To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.

OBJECTIVE To provide guidance for pharmaceutical care in patients with severe Cryptococcus gattii infections.METHODS Clinical pharmacists participated in the diagnosis and treatment of a male patient with disseminated Cryptococcus gattii infection (involving the lungs and central nervous system). Given the patient’s poor response to initial therapy, the clinical pharmacists, based on antimicrobial susceptibility test results, recommended voriconazole as an alternative azole. In response to refractory hypokalemia caused by liposomal amphotericin B, the clinical pharmacists assisted the physician in developing an intensive potassium replacement regimen. Regarding the interaction between voriconazole and nifedipine, the clinical pharmacists proactively identified the risk and intensified monitoring of the patient’s blood pressure. In response to the onset of hepatitis C during treatment, the clinical pharmacists thoroughly weighed the priority of infection treatment and recommended temporarily postponing antiviral therapy for hepatitis C. For the individualized use of voriconazole, the clinical pharmacists repeatedly performed therapeutic drug monitoring (TDM). Regarding the patient’s poor lesion resolution, the clinical pharmacists participated in multidisciplinary team discussions to clarify the indications for surgery.RESULTS The physician adopted the clinical pharmacists’ recommendations. Following pharmacological treatment and surgical intervention, the patient’s condition stabilized, and he was discharged while still on medication.CONCLUSIONS For severe Cryptococcus gattii infections, clinical pharmacists should focus on pathogen characteristics, strengthen adverse reaction management, pay close attention to drug interactions, conduct TDM, and actively participate in multidisciplinary team collaboration to ensure the safety and efficacy of patient treatment.

To guide clinical blood transfusion practices for pediatric patients, the National Health Commission has issued the health standard "Guideline for pediatric transfusion" (WS/T 795-2022). Critically ill children often present with anemia and have a higher demand for transfusions compared to other pediatric patients. This guideline provides guidance and recommendations for blood transfusions in cases of general critical illness, septic shock, acute brain injury, extracorporeal membrane oxygenation, non-life-threatening bleeding, and hemorrhagic shock. This article interprets the background and evidence of the blood transfusion provisions for critically ill and severely bleeding children in the "Guideline for pediatric transfusion", aiming to enhance understanding and implementation of this aspect of the guidelines. To guide clinical blood transfusion practices in pediatric patients, the National Health Commission has issued the health standard "Guideline for pediatric transfusion" (WS/T 795-2022). Children undergoing cardiac surgery are at high risk of bleeding, and the causes of perioperative anemia and coagulation disorders in neonates and children are complex and varied, often necessitating the transfusion of allogeneic blood components. This guideline provides direction and recommendations for specific measures in blood management for children undergoing cardiac surgery before, during, and after surgery. This article interprets the background and evidence for the formulation of the blood transfusion provisions for children undergoing cardiac surgery, hoping to facilitate the understanding and implementation of this guideline.

Objective To construct the performance evaluation index system of specialist cooperation alliance in hos-pital-community in order to scientifically evaluate the effect of the construction of the specialist cooperation alliance. Methods By searching literature related to the construction of specialized league, sort out national and local policy documents, and the"structure-process-result"model was adopted to initially construct the index system. The ex-pert letter consultation form of the indicator system was formed through the discussion of the special group, two rounds of Delphi expert consultation were carried out to determine the evaluation indicator system, and the weight coefficient of each indicator was determined by the simultaneous application of analytic hierarchy process. Results The effective recovery rates of the two rounds of questionnaire were 84.62%and 100.00%,respectively.The positive de-gree of experts was high, and the average authority degree of experts was 0.864.The importance,operability,sen-sitivity Kendall's W coefficient and x2 test value of all indexes were P<0.05.Finally,an evaluation index system covering 9 first-level indicators and 25 second-level indicators was established. Conclusion The constructed perfor-mance evaluation index is scientific and reliable. The practical application shows that it can be used as a tool to evalu-ate the effect of integrated, continuous and collaborative health service delivery in hospital-community.

Tumor 2025;45(1):12-21
Objective:To explore the effect of C1q like protein 4(C1ql4)on migration and invasion of breast cancer and its mechanism.Methods:Real-time fluorescence quantitative PCR was used to detect the expression level of C1ql4 mRNA in tumor tissues and paracancerous tissues from 50 cases of breast cancer,as well as the breast cells including T549 and MCF-7 breast cancer cells, and MCF-10A normal breast cells. The siRNA targeting C1ql4 gene(siC1ql4)was synthesized and transfected into BT549 breast cancer cells by liposome transfection, and the silencing efficiency was detected by Western blotting.MTT assay was used to detect the proliferative ability of the cells in each group, the invasive ability of the cells was detected by Transwell assay, and the migratory ability of the cells was detected by scratch healing assay.Western blotting was used to detect the proliferative ability of the cells in each group. The expression levels of Snail,Slug, nuclear factor-κB(NF-κB)and phospho-NF-κB(p-NF-κB)were detected. The immunofluorescence was used to localize the distribution of NF-κB.Real-time fluorescence PCR was used to detect the mRNA expression levels of the NF-κB target genes TNF-α and IL-1β.Results:The expression level of C1ql4 mRNA in breast cancer tissues was significantly higher than that in paracancerous tissues, and the expression level of C1ql4 mRNA in BT549 and MCF-7 breast cancer cells was significantly higher than that in MCF-10A normal breast cells(P<0.05).After silencing C1ql4 expression, the proliferation, invasion and migration abilities of BT549 cells were significantly decreased(all P<0.001);the mRNA and prorein expression levels of Snail and Slug were significantly down-regulated(both P<0.001),and the mRNA expression levels of TNFα and IL-1β were significantly reduced(both P<0.001).The amount of NF-κB entering the nucleus was reduced, and the ratio of p-NF-κB to NF-κB was reduced(both P<0.05).Conclusion:The C1ql4 gene may regulate migration and invasion of breast cancer cells and promote breast cancer progression through the NF-κB pathway.
