1.Chrysanthemum zawadskii Flower Extract Rescues Inflammatory Cognitive Impairment

Chrysanthemum zawadskii flower ethanol extract (CZ-F) was evaluated for its neuroprotective potential against inflammation-associated cognitive impairment. To assess its anti-inflammatory properties, we first conducted in vitro studies using LPS-stimulated BV2 microglial cells. CZ-F demonstrated strong antioxidant activity with IC₅₀ values of 186.04 µg/ml (DPPH) and 94.56 µg/ml (ABTS), suppressed the production of reactive oxygen species and nitric oxide, and downregulated the expression of iNOS and IL-1β, likely via inhibiting the NF-κB signaling pathway. CZ-F also decreased inflammation-induced acetylcholinesterase (AChE) expression and directly inhibited AChE activity. Among the constituents of CZF, quercetin and luteolin exhibited the strongest anti-inflammatory and antioxidant effects, while linarin most potently inhibited AChE activity. In an in vivo zebrafish model, CZ-F treatment ameliorated learning and memory impairments induced by sleep deprivation. These findings suggest that CZ-F can attenuate neuroinflammation and modulate cholinergic dysfunction, providing a potential therapeutic approach for inflammationrelated cognitive deficits.
2.Care burden, depression, quality of life, and institutionalization factors among family caregivers of long-term care recipients: A secondary data analysis

This study aims to identify caregiving burden, depression, quality of life, and institutionalization among family caregivers of home-based long-term-care insurance beneficiaries and to analyze various factors influencing these outcomes. Methods: A secondary data analysis was employed to investigate the impact of various factors on caregivers’ experiences. The study used data from the 2016 Long-Term Care Service Utilization and Care Needs Survey, targeting 1,767 family caregivers of long-term-care beneficiaries. The original survey data included the Zarit Burden Interview for caregiver burden, the Center for Epidemiologic Studies Depression Scale for depression, and the new five-level version of EQ-5D for quality of life. Institutionalization status was identified using the long-term care database until the end of November 2023. Results: A high caregiving burden was reported by 44% of caregivers and 47.6% of family caregivers were found to be depressed. The average quality of life score was 0.82±0.14, with differences observed based on the participants’ characteristics. Among the study participants, 29.4% of the care recipients were institutionalized. Compared to caregiving burden, depression, and quality of life, the health status of the beneficiaries and their long-term-care service utilization patterns were significant factors influencing institutionalization. Conclusion: To reduce family caregivers’ burden and to decrease institutionalization rates, it is crucial to enhance home-care services and offer a variety of community-based resources. Effective welfare policies should be established to support these efforts.
3.Role of Carotid Artery Stenting in Prevention of Stroke for Asymptomatic Carotid Stenosis: Bayesian Cross-Design and Network Meta-Analyses

BACKGROUND AND OBJECTIVES:
There is insufficient evidence regarding the optimal treatment for asymptomatic carotid stenosis.
METHODS:
Bayesian cross-design and network meta-analyses were performed to compare the safety and efficacy among carotid artery stenting (CAS), carotid endarterectomy (CEA), and medical treatment (MT). We identified 18 studies (4 randomized controlled trials [RCTs] and 14 nonrandomized, comparative studies [NRCSs]) comparing CAS with CEA, and 4 RCTs comparing CEA with MT from MEDLINE, Cochrane Library, and Embase databases.
RESULTS:
The risk for periprocedural stroke tended to increase in CAS, compared to CEA (odds ratio [OR], 1.86; 95% credible interval [CrI], 0.62–4.54). However, estimates for periprocedural myocardial infarction (MI) were quite heterogeneous in RCTs and NRCSs. Despite a trend of decreased risk with CAS in RCTs (OR, 0.70; 95% CrI, 0.27–1.24), the risk was similar in NRCSs (OR, 1.02; 95% CrI, 0.87–1.18). In indirect comparisons of MT and CAS, MT showed a tendency to have a higher risk for the composite of periprocedural death, stroke, MI, or nonperiprocedural ipsilateral stroke (OR, 1.30; 95% CrI, 0.74–2.73). Analyses of study characteristics showed that CEA-versus-MT studies took place about 10-year earlier than CEA-versus-CAS studies.
CONCLUSIONS
A similar risk for periprocedural MI between CEA and CAS in NRCSs suggested that concerns about periprocedural MI accompanied by CEA might not matter in real-world practice when preoperative evaluation and management are working. Maybe the benefits of CAS over MT have been overestimated considering advances in medical therapy within10-year gap between CEA-versus-MT and CEA-versus-CAS studies.
