Mesenchymal stem cells(MSCs)play a crucial role in tissue repair and regeneration, and the extracellular vesicle(EV)released by them holds great promise for applications in clinical biomarkers, vaccines, and drug delivery.However,MSCs-derived EV(MSCs-EV)face challenges such as low pro-duction yield, poor retention, and targeted delivery issues.There-fore,engineering MSCs-EV to enhance their performance and en-able visual research has become a hot topic.Curcumin(CUR),an active component in traditional chinese medicine, exhibits pharmacological effects but has limited bioavailability.Using MSCs-EV as a carrier for CUR delivery can address its solubility and bioavailability challenges. This article reviews the drug loading methods, engineering strategies of MSCs-EV,and their important applications in the delivery and treatment of CUR for cartilage injury diseases. It provides a basis for the clinical ap-plication of engineered MSCs-EV in CUR delivery for cartilage repair, offering potential solutions to the challenges in cartilage tissue repair.

Aim To investigate the effect of total fla-vones of Coreopsis tinctoria Nutt(CF)on cognitive im-pairment in vascular dementia(VD).Methods The VD rat models were established by modified bilateral common carotid arteries ligation method.SD rats were divided into the sham operation group, model group, positive control group(nicergoline),and low, medium, and high dose CF groups. After eight weeks of admin-istration,the short term memory and spatial learning and memory abilities were evaluated by the platform jumping test, dark avoidance test and Morris water maze test. The pathological changes of the hippocam-pal tissues were inspected by HE and Nissl staining. The contents of TNF-α and IL-1β in the hippocampal were examined by ELISA.The protein expression lev-els of TLR4,MyD88,NF-κB p65,and p-NF-κB p65 in the hippocampal were detected by Western blot. The mRNA expression levels of miR-93,TLR4,MyD88,and NF-κB p65 in the hippocampal were determined by qRT-PCR.Results CF obviously improved the short term memory and spatial learning and memory abilities of VD rats, and alleviated the pathological damage of the hippocampus.CF also obviously decreased the lev-els of TNF-α and IL-1β,declined the protein expres-sion levels of TLR4,MyD88,and p-NF-κB p65,and re-duced the miR-93,TLR4,and MyD88 mRNA expres-sion in the hippocampus. Conclusion CF has a nota-ble protective effect on the neuroinflammation and cog-nitive impairments in VD rats by inhibiting the miR-93-mediated TLR4 signaling pathway.

Chinese-foreign joint education program of clinical medicine is an important means to achieve the globalization of medical education. Chongqing Medical University and University of Leicester in the UK have jointly established a Chinese-foreign joint education program of clinical medicine to achieve the integration of Chinese and British cultivation concepts, management systems, teaching resources, teacher teams, evaluation systems, and multiculturalism. They have also constructed an internal teaching quality assurance system with the main contents of the improvement of management mechanisms, the formulation of training programs, the construction of teaching staff, the design of syllabuses, the curriculum assessment system, and teaching quality evaluation, as well as an external teaching quality assurance system with the core components of clinical medicine accreditation, Chinese-foreign joint education program evaluations, international quality audits, and professional quality monitoring. Both systems can help to comprehensively improve teaching quality.

Objective To investigate the impacts of epidural anesthesia with ropivacaine combined with dezocine on lower limb motor nerve block and maternal and infant outcomes in primipara undergoing painless delivery. Methods A total of 159 primiparas who delivered in Nanjing Jiangbei Hospital were selected as the research objects, and divided into the blank group(53 cases),the ropivacaine group(53 cases)and the combined group(53 cases)by the random number table method.Parturients in the blank group were given natural delivery mode, parturients in the ropivacaine group were given ropivacaine epidural anesthesia, and parturients in the combined group were given dezocine anesthesia on the basis of ropivacaine.Analgesic effect at different time points, time of the first, second and third stage of labor, pressing times of analgesic pump, lower limbs motor nerve block, maternal and infant outcomes, and adverse reactions of parturients were compared among the three groups. Results At 10 minutes after analgesia,60 minutes after analgesia, when the cervix was fully dilated and when the fetus was delivered, the VAS scores of the parturients in the ropivacaine group and the combined group were lower than those in the blank group(P<0.05),and the VAS scores of the parturients in the combined group were significantly lower than those in the ropivacaine group(P<0.05).There was no significant difference in the time of the first, second or third stage of labor of parturients among the three groups(P>0.05);The pressing times of analgesic pump of parturients in the combined group was significantly less than that in the ropivacaine group(P<0.05).There was no statistically significant difference in terms of low limb motor nerve block after painless labor of parturients among the three groups(P>0.05).There were no statistically significant differences in the perineal incision rate or the Apgar scores of newborns at 1 minute and 5 minutes after birth among the three groups(P>0.05).The usage rate of forceps and the rate of conversion to cesarean section in the combined group were significantly lower than those in the ropivacaine group and the blank group(P<0.05).There was no statistically significant difference in the incidence of total adverse reactions among the blank group, the ropivacaine group and the combined group(P>0.05).Conclusion The combination of ropivacaine and dezocine for epidural anesthesia has a better analgesic effect on primiparas with painless delivery, has a smaller impact on lower limb motor nerve block in parturients, and can achieve better maternal and infant outcomes.
