1.Amorphous calcium phosphate bladder stone: a case report

Amorphous calcium phosphate(ACP)is a component of urinary stones,primarily forming mixed stones with calcium oxalate,while pure ACP stones are relatively rare. This article reports a case of a patient with an ACP bladder stone who was admitted due to progressive dysuria over 5 years,which had worsened in the past months. Upon admission,tPSA was 29.63 ng/ml. CT and enhanced MRI revealed multiple bladder stones and prostatic hyperplasia. The patient underwent ultrasound-guided prostate biopsy and transurethral cystolithotripsy with pneumatic lithotripsy. Postoperative infrared spectroscopy confirmed the stone composition as ACP,and prostatic adenocarcinoma was diagnosed by prostate biopsy pathology. Endocrine therapy was administered postoperatively,and follow-up imaging at 3 months showed no stone. This article presents the first reported case of an ACP bladder stone coexisting with prostate cancer,providing important clinical insights into the etiology of such stones and the rare local manifestations of prostate cancer.

3.Research on the extraction process of standard decoction of Longan seed based on antioxidant activity

OBJECTIVE To optimize the extraction process of standard decoction of Longan seed.METHODS The amount of water added,extraction time and extraction times as the influencing factors,using the extract yield,gallic acid content,corilagin content,ellagic acid content,half clearance concentration(IC50)of 1,1-diphenyl-2-picryl-hydrazyl(DPPH)free radical and IC50 of 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid)ammonium salt(ABTS)free radical as the evaluation indexes,the analytic hierarchy process(AHP),criteria importarue though inter-criteria correlation(CRITIC)and AHP-CRITIC method were used to calculate the weight values of each index;the extraction process of Longan seed standard decoction was optimized by single factor test combined with orthogonal experiment;validation test was also performed.RESULTS According to the AHP-CRITIC method,the weights of the above indicators were determined to be 9.224%,11.784%,19.320%,11.206%,20.597%,27.869%,respectively.The best process of standard decoction of longan seed was to add 14 times water for the first extraction and extract for 30 minutes;and 12 times water for the second and third extractions,and extract for 20 minutes.Average comprehensive score of the 3 times validation experiments was 97.96 and the RSD was 0.97%(n=3).CONCLUSIONS The process is simple to operate,stable and feasible,which can provide a experimental basis for the further development and utilization of standard decoction of longan seed.
4.Whole-brain Mapping of Inputs and Outputs of Specific Orbitofrontal Cortical Neurons in Mice.

The orbitofrontal cortex (ORB), a region crucial for stimulus-reward association, decision-making, and flexible behaviors, extensively connects with other brain areas. However, brain-wide inputs to projection-defined ORB neurons and the distribution of inhibitory neurons postsynaptic to neurons in specific ORB subregions remain poorly characterized. Here we mapped the inputs of five types of projection-specific ORB neurons and ORB outputs to two types of inhibitory neurons. We found that different projection-defined ORB neurons received inputs from similar cortical and thalamic regions, albeit with quantitative variations, particularly in somatomotor areas and medial groups of the dorsal thalamus. By counting parvalbumin (PV) or somatostatin (SST) interneurons innervated by neurons in specific ORB subregions, we found a higher fraction of PV neurons in sensory cortices and a higher fraction of SST neurons in subcortical regions targeted by medial ORB neurons. These results provide insights into understanding and investigating the function of specific ORB neurons.

5.Integrated application of blockchain and artificial intelligence technology in the diagnosis and treatment of orthopedic trauma: a review

The incidence of orthopedic trauma-related diseases keeps rising annually, which brings an urgent need to optimize the diagnostic and treatment processes to enhance treatment efficiency and improve patients′ prognosis. Traditional diagnostic methods in traumatic orthopedics primarily rely on manual film interpretation and classification, resulting in a substantial workload for physicians and consequently a low efficiency. Furthermore, during multidisciplinary consultations and cross-hospital referrals for patients with orthopedic trauma, accessing medical records and facilitating information exchange can be challenging, leading to delays in surgical intervention. The rapid advancement of artificial intelligence (AI) technology, characterized by feature engineering, artificial neural networks, and deep learning, has transformed the landscape of rapid diagnosis and precision treatment for orthopedic trauma. Nonetheless, centralized storage during task training poses risks of privacy disclosure and security concerns that impede the widespread application of AI models. In contrast, the decentralized nature of blockchain technology offers a secure operational environment for AI-driven diagnostics and treatments and the integration of blockchain and AI can deliver more accurate, efficient, and safe services for patients with orthopedic trauma. Currently, challenges remain in the inter-institutional sharing of data, constant phenomenon of data silos and absence of standardized protocols for developing collaborative models in clinical settings. To address these challenges, the authors reviewed the research advancements in integrated application of blockchain technology and artificial intelligence in diagnosing and treating orthopedic trauma, aiming to provide insights into the development of a digital diagnostic system tailored to this field in China.
6.Comparison the efficacy and safety of Tip-flexible suction ureteral access sheath and ordinary ureteral access sheath combined with flexible ureteroscopy to treat upper urinary tract stones: a meta-analysis

