1.Efficacy and safety of Xuezhikang Capsule combined with low-dose statin therapy in treatment of carotid atherosclerotic plaques

Objective To explore the efficacy and safety of combined Xuezhikang Capsule with low-dose statin for patients with atherosclerotic plaques in carotid arteries.Methods A total of 240 inpatients and outpatients with carotid atherosclerosis admitted in Qingtian County Hospital of Traditional Chinese Medicine from May 2022 to March 2023 were recruited,and randomly divided into a control group and an observation group,with 120 patients in each group.The control group was given oral administration of atorvastatin,while the observation group was given same dose of atorvastatin in combination with Xuezhikang Capsule.The clinical efficacy,blood lipids before and after treatment,and safety were compared between the two groups.Results The LDL-C levels at 8 and 24 weeks after treatment were significantly lower in the observation group than the control group(P<0.05,P<0.01).The change in LDL-C level in the observation group was obviously higher than that of the control group after treatment(P<0.05).The alanine transaminase level was notably lower in the observation group than the control group after 24 weeks of treatment(26.74±7.66 U/L vs 31.53±10.18 U/L,P<0.01).There was no statistical difference in the inci-dence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of pa-tients with carotid atherosclerosis,Xuezhikang Capsule combined with low dose of statin can re-duce blood lipid with high safety.
2.Environmental object surfaces contamination with carbapenem-resistant gram-negative bacteria in intensive care units of tertiary hospitals in Shanghai

OBJECTIVE To investigate the current status of contamination with carbapenem-resistant gram-negative bacteria in environment of intensive care units(ICU)of tertiary hospitals in Shanghai and find out the potential contamination sources so as to provide bases for prevention and control of multidrug-resistant organisms infec-tions in the ICUs.METHODS The surroundings of the ICU patients detected with CRGNB and environmental ob-jects surfaces in public area were sampled by mSuperCARBA chromogenic media from Dec.2024 to Jan.2025,the strains were isolated,and the targeted strains were identified by matrix-assisted laser desorption/ionization time-of-flight(MALDI-TOF)mass spectrometer.RESULTS A total of 653 samples were collected in the survey,76 of which were positive for bacterial culture,60 were detected with CRGNB,and the isolation rate of CRGNB was 9.19%.The isolation rate of CRGNB was 53.40%in the water-source group,0.91%in the non-water-source group,and there was significant difference(x2=286.450,P<0.001).The result of whole genome sequencing for 17 strains of CRKP showed that ST11 and ST15 were the two major types of multilocus typing(MT),respective-ly carrying 2-12 types of drug resistance genes.CONCLUSIONS The CRGNB strains are detected in some environ-mental sites of the ICUs of 15 tertiary hospitals in Shanghai,and the isolation rate of CRKP is highest among them.The colonization rate of CRGNB is relatively low on the highly frequent-contact object surfaces of the ICUs,however,sink drain holes poses a risk of hospital-acquired CRGNB infections transmissions.Additionally,the ba-sins and towels of the CRGNB patients are hard to be thoroughly cleaned,disinfected and dried,resulting in a high contamination rate.
3.Scoping review of research on nurses' self-efficacy in disaster contexts

Objective To conduct a scoping review of studies on nurses' self-efficacy in disaster contexts,and to provide references for enhancing nurses' self-efficacy in disaster response.Methods Guided by the methodology of scoping review,we systematically searched PubMed,Web of Science,Cochrane Library,CINAHL,China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP Database,and China Biology Medicine disc(CBM)from inception to July 3,2025.Totally 2 researchers independently screened the search results,summarized them,and extracted relevant data.Results A total of 21 studies were included.Nurses' self-efficacy in disaster contexts was generally at a moderate level,influenced by multiple factors including sociodemographic characteristics,occupational background,and physical and mental health status.The main assessment tools were the General Self-Efficacy Scale and the Disaster Response Self-Efficacy Scale.Interventions included psychological first aid training,scenario simulation training,and immersive movie escape room training.Conclusion Nurses' self-efficacy in disaster contexts is generally at a moderate level with multiple influencing factors.Relevant assessment tools need further optimization,and interventions require further refinement.
4.Hip joint function recovery and prediction model construction after proximal femoral nail antirotation for intertrochanteric fractures

