1.Effect of Bushen Huoxue Prescription in Regulating PINK1/Parkin Pathway in Rat Model of Premature Ovarian Failure

ObjectiveTo investigate the mechanism of action of Bushen Huoxue prescription (BSHXP) in regulating premature ovarian failure in rats through the PTEN-induced kinase 1 (PINK1)/Parkinson's protein (Parkin) signaling pathway-mediated mitophagy. MethodsA total of 48 rats were randomly divided into a blank group consisting of eight rats, while the remaining 40 rats underwent modeling. The modeling group was intraperitoneally injected with 4 mg·kg-1 cisplatin solution, followed by a second injection one week later, for a total of two injections. The estrous cycle was observed through vaginal smears for 14 consecutive days to determine whether the modeling was successful. The successfully modeled rats were randomly divided into a model group, groups receiving low, medium, and high doses of BSHXP at 9.72, 19.44, and 38.88 g·kg-1·d-1 (BSHXP-L, BSHXP-M, and BSHXP-H groups), and a positive control group treated with estradiol valerate (0.09 mg·kg-1·d-1), for 21 consecutive days. The body weight of the rats was measured weekly. After the final administration, rats were anesthetized, and their blood and ovaries were collected. The ovarian weight was measured. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum levels of anti-Müllerian hormone (AMH), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and estradiol (E2). Assay kits were used to measure the levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in the rat serum. Hematoxylin-eosin (HE) staining was used to observe the morphological changes in the ovaries. Immunohistochemistry (IHC) was performed to detect microtubule autophagy-related protein 1 light chain 3B(LC3B) protein expression in ovarian tissue, and electron microscopy was employed to examine the mitochondrial and autophagosome changes in the rat ovaries. Western blot was used to detect the protein expression of PINK1, Parkin, LC3B, and p62. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expression of PINK1, Parkin, LC3B, and p62 in ovarian tissue. ResultsCompared with the blank group, the model group showed significant reductions in body weight, weight gain, and ovarian weight (P<0.01), along with decreased serum AMH and E2 levels (P<0.01), while FSH and LH levels were increased (P<0.01). Serum MDA levels were significantly increased (P<0.01), and SOD levels were significantly reduced (P<0.01). The ovarian tissue structure was disordered, and the zona pellucida was wrinkled into an irregular acidophilic annular object, accompanied by an increased number of closed follicles. Electron microscopy showed mitochondrial swelling, unclear structure, and no obvious autophagosomes and autolysosome structures. The proteins and mRNA expression levels of PINK1, Parkin, LC3B, and p62 in the ovarian tissue were significantly reduced (P<0.01). Compared with the model group, all treatment groups showed varying degrees of increases in body weight and ovarian weight (P<0.05, P<0.01). Except for the BSHXP-L group, all treatment groups showed increased body weight gain (P<0.01). All treatment groups showed significantly increased serum AMH and decreased FSH levels (P<0.01). Except for the BSHXP group, all treatment groups showed varying degrees of increase and decrease in serum E2 and LH levels (P<0.05, P<0.01). All treatment groups showed reduced serum MDA levels (P<0.01), while the BSHXP-M, BSHXP-H, and the positive control groups demonstrated improved serum SOD levels (P<0.05, P<0.01). All treatment groups showed an increased number of follicles at all stages, visible mature follicles, and a decreased number of closed follicles. Electron microscopy showed relieved mitochondrial swelling, morphology close to normal, clear structure, and visible formation of autolysosomes in all treatment groups. Additionally, the protein and mRNA expression levels of PINK1, Parkin, LC3B, and p62 in ovarian tissue were significantly increased (P<0.05, P<0.01). ConclusionBSHXP may improve ovarian function in rats with premature ovarian failure by regulating the PINK1/Parkin signaling pathway, activating mitochondrial autophagy, and reducing oxidative damage.

