ObjectiveTo observe the effects of Xiere Xingpi Yin (XRXPY) on the body mass and body fat distribution in patients with simple obesity, and to preliminarily explore whether the mechanism involves the regulation of specific obesity-related microRNAs (miRNAs). MethodsA randomized controlled trial design was employed to recruit patients with simple obesity diagnosed with the traditional Chinese medicine (TCM) syndrome of dampness-heat accumulating in the spleen. Using a randomized controlled trial design, 95 patients with simple obesity of dampness-heat encumbering the Spleen syndrome were recruited, after 3 were excluded, 92 were randomly assigned in a 1∶1 ratio by means of a random number table. On the basis of lifestyle intervention, the observation group received XRXPY granules, while the control group received a placebo. Serum samples and TCM symptom scores were collected before and after the 12-week observation period. Differences in body mass loss, body composition, lipid metabolism, and TCM symptom scores were compared between the groups. An obesity-related miRNA dataset was constructed by searching the Human MicroRNA Disease Database. Library construction, sequencing, and quality control were performed on 6 selected patients from each group to screen the differentially expressed miRNAs. The intersection of these miRNAs with the obesity-related miRNAs was verified by bioinformatics and correlation analysis. ResultsBaseline characteristics were comparable between the two groups before treatment. A total of 86 patients completed the study (42 cases in the observation group and 44 cases in the control group). The proportion of patients achieving ≥5% body weight reduction was significantly higher in the observation group than in the control group 81.0% (34/42) vs. 43.2% (19/44) (χ2=12.963, P<0.01). Compared with baseline within the observation group, body weight, waist circumference, and TCM syndrome score were significantly decreased (P<0.01), as were body fat percentage (P<0.01) and serum triglyceride level (P<0.05). Compared with the control group after treatment, waist circumference and TCM syndrome score were significantly lower in the observation group (P<0.01), and body weight showed a trend toward reduction. When changes from baseline were compared between the two groups, the magnitudes of decrease in body weight, waist circumference, and TCM syndrome score were all significantly greater in the observation group than in the control group (P<0.01). With respect to safety, no serious adverse events occurred in either group and no participant discontinued the study because of adverse events. Compared with baseline, alanine aminotransferase levels decreased significantly in both the control and observation groups (P<0.01), and urea decreased significantly in the observation group (P<0.01), indicating a favorable safety profile. Sequencing identified nine differentially expressed miRNAs. After intersecting with the obesity-related miRNA dataset (187 miRNAs), 3 miRNAs were identified: miR-221 (log2FC=-6.62, P=0.009), miR-29a (log2FC=6.33, P=0.016), and miR-10b (log2FC=-5.91, P=0.029). A total of 262 target genes of the 3 miRNAs were identified, which were significantly enriched in 67 pathways, including the lysine degradation pathway (P=0.001). Correlation analysis revealed a positive correlation between miR-221 and triglycerides (r=0.46, P=0.027), a negative correlation between miR-29a and waist-to-hip ratio (r=-0.41, P=0.047), and positive correlations of miR-10b with body fat percentage (r=0.49, P=0.014) and waist circumference (r=0.44, P=0.032). ConclusionXRXPY combined with lifestyle intervention can effectively reduce the body mass and improve body composition. The underlying mechanism may involve the regulation of the lysine degradation pathway via miR-221, miR-29a, and miR-10b, though further verification is required.

