1.Characterization of non-alcoholic fatty liver disease–related hepatocellular carcinoma on contrast-enhanced ultrasound with Sonazoid

Purpose:
This study aimed to evaluate the contrast-enhanced ultrasound with Sonazoid (Sonazoid-CEUS) features of hepatocellular carcinoma (HCC) in patients with non-alcoholic fatty liver disease (NAFLD).
Methods:
In this retrospective study, patients who underwent surgical resection and were histopathologically diagnosed with NAFLD or cirrhosis-related HCC were included. All patients received Sonazoid-CEUS examinations within 1 week prior to hepatic surgery. The enhancement patterns of HCC lesions were evaluated and compared between the two groups according to the current World Federation for Ultrasound in Medicine and Biology guidelines. Multivariate logistic regression analysis was used to assess the correlations between Sonazoid-CEUS enhancement patterns and clinicopathologic characteristics.
Results:
From March 2022 to April 2023, a total of 151 patients with HCC were included, comprising 72 with NAFLD-related HCC and 79 with hepatitis B virus (HBV) cirrhosis–related HCC. On Sonazoid-CEUS, more than half of the NAFLD-related HCCs exhibited relatively early and mild washout within 60 seconds (54.2%, 39/72), whereas most HBV cirrhosis–related HCCs displayed washout between 60 and 120 seconds (46.8%, 37/79) or after 120 seconds (39.2%, 31/79) (P<0.001). In the patients with NAFLD-related HCC, multivariate analysis revealed that international normalized ratio (odds ratio [OR], 0.002; 95% confidence interval [CI], 0.000 to 0.899; P=0.046) and poor tumor differentiation (OR, 21.930; 95% CI, 1.960 to 245.319; P=0.012) were significantly associated with washout occurring within 60 seconds.
Conclusion
Characteristic Sonazoid-CEUS features are useful for diagnosing HCC in patients with NAFLD.

3.Applicability of modified comprehensive interventions in the treatment of non-severe dry eye syndrome in military pilots

Objective:To explore the applicability of modified comprehensive interventions in the treatment of non-severe dry eye syndrome in military pilots.Methods:A total of 88 military pilots with non-severe dry eye syndrome admitted to the Special Service Department of the 988th Hospital of the Joint Logistic Support Force between December 2021 and December 2023 were divided into an intervention group and a control group using the random number table method, with 44 cases in each. The intervention group received modified comprehensive interventions, while the control group underwent conventional treatment. The Ocular Surface Disease Index (OSDI), break-up time, tear meniscus height, changes in meibomian gland function, and levels of satisfaction of military pilots were compared between the 2 groups. The correlations between the OSDI, break-up time, tear meniscus height and levels of satisfaction were analyzed.Results:Before treatment, there was no significant difference in the OSDI between the 2 groups ( P>0.05). After 4 weeks of treatment, the changes in the OSDI of military pilots were smaller in the intervention group than in the control group ( t=3.21, P=0.002). After 2 and 4 weeks of treatment, the break-up time (both P<0.001) and tear meniscus height ( P<0.001, =0.012) of pilots in the intervention group exceeded those of the control group. In both groups, the break-up time (all P<0.001) and tear meniscus height (all P<0.001) kept increasing after treatment. After 4 weeks of treatment, there were significant differences in the distribution of meibomian gland function between the 2 groups ( Z=-2.55, -2.41, -2.29, P=0.011, 0.016, 0.022). Clinical care, procedure flow, and health education scored higher in the intervention group than in the control group during the survey on levels of satisfaction with the treatment ( t=6.55, 6.77, 3.63, all P≤0.001). The OSDI was negatively correlated with clinical care, procedure flow and health education ( r=-0.286, -0.275, -0.363, P=0.007, 0.010, 0.001) while the break-up time was positively correlated with clinical care and procedure flow ( r=0.248, 0.278, P=0.020, 0.009). Conclusions:The implementation of modified comprehensive intervention measures for dry eye syndrome in military pilots can effectively improve clinical symptoms and leave military pilots more satisfied.

