1.80 kVp combined with artificial intelligence iterative reconstruction in lower extremity CT angiography of diabetic foot

Objective To observe the value of 80 kVp combined with artificial intelligence iterative reconstruction(AIIR)in lower extremity CT angiography(CTA)of diabetic foot.Methods Sixty patients with diabetic foot were prospectively enrolled and divided into experimental group and control group(each n=30).Lower extremity CTA was performed using 80 kVp combined with AIIR in experimental group,while using 120 kVp combined with hybrid iterative reconstruction(HIR)in control group.The subjective and objective evaluation results of imaging quality,as well as the radiation dose of scanning were compared between groups.Results Compared with control group,images in experimental group showed higher subjective scores(5[5,5]vs.4[4,4]),displayed more branches of lower limb arteries(both P<0.05),while the radiation dose decreased by 59.76% ([8.29±0.53]mSv vs.[20.60±2.42]mSv).CT values,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of each lower extremity artery segments were higher,while the noise of each lower extremity artery segments in experimental group were lower than those in control group(all P<0.05).Conclusion 80 kVp combined with AIIR could reduce radiation dose and improve imaging quality of lower extremity CTA of diabetic foot,hence being helpful to displaying small arteries of lower extremity,such as dorsalis pedis arteries and common plantar metatarsal arteries.
2.Analysis of expression and prognostic value of CD269 in patients with multiple myeloma

Objective:This study aimed to analyze the expression of B-cell maturation antigen (CD269) on myeloma cells in patients with newly diagnosed multiple myeloma (NDMM) and evaluate its prognostic value.Methods:The retrospective analysis was conducted on the clinical data of 154 NDMM patients admitted to the outpatient clinics and wards of Beijing Chaoyang Hospital, Capital Medical University from October 23, 2018 to December 25, 2023, including 90 males and 64 females, aged 60(55,66) years old. CD269 phenotype was analyzed using flow cytometry (FCM). Specifically, patients were divided into CD269 positive group (expression>20%, n=103) and CD269 negative group (expression≤20%, n=51) based on the flow cytometry prior to treatment. Initially, we assessed the effect of CD269 expression on progression-free survival (PFS) in patients with NDMM and compared the two groups. Through a cross-sectional analysis, we examined the basic clinical characteristics, 32 laboratory indicators, and 5 cytogenetic indicators. We identified differences between the two groups and analyzed the impact of various indicators on the PFS of patients in both the CD269 positive and negative groups. Furthermore, we employed the Cox proportional hazards regression model to evaluate the influence of CD269 expression and other disease-related indicators on patients′ PFS. Results:Among 154 NDMM patients, 103 were identified as CD269 positive (66.9%), while 51 were CD269 negative (33.1%). No statistically significant differences in clinical indicators were observed between the two groups. The PFS for CD269 positive group was significantly lower than it in the CD269 negative group [28.0(18.0,41.0) months vs 35.0 (27.0, -) months, HR=2.012, 95% CI 1.059-3.824, χ 2=4.554, P=0.033]. Cox proportional hazards regression analysis indicated that CD269 positivity and non-IgG subtypes were independent risk factors influencing PFS in NDMM patients( HR=2.395,95% CI 1.152-4.979, P=0.019; HR=0.425,95% CI 0.223-0.810, P=0.009). In the CD269 positive group, progression-free survival (PFS) was significantly shortened in patients with>65 years, non-IgG subtypes, International Staging System (ISS) stage Ⅲ, β2-microglobulin (β2-MG) levels≥5.5 mg/L, abnormal t(11;14) or ≥3 cytogenetic abnormalities. Conclusion:CD269 positivity serves as an independent risk factor influencing PFS in NDMM patients. Among the factors examined, older age, non-IgG subtype, ISS stage Ⅲ, elevated serum β2-MG, the presence of an abnormal t(11;14) translocation and≥3cytogenetic abnormalities, significantly impact the PFS of CD269 positive patients.

5.Comparison of different methods for HbA 1c measurement

Objectives:To compare the HbA 1c results on capillary electrophoresis (CE), cation exchange-high performance liquid chromatography (CE-HPLC), and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). Methods:902 normal samples without hemoglobin variants and 83 samples with hemoglobin variants were collected in 2020. The variants were divided into α-chain, β-chain, γ-chain, and δ-chain variants. The 902 samples without hemoglobin variants were measured by CE, CE-HPLC, and MALDI-TOF MS. Three methods were used for measuring α-chain, β-chain, and γ-chain compared with the liquid chromatography-tandem mass spectrometry (LC-MS/MS) reference method. According to the EP9-A3 guideline, Passing-Bablok regression and concordance correlation coefficient (CCC) were performed for correlation and consistency, respectively. The imprecision was assessed by coefficient of variance (CV). The mean relative bias was calculated and compared with the lowest biological variation bias of 2.3%.Results:All three methods met the acceptance of imprecision requirements (<2%). The R 2 and CCC for the normal samples were all above 0.95 between the pairwise comparison of CE, CE-HPLC, and MALDI-TOF MS. The mean relative bias between MALDI-TOF MS and CE-HPLC was higher than the lowest biological variation bias of 2.3%. The R 2 results between CE and LC-MS/MS results of α-chain, β-chain, γ-chain variants were>0.95, and the mean relative biases for γ-chain variants exceeded the lowest biological variation bias of 2.3%. A bad correlation ( R2=0.66) for β-chain variants was shown between CE-HPLC and LC-MS/MS and the mean relative biases of all variant samples exceeded 2.3%. MALDI-TOF MS showed good correlations with LC-MS/MS for three groups, and the mean relative biases for γ-chain variants were higher than 2.3%. Conclusion:MALDI-TOF MS and CE-HPLC with CE had good comparability in the measurement of HbA 1c in normal samples, butthe three methods showed interferences from different types of Hb variants and the CE method was affected with less interference.
6.Predictive study of pancreatic fistula after pancreaticoduodenectomy based on CT radiomics nomogram

