1.Early lactate/albumin ratio combined with quick sequential organ failure assessment for predicting the prognosis of sepsis caused by community-acquired pneumonia in the emergency department.

OBJECTIVE:
To investigate the predictive value of early lactate/albumin ratio (LAR) combined with quick sequential organ failure assessment (qSOFA) for the 28-day prognosis of patients with sepsis caused by emergency community-acquired pneumonia (CAP).
METHODS:
The clinical data of patients with sepsis caused by CAP admitted to the department of emergency of Beijing Haidian Hospital from June 2021 to August 2022 were retrospectively analyzed, including gender, age, comorbidities, lactic acid (Lac), serum albumin (Alb), LAR, procalcitonin (PCT) within 1 hour, and 28-day prognosis. Patients were divided into two groups based on 28-day prognosis, and risk factors affecting patients' prognosis were analyzed using univariate and multivariate Cox regression methods. Patients were divided into two groups according to the best cut-off value of LAR, and Kaplan-Meier survival curves were used to analyze the 28-day cumulative survival of patients in each group. Time-dependent receiver operator characteristic curve (ROC curve) were plotted to analyze the predictive value of sequential organ failure assessment (SOFA), acute physiology and chronic health evaluation II (APACHE II), and qSOFA+LAR score on the prognosis of patients with sepsis caused by CAP at 28 days. The area under the curve (AUC) was calculated and compared.
RESULTS:
A total of 116 patients with sepsis caused by CAP were included, of whom 80 survived at 28 days and 36 died, 28-day mortality of 31.0%. There were no statistically significant differences in age, gender, comorbidities, pH, platelet count, and fibrinogen between the survival and death groups, and there were significantly differences in blood urea nitrogen (BUN), white blood cell count (WBC), hemoglobin, Lac, Alb, PCT, D-dimer, LAR, as well as qSOFA score, SOFA score, and APACHE II score. Univariate Cox regression analyses showed that BUN, WBC, pH, Lac, Alb, PCT, LAR, qSOFA score, SOFA score, and APACHE II score were associated with mortality outcome. Multifactorial Cox regression analysis of the above variables showed that BUN, WBC, PCT, and APACHE II score were independent risk factors for 28-day death in the emergency department in patients with sepsis caused by CAP [hazard ratio (HR) were 1.081, 0.892, 1.034, and 1.135, respectively, all P < 0.05]. The best cut-off value of early LAR for predicting the 28-day prognosis of sepsis patients was 0.088, the Kaplan-Meier survival curve showed that the 28-day cumulative survival rate of sepsis patients in the LAR ≤ 0.088 group was significantly higher than that in the LAR > 0.088 group [82.9% (63/76) vs. 42.5% (17/40), Log-Rank test: χ2 = 22.51, P < 0.001]. The qSOFA+LAR score was calculated based on the LAR cut-off value and qSOFA score, and ROC curve analysis showed that the AUCs of SOFA score, APACHE II score, and qSOFA+LAR score for predicting 28-day death of patients with sepsis caued by CAP were 0.741, 0.774, and 0.709, respectively, with the AUC of qSOFA+LAR score slightly lower than those of SOFA score and APACHE II score, but there were no significantly differences. When the best cut-off value of qSOFA+LAR score was 1, the sensitivity was 63.9% and the specificity was 80.0%.
CONCLUSION
The qSOFA+LAR score has predictive value for the 28-day prognosis of patients with sepsis caused by CAP in the emergency department, its predictive value is comparable to the SOFA score and the APACHE II score, and it is more convenient for early use in the emergency department.

2.Knockout SQLE in melanoma cells potentiates anti-tumor immunity via improving CD8+T cell infiltration in tumor microenvironment

