1.Expert consensus on whole-process management of drug traceability codes in medical institutions of Sichuan province

OBJECTIVE To provide standardized whole-process guidance on drug traceability codes for medical institutions in Sichuan province, ensuring medication safety and compliance with medical insurance supervision requirements. METHODS Based on evidence-based principles and expert consensus, Expert Consensus on Whole-process Management of Drug Traceability Codes in Medical Institutions of Sichuan Province (hereinafter referred to as the Consensus) was formulated through systematic literature review, field investigations, establishment of a multidisciplinary expert committee and multiple rounds of questionnare consultation via the modified Delphi method, and finalized through consensus meetings. RESULTS & CONCLUSIONS The Consensus clarifies key operating procedures for code verification, code assignment and code return, whole-process operational standards for drug warehouse acceptance and storage, drug warehouse outbound delivery and pharmacy acceptance check, drug distribution and dispensing in pharmacy and intravenous admixture center, medication administration in nursing units and examination departments, as well as drug return process. Key recommendations are proposed such as improving the core functions of the drug traceability system, unifying the hospital-wide traceability code database, strengthening the management of traceability codes for backup medications, establishing a management organization and institutional framework, and optimizing the architectural design and data governance requirements of the drug traceability system. The release of the Consensus will provide scientific, standardized and implementable practical guidelines for medical institutions of Sichuan province, helping to improve closed-loop management of the drug traceability system, strengthen medication safety and fulfil medical insurance fund supervision.
2.Improvement effect of imperatorin on airway remodeling in bronchial asthma mice and its mechanism

Objective:To explore the effects of imperatorin(IMP)on airway remodeling in the bronchial asthma mice,and to elucidate the possible mechanisms.Methods:Forty SFP male BALB/c mice were randomly divided into control group,model group,low dose of IMP group(IMP-L group),high dose of IMP group(IMP-H group)and dexamethasone group,with 8 mice in each group.Except for contol group,the mice in the other groups were injected with an ovalbumin(OVA)suspension intraperitoneally to induce the asthma models.After one week,the daily asthma symptoms of the mice were observed and scored.After 8 weeks,the enhanced pause(Penh)values of the mice in various groups were detected to evaluate the airway reactivities.The percentages of eosinophils in the bronchoalveolar lavage fluid(BALF)of the mice in various groups were detected by flow cytometry.The levels of serum IgE,interleukin interferon-gamma(IL)-13,IL-5,IL-4 and interferon-gamma(IFN-γ)in BALF of the mice in various groups were measured by enzyme-linked immunosorbent assay(ELISA)method.HE,PAS and Masson staining were applied to observe the pathomorphology,the number of goblet cells and collagen deposition of the lung tissue of the mice in various groups.Immunohisto chemistry method was applied to detect the expressions of α-smooth muscle actin(α-SMA)and mouse mammary tumor virus(MMTV)wingless type MMTV intergration site family member 5A(Wnt5A)proteins in lung tissue of the mice in various groups.The expression levels of Wnt5A,cellular myelocytomatosis oncogene(c-Myc),β-catenin and α-SMA in lung tissue of the mice in various groups were detected by Western blotting method.The expression levels of α-SMA protein in lung tissue of the mice in various groups were detected by immunofluorescence method.Results:Compared with control group,the score of asthma symptoms of the mice in model group was increased(P<0.01);the Penh value was significantly increased(P<0.01);the serum IgE levels and the levels of IL-13,IL-5,IL-4 in BALF,as well as the percentage of eosinophils(EOS)in BALF were significantly increased(P<0.05 or P<0.01),and the level of IFN-γ was reduced(P<0.05);the expression levels of α-SMA and Wnt5A proteins in lung tissue were markedly increased(P<0.01);the expression levels of proteins associated with the Wnt/β-catenin signaling pathway in the lung tissue were significantly increased(P<0.01);the immofluorescence method results showed the expression level of α-SMA protein in lung tissue was significantly increased(P<0.01).Compared with model group,the scores of asthma symphtoms of the mice in IMP-L group,IMP-H group,and dexamethasone group were decereased(P<0.01),and the Penh values of the mice in IMP-H group were decreased(P<0.05);the serum IgE levels and the levels of IL-13,IL-5,IL-4 in BALF,as well as the percentages of EOS in BALF of the mice in IMP-L group,IMP-H group,and dexamethasone group were decreased(P<0.05 or P<0.01),and the levels of IFN-γ were increased(P<0.05);the expression levels α-SMA and Wnt5A proteins in lung tissue were decreased(P<0.05 or P<0.01);the expression levels of proteins related to the Wnt/β-catenin signaling pathway in the lung tissue were decreased(P<0.05 or P<0.01);the immunofluorescence method results showed that expression levels of the α-SMA protein in the lung tissue were reduced(P<0.05 or P<0.01).Conclusion:IMP has an improving effect on airway remodeling in the asthmatic mice and can inhibit the expression levels of Wnt/β-catenin pathway-related proteins.
3.Analysis on the Effect of Intelligent and Information Management of Labor Analgesia

