1.Research progresses in association between abnormal uterine artery and adverse pregnancy events

Adverse pregnancy events is an important factor affecting maternal and infant health,and the research of relative mechanisms has deepened in recent years,with particular attention given to the role of uterine artery.Hemodynamic changes in the uterine artery can directly affect blood supply to uterus and placenta as well as fetal development,which are closely related to pregnancy outcomes.As a non-invasive prenatal assessment tool,ultrasound technology have unique advantages in monitoring and evaluating changes of uterine artery blood flow.The research progresses in association between abnormal uterine artery and adverse pregnancy events were reviewed in this article.
2.Analysis of the predictive value of Visually Accesable Rembrandt Images MRI feature model in high-grade glioma with IDH-1 mutations

Objective To explore the application value of constructing Visually Accesable Rembrandt Images(VASARI)MRI feature model for the prediction of isocitrate dehydrogenase-1(IDH-1)mutations in high-grade glioma(HGG).Methods A total of 242 patients with HGG were randomly divided into training set and validation set according to the ratio of 7︰3,the VASARI features of the patients were extracted to evaluate the statistical difference between IDH-1 mutant and wild-type VASARI features of the HGG.The least abso-lute shrinkage and selection operator(LASSO)regression method was used to reduce the dimension of VASARI features and the logistic regres-sion(LR)machine learning model was used to construct the prediction model for single VASARI feature and combined features.The receiver operating characteristic(ROC)curve was used to evaluate the predictive performance of the model.Results A total of 11 VASARI fea-tures were associated with IDH-1 mutations in HGG,with statistically significant differences(P<0.05).Seven closely related fea-tures were finally screened out,and the area under the curve(AUC)of the prediction model established after the combination of the seven VASARI features was higher,and the AUC of training set was 0.908,and the AUC of validation set was 0.872.Conclusion The VASARI MRI feature model can better predict IDH-1 mutations of HGG with high predictive efficacy and has greater application value.
3.Key mediating factors of ABO glycosyltransferases affecting human diseases: ABH blood group antigens and von Willebrand factor

Epidemiology research has found that ABO blood group and the gene coding ABO glycosyltransferases are associated with many human diseases. The activity of ABO glycosyltransferases varies with different blood types, mediating different glycosylation modifications. The variation in glycosylation level might be the risk factor of specific disease. Based on the literature retrieval and analysis, glycosylation levels regulated by ABO glycosyltransferases mainly affect the ABH blood group antigens and von Willebrand factor (vWF). By modulating key glycosylation components, ABO glycosyltransferases partly determine the activity or expression levels of the ABH antigens and vWF, thereby affecting the development and progression of diseases. Exploring the pathogenic mechanisms of ABO glycosyltransferases can improve the understanding of the molecular pathology of related diseases and provide reference for clinical research and application.
4.Surgical versus medical castration following radical prostatectomy in patients with very high-risk prostate cancer

Objective To compare the efficacy,economic burden,psychological impact,and quality of life between surgical and medical castration following radical prostatectomy(RP)in patients with very high-risk prostate cancer(VHR PCa).Methods Clinical data of 167 patients with VHR PCa who underwent RP in the Department of Urology,the First Affiliated Hospital of Anhui Medical University during Jul.2019 and Mar.2024 were retrospectively collected.Patients were divided into two groups:the surgical castration group(n=44)and medical castration group(n=123).The effects of different castration methods on the biochemical recurrence(BCR)were analyzed with Cox proportional hazards models.The survival curves of BCR-free and progress to castration-resistant prostate cancer(CRPC)were plotted with the Kaplan-Meier method.The differences in functional assessment of cancer therapy-prostate(FACT-P)and hospital anxiety and depression scale(HADS)between the two groups were evaluated with linear regression model.Results The total costs were significantly lower in the surgical castration group than in the medical castration group[(47 422.0±3 998.3)yuan vs.(59 017.2±8 014.1)yuan,P<0.001].One month postoperatively,the surgical castration group had significantly lower prostate-specific antigen(PSA)level[0.028(0.010,0.159)ng/mL vs.0.100(0.029,0.895)ng/mL,P=0.002].However,no significant differences were observed in the PSA level between the two groups at 3,6,and 12 months postoperatively,or in PSA nadir and time to nadir(P>0.05).Cox regression analysis suggested a potentially higher risk of BCR in the medical castration group(HR=2.23),but the difference was not statistically significant(P=0.112).The 1-and 3-year BCR-free survival rates were higher in the surgical castration group(90.9%vs.85.4%;86.4%vs.70.7%,respectively),whereas 1-and 3-year progression-free survival rates were comparable between the two groups(97.7%vs.97.6%;95.5%vs.91.9%),with no significant differences(P>0.05).No significant differences were found in FACT-P[(57.3±10.2)vs.(57.3±7.6)]or HADS[(12.6±5.1)vs.(11.3±4.8)]scores between the two groups(P>0.05).Conclusion In VHR PCa patients,surgical castration performed following RP is not inferior to drug castration in terms of PSA control,and potential delay of BCR.It had a lower cost and does not significantly increase the psychological burden.As an underutilized strategy,surgical castration can become an optional option for individualized treatment.
5.Clinical Efficacy of Modified Huangqi Chifengtang in Treatment of IgA Nephropathy Patients and Exploration of Dose-effect Relationship of Astragali Radix

