1.New Progress in the Prevention and Treatment of Depression by Regulating Intestinal Flora with Traditional Chinese Medicine

The incidence of depression is increasing year by year,seriously affecting people's work and life.With the continuous in-depth study of its pathogenesis,it is found that depression is closely related to intestinal flora.The imbalance of intestinal flora can cause central nervous system diseases through the microbial-brain-gut axis,which involves neurotransmitters,neuroendocrine,neurotrophic factors,inflammatory response and other mechanisms.This paper introduced intestinal flora and relevant regulatory factors related to various mechanisms of depression,listed experimental examples of intestinal flora imbalance leading to depression,and then summarized relevant studies on the anti-depression mechanism of traditional Chinese medicine through regulation of intestinal flora according to the mechanism of action,among which the regulation of microbial metabolism,inflammatory factors and neurotransmitters was mainly studied.The conclusion is expected to provide some reference for the study of anti-depression effect of Chinese medicine.

3.The characteristic of cognitive impairments in patients with bipolar Ⅱ depression

Objective:To investigate the character and prevalence of cognitive impairment of patients with bipolar Ⅱ depression (BD-Ⅱ).Methods:124 patients diagnosed as bipolar Ⅱ depression according to the DSM-5 criteria and 124 demographically matched healthy subjects were recruited. Seven cognitive functions were assessed with the Measurement and Treatment Research to Improve Cognition in Schizophrenia (MATRICS) Consensus Cognitive Battery(MCCB) in all participants, including speed of processing (SOP), attention vigilance (AV), working memory (WM), verbal learning (VER), visual learning (VIS), reasoning problem solving (RPS), and social cognition (SC), and the composite. Analysis of covariance was used to test the differences in cognitive function. The number and percentage of cognitive domains impairment which was defined as the cognitive domains scored below standard values by 1, 1.5 and 2 standard deviation (SD) were explored.Results:(1) BD-Ⅱ patients were significantly impaired on seven MCCB domains and the composite scores compared with HC (all P<0.01). Correlation analysis showed that the scores of VER, RPS negatively correlated to the number of episodes ( r=-0.212, P=0.018; r=-0.183, P=0.042); (2) Most healthy control participants were not impaired on any 2 cognitive domains at 1.5 SD (79.84%,99/124) and 2 SD (92.74%,115/124) cut-offs, with the 2.42%-6.45% cognitive impairment at the 1.5 SD cut-off, and 0-4.84% at the 2 SD cut-off accordingly. (3) At the 1.5 SD cut-off, 33.06%,41/124 of the BD-Ⅱ patients were cognitively impaired in two or more domains, while at the 2.0 SD cut-off, 14.52%,18/124 of patients were cognitively impaired. Meanwhile, the incidence of impairment in various cognitive domains was 9.68%-24.19% and 3.23%-15.32%, of which the incidence rate of visual learning impairment was 12.90%, and the incidence rate of impairment in working memory and social cognition was 24.19%. Conclusions:Participants with BD-Ⅱ depression were generally impaired on a greater number of cognitive domains with a higher percentage than the healthy controls, especially on the cognitive domains of working memory, visual learning, and social cognition. And the domains of verbal learning and reasoning problem solving were negatively correlated with the number of episodes.

5.Correlation between serum LN, Ⅳ C, P Ⅲ NP, HA and the severity as well as the prognosis of idiopathic pulmonary fibrosis

Objective To investigate the correlation between serum laminin (LN),type collagen (ⅣC),type procollagen N-terminal peptide (PⅢNP),hyaluronic acid (HA) and the severity as well as the prognosis of idiopathic pulmonary fibrosis (IPF).Methods From February 2015 to July 2016,160 IPF patients and 160 healthy subjects as controls were enrolled in this retrospective study.Serum LN,ⅣC,PⅢNP,and HA were analyzed in IPF patients and healthy controls.Pulmonary function test and chest high resolution computed tomography (HRCT) were carried out in IPF patients.Demographics and clinical characteristics,the percentage of forced vital capacity in the prediction value (FVC%pred),the percentage of diffusing capacity of the lung for carbon monoxide in the prediction value (DLCO%pred),and HRCTscore were collected.IPF patients were followed up.Results (1)There were no significant difference between two groups in age and sex ratio.The proportion of smoker in IPF patients was significantly higher than that in healthy controls(P < 0.01).(2)Serum LN(P < 0.01),ⅣC(P < 0.01),PⅢNP(P < 0.01),and HA(P < 0.01) were significantly increased in the patients with IPF compared with the healthy controls.(3)Serum LN,ⅣC,PⅢNP,and HA of IPF patients positively correlated with HRCT score,all P < 0.01,and negatively correlated with FVC%pred and DLCO%pred (all P < 0.05).(4)Serum LN(P < 0.01),ⅣC(P < 0.05),PⅢNP(P < 0.01),and HA(P < 0.01) in acute exacerbation IPF patients were significantly higher than those in the stable IPF patients.(5)Serum LN(P < 0.01),ⅣC(P <0.01),PⅢNP(P < 0.01),and HA(P < 0.01) in the survived patients were significantly lower than those inthe dead patients.Conclusions Serum LN,ⅣC,PⅢNP,and HA may reflect IPF prognosis and the severity of IPF.

