Radiofrequency ablation (RFA) is a minimally invasive treatment modality used as an alternative to surgery in patients with benign thyroid nodules, recurrent thyroid cancers (RTCs), and primary thyroid microcarcinomas. The Korean Society of Thyroid Radiology (KSThR) initially developed recommendations for the optimal use of RFA for thyroid tumors in 2009 and revised them in 2012 and 2017. As new meaningful evidence has accumulated since 2017 and in response to a growing global interest in the use of RFA for treating malignant thyroid lesions, the task force committee members of the KSThR decided to update the guidelines on the use of RFA for the management of RTCs based on a comprehensive analysis of current literature and expert consensus.

Objective: To prospectively evaluate the effect of accelerated deep learning-based reconstruction (Accel-DL) on improving brain magnetic resonance imaging (MRI) quality and reducing scan time compared to that in conventional MRI. Materials and Methods: This study included 150 participants (51 male; mean age 57.3 ± 16.2 years). Each group of 50 participants was scanned using one of three 3T scanners from three different vendors. Conventional and Accel-DL MRI images were obtained from each participant and compared using 2D T1- and T2-weighted and 3D gradient-echo sequences. Accel-DL acquisition was achieved using optimized scan parameters to reduce the scan time, with the acquired images reconstructed using U-Net-based software to transform low-quality, undersampled k-space data into high-quality images. The scan times of Accel-DL and conventional MRI methods were compared. Four neuroradiologists assessed the overall image quality, structural delineation, and artifacts using Likert scale (5- and 3-point scales). Inter-reader agreement was assessed using Fleiss’ kappa coefficient. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated, and volumetric quantification of regional structures and white matter hyperintensities (WMHs) was performed. Results: Accel-DL showed a mean scan time reduction of 39.4% (range, 24.2%–51.3%). Accel-DL improved overall image quality (3.78 ± 0.71 vs. 3.36 ± 0.61, P < 0.001), structure delineation (2.47 ± 0.61 vs. 2.35 ± 0.62, P < 0.001), and artifacts (3.73 ± 0.72 vs. 3.71 ± 0.69, P = 0.016). Inter-reader agreement was fair to substantial (κ = 0.34–0.50). SNR and CNR increased in Accel-DL (82.0 ± 23.1 vs. 31.4 ± 10.8, P = 0.02; 12.4 ± 4.1 vs. 4.4 ± 11.2, P = 0.02). Bland-Altman plots revealed no significant differences in the volumetric measurements of 98.2% of the relevant regions, except in the deep gray matter, including the thalamus. Five of the six lesion categories showed no significant differences in WMH segmentation, except for leukocortical lesions (r = 0.64 ± 0.29). Conclusion Accel-DL substantially reduced the scan time and improved the quality of brain MRI in both spin-echo and gradientecho sequences without compromising volumetry, including lesion quantification.

Purpose: This study aimed to investigate the effects of postnatal systemic corticosteroids on neurodevelopment in very low birth weight (VLBW) preterm infants. Methods: This was a population-based study of the Korean Neonatal Network of VLBW infant born at 23+0 and 31+6 weeks of gestation between 2013 and 2020. VLBW preterm infants assessed using the Bayley Scales of Infant and Toddler Development, third edition (BSID-III) at 18–24 months of corrected age and 3 years of age were enrolled. The primary outcomes were BSID-III scores and neurodevelopmental delays, with scores of <85. Socioeconomic status and clinical variables were adjusted for using multivariate regression analyses. Results: In total, 517 infants were enrolled in this study. Among the 216 (41.8%) infants who received postnatal systemic corticosteroids, the rate of cognitive delay was significantly higher at 18–24 months of corrected age than at 3 years of age. The rates of language and motor delays were significantly higher both at 18–24 months of corrected age and at 3 years of age. When multivariate logistic regression was performed, postnatal systemic corticosteroid use was significantly associated with cognitive delay at 18–24 months of corrected age, but not at 3 years of age. There was no significant association between postnatal systemic corticosteroid use and language or motor delay at 18-24 months of corrected age or at 3 years of age after multivariate logistic regression. Conclusion Postnatal systemic corticosteroid use in VLBW preterm infants increased the risk of cognitive delay at 18–24 months of corrected age, but not at 3 years.

