1.Optimization of honey processing technology of Phellinus igniarius by multi-index comprehensive score combined with analytic hierarchy process

OBJECTIVE To optimize the honey processing technology of Phellinus igniarius. METHODS The key factors of honey processing technology of P. igniarius (honey-water ratio, the mass ratio of honey-water to P. igniarius, the frying temperature and the frying time) were investigated by orthogonal test combined with analytic hierarchy process to determine the optimal technological parameters, using the internal quality (the contents of ergosterol, protocatechuic aldehyde and protocatechuic acid) and appearance traits as evaluation indexes. RESULTS The optimal process of honey-roasting P. igniarius was to take raw P. igniarius (1 cm3 square block), add the appropriate amount of auxiliary materials (with 25 kg of refined honey and water for every 100 kg of P. igniarius), mix well, moisten for 2 h until the auxiliary materials were completely absorbed; put it in a frying container, fry at the frying temperature of 130-140 ℃ for 5 min; take it out, put it in an oven at 50 ℃ for 2 h; take it out, and let it cool. The RSD of the results of three validation experiments was 0.68%. CONCLUSIONS The optimized honey processing technology of P. igniarius is stable and feasible.

3.Summary of best evidence for sexual health management in cervical cancer patients

Objective:To systematically search, evaluate, and summarize the best evidence on sexual health management for cervical cancer patients to provide evidence-based guidance for clinical practice.Methods:According to the "6S" pyramid model, a systematic search was conducted for evidence on sexual health management in cervical cancer patients, including clinical decisions, guidelines, evidence summaries, expert consensus, and systematic reviews, from domestic and international guidelines, professional association websites, and comprehensive databases. The search covered literature from the inception of the databases to November 30, 2023. Two researchers trained in evidence-based nursing independently evaluated the methodological quality of the included studies and extracted and summarized the evidence.Results:A total of 13 articles were included, consisting of 3 clinical decisions, 5 guidelines, 1 expert consensus, and 4 systematic reviews. A total of 27 pieces of evidence were extracted and summarized, covering six aspects: screening and assessment, psychological interventions, device and exercise interventions, non-hormonal interventions, hormonal interventions, and follow-up.Conclusions:The best evidence summarized in this study for sexual health management in cervical cancer patients can provide evidence-based guidance for clinical healthcare providers to standardize the sexual health management of cervical cancer patients.
4.Construction of the evaluation index system for comprehensive budget management in public hospitals

Objective:To construct an evaluation index system for comprehensive budget management at public hospitals and conduct empirical research for reference in evaluation of comprehensive budget management at public hospitals.Methods:From May to June 2022, based on the Implementation Measures for the Comprehensive Budget Management System of Public Hospitals and its interpretations, literature analysis, and expert interviews, a preliminary evaluation index system was developed for comprehensive budget management of public hospitals. Then the Delphi method was applied for two rounds of expert consultation, with experts invited to evaluate the importance of indexes, while the analytic hierarchy process was used to calculate the weight values of indexes. Based on such a comprehensive budget management evaluation index system, a questionnaire was developed, and tertiary public hospitals were selected based on the principle of convenient sampling. Then a questionnaire survey was conducted from June to October 2022. The questionnaire was filled out by hospital level leaders(chief accountants), finance/audit leaders, and directors of finance/audit/operation management departments from the sample hospitals with professional knowledge and rich operational management experience in budget management. The comprehensive budget management level of the sample hospital was thus evaluated based on the survey results. Results:The comprehensive budget management evaluation index system for public hospitals was composed of 2 first-level indexes, 4 second-level indexes, and 20 third-level ones. The weights for the 2 first-level indexes of target building and management orientation were both 0.500, while the weights for the 4 second-level indexes, namely strategic implementation, comprehensive coverage, performance evaluation, and adaptation matching were all 0.250. Among the third-level indexes, the weights of budget execution, system improvement, system establishment, organizational construction, and revenue and expenditure business ranked top five, with weights of 0.070, 0.069, 0.066, 0.066, and 0.064, respectively. A total of 95 hospitals were selected for questionnaire survey, and the evaluation found 46 of them with a comprehensive budget management level of " excellent" and " good" , 27 with a " moderate" level, and 22 with a " poor" and " very poor" level. Scores of the third-level indexes were between 39-94 points, while the score of 6 third-level indexes were below 60 points, namely interconnection, budget analysis, compatibility and adaptation, system improvement, participation level, and assessment application.Conclusions:The evaluation index system for comprehensive budget management at public hospitals in this study proves rational and practical, serving as a reference for the evaluation of comprehensive budget management at public hospitals and related researches.
5.A method for immortalizing swine monoclonal B cells secreting anti-PRRSV antibodies.