ObjectiveTo investigate the diagnostic accuracy and grading capability of ultrasound-derived fat fraction (UDFF), controlled attenuation parameter (CAP), and hepatic/renal ratio (HRR) in assessing hepatic steatosis in metabolic associated fatty liver disease (MAFLD) with magnetic resonance imaging-proton density fat fraction (MRI-PDFF) as the gold standard. MethodsA total of 150 patients with MAFLD who attended The First Hospital of Hebei Medical University from January 2023 to December 2024 were enrolled, and 148 healthy volunteers were recruited. All subjects underwent MRI-PDFF, UDFF, CAP, and HRR examinations. Hepatic steatosis was graded based on MRI-PDFF (S0:148 cases; S1:92 cases; S2:21 cases; S3:37 cases), and the MAFLD patients with different grades of hepatic steatosis were compared in terms of UDFF, CAP, HRR, and clinical features. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups and the Tukey HSD test was used for further comparision between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Mann-Whitney U test was used for further comparison between two groups; the chi-square test was used for comparison of categorical data between groups. The Spearman correlation analysis was used to investigate the correlation between UDFF, CAP, HRR, and MRI-PDFF in different grades of MAFLD; the receiver operating characteristic (ROC) curve was used to investigate the efficacy of UDFF, CAP, and HRR in the diagnosis of different degrees of hepatic steatosis in MAFLD; the Bland-Altman difference plot was used to analyze the consistency between UDFF and MRI-PDFF in different degrees of hepatic steatosis in MAFLD. ResultsUDFF measurement gradually increased with the increase in the grade of fatty liver (H=201.52,P0.001). The Spearman correlation analysis showed that there was a strong correlation between any two indicators of UDFF, CAP, HRR, and MRI-PDFF in S1, S2, and S3 MAFLD (all P0.001), with the strongest correlation between UDFF and MRI-PDFF (rs1=0.884,rs2=0.962,rs3=0.929, all P0.001). The ROC curve analysis showed that UDFF had a larger area under the ROC curve (AUC) than CAP and HRR in the graded diagnosis of S1 and S3 (all P0.05), while in the diagnosis of S2 MAFLD, UDFF had a significantly larger AUC than HRR (P0.05) and a similar AUC to CAP (P0.05). The Bland-Altman difference plot showed good consistency between UDFF and MRI-PDFF in different degrees of hepatic steatosis in MAFLD. ConclusionCompared with CAP and HRR, UDFF can accurately measure liver fat content and has good efficacy in identifying varying degrees of hepatic steatosis in MAFLD.
6.Level of TyG index in breast cancer and its value in predicting breast cancer

Objective:To explore the level of triglyceride-glucose (TyG) index and its effect and clinical value in predicting breast cancer.Methods:A retrospective case-control study was conducted. A total of 700 patients with breast tumors who underwent surgical treatment in the Inner Mongolia Autonomous Region People's Hospital from July 2021 to June 2024 were selected, and finally 625 patients were included, including 388 patients with benign breast tumors and 237 patients with breast cancer, and all lesions were determined to be benign or malignant by pathology after surgery. Triglyceride and blood glucose data were collected to calculate the TyG index. The correlation between TyG index and occurrence of breast cancer was analyzed by univariate and multivariate binary logistic regression models. With postoperative pathological diagnosis as the gold standard, the receiver operating characteristic (ROC) curve was used to evaluate the efficacy of TyG index in distinguishing breast cancer from benign breast tumors.Results:The age of patients in the breast cancer group and the benign breast tumor group was (57±12) years old and (42±12) years old, respectively. There were statistically significant differences in the distributions of patients between the two groups in terms of nationality, marital status, height, body mass, body mass index (BMI), cholesterol, low-density lipoprotein cholesterol, triglyceride, blood glucose levels, and color ultrasound Breast Imaging Report and Data System (BI-RADS) classification (all P < 0.05), and there was no statistically significant difference in the level of high-density lipoprotein cholesterol between the two groups ( P > 0.05); the TyG index in the breast cancer group was higher than that in the benign breast tumor group (8.6±0.6 vs. 8.4±0.6), and the difference was statistically significant ( P <0.001). Logistic regression analysis showed that in the model with only the TyG index as a variable, the risk of breast cancer increased with the increase of TyG index [ OR = 1.97, 95% CI: 1.48-2.63, P < 0.001, Hosmer-Lemeshow (HL) test P = 0.077]; in the model with 6 variables including TyG index, nationality, marriage, height, body mass and BMI, the risk of breast cancer increased with the increase of TyG index ( OR = 1.47, 95% CI: 1.08-2.00, P = 0.015, HL test P = 0.832); however, after adding age and BI-RADS classification variables, although the TyG index was still an independent influencing factor for the occurrence of breast cancer ( OR = 0.58, 95% CI: 0.35-0.96, P = 0.033, HL test P = 0.165), the rise of TyG index was an independent protective factor for the occurrence of breast cancer; in the two variable models including TyG index and BI-RADS classification, the TyG index was not an independent influencing factor for the occurrence of breast cancer ( OR = 1.10, 95% CI: 0.72-1.70, P = 0.659), suggesting that the TyG index may not be able to assist in the diagnosis of breast cancer by ultrasound. ROC curve analysis showed that the area under the curve to distinguish breast cancer and benign breast tumor according to the TyG index was 0.623, the optimal critical value of the TyG index was 8.51, the corresponding sensitivity was 59.5%, and the specificity was 62.4%. Conclusions:The TyG index of breast cancer patients is higher than that of benign breast tumor patients. Breast tumor patients with elevated TyG index have an increased risk of breast cancer, which may be a potential non-invasive biomarker of breast cancer.
7.Progress of insulin-like growth factor receptor 2 in the cardiac adverse reactions of breast cancer induced by anthracycline

