1.Molecular diversity and evolutionary mechanisms of bacterial nucleic acid sensing systems: a focus on the CBASS innate immune system

The bacterial CBASS(cyclic-oligonucleotide-based anti-phage signaling system) represents a novel innate immune defense mechanism mediated by cyclic nucleotides.The system employs cGAS/DncV-like nucleotidyltransferases (CD-NTases) to recognize exogenous nucleic acids, catalyzing the production of second messenger molecules such as cyclic GMP-AMP (cGAMP) to activate effector proteins (e.g., nucleases), thereby triggering "cell suicide" to combat phage infection. The molecular mechanisms and evolutionary features of CBASS not only uncover the diversity of bacterial immune defenses but also provide critical insights into the conservation of prokaryotic-eukaryotic immune pathways. Notably, the bacterial CBASS system exhibits profound homology with the eukaryotic cGAS-STING (Cyclic GMP-AMP Synthase—Stimulator of Interferon Genes) pathway across multiple dimensions, including the catalytic domains of CD-NTase/cGAS and cyclic nucleotide signaling mechanisms. This finding supports the hypothesis that eukaryotic innate immunity may have originated from prokaryotic horizontal gene transfer and highlights an evolutionary shift in defense strategies from bacterial "population lysis" to "individual inflammatory activation" in higher organisms.This review systematically synthesizes the functional architecture and mechanisms of the CBASS system, with a deep exploration of its evolutionary connections to the eukaryotic cGAS-STING pathway. The insights gained herein offer fresh perspectives for understanding the origin and evolution of the innate immune system, and for driving advancements in biotechnological research and development.
2.Research progress on the mechanism of low-density lipoprotein transendothelial transport

The transendothelial transport of low-density lipoprotein(LDL)and its subendothelial accumulation represent a central pathogenic mechanism in atherogenesis.This process involves a variety of receptors and molecular mechanisms that work together to regulate the transport of LDL in endothelial cells and its accumulation in blood vessel wall.In addition to receptor-mediated transport mechanisms,physiological and pathological factors such as hormones,inflammatory mediators and metabolites also play an important role in it.Recent studies have further elucidated the critical regulatory roles of these factors in LDL transendothelial transport,suggesting that targeting LDL transcytosis may represent an effective strategy for the prevention of early atherosclerosis and the treatment of related vascular diseases.This review summarizes recent advances in this field and discusses their potential clinical applications.
3.Latent profile analysis and influencing factors of intrinsic ability among elderly patients with ischemic stroke

Objective:To explore the potential categories of internal ability of elderly patients with ischemic stroke, identify the influencing factors and propose corresponding nursing interventions.Methods:This was a cross-sectional study. From January 2025 to May 2025, the elderly patients with ischemic stroke in Tianjin Huanhu Hospital were selected by convenient sampling method. The general information questionnaire, the Internal Capacity Assessment Scale for the Older People, the Health Literacy Scale for stroke patients and the Perceived Social Support Scale were used to investigate. To determine latent categories of patients' intrinsic capacity, potential profile analysis was employed. Subsequently, multivariable Logistic regression examined factors associated with these categories.Results:A total of 260 survey questionnaires were distributed, and 256 valid questionnaires were finally collected, with an effective response rate of 98.46% (256/260). Among of them, there were 166 males and 90 females, aged 68.00 (63.00, 74.00) years.The intrinsic ability of elderly patients with ischemic stroke could be divided into three potential categories: low sensation-low exercise group (27.0%, 69/256), relatively stable intrinsic ability group (37.5%, 96/256) and low cognition-low psychology group (35.5%, 91/256). Multivariable Logistic regression showed that advanced age (compared to the low sensation-low exercise group, OR=0.902; compared to the relatively stable intrinsic ability group, OR=0.813), smoking (compared to the low sensation-low exercise group, OR=0.459; compared to the relatively stable intrinsic ability group, OR=0.442), the lower the Barthel index (compared to the low sensation-low exercise group, ≤40 points with OR=0.157; 41-60 points with OR=0.285) were more likely to enter the low cognition-low psychology group (all P<0.05); other chronic disease types ≤1 (compared to the low cognition-low psychology group, OR=2.630), higher health literacy scores (compared to the low cognition-low psychology group, OR=1.033) were more likely to enter the relatively stable intrinsic ability group (both P<0.05); and stroke frequency was the first occurrence (compared to the low cognition-low psychology group, OR=2.725) was more likely to enter the low sensation-low exercise group ( P<0.05). Conclusions:In older adults with ischemic stroke, the characteristics of intrinsic ability are clearly categorized. To enhance patient outcomes, healthcare professionals are advised to tailor nursing interventions based on the unique features and specific influencing factors associated with each potential category.

