In patients with differentiated thyroid cancer (DTC), initial recurrence risk stratification based on clinical, histopathological, and perioperative data remains the key determinant for guiding management strategies during the first 1-2 years post-treatment. However, the adoption of ongoing risk stratification (ORS), which dynamically reassesses risk by integrating longitudinal clinical data and treatment response, enables more precise long-term prognostic assessment and facilitates highly individualized management. Building upon recent guidelines, the 2026 KTA guideline has been further refined by incorporating robust evidence from large-scale national cohorts and comprehensive systematic reviews. These updated recommendations outline contemporary concepts of ORS, risk-adapted TSH suppression targets, optimized surveillance modalities for recurrence detection, and disease-specific long-term follow-up strategies. Reflecting the paradigm shift toward de-escalated treatment, this revision integrates evolved perspectives on TSH suppression intensity, the clinical interpretation of thyroglobulin levels, and tailored follow-up intervals. These evidence-based recommendations aim to minimize unnecessary treatment and excessive surveillance in the large proportion of patients with excellent prognosis after initial therapy, while ensuring that each patient receives appropriately tailored and effective long-term management.

Methods: This study analyzed 1,774 adults aged 31–79 years with T2D who participated in the 8th Korea National Health and Nutrition Examination Survey. The diet quality was examined by the Korean Healthy Eating Index (KHEI), and the dependent variables were the risk factors for diabetes complications (overweight/obesity, hyperglycemia, dyslipidemia, and hypertension) and related markers (HbA1c, fasting plasma glucose [FPG], triglycerides, low-density lipoprotein-cholesterol, high-density lipoprotein-cholesterol [HDL-C], systolic blood pressure, and diastolic blood pressure). The data were analyzed using SPSS v. 29.0. Results: The total KHEI scores were Q1 45.80, Q2 58.98, Q3 67.91, and Q4 77.90 in men and Q1 48.96, Q2 60.79, Q3 69.62, and Q4 80.34 in women. The odds ratio [OR]s of the adverse level HDL-C were significant for men in KHEI Q1 (OR, 1.844; 95% confidence interval [CI], 1.115–3.052), Q2 (OR, 1.777; 95% CI, 1.085–2.911), and Q3 (OR, 1.982; 95% CI, 1.220–3.220) compared to those in Q4. The KHEI ‘have breakfast’ score was inversely and significantly associated with the adverse level FPG in men (OR, 0.942; 95% CI, 0.891–0.996) and women (OR, 0.935; 95% CI, 0.883–0.990). The ORs of dyslipidemia were significant for men in KHEI Q2 (OR,2.026; 95% CI, 1.179–3.483) and Q3 (OR, 1.817; 95% CI, 1.062–3.110) compared to those in Q4. Conclusion A good diet quality can help with the proper control of HDL-C in men with T2D, and having breakfast can help control the FPG level in people with T2D.

Fifteen leading hospitals and fourteen academic associations in Korea are engaged in constructing CCLDs for 10 primary cancer types. For each cancer type-specific CCLD, cancer data experts determine key clinical data items essential for cancer research, standardize these items across cancer types, and create a standardized schema. Comprehensive clinical records covering diagnosis, treatment, and outcomes, with annual updates, are collected for each cancer patient in the target population, and quality control is based on six-sigma standards. To protect patient privacy, CCLDs follow stringent data security guidelines by pseudonymizing personal identification information and operating within a closed analysis environment. Researchers can apply for access to CCLD data through the K-CURE portal, which is subject to Institutional Review Board and Data Review Board approval. The CCLD is considered a pioneering standardized cancer-specific database, significantly representing Korea’s cancer data. It is expected to overcome limitations of previous CDMs and provide a valuable resource for multicenter cancer research in Korea.

Background: Staged operations are commonly employed in the management of high-energy tibial plateau fractures (TPF) complicated by acute compartment syndrome (ACS); however, complications, such as nonunion, deep wound infection, and traumatic arthritis, often occur due to severe bone and soft-tissue damage. We aimed to report the radiological and clinical outcomes of staged surgical interventions performed following complete closure of the fasciotomy wound for the treatment of TPF complicated by ACS.Additionally, we analyzed factors associated with complications arising from these procedures. Methods: Thirty patients with TPF and ACS were included (23 men and 7 women; average age, 59.7 years). The mean followup period was 33.2 months (range, 12–85 months). An external fixator was initially applied with emergency fasciotomy, and open reduction and plate fixation were performed after complete closure of the fasciotomy wound and soft-tissue stabilization (mean, 31 days; range, 9–55 days). Radiological evaluation of bone union and alignment was conducted, functional evaluation of the knee and ankle joints was performed using the Knee Society and American Orthopedic Foot and Ankle Society (AOFAS) scores, and complications and related factors were analyzed. Results: Primary bone union was achieved in 29 of the 30 cases (96.7%) at an average of 20.8 weeks (range, 12–35 weeks). Malalignment was not observed in any case. At the final follow-up examination, the mean Knee Society and AOFAS scores were 92.5 (range, 65–100) and 95.5 (range, 74–100), respectively. Complications included 1 case of nonunion (3.3%), 2 cases of deep wound infection (6.7%), and 5 cases of traumatic arthritis (16.7%). A statistically significant correlation was noted between complications and patients who underwent dual approaches for the fixation of bicondylar TPFs. Conclusions A staged operation coupled with sufficient soft-tissue healing can achieve excellent bone union and functional outcomes in patients with TPF and ACS. However, complications may occur more often in patients undergoing dual approaches for bicondylar TPFs, necessitating vigilant monitoring and management.
