To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.

Objective:To conduct a study on pricing by service unit to address the problems of hospice and palliative care pricing and fee system in China. Methods:Combining theoretical research and empirical evidence, this study organized the pricing mechanism of initiative hospice and palliative care services and established a graded and categorized pricing strategy.Empirical research was conducted based on real-world data from 36 pilot institutions in typical areas. Results:This study developed a comprehensive pricing framework for value-based classification price standard of initiative hospice and palliative care services from the perspective of incentive regulation. We proposed a pricing plan based on service units, with inpatient bed fee ranging from 459 to 606 yuan or 459 to 1 102 yuan, and home visit fee ranging from 89 to 264 yuan. Conclusions and suggestions:This study proposes a pricing scheme based on the technique and service value with a gradient fluctuation by service unit, and forms a set of price standards with high economic and technical feasibility, which can provide scientific evidences for solving the pricing problem of hospice care.In addition, there is still a need to establish a multi-level incentive compensation mechanism to motivate all levels and types of organisations and healthcare provider, and to promote the high-quality and sustainable development of hospice and palliative care.

This study was aimed at establishing a sensitive and specific sandwich ELISA detection method for Brucella. We screened monoclonal capture antibodies and detection antibodies for Brucella detection, and optimized and determined the opti-mal antibody coating time and concentration,as well as the optimal blocking solution, blocking time, and yin-yang critical val-ue.The specificity of this method was verified by examination of other bacteria prone to cross-reacting with Brucella. The sen-sitivity of the method was verified by detection of a gradient dilution of inactivated Brucella. Moreover, the sandwich ELISA detection results were compared with test tube agglutination and qPCR results. The selected capture antibody was 4A12,and the selected detection antibody was 6C12.Experimental analysis indicated that the optimal coating concentration for the 4A12 capture antibody was 5 μg/mL,and the optimal dilution ratio for the 6C12 detection antibody was 1∶2000.The optimal coating conditions were overnight at 4℃,and blocking with 5%skim milk powder for 2 hours. The established double antibody sand-wich ELISA method reacted with only Brucella but not other bacteria, thus demonstrating the method's good specificity.Inac-tivated Brucella solution was still detectable after dilution to 1 × 105 CFU/mL,thus demonstrating the method's good sensitiv-ity.The intra-and inter batch coefficients of variation were both below 10%,thus indicating the method's good repeatability. Thus, this study successfully established a dual antibody sandwich ELISA method for Brucella detection, which has good spe-cificity and sensitivity, and might provide an effective approach for the precise diagnosis and effective prevention and control of brucellosis.

Objective:Invasive pulmonary mucormycosis(PM)is a rare but highly lethal opportunistic infection.COVID-19 associated mucormycosis(CAM)is difficult to diagnose, often leading to misdiagnosis or missed diagnosis, and has poor treatment outcomes. This study reports a case of successfully treated CAM and explores optimized diagnostic and therapeutic strategies. Methods:A retrospective analysis of the diagnosis and treatment process in a 50-year-old female patient with COVID-19 associated with diabetic ketoacidosis(DKA)and invasive pulmonary mucormycosis was conducted.Combined with a literature review, the therapeutic efficacy of local bronchoscopic instillation in conjunction with systemic treatment using liposomal Amphotericin B(L-AmB)was specifically evaluated. Results:The patient was rapidly diagnosed with Rhizopus microsporus infection through metagenomic next-generation sequencing(mNGS).She subsequently received antifungal treatment with intravenous L-AmB combined with local bronchoscopic instillation. After treatment, the patient was significantly improved, with imaging studies showing gradual absorption of the lesions.Follow-up at six months revealed no recurrence.A literature review suggests that early diagnosis and multimodal therapy are key to improving survival rates in patients with CAM.Conclusion:mNGS can significantly improve the early diagnosis rate of CAM.The combination of local and systemic treatment with L-AmB is valuable in improving prognosis.Early diagnosis, multimodal antifungal therapy, and individualized management are key to increasing the survival rate of patients with CAM.