Objective To evaluate the effect difference between intelligent information management system and traditional management mode in labor analgesia,and to discuss its influence on labor pain control and clinical application.Methods The study included 200 eligible pregnant women admitted to Xiamen Humanity Maternity Hospital from April 2022 to April 2023,who were randomly divided into an experimental group(Group A)and a control group(Group B),with 100 patients in each group.Group A utilized an intelligent information management system(REHN I analgesia pump system),while Group B employed a traditional electronic analgesia pump.The primary outcomes included Numerical Rating Scale(NRS)scores before analgesia,15 minutes after analgesia,and at full cervical dilation,as well as the proportion of patients with NRS≥7.Secondary outcomes included the number of pump presses,the number of effective presses,the proportion of active versus passive rescue measures,adverse reactions,and maternal satisfaction.Results Compared with Group B,Group A had significantly lower NRS scores at full cervical dilation(P<0.05),and a significantly lower proportion of patients with NRS≥7[10 cases(10.4%)vs 24 cases(25.2%),P<0.05].Group A also showed a higher proportion of active rescue measures(P<0.05)and significantly more effective presses(P<0.05).There were no significant differences between the groups in terms of adverse reactions and maternal satisfaction(P>0.05).Conclusion The intelligent information management system can significantly enhance the effectiveness of labor analgesia,decrease the incidence of severe maternal pain,reduce the frequency of active interventions by anesthesiologists,and improve the efficiency of analgesic management.Its application holds substantial value and potential for promotion.
4.Correlation Analysis between Serum lncRNA BIRF and lncRNA FAL1 Levels Expression and Degree of White Matter Lesions in Patients with Cerebral Small Vessel Disease

