1.Development and effectiveness of double-set endotracheal intubation

Objective:To develop a double-set endotracheal intubation and evaluate its effectiveness.Methods:The development of a double-set endotracheal intubation was achieved by integrating endotracheal intubation with dental pads. From November 2023 to July 2024, convenience sampling was used to select 60 patients with endotracheal intubation admitted to the ICU of the First Hospital of Jiaxing as participants. Patients were divided into an experimental group ( n=30) and a control group ( n=30) using a random number table method. Experimental group was treated with a double-set endotracheal intubation, while control group was treated with a regular endotracheal intubation. The incidence of oral mucosal membrane pressure injury and oral ulcers during endotracheal intubation, the Chinese version of the modified Beck Oral Assessment score, plaque index, and endotracheal intubation duration were compared between two groups of ICU patients. Results:The duration of endotracheal intubation in experimental group was shorter than that in control group ( P<0.05), and the difference was statistically significant ( P<0.05). The number of cases with oral mucosal membrane pressure injury and oral ulcers in experimental group, as well as the Chinese version of the modified Beck Oral Assessment score, were lower than those in control group, and the differences were statistically significant ( P<0.05). There was no statistically significant difference in the plaque index between two groups ( P>0.05) . Conclusions:Double-set endotracheal intubation can effectively improve the efficiency of intubation in ICU patients, reduce the local pressure on oral tissues caused by intubation, improve the oral condition of patients, and have good clinical effects.

3.Inhibition of subicular seizure-labeled c-fos+ neurons alleviates cognitive deficit in epilepsy

OBJECTIVE Cognitive deficit is a com-mon comorbidity in temporal lobe epilepsy(TLE)and that is not well controlled by current therapeutics.Currently,how epileptic seizure affects cognitive performance remains largely unclear.The subiculum is the major out-put of the hippocampus,which projects to entorhinal cor-tex and other more distinct brain regions.Physiologically,the subiculum codes spatial working memory and naviga-tion information including place,speed,and trajectory.Importantly,prior studies have noted the importance of the subiculum in the beginning,spreading,and generaliz-ing process of hippocampal seizure.How seizure-activated neurons in subiculum participate in cognitive impairment remains largely elusive.METHODS In this study,we sought to label the subicular seizure-activated c-fos+ neu-rons with a special promoter with enhanced synaptic activity-responsive element E-SARE in the subiculum,combined with chemogenetics and designer receptors exclusively activated by designer drugs(DREADDs),Ca2+ fiber photometry approaches,and behavioral tasks,to reveal the role of these neurons in cognitive impairment in epilepsy.RESULTS We found that chemogenetic inhibi-tion of subicular seizure-tagged c-fos+ neurons(mainly CaMK Ⅱ α+ glutamatergic neurons)alleviates seizure generalization and improves cognitive performance in the hippocampal CA3 kindling TLE model.While inhibition of seizure-labeled c-fos+ GABAergic interneuron shows no effect on seizure and cognition.As a comparison,che-mogenetic inhibition of the whole subicular CaMK Ⅱ α+ neuron impairs cognitive function in na?ve mice in basal condition.Notably,inhibition of subicular seizure-tagged c-fos+ neurons enhances the recruitment of cognition-responsive c-fos+ neurons via increasing neural excitability during cognition tasks.CONCLUSION Our results dem-onstrate that subicular seizure-activated c-fos+ neurons contribute to cognitive impairment in TLE,suggesting sei-zure-tagged c-fos+ neurons as the potential therapeutic target to alleviate cognitive impairment in TLE.
4.Construction of core competence evaluation index system for nurses in Anesthesia Intensive Care Unit

Objective:To construct a core competence evaluation index system for Anesthesia Intensive Care Unit (AICU) nurses, and to provide reference for the career orientation, training and management of AICU nurses.Methods:The first draft of the core competence evaluation index system of AICU nurses was developed by literature evaluation, semi-structured interview and group discussion. Using the purposive sampling method, 26 experts were selected for 2 rounds of expert consultation from August to September 2021 to determine the evaluation index system of core competence of AICU nurses, and the weights of indicators at all levels were determined by analytic hierarchy process.Results:The effective recovery rates of the 2 rounds of expert letter consultation questionnaire were respectively 92.31% (24/26) and 91.67% (22/24) , the expert authority coefficients were respectively 0.93 and 0.93, and the Kendall's harmony coefficients were respectively 0.28 and 0.19 (all P<0.01) . The final evaluation index system of AICU nurses' core competence included 4 first-level indicators (theoretical knowledge, nursing practice, comprehensive ability, personal characteristics) , 14 second-level indicators and 52 third-level indicators. Conclusions:The constructed evaluation index system of core competence of AICU nurses is scientific and reliable, which can provide a reference for the career orientation, training and management of AICU nurses.
6.Application of enclosed safety blood sampling device in prevention of arterial catheter-related bloodstream infection

Objective To evaluate the effect of enclosed safety blood sampling device in prevention of arterial catheter-related bloodstream infection.Methods Totally 510 patients who received successful radial and brachial artery cannulation in ICU of the Second Hospital of Jiaxing between June and December 2016 were selected according to the table of random number and randomly and equally divided into a control group and an experimental group. The patients in the control group received conventional blood sampling with arterial transducers, while the patients in the experimental group received blood sampling with Safedraw enclosed safety blood sampling devices. Their incidence of complications and AC-BSI related to arterial catheter were observed. Results The incidence of complications related to arterial catheter of the patients in the experimental group was 31.76%, lower than that of the patients in the control group, 42.74% (x2=6.576,P<0.05); the incidence of AC-BSI of the patients in the experimental group was 3.92%, lower than that of the patients in the control group, 8.23% (x2=4.156,P<0.05).Conclusions Enclosed safety blood sampling device used in pressure measurement for artery cannulation can effectively reduce the incidence of complications and AC-BSI related to arterial catheter, thus worthy of clinical expansion.
7.Effect of Rehabilitation on Deaf Children with Multichannel Cochlear Implant

ObjectiveTo study the validation of rehabilitation for deaf children with multichannel cochlear implant and to explore the relationship between the age and the rehabilitation effectiveness, and the change of the effectiveness over time.Methods24 prelingually deafened children with multichannel cochlear implant were investigated. And they were divided into two groups (more than 3 years old and less than 3 yeas old). The validation of rehabilitation was assessed in different times before the operation, 3 months, 6 months, 9 months, 12 months after the operation.ResultsThe longer the rehabilitation, the better the validation(P<0.05). The speed of the first group (more than 3 yeas old) is slower than the other group by the time(P<0.05).ConclusionDeaf children with cochlear implant may take more rehabilitation training after the operation.