Breast cancer is the most common type of cancer among women. Although chemotherapy has extended the survival time of patients with breast cancer, the cardiovascular complications caused by chemotherapy drugs are on the rise, which limits the clinical use of anthracycline drugs. At present, it is believed that the mechanisms of cardiac adverse reactions induced by anthracycline drugs may be related with oxidative stress, topoisomerase inhibitor 2, abnormal expression of non-coded RNA in cells, induction of autophagy and apoptosis in myocardial cells, and other various cell death pathways. This article reviews the relationship between insulin-like growth factor receptor 2 and cardiac adverse reactions induced by anthracycline drugs.

OBJECTIVE:
Peritoneal fibrosis (PF) is an adverse event that occurs during long-term peritoneal dialysis, significantly impairing treatment efficiency and adversely affecting patient outcomes. Astragaloside IV (AS-IV), a principal active component derived from Astragalus membranaceus (Fisch.) Bunge, has exhibited anti-inflammatory and antifibrotic effects in various settings. This study aims to investigate the potential therapeutic efficacy and mechanism of AS-IV in the treatment of PF.
METHODS:
The PF mouse model was established by intraperitoneal injection of 4.25% peritoneal dialysis fluid (100 mL/kg). The epithelial-mesenchymal transition (EMT) of HMrSV5 cells was induced by the addition of 10 ng/mL transforming growth factor β (TGF-β). The differentially expressed genes in HMrSV5 cells treated with AS-IV were screened using transcriptome sequencing analysis. The potential targets of AS-IV were screened using network pharmacology and analyzed using molecular docking and molecular dynamics simulations.
RESULTS:
Administration of AS-IV at doses of 20, 40, or 80 mg/kg effectively mitigated the increase in peritoneal thickness and the development of fibrosis in mice with PF. The expression of the fibrosis marker α-smooth muscle actin in the peritoneum was significantly decreased in AS-IV-treated mice. The treatment of AS-IV (10, 20, and 40 μmol/L) significantly delayed the EMT of HMrSV5 cells induced by TGF-β, as demonstrated by the decreased number of 5-ethynyl-2'-deoxyuridine-positive cells, reduced migrated area, and decreased expression of fibrosis markers. A total of 460 differentially expressed genes were detected in AS-IV-treated HMrSV5 cells through transcriptome sequencing, with notable enrichment in the phosphatidylinositol-4,5-bisphosphate 3-kinase (PI3K)-AKT serine/threonine kinase 1 (AKT) signaling pathway. The reduced levels of phosphorylated PI3K (p-PI3K) and p-AKT were detected in HMrSV5 cells with AS-IV treatment. Epidermal growth factor receptor (EGFR) was predicted as a direct target of AS-IV, exhibiting strong hydrogen bond interactions. The activation of the PI3K-AKT pathway by the compound 740Y-P, and the activation of the EGFR pathway by NSC 228155 each partially counteracted the inhibitory effect of AS-IV on the EMT of HMrSV5 cells.
CONCLUSION
AS-IV delayed the EMT process in peritoneal mesothelial cells and slowed the progression of PF, potentially serving as a therapeutic agent for the early prevention and treatment of PF.
