalization volume and increase in CVT-related mortality, partially attributable to VITT.
6.A dynamic study of sentinel surveillance for occupational hazard in typical industrial enterprises in Guangzhou, China, from 2012 to 2014.

OBJECTIVETo perform a sampling survey of occupational hazard in typical industrial enterprises in Guangzhou, China, by means of sentinel surveillance, to understand the classification, distribution, and concentration/intensity of occupational hazard as well as the characteristics and development of occupational health management in Guangzhou, and to provide a scientific basis for occupational health supervision.
METHODSFifteen enterprises in information technology (IT), shipbuilding, chemical, leather and footwear, and auto repair industries were enrolled as subjects. Dynamic surveillance for occupational hazard and occupational health management was performed in workplaces of those enterprises.
RESULTSThe overall overproof rate of occupational hazard in the 15 sentinel enterprises from 2012 to 2014 was 6.16% (45/731). There was no significant difference in the overproof rate between the three years (P > 0.05). During the three years, enterprises in shipbuilding industry had significant higher overproof rates than those in other industries (P < 0.05). According to the results of occupational health management questionnaire, the overall coincidence rate of survey items was 57.88% (393/679); enterprises in IT industry had significant lower coincidence rates than those in other industries in 2012 and 2014 (47.62%, 29.63%; P < 0.05), while enterprises in leather and footwear industry had significant lower coincidence rates than those in other industries in 2013 (40.63%; P < 0.05).
CONCLUSIONThe enterprises in shipbuilding industry are the key to the prevention and control of occupational hazard in Guangzhou. To strengthen surveillance for occupational health in workplaces in Guangzhou, it is important to enhance occupation health supervision among small and micro enterprises and develop continuous sentinel surveillance for occupational hazard in key industries.

7.Analysis of cytochrome P450 2E1 RsaI/PstI and DraI polymorphisms in workers exposed to benzene.

OBJECTIVETo investigate the cytochrome P450 2E1 (CYP2E1) RsaI/PstI and DraI polymorphisms in workers exposed to benzene.
METHODSA cross-sectional survey was carried out. A total of 71 workers exposed to benzene were included in observation group and the same number of people without occupational benzene exposure were included in control group. Blood samples from the two groups were collected and genotyping for CYP2E1 RsaI/PstI and DraI were conducted using the polymerase chain reaction-restriction fragment length polymorphism.
RESULTSThere were no significant differences in CYP2E1 DraI genotype and allele distributions between the observation group and the control group (χ² = 2.374, P > 0.05; χ² = 2.113, P > 0.05). Significant differences in CYP2E1 RsaI/PstI genotype and allele distributions between the two groups were observed (χ² = 9.129, P < 0.01; χ² = 6.028, P < 0.05).
CONCLUSIONMutations at CYP2E1 RsaI/PstI can enhance the expression of CYP2E1 and this suggests individuals with the mutated gene have increased susceptibility to chronic benzene poisoning.

9.Dynamic observation of clinical course in patients with subacute 1, 2-dichloroethane poisoning.

OBJECTIVETo observe the clinical characteristics and regular patterns of subacute 1, 2-dichloroethane poisoning patients for providing evidences to it's diagnosis, treatment and prognosis.
METHODS51 cases of subacute 1, 2-dichloroethane poisoning analyzed. They were divided into 3 groups according to their main clinical manifestation: group A mainly with intracranial hypertension (n = 25), group B with limbs tremor (n = 18), group C with mental and behavior disorder (n = 8). All cases' clinical symptoms, cranial computer tomography, cerebrospinal pressure (Group A) were observed, the durations of the onset, deterioration, improvement, recovery and whole course of the disease were compared between groups and in each group.
RESULTSIn all of 51 cases, only the differences between the deterioration duration of cranial CT and symptom was significantly (t = 2.555, P<0.05), which indicate the deterioration of symptom was earlier than radiological change. The symptom deterioration of group C was the fastest than group A and group B (P<0.00). As to the change of symptom duration, group B's improvement, recovery and whole course was the longest comparing with group A and group C (P<0.05). As to the change of cranial CT duration, group B's recovery duration was the shortest and group A's recovery duration was the longest (P<0.01); group B's whole course was also the shortest and group A's whole course was the longest (P<0.05). The clinical course of symptoms, cranial computer tomography, cerebrospinal pressure (Group A) was compared in each group, in group A, the duration of improvement and whole course of the cranial CT and cerebrospinal pressure change was longer than that of the symptom change (P<0.01), this indicated that group A has longer asymptomatic intracranial hypertension and their cranial radiography recover slowly. In group B, their symptoms (3.94 ± 4.31 days) deteriorated is earlier than cranial CT changes (P<0.05), the recovery (92.39 ± 55.04 days) and whole course of symptom was longer than cranial CT change (all P<0.01). In group C, symptom deterioration was earlier than CT deterioration (P< 0.05).
CONCLUSIONThe clinical characteristic of subacute 1, 2- dichloroethane poisoning is central nervous system damage, it differs according to the different stage of course, the regions and severity of pathology lesions.