PURPOSE: Three-dimensional (3D) printing must go through a post-processing procedure. This study aimed to evaluate the effects of different cleaning methods on 3D printed three-unit fixed partial dentures using 3D analysis. MATERIALS AND METHODS: A three-unit fixed partial denture was designed and printed using two different printers. The evaluation was taken regarding cleaning method with ethanol (ETH) or isopropanol (IPA), and combining ultrasonic (US) technique for the following groups: group 1, ETH; group 2, ETH+US; group 3, IPA; group 4, IPA+US; and group 5, US. The 3D evaluation was conducted by calculating the mean distance between two specific points and analyzing various angular deviations, utilizing the geometry-embedded library. In addition, visual analysis was performed by creating sectional planes in different points of view. Normality test and independent t-test were performed to compare results between the two printers used in this study. In addition, one-way ANOVA was performed to compare cleaning method groups. RESULTS: Mean distance results showed statistical differences between printers for groups IPA, US, and IPA+US, whereas no significant differences were found among cleaning methods. Angular measurements were taken at the bucco-lingual, proximal, and occlusal planes to assess deviations in the internal surfaces. Overall results showed a tendency of shrinkage of the printed three-unit fixed partial denture. CONCLUSION Ultrasonic treatment may impact the washing efficiency when used with other cleaning solutions regarding angular and longitudinal deformation. Adequate selection of post-processing methods is crucial to decrease morphological deformities in 3D printed materials.

Background/Aims: Portal vein thrombosis (PVT) frequently occurs in patients with inflammatory bowel disease (IBD), particularly when influenced by factors such as abdominal infections, IBD flare-ups, or surgical procedures. The implications of PVT range from immediate issues such as intestinal ischemia to long-term concerns including portal hypertension and its complications. However, there is a notable gap in comprehensive studies on PVT in IBD, especially with the increasing incidence of IBD in Asia. This research aimed to evaluate the clinical features and outcomes of PVT in patients with IBD at a leading hospital in South Korea. Methods: This retrospective analysis reviewed adult patients diagnosed with both IBD and PVT from 1989 to 2021 at a renowned South Korean medical center. The study focused on patient characteristics, specifics of PVT, administered treatments, and outcomes, all confirmed through enhanced CT scans. Results: A total of 78 patients met the study’s criteria. Notably, only 20.5% (16/78) were treated with oral anticoagulants; however, a vast majority (96.2%; 75/78) achieved complete radiographic resolution (CRR). When comparing patients receiving anticoagulants to those who did not, a significant preference for anticoagulant use was observed in cases where the main portal vein was affected, as opposed to just the left or right veins (p = 0.006). However, multivariable analysis indicated that neither anticoagulant use nor previous surgeries significantly impacted CRR. Conclusions Patients with IBD and PVT generally had favorable outcomes, regardless of anticoagulant use.

Background: Respiratory infections play a major role in acute exacerbation of chronic obstructive pulmonary disease (AECOPD). This study assessed the prevalence of bacterial and viral pathogens and their clinical impact on patients with AECOPD. Methods: This retrospective study included 1,186 patients diagnosed with AECOPD at 28 hospitals in South Korea between 2015 and 2018. We evaluated the identification rates of pathogens, basic patient characteristics, clinical features, and the factors associated with infections by potentially drug-resistant (PDR) pathogens using various microbiological tests. Results: Bacteria, viruses, and both were detected in 262 (22.1%), 265 (22.5%), and 129 (10.9%) of patients, respectively. The most common pathogens included Pseudomonas aeruginosa (17.8%), Mycoplasma pneumoniae (11.2%), Streptococcus pneumoniae (9.0%), influenza A virus (19.0%), rhinovirus (15.8%), and respiratory syncytial virus (6.4%). Notably, a history of pulmonary tuberculosis (odds ratio [OR], 1.66; p=0.046), bronchiectasis (OR, 1.99; p=0.032), and the use of a triple inhaler regimen within the past 6 months (OR, 2.04; p=0.005) were identified as significant factors associated with infection by PDR pathogens. Moreover, patients infected with PDR pathogens exhibited extended hospital stays (15.9 days vs. 12.4 days, p=0.018) and higher intensive care unit admission rates (15.9% vs. 9.5%, p=0.030). Conclusion This study demonstrates that a variety of pathogens are involved in episodes of AECOPD. Nevertheless, additional research is required to confirm their role in the onset and progression of AECOPD.

Irritable bowel syndrome (IBS) is a chronic, disabling, and functional bowel disorder that significantly affects social functioning and reduces quality of life and increases social costs. The Korean Society of Neurogastroenterology and Motility published clinical practice guidelines on the management of IBS based on a systematic review of the literature in 2017, and planned to revise these guidelines in light of new evidence on the pathophysiology, diagnosis, and management of IBS. The current revised version of the guidelines is consistent with the previous version and targets adults diagnosed with or suspected of having IBS. These guidelines were developed using a combination of de novo and adaptation methods, with analyses of existing guidelines and discussions within the committee, leading to the identification of key clinical questions. Finally, the guidelines consisted of 22 recommendations, including 3 concerning the definition and risk factors of IBS, 4 regarding diagnostic modalities and strategies, 2 regarding general management, and 13 regarding medical treatment. For each statement, the advantages, disadvantages, and precautions were thoroughly detailed. The modified Delphi method was used to achieve expert consensus to adopt the core recommendations of the guidelines. These guidelines serve as a reference for clinicians (including primary care physicians, general healthcare providers, medical students, residents, and other healthcare professionals) and patients, helping them to make informed decisions regarding IBS management.