Objective To investigate the role of squalene epoxidase(SQLE)knockout in anti-tumor effect vial im-proving CD8+T cell infiltration in melanoma tumor microenvironment.Methods Both immunodeficient and immu-nocompetent mice were inoculated with SQLE knockout B16F10 cells to determine the cell-autonomous and non-au-tonomous regulation of malignancy.Antibody blockade,Luminex multiplex assays,and flow cytometry were em-ployed to explore the impact of SQLE gene knockout on the secretion of cytokines/chemokines and immune cell in-filtration.Bioinformatics analysis was conducted to validate the correlation between SQLE expression and immune infiltration as well as clinical prognosis in melanoma patients.Results Compared with immunodeficient mice,SQLE knockout significantly inhibited melanoma proliferation in immunocompetent mice and prolonged their surviv-al.SQLE knockout induced the secretion of cytokines and chemokines from tumor cells,improved CD8+T cell in-filtration in the tumor microenvironment,thereby potentiating anti-tumor immunity.Bioinformatics analysis sugges-ted a significant correlation between SQLE and its corresponding immune infiltration markers with the prognosis of melanoma patients.Conclusion SQLE regulates anti-tumor immunity by controlling cytokines and chemokines re-leasing in tumor microenvironment,thus holding promise as a novel tumor immunotherapy target and efficacy predic-tion molecular indicator.
3.Analysis on correlation between systemic inflammatory indicators with severity of disease condition and cognitive dysfunction in patients with schizophrenia

Objective To analyze the value of systemic inflammatory indicators in diagnosing schizo-phrenia and their correlation with disease condition severity and cognitive dysfunction.Methods A total of 47 patients with schizophrenia visited and treated in this hospital from October 2021 to March 2023 were selected as the study subjects,and 47 healthy volunteers during the same period served as the control group.The morn-ing fasting venous blood was collected in all research subjects,and the blood routine and biochemical indicators were measured.The SII,SIRI,NHR and LHR were calculated.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of SII,SIRI,NHR and LHR in the patients with schizophrenia.The correlation adopted the Pearson correlation analysis.Results The levels of SII,SIRI,NHR and LHR in the schizophrenia group were significantly increased compared with the control group,and the difference was statistically significant(P<0.05).The ROC curve analysis demonstrated that the area under the curve(AUC)of SII,SIRI,NHR and LHR detection alone in diagnosing schizophrenia were 0.683,0.686,0.774 and 0.766 respectively.AUC of their combination detection was 0.886.SII was positively correlated with the gen-eral symptoms and the positive and negative symptom scale(PANSS)total score,and negatively correlated with the spatial structure,linguistic function and delayed memory(P<0.05);SIRI was positively correlated with the negative symptoms,general symptom and PANSS total score,and negatively correlated with the im-mediate memory,spatial structure,linguistic function,attention;NHR was positively correlated with the nega-tive symptoms,positive symptoms,general symptoms and PANSS total score,and negatively correlated with immediate memory,linguistic function,attention and delayed memory(P<0.05).LHR was positively corre-lated with the positive symptoms,general symptoms and PANSS total score,and negatively correlated with the spatial structure,linguistic function,attention and delayed memory(P<0.05).Conclusion SII,SIRI,NHR and LHR combined detection has higher diagnostic value for the patients with schizophrenia.Further-more,the SII,SIRI,NHR and LHR levels are closely correlated with the disease condition severity and cogni-tive function impairment degree.
4. Application of 4+ 1 photography in determining the end point of double eyelid surgery

Objective:
To explore the practicability and reproducibility of judgment method and assessment indexes for the end point of double eyelid surgery using 4+ 1 photography in supine position.
Methods:
From 2017 October to 2018 October, 158 patients were included and randomly divided into 2 groups. Photos were taken by 4 + 1 photography in supine position of 79 patients, while other 79 patients in control group were evaluated by traditional observation. By 4+ 1 photography in supine position, the surgeon stood at the head side of the patient, taking photos with eyes movement: looking straight forward, looking up, looking downward and eye-closed. It was to observe the upper eyelid creases, upper and lower tissues of double eyelid creases, and upper and lower eyelid margo palpebrae. In addition, the surgeon looked from patient′s feet to observe the indexes such as upper margo palpebrae, to make a decision whether the surgery could finish. In control group, the surgeon observed the upper eyelid creases, upper and lower tissues of double eyelid creases and upper and lower eyelid margo palpebrae. The patient has to sit if necessary.
Results:
All 158 patients were performed double eyelid surgery successfully. Average times of valuating end point was 1.20 by 4+ 1 Photography in supine position, and 1.53 in control group. The operation time of 4+ 1 photography group is (151.65±21.58) s, and control group were (241.53±33.53) s. The satisfaction level was increased to 89.87% in 4+ 1 photography group, compared to 78.48% in control group.
Conclusions
The 4+ 1 Photography method is simple and easy to practice. The observation indexes are accurate and comprehensive. It is useful in determining the end point of double eyelid plasty in supine position.