Background and purpose:As one of the most fatal malignant tumors in China, the morbidity and mortality of lung cancer remain high. Early diagnosis and normative treatment is the key to improve the prognosis of lung cancer. The aim of this study was to explore the practice of early lung cancer screening with low-dose spiral computed tomography (CT) based on the current situation in community health service, with integration of superior resources of med-ical institutions at all levels in Shanghai.Methods:From Aug. 2013 to Aug. 2014, we screened high-risk population in selected communities of Minhang District, Shanghai, for early diagnosis of lung cancer with low-dose spiral CT combined with multidisciplinary comprehensive treatment models including minimally invasive surgery, exploring the medical service network covering prevention, diagnosis, treatment, rehabilitation and follow-up.Results:Screening population is 11 332 (male 7 144, female 4 188); Twenty-nine cases with pathological diagnosis of malignant tumor, including 27 cases of pri-mary lung cancer, 1 case of lung metastasis, 1 case of breast cancer. The morbidity of primary lung cancer is 238.26×10-5. There were 22 cases of Stage 0-Ⅰ lung cancer accounting for 81.48% of all diagnosed primary lung cancer.Conclusion:Based on community health service, screening with low-dose spiral CT could improve the early diagnosis rate of lung can-cer with feasibility and validity, which could be applicable in qualified eligible medical center and community in China.
7.Role of the Mac-2 binding protein glycosylation isomer in diagnosis of chronic liver diseases

Mac-2 binding protein is a type of glycoprotein which exits in cytoplasm, interstitial fluid, and plasma and is secreted by a variety of tissues and organs. Mac-2 binding protein glycosylation isomer (M2BPGi) is synthesized by hepatic stellate cells. This article briefly introduces the research advances in the role of M2BPGi in chronic liver diseases and finds out that the serum level of M2BPGi helps to increase the specificity and sensitivity of the diagnosis and monitoring of liver diseases, has certain advantages over other noninvasive methods, and may become a new biological marker for the diagnosis of liver diseases.
8.Endoscopic resection using the new duette multiband mucosectomy kit for esophageal disease

Background and purpose:Endoscopic treatment is a promising therapeutic option for superifcial lesions throughout the gastrointestinal tract, this study was aimed to evaluate the efficacy of endoscopic resection (ER) using the new Duette multiband mucosectomy kit (DT-6) on treating esophageal disease. Methods:Since Jun. 2011, ER using DT-6 has been performed on 100 patients in a tertiary medical center. Data from those who have been followed up for over 6 months was analyzed. ER and esophagectomy were compared on treating high grade dysplasia (HGD) lesions and early esophageal cancer. Results:From Jun. 2011 to Jan. 2012, a total of 32 patients with esophageal lesions underwent 34 ER using DT-6 (22 male and 10 female, mean age 59.0 years, range 25 to 83 years). There were (3.4±1.0) specimen resected per operation, and the average greatest diameter was (11.8±2.7)mm. Intraoperative blood loss was (5.45±1.47)mL. The median follow-up period was 8.2 months with a 100%half-year-follow-up rate. Except one pneumothorax occurred during one endoscopic submucosal dissection (ESD), no other complications happened. When Comparing ER and esophagectomy on treating HGD and early esophageal cancer, ER showed advantages in terms of operation time, intraoperative blood loss, hospital stay and complications. Conclusion:ER using DT-6 is safe, simple, minimally invasive and effective for esophageal disease. Prospective study and long follow-up are needed to compare endoscopic resection and esophagectomy for HGD and early esophagus cancer.