ObjectiveTo explore the dose-effect relationship and safety of high, medium, and low doses of raw Astragali Radix in the modified Huangqi Chifengtang (MHCD) for treating proteinuria in immunoglobulin A (IgA) nephropathy, and to provide scientific evidence for the clinical use of high-dose Astragali Radix in the treatment of proteinuria in IgA nephropathy. MethodsA total of 120 patients with IgA nephropathy, diagnosed with Qi deficiency and blood stasis combined with wind pathogen and heat toxicity, were randomly divided into a control group and three treatment groups. The control group received telmisartan combined with a Chinese medicine placebo, while the treatment groups were given telmisartan combined with MHCD containing different doses of raw Astragali Radix (60, 30, 15 g). Each group contained 30 patients, and the treatment period was 12 weeks. Changes in 24-hour urinary protein (24 hUTP), traditional Chinese medicine (TCM) syndrome scores, effective rate, and renal function were observed before and after treatment. Safety was assessed by monitoring liver function and blood routine. ResultsAfter 12 weeks of treatment, 24 hUTP significantly decreased in the high, medium, and low-dose groups, as well as the control group (P<0.05, P<0.01). The TCM syndrome scores in the high, medium, and low-dose groups also significantly decreased (P<0.01). Comparisons between groups showed that the 24 hUTP in the high-dose group was significantly lower than in the medium, low-dose, and control groups (P<0.05, P<0.01), and the 24 hUTP in the medium-dose group was significantly lower than in the control group (P<0.05). The TCM syndrome scores in the high and medium-dose groups were significantly lower than in the low-dose and control groups (P<0.05, P<0.01). The total effective rates for proteinuria in the high, medium, low-dose, and control groups were 92.59% (25/27), 85.19% (23/27), 60.71% (17/28), and 57.14% (16/28), respectively. The effective rates in the high and medium-dose groups were significantly higher than in the low-dose and control groups (χ2=13.185, P<0.05, P<0.01). The effective rates for TCM syndrome scores in the high, medium, low-dose, and control groups were 88.89% (24/27), 81.48% (22/27), 71.43% (20/28), and 46.43% (13/28), respectively. The efficacy of TCM syndrome scores in the high and medium-dose groups was significantly higher than in the control group (χ2=14.053, P<0.01). Compared with pre-treatment values, there was no statistically significant difference in eGFR and serum creatinine in the high and medium-dose groups. However, eGFR significantly decreased in the low-dose and control groups after treatment (P<0.05), and serum creatinine levels increased significantly in the control group (P<0.05). No statistically significant differences were observed in urea nitrogen, uric acid, albumin, total cholesterol, triglycerides, liver function, and blood routine before and after treatment in any group. ConclusionThere is a dose-effect relationship in the treatment of IgA nephropathy with high, medium, and low doses of raw Astragali Radix in MHCD. The high-dose group exhibited the best therapeutic effect and good safety profile.

10.Application of Near-Infrared Autofluorescence Imaging Technology in Assisting Adrenal Gland Localization During Adrenal Tumor Resection:A Case Report and Literature Review

The adrenal gland is a crucial endocrine organ, and adrenal tumors, mostly benign, often require surgical treatment. However, intraoperative localization of the adrenal gland is challenging due to its anatomical features. Laparoscopic surgery is widely used, but current laparoscopes lack real - time adrenal navigation. Near-infrared autofluorescence imaging (NIRAF) is a non-invasive technology that shows potential in adrenal localization, with limited reports globally and none in China. A 37-year-old female with a left adrenal nodule underwent retroperitoneoscopic left adrenal tumor resection. NIRAF technology, combined with laparoscopy, was used. The equipment had white light and near-infrared light modes.In the near-infrared light mode, the adrenal gland's autofluorescence was detected for intraoperative identification. Intraoperative NIRAF laparoscopy showed high-intensity fluorescence of the left adrenal tissue, clearly differentiating it from surrounding tissues.The surgery was completed within one hour without damage to surrounding organs. Postoperative monitoring of indicators such as blood pressure, aldosterone, adrenocorticotropic hormone, and white blood cell count was carried out. NIRAF technology has advantages in adrenal tumor resection, like improving surgical efficiency, reducing tissue manipulation, and lowering bleeding and infection risks. However, it has limitations such as weak light penetration and limited research. Further studies are needed to clarify its autofluorescence mechanism and evaluate its long-term effects.
