1.Serum Metabolomics Analysis of Baicalein for Prevention and Treatment of Radiation-Induced Intestinal Damage in Mice Based on GC-MS

OBJECTIVE To investigate the changes in serum metabolites of mice with radiation-induced intestinal injury under the intervention of baicalein and the changing characteristics of endogenous biological small molecules during the process of baicalein's participation in the prevention and treatment of radiation-induced intestinal injury through the metabolomics method based on GC-MS technology,in order to explore the potential regulatory mechanism of baicalein.METHODS A mouse radioactive intestinal injury model was established and randomly divided into normal control group,model group,low-dose baicalein group and high-dose baica-lein group.Baicalein was administered by gavage.Gas chromatography-mass spectrometry(GC-MS)technology was used to analyze the serum samples of mice in each group,and differential metabolites were screened through partial least squares-discriminant analysis(PLS-DA).Potential metabolic pathways were analyzed with MetaboAnalyst.RESULTS The pathological sections of mouse intesti-nal tissue showed that the high-dose and low-dose baicalein groups had a certain protective effect on radiation-induced intestinal dam-age.Metabolomic analysis showed that there were significant differences in the metabolic profiles of the blank control group,model group,low-dose and high-dose baicalein administration groups.After intragastric administration of baicalein,the endogenous metabo-lites in mice with radiation intestinal injury tended to normalize.The study screened out a total of 11 potential metabolic markers and 5 related metabolic pathways,among which pathways related to glucose metabolism,glutathione pathway,and ammonia metabolism were particularly significant.CONCLUSION Baicalein has a certain preventive and therapeutic effect on radiation-induced intestinal in-jury;baicalein participates in glucose metabolism and glutathione metabolism,and improving the endogenous substance disorder caused by radiation is its potential mechanism of action.
2.Furazolidone-induced interstitial lung disease: a case series analysis and literature review

Objective:To explore the clinical characteristics of interstitial lung disease (ILD) caused by furazolidone.Methods:Clinical data of 7 patients with furazolidone-related ILD (hospital cases) hospitalized in the Department of Respiratory Medicine, Huazhong University of Science and Technology Union Shenzhen Hospital from April 1, 2022 to April 1, 2023 were collected and retrospectively analyzed, including patient basic information, medication treatments, and characteristics, treatment and outcomes of ILD. Relevant databases at home and abroad were searched, and case reports of furazolidone-related ILD (literature cases) were collected. The basic information of literature, involving patient information, indications and treatment doses of furazolidone, as well as the latentperiod, main symptoms, laboratory tests, and auxiliary examination related to ILD was descriptively analyzed.Results:Among the 7 hospital cases, 2 were males and 5 were females, with a median age of 34 years. All patients were treated with a bismuth quadruple therapy containing furazolidone (0.2 g, once daily) due to Helicobacter pylori (HP)-associated gastritis. The median time from taking medicine to the onset of clinical symptoms of interstitial lung disease was 10 days. All patients had fever, and 6 had respiratory symptoms (dry cough, dyspnea, chest pain, etc.). ILD changes were found in all patients in their chest imaging, and pleural effusion was also found in 1 patient. Seven patients had normal eosinophil counts, and 1 had mild hypoxemia. The severity of ILD was grade 2 in 6 patients and grade 3 in 1 patient. All 7 patients stopped using furazolidone, and 4 received glucocorticoids additionally. After the hospitalization of 3-10 days, all the 7 patients recovered. Eighteen literature cases of furazolidone-related ILD were retrieved, of which 15 were from China and 14 used furazolidone to treat HP infection. The time from taking medicine to the onset of clinical symptoms was 2-14 days, and the time was >7 days in 13 patients. Sixteen patients had fever, 14 had respiratory symptoms, and 1 had respiratory failure. The peripheral blood eosinophil count or percentage increased in 12 patients. All 18 patients stopped using furazolidone; 7 received glucocorticoids and 1 received ventilator assisted ventilation. All patients recovered or were improved. ILD recured in rechallenge with furazolidone in 2 patients, and 1 of them had a strong positive reaction to furazolidone in vitro lymphocyte transformation test. Conclusions:ILD is a rare adverse reaction of furazolidone, main manifests as fever and respiratory symptoms and mostly occurs 7 days after furazolidone treatment. Early detection and timely withdrawal of furazolidone generally lead to a good prognosis.