Objective:To investigate the effects of Vibrio vulnificus ( Vv) quorum-sensing protein LuxS on the homeostasis of pulmonary innate immune cells in sepsis-induced acute lung injury. Methods:This study constructed luxS knockout and complemented Vv strains. The time required for wild type, luxS knockout, and complemented Vv strains to grow to an absorbance of 600 nm in liquid medium was measured using a spectrophotometer. Iron-overloaded mice were intraperitoneally infected with 1×10 5 CFU of the above three kinds of Vv strains, respectively. Clinical scoring for sepsis-induced dyspnea was used to evaluate the respiratory quality in mice. At 7 h after infection, the pathological changes in lung tissues were observed by HE staining; the bacterial loads in lung tissues were measured; the single-cell suspension of lung tissues was analyzed by flow cytometry. Uniform manifold approximation and projection (UMAP) was used to reduce the dimension of the distribution of CD45 + immune cells in lung tissues of mice in the PBS control group and infection groups with different strains. The frequency and absolute number of innate immune cells in lung tissues were analyzed by multicolor flow cytometry. One-way analysis of variance and t test were used for statistical analysis. Results:There was no significant difference in the growth rate of wild type, luxS knockout, and complemented Vv strains in liquid medium. Compared with the mice infected with the wild type or complemented strain, the mice infected with the luxS knockout strain exhibited overall alleviated respiratory difficulty, decreased inflammatory cell infiltration in lung tissues, and reduced bacterial load in lung tissues ( P<0.05). Besides, there was no significant difference in clinical respiratory scores, inflammatory cell infiltration, or bacterial loads between the mice infected with the complemented strain and wild type strain. UMAP analysis showed that compared with the mice infected with the luxS knockout strain, the mice infected with the wild type or complemented strain showed increased proportions of neutrophils and eosinophils in lung tissues. Results of multicolor flow cytometry analysis further verified that the proportions of neutrophils and eosinophils were significantly lower in the mice infected with the luxS knockout strain than in the mice infected with wild type or complemented strain ( P<0.01, P<0.000 1), while the proportion of alveolar macrophages was significantly higher as compared with that in the mice infected with wild type or complemented strain ( P<0.01). Conclusion:During Vv infection, LuxS may promote acute lung injury by affecting the homeostasis of neutrophils, eosinophils and resident macrophages in lung tissues.

Objective:To analyze the clinico pathological features, differential diagnosis and prognosis of metastatic renal cell carcinomas. Methods:The clinical data, histology, immunophenotype and follow-up data of 196 patients with metastatic renal cell carcinoma diagnosed from 1994 to 2017 at the Department of Pathology, Changhai Hospital, Naval Military Medical University, Shanghai, China were analyzed retrospectively. Results:There were 142 males and 54 females, with a median age of 61 years. The top three metastatic sites for the 196 cases of metastatic renal cell carcinoma were lung (31.1%, 61/196), bone (29.1%, 57/196) and digestive system (19.4%, 38/196). Among the pathological subtypes of metastasis, the proportion of clear cell renal cell carcinoma was 94.4% (185/196) and that of type II papillary renal cell carcinoma was 3.6% (7/196). The TFE3 translocated renal cell carcinoma and congestive tubular carcinoma were rare, with 3 cases and 1 case, respectively. CK, vimentin, CAⅨ and CD10 were expressed in all metastatic clear cell renal cell carcinomas. CK7, CD10 and P504s were expressed in papillary renal cell carcinomas. TFE3 was expressed in TFE3 translocated renal cell carcinoma. The collecting duct carcinoma was positive for HCK.Conclusions:Lung metastasis and bone metastasis are still the most frequent metastatic sites of renal cell carcinoma. Five years after primary lesion resection may be the high risk time for metastasis. Most of the metastases are solitary when they are first identified. To better diagnose and identify the renal origin of a metastatic renal cell carcinoma, one should consider morphological characteristics, clinical history information of the metastasis and the combined immunohistochemistry of CK, vimentin, CD10, CK7, TFE3, PAX2 and PAX8.

Objective To study the effects of one finger massage on sciatic nerve injury rats. Methods The sciatic nerves were exposed, and the sciatic nerve was held by micro needles to make the sciatic nerve injury model. SD rats were randomly divided into sham-operation group, model group and massage group. In the sham-operation group, the sciatic nerves were exposed but not held. 7 d after the establishment of modeling, rats in massage group received one finger massage for 30 d. After 30 d, SFI and BBB of sciatic nerves were detected. HE staining, transmission electron microscope and immunochemistry assay were used to measure the changes of sciatic nerves. Results Compared with model group, massage group could speed up the recovery of SFI and increase BBB, promote the recovery of sciatic nerve morphology, increased protein level of S-100β, and enhance ultrastructure of newborn nerve growth and recovery. Conclusion One finger massage can effectively promote neurological and functional recovery after sciatic nerve injury in rats.