BACKGROUND:At present,domestic and foreign studies mainly focus on the comparison of different operation methods for intertrochanteric fracture of femur and the risk of failure in proximal femoral nail antirotation operation.There are few studies on the prognosis of hip function of affected limb after proximal femoral nail antirotation.OBJECTIVE:To analyze the influencing factors of poor hip function recovery in elderly patients with intertrochanteric fracture after proximal femoral nail antirotation surgery,and to construct a score system for predicting hip function after surgery and explore its value.METHODS:A total of 150 patients with intertrochanteric fracture of femur who received proximal femoral nail antirotation surgery in the Affiliated Hospital of Xuzhou Medical University from June 2021 to June 2023 were selected and divided into groups according to the Harris hip function score during postoperative follow-up.A score ≥80 was considered as the good group,and a score<80 was considered as the poor group.Univariate and binary regression analyses were used to explore the risk factors leading to postoperative hip dysfunction.A score scale was established according to the risk factors.The value of this scoring system in predicting hip function after proximal femoral nail antirotation was investigated by using receiver operating characteristic curve.RESULTS AND CONCLUSION:(1)Among the 150 patients,according to the Harris score standard of the affected hip joint at 1 year follow-up,there were 52 cases in the group with poor functional recovery and 98 cases in the group with excellent functional recovery,with an unsatisfactory rate of 34.7%.(2)The results of univariate comparison between the two groups showed that there were significant differences in age,bone mineral density,number of preoperative underlying complications,type of lateral wall,position of spiral blade,quality of reduction and time of first exercise after surgery between the poor group and the good group(P<0.05).(3)The results of binary Logistic regression analysis showed:Age ≥75 years old(OR=2.834),osteoporosis(OR=3.002),number of preoperative basic complications>2(OR=4.024),lateral wall rupture(OR=2.999),position difference of spiral blade(OR=4.025),and time to exercise for the first time after surgery>4 weeks(OR=3.153)were independent risk factors for hip dysfunction after proximal femoral nail antirotation for the intertrochanteric fractures(P<0.05);poor reduction quality(OR=1.026)was not an independent risk factor(P>0.05).(4)Based on the results of binary regression analysis,a score system for predicting good hip function after surgery was established.Receiver operating characteristic curve analysis showed that the threshold for predicting poor hip function after surgery was 4.5 points;the area under the curve was 0.797;the sensitivity was 83.7%and the specificity was 65.4%.(5)These results suggested that age ≥75 years old,osteoporosis,number of preoperative basic comorbidities>2,lateral wall rupture,poor position of spiral blade,and first time out of bed exercise>4 weeks after intertrochanteric fracture were risk factors for hip dysfunction after proximal femoral nail antirotation.The establishment of a score prediction system can provide reference value for early clinical identification of high-risk patients with postoperative hip dysfunction,and is conducive to guiding early clinical intervention,adopting more personalized treatment and rehabilitation programs,and promoting the recovery of hip function in patients after surgery.
5.Mapping the symptom management journey of oral cancer patients and nursing response

Objective To identify the multi-dimensional needs of symptom management for oral cancer patients based on the journey map,and provide references for optimizing the symptom management of oral cancer patients.Methods From September 2023 to March 2024,the purposive sampling was used to select 15 perioperative oral cancer patients from a tertiary A general hospital in Zhengzhou for semi-structured interviews.The content analysis method was used to analyze the data and create a patient journey map.Results According to the time axis of diagnosis and treatment,the symptom management of patients with oral cancer was subdivided into 22 themes including identifying abnormal symptoms,clarifying diagnosis,anxiety,fear and uncertainty,guilt,high-risk behavioral inertia solidification,diagnostic trust crisis and so on from 3 dimensions of task,emotion,and pain point,and a journey map was formed.Conclusion The journey of symptom management for patients with oral cancer is long and complex,and the needs of patients'physiological and psychological symptom management are dynamically changing.In the future,the digital intelligence of big data technology can be combined to achieve whole-process,personalized and precise symptom management to improve the quality of life of oral cancer patients.