Objective To investigate the value of radiomics nomogram model based on enhanced CT for predicting postoperative pancreatic fistula(POPF)in patients undergoing pancreaticoduodenectomy(PD).Methods The clinical and abdominal imaging data of 82 patients who underwent PD were analyzed retrospectively.They were divided into training set and validation set at a ratio of 3∶2.In the venous phase of CT,the maximum slice of the pancreatic duct was selected to delineate the region of interest(ROI),and the features were extracted and screened to construct the radiomics model.The independent risk factors associated with POPF were screened by univariate and multivariable logistic regression analyses to construct the clinical model.A nomogram model was created by combining Radiomics score(Radscore)and clinical risk factors.The diagnostic performance of the model was verified by the vali-dation set.Results The nomogram model demonstrated significant predictive power,with an area under the curve(AUC)of 0.862 and 0.806 in the training set and validation set,respectively.Conclusion The nomogram model exhibits excellent predictive performance and outperforms the clinical model and radiomics model in predicting POPF,which can provide important guidance for clinical deci-sion-making.

Objective Based on R language and other data mining visualization software,to visually display the clinical case set of Professor Zheng Hongxin in the treatment of depression-related insomnia,analyze its core prescription and medication characteristics,in order to promote the clinical application of such diseases.Methods During 2020.4.28-2024.1.24,Professor Zheng Hongxin treating patients in the Affiliated Hospital and the 4th Affiliated Hospital of Liaoning TCM University,we collected 237 effective cases of"depression-related insomnia".We input patient diagnosis and treatment information,establishing Excel database,in order to analyz the informaton and summarize the core prescription and Professor Zheng Hongxin's medication characteristics by RStudio(4.3.3),IBM SPSS Modeler18.0,Cytoscape,Origin.2021,Gephi and so on statistical and visualization software.Results Through the data mining of Professor Zheng Hongxin's medical record database,it is concluded that professor Zheng medication core association rules from"Treatise on Exogenous Febrile Disease"Chaihu Longgu Muli Decoction from the perspective of coordinating Shaoyang's activities of qi and"Valuable Prescriptions for Emergency"Kaixinsan from the perspective of inducing resuscitation and relieve uneasiness of mind and body.And the drug changes of"depression-related insomnia"can be divided into 10 methods invigorating spleen qi,regulating qi and stomach,tonifying liver and kidney,detoxification and eliminating carbuncle,making the stomach creep normally,inhibiting the production of gastric acid and pain relief,damping down the fire of heart and eliminate annoyance,calming heart down,nourishing blood and reducing sweat,nourishing the mind to calm the nerves.Conclusion The core prescription of Professor Zheng's treatment of depression-related insomnia is the changes of Chaihu Longgu Muli Decoction and Kaixinsan,and the therapeutic characteristic is to emphasize the coordination of the spleen and stomach,qi and blood and the viscera.

9.Evaluation of measurement uncertainty for HbA1c by four approaches in clinical laboratory

Objective:To compare results of four glycosylated hemoglobin A1c (HbA1c) detection methods and to evaluate the uncertainty of HbA1C results in clinical laboratory, and to provide method for clinical laboratory on the evaluation of uncertainty.Methods:According to the four uncertainty evaluation methods, which were recommended by "CNAS-TRL-001, the evaluation and expression of measurement uncertainty in medical laboratory", the relative and absolute uncertainty of low, medium and high HbA1c in 33 clinical laboratories measured in 2019 and 35 clinical laboratories measured in 2020 was evaluated by more than 6 months of internal quality control (IQC) data, trueness verification and external quality assessment (EQA) data. The four uncertainty evaluation methods were: IQC data and trueness verification data (method 1), only trueness verification data (method 2), IQC and EQA data (method 3) and only EQA data (method 4). The related statistical methods used in this analysis were Friedman and Wilcoxon signed rank test.Results:For method 1, the median range of relative and absolute uncertainty of low, medium and high HbA1c detection in 2019 and 2020 ranged from 4.21% to 9.24% and from 0.27% to 0.64%, respectively. Compared to method 1, the relative and absolute uncertainties obtained by method 2 were smaller, and the differences were statistically significant ( P<0.016 7, P<0.05). Compared to method 1, the relative uncertainties obtained by method 3 and method 4 were smaller, except for the high concentration of HbA1c level in 2020. Among the 6 pairs of comparisons (low, medium and high HbA1c in 2019 and 2020), there were 3 pairs (high HbA1c in 2019, low and medium HbA1c in 2020) and 2 pairs (low and high HbA1c in 2020) of differences with statistical significance (all P<0.016 7). Conclusion:The uncertainty evaluation of HbA1c detection in clinical laboratory should be evaluated based on IQC and trueness verification data.