3.Association of physical activity levels and sarcopenia and frailty in older adults in rural communities: a cross-sectional study

Objective:To assess the physical activity levels(PA)and patterns among older adults in rural areas, as well as to evaluate the association between PA levels and the conditions of sarcopenia and frailty.Methods:A cross-sectional survey was conducted involving 690 rural individuals aged 60 and above.Data on socio-demographic characteristics were collected, while Appendicular Skeletal Muscle Mass Index(ASMI)and grip strength were measured.The Physical Activity Scale for the Elderly(PASE)was employed to evaluate PA levels.Based on their PASE scores, participants were categorized into three groups: low PA level, medium PA level, and high PA level.Sarcopenia was defined according to the 2019 criteria established by the Asian Working Group for Sarcopenia(AWGS), and frailty status was assessed using the Frail Scale.Results:Among the study sample, 38.0% exhibited a low level of physical activity, 46.2% had a medium level, and 15.8% engaged in high levels of activity.As age increased, the level of physical activity among older adults significantly declined.Most of the physical activity reported by participants was attributed to household chores and farming-related activities.Participation in structured exercise among older adults was notably low, with only 1.3% engaging in muscle-strengthening exercises on a weekly basis.The majority(53.9%)reported walking as their preferred form of weekly exercise.After adjusting for confounding factors, the prevalence of sarcopenia was found to be 0.40(95% CI: 0.26-0.62)times lower in the medium physical activity group, and 0.56(95% CI: 0.31-1.01)times lower in the high physical activity group, compared to those with a low physical activity level.Similarly, regarding frailty as a negative outcome, the prevalence was 0.66(95% CI: 0.51-0.84)times lower in the medium physical activity group and 0.46(95% CI: 0.30-0.73)times lower in the high physical activity group, relative to the low physical activity group.When using PASE scores as a continuous variable, the results remained consistent. Conclusions:The physical activity levels of rural older adults are inadequate, and participation in multicomponent exercise programs is notably low.A lower level of physical activity is significantly associated with a higher prevalence of sarcopenia and frailty.Our findings indicate that it is essential to implement physical activity education and interventions to enhance exercise health literacy and to prevent sarcopenia and frailty among rural older adults.
4.Analysis of lipid metabolism gene mutations and pathogenicity in patients with hypertriglyceridemia-associated acute pancreatitis

Objective:To investigate lipid metabolism gene mutations and pathogenicity of hypertriglyceridemia acute pancreatitis (HTG-AP) patients.Methods:Clinical data of 495 HTG-AP patients admitted from June 2018 to June 2020 in the center for severe acute pancreatitis of Eastern Theater General Hospital were retrospectively analyzed. Whole-exome sequencing and mutation verification were performed by next-generation sequencing technology and Sanger sequencing. The pathogenicity of gene mutation was analyzed by population mutation ratio, pathogenicity prediction software, conservation scoring software, protein structure prediction, and in vitro experiments. Results:The mutation ratio of lipid metabolism-related genes, namely LPL, APOA5, LMF1, GPIHBP1, and APOC2, were 14.81%, 55.78%, 43.61%, 1.62%, and 0.61%, respectively. Among them, 44 heterozygous mutations in LPL gene were detected including 36 missense mutations, 5 nonsense mutations and 3 frameshift mutations, which were all rarely carried in single patient. Six HTG-AP patients carried the LPL gene heterozygous mutation c.835C>G (p.Leu279Val). The mean level of serum triglyceride at the onset of HTG-AP was 27.4 mmol/L. All of them had a history of recurrent HTG-AP, and most of them had severe acute pancreatitis. The serum LPL concentration and activity were lower than the normal level. The pathogenicity analysis results suggested that the LPL p.Leu279Val was a rare, highly possible pathogenic and highly conserved gene mutation. The in vitro results showed that the LPL p.Leu279Val could significantly reduce the synthesis and secretion ability of LPL as well as its enzymatic activity. Conclusions:The mutation ratio of lipid metabolism-related genes, including LPL, APOA5, LMF1, GPIHBP1, and APOC2, are relatively high in the HTG-AP patients. The LPL p.Leu279Val is a rare and highly possible pathogenic gene mutation, which may lead to recurrent episodes of HTG-AP.

7.Clinical study on treatment of Diabetic foot with negative pressure of wound surface combined with periosteal distraction

Objective To explore the clinical study of the treatment of Diabetic foot with wound negative pressure treatment(NPWT)combined with periosteal distraction.Methods 129 patients with Diabetic foot admitted to our hospital from June 2022 to April 2023 were randomly divided into a combination group(n=43),a negative pressure group(n=43),and a control group(n=43).The control group was treated with conventional treatment,the negative pressure group was treated with NPWT,and the combined group was combined with periosteal distraction technique on the basis of the negative pressure group.The treatment efficiency,inflammatory index level,peripheral blood picture level,visual pain(VAS)score,and foot related index level of the three groups were compared.Results After 4 weeks of treatment,the scores of pain scale(VAS),white blood cell count(WBC),Calcitonin(PCT),Interleukin 6(IL-6),and high-sensitivity C-reactive protein(hs CRP)in the combined group were lower than those in the other two groups(P<0.05);The effective rate,foot temperature,toe blood oxygen,Ankle-brachial pressure index,vascular diameter,blood flow velocity,and perfusion index(PI)of the combined group were higher than those of the other two groups(P<0.05).Conclusion The combination of wound negative pressure therapy and periosteal distraction surgery can effectively alleviate pain,improve foot blood supply,and alleviate inflammatory reactions.
8.Expression of alcohol dehydrogenase 1 A and vascular endothelial growth factor-A in hepatocellular carcinoma