Porcine reproductive and respiratory syndrome (PRRS) is a highly contagious disease caused by porcine reproductive and respiratory syndrome virus (PRRSV), which causes great economic losses. At the moment, no effective neutralizing antibody is available for scientific research and treatment. Therefore, developing a method for screening the neutralizing monoclonal antibodies is of great significance for the prevention and treatment of PRRSV and the screening of antigen sites. Monoclonal antibodies have been widely used in the treatment and diagnosis of many human and animal diseases. Therefore, screening effective neutralizing antibodies for different pathogens is an urgent task. Among the methods for monoclonal antibody screening, B cell immortalization is an effective method to obtain neutralizing monoclonal antibody. Specifically, in this study, the bcl-6 and bcl-xl genes were connected by f2a and then the yielded product was ligated to a vector for retrovirus packaging. The swine lymphocytes immunized with PRRSV were infected the yielded mature viruses and cultured in the complete medium containing CD40L and IL21 cytokines. Then, CD21 was used as the marker to screen B cells with the magnetic bead method. Finally, monoclonal B cells were obtained and the secretion of antibodies was tested. The results showed that the plasmid, either being transfected alone or with the packaged plasmids, could be expressed, and that the packaged retrovirus could infect the cells. Moreover, the infected lymphocytes secreted antibodies, so did the screened B cells. Therefore, the method for screening monoclonal antibody against PRRSV was successfully established.

6.Role of nicotinamide phosphoribosyltransferase in delaying smooth muscle cell senescence and protecting abdominal aortic aneurysm

Objective:To investigate the protective effect of nicotinamide phosphoribosyltransferase (NAMPT) on abdominal aortic aneurysm by delaying the senescence of aortic vascular smooth muscle cells (VSMC).Methods:The primary VSMC cells from normal and patients with abdominal aortic aneurysm were cultured by tissue adherence method. Cells were divided into normal human-derived VSMC group (Ctrl-VSMC group), abdominal aortic aneurysm patient-derived VSMC group (AAA-VSMC group), and angiotensinⅡ(AngⅡ) in vitro abdominal aortic aneurysm model group (AngⅡ-VSMC group, 100 nmol/L AngⅡ treated normal human-derived VSMC for 48 hours), AngⅡ+P7C3 group and AAA+P7C3 group after NAMPT agonist P7C3 intervention (adding 5 μmol/L P7C3 on the basis of AngⅡ-VSMC group and AAA-VSMC group, respectively). Immunofluorescence staining was used to identify VSMC; cell proliferation-associated antigen Ki67 staining was used to detect cell proliferation; senescence associated β-galactosidase (SA-β-gal) staining was used to detect cell senescence in each group; Western blotting was used to detect the protein expression levels of senescence-related proteins p21, p16 and NAMPT in each group. Results:Compared with the Ctrl-VSMC group, the positive rate of SA-β-gal staining and the expression levels of senescence-related proteins p21 and p16 in the AAA-VSMC group and AngⅡ-VSMC group were significantly increased [SA-β-gal staining positive rate: (74.1±4.4)%, (68.6±5.5)% vs. (36.8±10.3)%, p21/GAPDH: 0.61±0.07, 0.51±0.03 vs. 0.31±0.03, p16/GAPDH: 0.77±0.03, 0.72±0.06 vs. 0.33±0.26, all P < 0.01]. However, the expression of NAMPT was significantly decreased (NAMPT/GAPDH: 0.88±0.07, 0.79±0.14 vs. 1.29±0.02, both P < 0.01). Compared with the AngⅡ-VSMC group, the positive rate of SA-β-gal staining and the expressions levels of senescence-related proteins p21 and p16 in the AngⅡ+P7C3 group were significantly lower [SA-β-gal staining positive rate: (49.1±3.2)% vs. (68.6±5.5)%, p21/GAPDH: 0.35±0.06 vs. 0.51±0.03, p16/GAPDH: 0.47±0.08 vs. 0.72±0.06, all P < 0.05], while the expression of NAMPT was significantly increased (NAMPT/GAPDH: 1.15±0.06 vs. 0.79±0.14, P < 0.01). Compared with the AAA-VSMC group, the positive rate of SA-β-gal staining and the expression levels of senescence-related proteins p21 and p16 in the AAA+P7C3 group were significantly lower [SA-β-gal staining positive rate: (54.1±6.0)% vs. (74.1±4.4)%, p21/GAPDH: 0.38±0.02 vs. 0.61±0.07, p16/GAPDH: 0.50±0.13 vs. 0.77±0.03, all P < 0.05], but the expression of NAMPT was significantly increased (NAMPT/GAPDH: 1.25±0.28 vs. 0.88±0.07, P < 0.01). Conclusion:NAMPT agonist P7C3 can delay the senescence of VSMC and play a protective role in abdominal aortic aneurysm.