Objective To explore the correlation between the expression of long non-coding RNA(lncRNA)brain ischemia-related factor(BIRF)and focally amplified lncRNA on chromosome 1(lncRNA FAL1)in serum and the degree of white matter lesions(WML)in patients with cerebral small vessel disease(CSVD).Methods From June 2021 to June 2023,102 CSVD patients admitted to Affiliated Hospital of Chengde Medical University were collected,and these patients were grouped into WML group(n=72)and non WML group(n=30)based on WML diagnostic criteria.According to the Fazekas score,the WML group was further grouped into mild WML group(n=24),moderate WML group(n=36)and severe WML group(n=12).Real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)was applied to detect the levels of lncRNA BIRF and lncRNA FAL1 in serum.Pearson correlation was applied to analyze the correlation between serum lncRNA BIRF and lncRNA FAL1 levels.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum lncRNA BIRF and lncRNA FAL1 levels for severe WML in CSVD patients.Results The age(70.50±5.86 years),history of hypertension(Yes/No,43/29),history of diabetes(Yes/No,45/27),IL-33(68.35±6.80 pg/ml),IL-18(97.78±9.65 ng/L),ubiquitin carboxyl terminal hydrolase L1(UCH-L1)(0.29±0.10 μg/L)and lncRNA BIRF level(2.45±0.30)of patients in the WML group were higher than those in the non WML group(67.10±5.76 years,11/19,9/21,62.48±6.13 pg/ml,92.56±9.37 ng/L,0.24±0.06 μg/L,1.02±0.11),while the expression of serum lncRNA FAL1(0.52±0.10)was lower than that in the non WML group(1.04±0.15),with significant differences(t=2.683,4.518,8.978,4.085,2.510,2.550,25.346,20.500,all P<0.05).The serum lncRNA BIRF levels of CSVD patients in the mild,moderate and severe WML groups(2.23±0.23,2.47±0.31,2.82±0.42)were increased sequentially,while the expression of serum lncRNA FAL1(0.60±0.15,0.51±0.09,0.40±0.04)was decreased sequentially,with significant differences(F=14.913,13.899,all P<0.05).Pearson correlation analysis,the serum levels of lncRNA BIRF and lncRNA FAL1 in patients with WML were negatively correlated(r=-0.603,P<0.001),serum lncRNA BIRF was positively correlated with Fazekas score in WML patients(r=0.483,P<0.001),but serum lncRNA FAL1 was negatively correlated with Fazekas score(r=-0.507,P<0.001).The AUCs of serum lncRNA BIRF and lncRNA FAL1 levels alone and both combination for predicting severe WML in CSVD patients were 0.756(0.641~0.850),0.839(0.733~0.915)and 0.892(0.796~0.953),respectively,and the combination of the two was superior to the detection of serum lncRNA BIRF alone(Z=2.111,P=0.035).Conclusion The serum lncRNA BIRF level is increased and lncRNA FAL1 is reduced in patients with CSVD and WML,and both are related to the degree of WML in CSVD patients.
5.Analysis of human plague in Inner Mongolia Autonomous Region from 1970 to 2022

Objective:This study aims to analyze the epidemic situation and characteristics of human plague in Inner Mongolia Autonomous Region, in order to provide scientific basis for formulating human plague prevention and control strategies.Methods:The epidemic data of human plague in Inner Mongolia Autonomous Region from 1970 to 2022 were collected, and descriptive analysis of its distribution, sources of infection and route of transmission, and case classification were conducted. The epidemic data came from the Plague Prevention and Control Management Information System form the Chinese Center for Disease Control and Prevention, as well as the summaries of plague surveillance work from 1970 to 2022, the compilation of historical data and other relevant materials in Inner Mongolia Autonomous Region.Results:From 1970 to 2022, all cases of human plague in Inner Mongolia Autonomous Region occurred in the plague foci of Meriones unguiculatus, with 18 cases of disease and 5 deaths, and the case fatality rate was 27.78%. In 2019, there were 4 cases. Three cases each in Sunitezuo Banner, Wulateqian Banner, and Etuokeqian Banner. Two cases each in Wulatezhong Banner and Siziwang Banner. One case each in Shangdu Banner, Suniteyou Banner, Xianghuang Banner, Damao Banner, and Etuoke Banner. May, and from July to November, the number of cases was 4, 4, 4, 1, 2, and 4. Of the 18 patients, 13 were males and 5 were females, age ranging from 6 years to 70 years. Three patients aged 1 - 18, 11 patients aged 19 - 59, and 4 patients aged 60 - 70. The largest number of patients (13 cases) were pastoralists by occupation, followed by field workers (3 cases). Thirteen patients contracted the disease through flea bites. Among the plague subtypes, bubonic plague was the most common with 13 cases.Conclusions:From 1970 to 2022, patients with human plague in Inner Mongolia Autonomous Region were predominantly young adult males. Herdsmen and outdoor workers were more susceptible to human plague. The most common mode of transmission was through flea bites, and the predominant subtype of plague was bubonic plague.