Objective To investigate the expression,synergistic relationship and clinical significance of alcohol de-hydrogenase(ADH1A)and vascular endothelial growth factor-A(VEGFA)in hepatocellular carcinoma(HCC).Methods The expression and correlation of ADH1A and VEGFA in HCC and adjacent normal tissues were ana-lyzed by GEPIA.TCGA and GSEA were used to analyze the pathway of ADH1A in HCC.The clinical and patho-logical data of 84 patients with HCC were collected,and 54 patients with paracancer normal tissue samples were se-lected as controls to analyze the correlation between ADH1A and VEGFA and clinicopathological parameters of HCC.Immunohistochemistry was used to detect the protein expression of ADH1A and VEGFA in cases and con-trols,and the correlation between the expression of ADH1A and VEGFA and the clinical progression and prognosis of patients with HCC was analyzed based on clinical pathological parameters and Kaplan-Meier.Results Bioinfor-matics analysis found that ADH1A was low-expressed in HCC and VEGFA was highly expressed in HCC,and there was a negative correlation between the two(P<0.001);immunohistochemical detection results showed that the expression of ADH1A in HCC tissue was lower than that in normal tissue adjacent to cancer(P<0.01)while the expression rate of VEGFA in HCC tissue was significantly higher than that of normal tissue adjacent to cancer(P<0.01);The recurrence rate of vascular thrombus and HCC patients in HCC group with high expression of ADH1A was lower(P<0.05).The proportion of tumor diameter>5 cm,high TNM stage,microsatellite and G2-G3 dif-ferentiation in HCC tissues in VEGFA high expression group was higher(P<0.05).Kaplan-Meier survival analy-sis showed that patients with high ADH1A expression and low VEGFA expression had a higher five-year survival rate.Conclusion Low expression of ADH1A and high expression of VEGFA in tumor tissues of patients with HCC indicate tumor progression and can be used as one of the prognostic evaluation indicators for patients with HCC.
9.Clinical efficacy of umbilical moxibustion combined with stone needle acupoint massage for elderly patients with Qi deficiency constipation

Objective:To explore the clinical efficacy, recurrence rate, and quality of life of elderly patients with Qi deficiency constipation treated with umbilical moxibustion combined with stone needle acupoint massage.Methods:Convenience sampling method was used to select 160 elderly patients with qi deficiency constipation who were hospitalised in the Department of Respiratory Medicine of Dongfang Hospital, Beijing University of Chinese Medicine, Xiyuan Hospital of China Academy of Chinese Medical Sciences, and Guang'anmen Hospital, China Academy of Chinese Medical Sciences from June 2021 to January 2023 as the study subjects. A total of 160 elderly patients with Qi deficiency constipation were randomly divided into four groups by using the method of randomisation of district groups, the control group ( n=40) received routine health education and nursing, the massage group ( n=40) was treated with needle stone acupoint massage, the umbilical moxibustion group ( n=40) received umbilical moxibustion with moxa and ginger, and the massage and umbilical moxibustion group ( n=40) was treated with needle stone acupoint massage combined with umbilical moxibustion. All patients were treated continuously for two weeks. This study compared the clinical efficacy, constipation symptom score, traditional Chinese medicine syndrome score, score of Patient Assessment of Constipation-Quality of Life (PAC-QOL) and the recurrence rate after three months of treatment among four groups of patients. Results:After treatment, compared with the other three groups, the massage and umbilical moxibustion group had the highest total clinical effective rate, with the lowest scores for constipation symptoms, traditional Chinese medicine syndromes, and PAC-QOL, with statistical differences ( P<0.05). There was a statistically significant difference in the recurrence rate among the four groups ( P<0.05) . Conclusions:Needle stone acupoint massage and umbilical moxibustion can both improve the clinical symptoms of elderly patients with Qi deficiency constipation, and combination of the two has a better effect, reducing recurrence rate, and improving quality of life.