Objective:To investigate the clinical manifestations, treatments and prognosis of subacute arsenic poisoning.Methods:In January 2020, a retrospective analysis was carried out on 11 patients hospitalized with subacute arsenic poisoning caused by arsenic contaminated drinking water. We observed manifestations, treatments and prognosis.Results:The main clinical presentations of subacute arsenic poisoningin were gastroenteritis in early phase, some of them had other organ damage, such as skin, blood, liver, kidney, cardiovascular and so on. The later phase was mainly peripheral nervous system damage. The treatment was mainly to chelate arsenic, protect target organs and treat toxic peripheral neuropathy. Most were significantly recoveried, but the recovery of severe toxic peripheral neuropathy was tardy.Conclusion:Acute gastroenteritis is the mainly early manifestation of subacute arsenic poisoning caused by digestive tract, and toxic peripheral neuropathy in the later phase. The prognosis is good, but the recovery of severe toxic peripheral neuropathy is tardy.

9.Construction of risk prediction model for acute type A aortic dissection

Objective To analyze the risk factors associated with acute type A aortic dissection,and to establish an effective predictive scoring model for early clinical intervention.Methods A retrospective review of 200 patients diagnosed as acute type A aortic dissection in the Department of Emergency,Guangdong Provincial People's Hospital between April 1,2012 and March 31,2017 was conducted as a modeling group.There were 160 males and 40 females with age of (53.30 ±13.19) years.According to the criteria of dissection rupture within 72 h after CTA examination,patients were divided into a rupture group (100 cases) and an unruptured group (100 cases).A prediction model of rupture risk was established based on the multivariate Logistic regression analysis of the risk factors of acute A-type aortic dissection in the early stage.After the establishment of the model,a prospective review of 80 patients diagnosed as acute type A aortic dissection in our hospital between April 1,2017 and May 31,2018 was conducted as a validation group.There were 57 males and 23 females with age of (54.00 ±13.68) years.In the validation group,there were 53 patients with rupture and 27 patients without rupture.Furthermore,this model was verified in the validation group.The scores were divided into low-risk rupture and high-risk rupture according to the best cut-off value of the receiver operating model curve of the modeling group.Results Logistic regression analysis showed that 10 factors were finally established the rupture risk prediction score model.The factors were as following:age >63 years (2 points),women (2 points),ventilator assisted ventilation (3 points),AST >80 U/L (2 points),no distortion of the inner membrane (2 points),diameter of the aortic sinus >41 mm (1 point),maximum diameter of the ascending aorta >48 mm (1 point),ratio of false lumen area to true lumen area >2.12 (2 points),Lac >1.9 mmol/ L (3 points),and WBC >14.2×109/L (1 point).The results of validation showed that the scoring model had a high predictive value (AUC =0.928,95% CI:0.872-0.984,P < 0.001) and goodness of fit (Hosmer-Lemeshow x2 =8.331,P =0.402).Moreover,the scoring model was further divided into low-risk (0-6 points) and high-risk (7-19 points),with sensitivity of 83.0% and specificity of 86.0%.Conclusions The predictive model of dissection risk in the acute type A aortic dissection constructed within 72 h after CTA is helpful for the evaluation of the disease and early intervention,and has certain clinical application value.
10.Evidence summary of procedural pain management for pediatric patients over three years old

Objective? To search and summarize the best evidence of nursing management of procedural pain in pediatric patients so as to provide guidance for clinical nursing staff in the standardized prevention and management of procedural pain. Methods? The Cochrane Database of Systematic Reviews, JBI Database of Systematic Reviews, National Guideline Clearinghouse(NGC), the National Institute for Health and Care Excellence(NICE) and Scottish Intercollegiate Guideline Network(SIGN) were searched on computer to collect literature including clinical practice guidelines, evidence summary and systemic reviews about the management of procedural pain among children 3 years old above. Two researchers independently assessed the quality and extracted data and conducted evidence extraction from the articles that met the standard of quality. Results? Totally eleven articles were enrolled including three best practice guidelines, three systematic reviews and five evidence summaries. Conclusions? Cognitive behavioral intervention, topical application of epidermal anesthetics or local cold compress, and combined operation of distraction can effectively reduce the pain and anxiety caused by procedures. The knowledge and skills of procedural pain management should be included in the education and training courses of medical institutions. Evidence should be selected pertinently to participate in the management of procedural pain in the mode of multidisciplinary cooperation.