4.Efficacy of vitamin D adjuvant therapy for prevention of spontaneous bacterial peritonitis in patients with decompensated cirrhosis of hepatitis B

Objective:To evaluate the efficacy of vitamin D supplementation for prevention of spontaneous bacterial peritonitis (SBP) in patients with decompensated liver cirrhosis of hepatitis B.Methods:A total of 172 patients with decompensated cirrhosis of hepatitis B admitted in Jinhua Hospital affiliated to Zhejiang University School of Medicine from January to December 2021 were randomly divided into two groups with 86 cases in each group. Patients in both groups received conventional antiviral and symptomatic treatment; while patients in the intervention group received additinal oral vitamin D drops (800 IU/d) for 6 months. After 6 months of treatment, the incidence of SBP and the serum biochemical indexes were compared between two groups. SPSS 21.0 statistical software was used for data analysis.Results:After 6 months of treatment, the incidence of SBP in the intervention group(5.81%, 5/86) was significantly lower than that in control group(30.23%, 26/86)( χ2=19.210, P<0.01). The serum 25-(OH)D level in intervention group was significantly higher than that in the control group ( t=13.425, P=0.018), while the levels of CRP, PCT and IL-6 in intervention group were significantly lower than those in control group ( t=17.312, 10.353 and 12.218, P<0.01 or <0.05). Conclusion:Vitamin D adjuvant therapy can increase serum 25-(OH)D level, decrease serum CRP, PCT and IL-6 levels, and effectively reduce the incidence of SBP in patients with decompensated cirrhosis of hepatitis B.
5.Roles of multidisciplinary team in diagnosis and treatment of suspected cases of COVID-19.

OBJECTIVE:
To analyze the roles of multidisciplinary team (MDT) in the diagnosis and treatment of suspected cases of coronavirus disease 2019 (COVID-19).
METHODS:
The clinical data of 48 patients with suspected COVID-19 admitted in Jinhua Municipal Central Hospital from January 21, 2020 to March 20, 2020 were retrospectively analyzed.
RESULTS:
In the 48 suspected cases, 18 were diagnosed with COVID-19, and 30 were excluded. Each of the confirmed cases were discussed among MDT for 2 to 12 times with an average of (4.7±3.2) times; while for non-COVID-19 patients were discussed for 2 to 4 times with an average of (2.3±0.6) times. With the guidance of MDT, one COVID-19 patient was transferred to designated provincial hospital after effective treatment; one patient complicated with acute cholecystitis underwent gallbladder puncture and drainage; and COVID-19 was excluded in a highly suspected patient after alveolar lavage fluid examination. Except one transferred patient, all 17 confirmed COVID-19 patients were cured and discharged. There was no cross-infection occurred in suspected patients during the hospitalization. There were no deaths and no medical staff infections.
CONCLUSIONS
The efficiency of diagnosis and treatment for suspected COVID-19 patients can be improved with MDT, particularly for complicated cases.

7.The therapeutic effects of interleukin 10 gene-modified bone marrow-derived dendritic cells in a murine model of liver fibrosis

Objective To investigate the therapeutic effects and mechanisms of interleukin-10 (IL-10) gene-modified dendritic cells (DC-IL-10) in mice with liver fibrosis. Methods DC-IL-10 was constructed in vitro, the phenotype and function of which were evaluated by flow cytometry. BALB/c mice were treated with intraperitoneal injection of carbon tetrachloride(CCl4)to establish liver fibrotic model. DC-IL-10 was administrated via tail vein. Animals were divided into 4 groups including normal dendritic cell (DC) control , liver fibrosis only, negative lentiviral transfection DC (DC-mock) and DC-IL-10. Liver function, cytokine secretion, T lymphocyte differentiation and liver histomorphology were tested. Real-time PCR and western blot were used to analyze the effect of DC-IL-10 on Wnt/β-catenin signaling pathway and its role in liver fibrosis. Results When compared with DC control and DC-mock, the expression of DC-IL-10 surface stimulating molecules (major histocompatibity complex-Ⅱ, CD80, CD86) were significantly decreased (F=14.708, 22.503, 12.595, respectively, all P<0.05), and DC-IL-10 significantly inhibited T lymphocyte proliferation (F=50.295, P<0.05). When compared with liver fibrosis group, serum alanine aminotransferase and aspartate transaminase were decreased in DC-IL-10 treated group (all P<0.05), other parameters including inflammatory factors (tumor necrosis factor α, IL-6, IL-1β) reduced (all P<0.05), the proportion of regulatory T cells (Treg) increased (F=6.742, P<0.05), pathological damage improved, the expression of Wnt3a, α-SMA and β-catenin mRNA and protein significantly reduced in DC-IL-10 treatment group(all P<0.001). Conclusions DC-IL-10 induces elevation of Treg for immune tolerance, as well as inhibition of inflammatory response, block of Wnt/β-catenin signaling pathway, which translates into improvement of liver fibrosis.
8.Analysis of the Results of Serum Pre-S1 Antigen and HBV DNA Detection

To study the relationship between serum Pre-S1 antigen and HBV DNA, serum Pre-S1 antigen and HBV DNA marker in 229 patients positive for HbsAg were detected by using ELISA method and compared. It was found that both Pre-S1 antigen and HbeAg were correlated with HBV DNA with the coefficient being 0. 9812 and could be used to reflect the existence or reproduction of HBV DNA well. Although there was statistical correlation between them, their clinical implication was not completely the same. Therefore, a conclusion was drawn that combined detection of Pre-S1 antigen, HBV and HBV DNA in the patients with different kinds of HBV infection was helpful to clinical diagnosis, therapeutic effect evaluation and prognosis judgement.