Outpatient humanistic care refered to providing a full process of caring medical services to outpatients. In order to standardize the human caring services for outpatients in medical institutions, promote the comprehensive service level of outpatient services, and improve the patient′s medical experience, Chinese Association for Life Care issued the group standard of Management Norms for Human caring of Outpatients in April 2023. This standard clarified the relevant terms and definitions of human caring for outpatients, specified the basic requirements for human caring, the humanistic quality and care responsibilities of outpatient staff, the outpatient care environment and facilities, the outpatient care process and measures, and quality management. It designed standardized and personalized full process care service norms, providing references for medical institutions at all levels to promote the development of human caring for outpatients.
7.Analysis of a child with developmental disorder and epilepsy due to a homozygous variant of SLC25A12 gene

Objective:To explore the genetic basis for a child featuring global developmental delay and epilepsy.Methods:A child who had presented at Guangzhou Women and Children′s Medical Center Liuzhou Hospital on February 19, 2023 was selected as the study subject. Clinical data of the child was collected. The child was subjected to whole exome sequencing, and candidate variant was validated by Sanger sequencing and bioinformatic analysis.Results:The child, an 8-month-old girl, had manifested with global developmental delay, epilepsy, and hyperlactacidemia. Cranial MRI revealed diverse hypomyelinating leukodystrophies. Electroencephalogram showed slow background activities. Genetic testing revealed that she has harbored a homozygous variant of the SLC25A12 gene, namely c. 115T>G (p.Phe39Val), for which both of her parents were heterozygous carriers. Based on the guidelines from the American College of Medical Genetics and Genomics, the variant was predicted to be of uncertain significance (PM2_Supporting+ PM3_Supporting+ PP3_Moderate+ PP4_Moderate). I-Mutant v3.0 software predicted that the variant may affect the stability of protein product. Conclusion:The homozygous c. 115T>G (p.Phe39Val) variant of the SLC25A12 gene probably underlay the pathogenesis of the disease in this child.
8.Concentrated growth factor and collagen as barrier materials in alveolar ridge preservation for posterior teeth:a prospective cohort study with one-year follow-up

Objective This study aims to evaluate the efficacy of concentrated growth factor(CGF)membrane and collagen as barrier materials in sealing the alveolar socket in alveolar ridge preservation(ARP)in the posterior region during a one-year follow-up.Methods A total of 24 pa-tients who underwent ARP in the posterior region were selected for inclusion and randomly assigned to the CGF group(12 cases)and Collagen group(12 cases).The pa-tients in both groups underwent extraction of posterior teeth.The extraction sockets were filled with a bone sub-stitute to the level of the pre-extraction buccal and lingual or palatal alveolar bone plates.The wounds in the CGF group were closed with a fabricated CGF overlaying the upper edge of the bone substitute material,whereas those in the Colla-gen group were closed with Bio-Oss Collagen.The implants were placed after 6 months.The evaluation was based on implant retention,re-grafting rate,and vertical and horizontal alveolar ridge bone volume changes measured by cone beam computed tomography(CBCT).Data were statistically analyzed using SPSS 28.0 software.Results No patient withdrew throughout the follow-up period.No implant failure and no severe peri-implant or mucosal soft tissue compli-cations were observed.Six months after the operation,the degree of vertical alveolar ridge height resorption in the CGF group was lower than that in the Collagen group(P<0.05).There were no statistically difference between the groups at 1 year after the operation(P>0.05).The amount of bone reduction in horizontal alveolar ridge width showed no difference between the groups at 6 months and 1 year after surgery(P>0.05).Conclusion CGF membrane and Bio-Oss Collagen as barrier materials for posterior ARP inhibited reduction in alveolar ridge bone mass.
9.Study on the prediction of echocardiography combined with diaphragmatic ultrasound on the outcome of removing machine for patients with cardiac valve replacement for mechanical ventilation