Purpose: This study examined the diet quality and its relationship with risk factors for diabetes complications and the related markers in Korean adults with type 2 diabetes (T2D). Methods: This study analyzed 1,774 adults aged 31–79 years with T2D who participated in the 8th Korea National Health and Nutrition Examination Survey. The diet quality was examined by the Korean Healthy Eating Index (KHEI), and the dependent variables were the risk factors for diabetes complications (overweight/obesity, hyperglycemia, dyslipidemia, and hypertension) and related markers (HbA1c, fasting plasma glucose [FPG], triglycerides, low-density lipoprotein-cholesterol, high-density lipoprotein-cholesterol [HDL-C], systolic blood pressure, and diastolic blood pressure). The data were analyzed using SPSS v. 29.0. Results: The total KHEI scores were Q1 45.80, Q2 58.98, Q3 67.91, and Q4 77.90 in men and Q1 48.96, Q2 60.79, Q3 69.62, and Q4 80.34 in women. The odds ratio [OR]s of the adverse level HDL-C were significant for men in KHEI Q1 (OR, 1.844; 95% confidence interval [CI], 1.115–3.052), Q2 (OR, 1.777; 95% CI, 1.085–2.911), and Q3 (OR, 1.982; 95% CI, 1.220–3.220) compared to those in Q4. The KHEI ‘have breakfast’ score was inversely and significantly associated with the adverse level FPG in men (OR, 0.942; 95% CI, 0.891–0.996) and women (OR, 0.935; 95% CI, 0.883–0.990). The ORs of dyslipidemia were significant for men in KHEI Q2 (OR,2.026; 95% CI, 1.179–3.483) and Q3 (OR, 1.817; 95% CI, 1.062–3.110) compared to those in Q4. Conclusion A good diet quality can help with the proper control of HDL-C in men with T2D, and having breakfast can help control the FPG level in people with T2D.

Purpose: A one-year blood analysis and dietary intake survey was conducted on postmenopausal women living in a large city in Korea to analyze the atherogenic index of plasma (AIP), a predictive factor for cardiovascular disease, and the relationship between the AIP and blood or nutritional indices was analyzed. Methods: The study subjects were 92 women aged 45 to 69 years who lived in Daejeon and had been through menopause for more than one year. Blood samples were collected twice a year, in the fall and spring, and dietary intake surveys were conducted four times a year, once each season, from September 2021 to August 2022. The subjects’ drinking, exercise status, supplement intake, body mass index, blood sugar, and lipid profiles were investigated. Results: The mean AIP of the study participants was −0.30±−0.55, and 76% were in the low-risk group for cardiovascular disease. The body mass index, body weight, total body fat ratio, fasting blood sugar, and glycated hemoglobin of the study participants increased as the AIP quartile increased. A lower AIP quartile means a higher nutrient intake of omega-3 fatty acids, calcium, and iron and a higher intake of vegetables and oily fish among food groups. Conclusion Weight and blood sugar control are essential to prevent cardiovascular disease in postmenopausal Korean women, and it is necessary to consume more than two servings (approximately 140 g) of oily fish, a source of omega-3 fatty acids, per week and at least four to five servings/day of vegetables, including kimchi.

Objective: Using a nationally representative cohort of medical claims data in Korea, this study aimed to analyze the association between the use of various anti-rheumatic agents and the risk of acute myocardial infarction (AMI) in patients with rheumatoid arthritis (RA). Methods: This nested case-control study used the Korean Health Insurance Review and Assessment data of 35,133 patients newly diagnosed with RA between 2011 and 2020. Incident AMI patients were identified and matched at a 1:4 ratio with randomly selected controls. The usage of anti-rheumatic agents was measured from the date of RA diagnosis to the index date and stratified based on exposure time and duration. The risk of AMI associated with each anti-rheumatic agent was estimated using conditional logistic regression, adjusted for comorbidities and concomitant drug use. Results: Of the 35,133 patients with RA, 484 were diagnosed with AMI. In total, 484 AMI patients and 1,924 controls with newly diagnosed RA were included in the analysis. Current exposure and long-term exposure to glucocorticoids (adjusted odds ratio [aOR]: 2.301, 95% confidence interval [CI]: 1.741~3.041; aOR: 1.792, 95% CI: 1.378~2.330) and leflunomide (aOR: 1.525, 95% CI: 1.196~1.944; aOR: 1.740, 95% CI: 1.372~2.207) were associated with an increased risk of AMI. Conclusion The study demonstrates a significant association between the current and long-term use of glucocorticoids and leflunomide and an increased risk of AMI in patients with RA. These findings underscore the importance of careful consideration of cardiovascular risks when selecting anti-rheumatic agents for RA treatment.