Objective:To systematically evaluate of the efficacy and safety of Tip-flexible suction ureteral access sheath and ordinary ureteral access sheath combined with flexible ureteroscopy to treat upper urinary tract stones.Methods:The databases CNKI, Wanfang, VIP, CBM, PubMed, Cochrane, Embase, Web of Science, MEDLINE and Google Scholar were searched from their inception to May 31, 2024 for related studies about Tip-flexible suction ureteral access sheath compared with ordinary ureteral sheath combined with flexible ureteroscopy to treat upper urinary tract stones. Literature screening and data extraction were performed independently by two researchers, the quality assessment of randomized controlled trials was assessed using the Cochrane risk of bias assessment scale, the Newcastle-Ottawa risk of bias assessment scale was used to assess the quality of retrospective studies, and finally Meta-analysis of data was conducted using Review Manager 5.3 software.Results:A total of 14 literatures involving 1947 patients with upper urinary tract stones (1017 in the Tip-flexible suction sheath group and 930 in the ordinary sheath group) were included. Meta-analysis showed that the stone free rate of the Tip-flexible suction sheath group was higher than that of the ordinary sheath group (immediate postoperative stone free rate 80.7% vs. 60.0%, OR = 3.04, 95% CI 2.30-4.03, P<0.05; 1 month after surgery stone free rate 94.4% vs. 79.4%, OR=4.39, 95% CI 3.12-6.19, P<0.05). The total complication rate of the Tip-flexible suction sheath group was lower than that of the ordinary sheath group (5.4% vs. 17.3%, OR=0.29, 95% CI 0.21~0.40, P<0.05). There was no significant difference in the decrease of hemoglobin level between the Tip-flexible suction sheath group and the ordinary sheath group at 1 day after surgery( MD=-0.30, 95% CI -2.61-2.02, P>0.05). The postoperative fever in the Tip-flexible suction sheath group was less than that in the ordinary sheath group (1.9% vs. 7.5%, OR=0.28, 95% CI 0.15-0.54, P<0.05). There was no significant difference in operation time between the Tip-flexible suction sheath group and the ordinary sheath group( MD=-4.93, 95% CI -11.48-1.62, P>0.05). The postoperative hospital stay in the Tip-flexible suction sheath group was shorter than that in the ordinary sheath group ( MD=-0.20, 95% CI -0.25--0.16, P <0.05). Conclusions:Compared with ordinary ureteral access sheath combined with flexible ureteroscope to treat upper urinary tract stones, Tip-flexible suction ureteral access sheath has a higher stone free rate, less postoperative fever, low total complication rate, shorter postoperative hospital stay, which is conducive to postoperative recovery and is safer and more effective.
7.Apigenin alleviates renal cell injury and oxidative stress induced by high glucose by activating Keap1-Nrf2-ARE signaling pathway

Objective To investigate the protective effect of apigenin on renal tubular epithelial cells(HK-2)induced by hyperglycemia and its potential mechanism.Methods HK-2 cells were incubated with d-glucose to establish an in vitro diabetic nephropathy model.Cell viability,apoptosis and oxidative stress were assessed.The mRNA levels of nuclear factor erythroid-2 related factor 2(Nrf2),heme oxygenase-1(HO-1)and epoxy chloropropane Kelch sample related protein-1(Keap1)were measured by quantitative real-time-PCR(q-PCR).Western blot analysis was performed to measure the protein expression of Nrf2.Results In HK-2 cells,high glucose decreased cell viability in a concentration-and time-dependent manner.Apigenin improved the viability of HK-2 cells under high glucose stress,reduced apoptosis and proinflammatory cytokine production,and inhibited the oxidative stress.Apigenin increased mRNA expression of Nrf2 and HO-1,increased expression of Nrf2 protein and reduced the mRNA level of Keap1.Inhibition of Nrf2 using small interfering RNA(siRNA)abolished the protective effects of apigenin on HK-2 cells under high glucose stress.Conclusion Apigenin treatment can alleviate the damage of HK-2 cells under high glucose stress,and exert the anti-oxidation and anti-inflammation effects through Keap1-Nrf2-ARE pathway.