Objective:To analyze the predictive value of echocardiography combined with diaphragmatic ultrasound on the outcome of removing machine for patients who underwent cardiac valve replacement for mechanical ventilation.Methods:Retrospectively,a total of 57 patients who adopt mechanical ventilation after underwent cardiac valve replacement in the First People's Hospital of Yibin from January 2022 to March 2023 were selected as the study subjects.According to the results of removing machine,the patients were divided into failed group(11 cases)and successful group(46 cases).All patients underwent echocardiography combined with diaphragmatic ultrasound examination.The indicators of echocardiography,included left ventricular ejection fractions(LVEF),right ventricular fractional area change(RVFAC)and systolic myocardial velocity(Sa),between different groups were compared.The early diastolic mitral annular tissue velocity(e')was recorded to calculate the ratio of early diastolic transmitral flow velocity(E)to e'(E/e'),and the indicators of echocardiography and diaphragmatic ultrasound.Logistic regression analysis was performed to analyze the factors affecting the failure of removing machine.Receiver operating characteristic(ROC)curve was drawn to evaluate the predictive values of echocardiography and diaphragmatic ultrasound indicators for the failure of removing machine in mechanical ventilation.Results:The differences of the differences of LVEF,RVFAC and Sa between failed group and successful group were no significant(P>0.05).The E/e'value of the successful group was 10.06±1.30,which was significantly lower than 12.69±2.96 of the failed group,and the difference was statistically significant(t=2.084,P<0.05).The diaphragm thickening fraction(DTF)and diaphragm excursion(DE)values of the successful group were respectively 41.34±10.74 and 13.04±1.18,which were significantly higher than 19.67±5.37 and 11.27±0.94 of the failed group,respectively,and the differences between the two groups were statistically significant(t=2.148,2.776,P<0.05).The results of logistic regression analysis showed that low expression of DTF and DE,as well as high expression of E/e',were all influence factors for the failure of removing machine for patients adopted mechanical ventilation after underwent cardiac valve replacement.The ROC results showed that the best cut-off value of the prediction model was 0.0893,and the area under curve(AUC)values were 0.713(95%CI:0.646~0.758),0.710(95%CI:0.651~0.779),0.752(95%CI:0.657~0.805)and 0.886(95%CI:0.782~0.991).Conclusion:The combination of echocardiography and diaphragm ultrasound has better prediction for the outcome of removing machine,which high higher clinical application value.
10.Predictors for Failed Removal of Nasogastric Tube in Patients With Brain Insult

Objective:
To construct a prognostic model for unsuccessful removal of nasogastric tube (NGT) was the aim of our study.
Methods:
This study examined patients with swallowing disorders receiving NGT feeding due to stroke or traumatic brain injury in a regional hospital. Clinical data was collected, such as age, sex, body mass index (BMI), level of activities of daily living (ADLs) dependence. Additionally, gather information regarding the enhancement in Functional Oral Intake Scale (FOIS) levels and the increase in food types according to the International Dysphagia Diet Standardization Initiative (IDDSI) after one month of swallowing training. A stepwise logistic regression analysis model was employed to predict NGT removal failure using these parameters.
Results:
Out of 203 patients, 53 patients (26.1%) had experienced a failed removal of NGT after six months of follow-up. The strongest predictors for failed removal were age over 60 years, underweight BMI, total dependence in ADLs, and ischemic stroke. The admission prediction model categorized patients into high, moderate, and low-risk groups for removal failure. The failure rate of NGT removal was high not only in the high-risk group but also in the moderate-risk groups when there was no improvement in FOIS levels and IDDSI food types.
Conclusion
Our predictive model categorizes patients with brain insults into risk groups for swallowing disorders, enabling advanced interventions such as percutaneous endoscopic gastrostomy for high-risk patients struggling with NGT removal, while follow-up assessments using FOIS and IDDSI aid in guiding rehabilitation decisions for those at moderate risk.