6.The Effects of Reminazolam and Midazolam on Postoperative Delirium in Patients with Congenital Heart Disease

Objective The purpose of this study is to explore the effects of reminazolam and midazolam on the occurrence of deliri-um in postoperative patients with congenital heart disease.Methods A total of 131 postoperative patients with congenital heart disease were admitted to our hospital from March to August 2023,they were randomly divided into a reminazolam group(n=66)and a midazolam group(n=65).The Cornell Delirium Assessment Scale for Children was used to evaluate postoperative delirium.We analyzed and com-pared the incidence,occurrence time,and duration of the two groups.We also recorded the patient's adverse drug reactions,mechanical ventilation time,cardiac intensive care unit(CICU)stay time,and hospitalization time.Results The incidence of delirium was 51.7%in the midazolam group and 40.0%in the reminazolam group,with no statistically significant difference.However,there were significant differences in the timing of delirium occurrence between the two groups(x2=12.400,P=0.030).The duration of delirium in the remi-nazolam group was significantly shortened(x2=14.773,P=0.034).There was no statistically significant difference between the two groups in mechanical ventilation and CICU retention time.There was a difference in hospitalization time between the two groups(z=0.096,P=0.016).There were 2 cases of adverse reactions(3.08%)in the midazolam group and 5 cases(7.58%)in the reminazolam group,with no statistically significant difference.Conclusion As a benzodiazepine drug,reminazolam has a certain protective effect on postoperative delirium in patients undergoing congenital heart surgery.However,further research is needed on the application of reminazo-lam in children,including drug dosage and adverse reactions.
7.Preliminary research on clinical application of a new flexible patch ECG

Objective To explore the diagnostic value of a wearable flexible patch ECG instrument in arrhythmia,the alarm situation during clinical application,patient satisfaction and safety.Methods A total of 1 443 subjects wore flexible patch ECG and conventional dynamic ECG(control)for 24 h to test the validity and consistency of arrhythmia diagnosis,and counted the alarm of remote ECG and the occurrence of related adverse events during the wearing of the instrument.Results There were 987 cases of arrhythmia detected by flexible patch ECG and 992 cases by conventional dynamic ECG.The total coincidence rate of arrhythmia diagnosis was 98.7%.The mean heart rate was measured by flexible patch ECG(75.4±11.4)times/min,conventional dynamic heart rate(71.5±12.1)times/min,the intra-group correlation coefficient(ICC)of 2 instruments was 0.892(95%CI:0.537-0.956),with good repeatability.The correct alarm rate of flexible patch ECG was 100%.The incidences of skin pruritus(0.28%vs.1.32%),skin allergy,redness and swelling(0.14%vs.0.69%)and electrode strip shedding(0 vs.0.28%)during wearing the flexible patch electrocardiogram were lower than those of the conventional holter electrocardiogram(P<0.05).Conclusion The flexible patch ECG has few adverse reactions,high comfort,good safety and clinical applicability.
8.Progress in the treatment of chronic spontaneous urticaria: insights from the changes in the understanding of its pathogenesis

Mast cells, autoantibodies, inflammatory cells, coagulation cascade, complement system and nervous system are all involved in the complex pathogenesis of chronic spontaneous urticaria (CSU) , while mast cells play a pivotal role in it. With deeper understanding of the pathogenesis of CSU, cutting-edge therapeutic methods are gradually being used in clinical practice. Nowadays, pharmacotherapyeutic studies are more focused on accurately modulating the pathological state of mast cells. This review summarizes recent advances in the pathogenesis of and medicines for CSU.
9.Research on the performance evaluation index system for Party committee secretaries of public hospitals

Objective:Under the guidance of " Opinions on Strengthening Party Construction in Public Hospitals" enacted by the Central Committee of the Communist Party of China, the performance evaluation index system for Party-committee secretaries has been released to reflect the overall leadership responsibility of Party committees at public hospitals.Methods:By means of literature review, policy learning and the preliminary work performance of Shanghai Hospital Development Center(SHDC), the framework of the target system was constructed. Three rounds of Delphi method were conducted to score 138 indicators based on eight aspects: result, objectivity, statistics, sensitivity, accessibility, quantifiability, importance and score-oriented. The final indicators were selected and statistically weighted. SPSS 20.0 software was used for descriptive statistical analysis, and Cronbach α evaluation coefficient method and Pearson correlation coefficient were also used to analyze the reliability and validity of indicators. Results:Following three rounds of Delphi research, 18 experts finally constructed a performance evaluation index system for Party committee secretaries that is composed of 4 level-1, 13 level-2 and 25 level-3 indexes. The new index system was used to simulate the scores of 14 hospitals, which was positively correlated with the survey results of third-party patient satisfaction( r=0.897, P<0.01), and was better than the original index system. Conclusions:The index system constructed in this study proves better quantitative objectivity and stronger operability. It has a good reliability and validity among the hospitals that belongs to SSHDC, and could be used for the evaluation of performance of Party-committee secretaries at public hospitals.

Objective To promote the publicizing and implementation of “The technical specifications of hyperbaric oxygen clinical application”.Methods Special investigations and analyses were made in the medical facilities in shanghai which were conducting HBO therapy .Results Most medical facilities are still lacking in appropriate management systems .The HBO clinical application involves multi-disciplines, the technical level of professionals needs further improved .Conclusions Greater efforts should be made to publicize “The technical specifications of hyperbaric oxygen clinical application ” , perfect related management system and enhance training of HBO medical professionals .
