1.Retrospective analysis of adverse events associated with traditional Chinese medicine formula granules and decoction pieces in hospitalized patients using the global trigger tool

OBJECTIVE To provide technical support for improving recognition rate of adverse drug events (ADEs) related to traditional Chinese medicine (TCM) formula granules and decoction pieces among inpatient patients. METHODS By referencing the global trigger tool (GTT) whitepaper, literature on adverse reactions to TCM, and expert review opinions, ADE trigger items for TCM formula granules and decoction pieces used in the inpatients were established. GTT was applied to analyze ADEs in inpatients who had used TCM formula granules and decoction pieces in our hospital from August 2013 to August 2023, utilizing the Chinese Hospital Pharmacovigilance System. The effectiveness of GTT and the characteristics of these ADEs were analyzed. RESULTS A total of forty-eight triggers were established, including thirty-two laboratory test indexes, thirteen clinical symptoms, and three antidotes. Among the 1 682 patients included, GTT identified 652 potential ADEs, 284 true positive ADEs,with a trigger rate of 38.76% and a positive predictive value of 43.56%. After review by the auditor, 278 cases of ADEs were finally confirmed, with an incidence rate of 16.53%, significantly higher than the number of spontaneously reported ADEs during the same period (0). The 278 cases of ADEs were mostly grade 1 (223 cases), mainly involving hepatobiliary system, gastrointestinal system, blood- lymphatic system, etc;a total of 219 types of TCMs are involved,and the top five suspected TCMs used at a frequency higher than 1% were Poria cocos, Codonopsis pilosula, Atractylodes macrocephala, fried Glycyrrhiza uralensis, and Scutellaria baicalensis. CONCLUSIONS The established GTT can improve the recognition rate of ADEs for hospitalized patients using traditional Chinese medicine formula granules and decoction pieces.
2.Research progress on antimicrobial peptides against methicillin-resistant Staphylococcus aureus

Staphylococcus aureus is a Gram-positive bacterium with strong pathogenicity. With the widespread use of antibiotics, its multi-drug resistance has gradually increased. Among them, methicillin-resistant S. aureus (MRSA) is one of the main pathogens of hospital and community infections. Antimicrobial peptides are short-chain peptides with good antibacterial effects and low drug resistance, which have been widely studied in recent years. This study summarizes the mechanism of action of antimicrobial peptides and related study on antimicrobial peptides against MRSA from different sources. It is found that the mechanisms of action of antimicrobial peptides include targeting bacterial cell membranes, bacterial cells, and bacterial cell walls, etc. Besides isolating antimicrobial peptides with anti-MRSA activity from animals, plants, and microorganisms, antimicrobial peptides can also be obtained through synthetic methods. Among them, GHa-derived peptides from animal sources, Ib-AMP4 from plant sources, Ph-SA from microbial sources, the synthetic peptide LLKLLLKLL-NH2, and so on, due to their effective antibacterial activity, rapid bactericidal speed, and low toxicity, are promising candidates for anti-MRSA drugs.
3.Logic-gated drug delivery systems in cancer immunotherapy

Immunotherapy has become a mainstream treatment for cancer.However,the immunosuppressive tumor microenvironment hinders the antigen extraction and presentation by immune cells,resulting in insufficient infiltration and activation of cytotoxic T cells.Moreover,patient variability and the systemic distribution of immunotherapeutic agents can cause toxic side effects,such as excessive activ-ation or immunosuppression.The extratumoral toxicity and challenges in penetrating the tumor microenvironment hinder the effectiveness of immunotherapy on solid tumors.Recently,researchers have developed immunotherapy platforms using polymers,nucleic acids,and cells capable of logical processing based on signal inputs.These platforms can target tumors and perform logical processing based on various in-puts from the tumor microenvironment or external signals.This review briefly introduces logic-gated platforms designed using synthetic nanocarriers,nucleic acids,and chimeric antigen receptor T-cells(CAR-T)and discusses research progress in immunotherapy,aiming to provide foresight by comparing the advantages and disadvantages of each platform.
4.Application and mechanism of induced pluripotent stem cells in inherited heart disease models

BACKGROUND:Inherited heart disease has a high prevalence and mortality rate,but its pathogenesis has not yet been clarified.Although relevant animal models have been established to provide a foundation for the pathogenesis research of inherited heart disease,the value of these research results has been significantly reduced due to differences among species.Therefore,a new model is needed to explore its occurrence and development. OBJECTIVE:To review the current role of induced pluripotent stem cells in disease modeling and potential application prospects in various inherited heart diseases. METHODS:The first author searched the relevant articles published nearly 13 years in PubMed from January to March 2023.The search terms were"induced pluripotent stem cell,inherited heart disease,congenital heart disease".Finally,76 articles were included for analysis. RESULTS AND CONCLUSION:Since 2007,when induced pluripotent stem cells were induced from human somatic cells,many studies have been reported on disease-specific induced pluripotent stem cells.Due to the ability of disease-specific induced pluripotent stem cells to reproduce disease phenotypes,they are expected to become a new research tool for in vitro disease modeling,used to analyze pathogenesis and develop auxiliary drugs.In the research of cardiovascular genetic diseases,cardiomyocytes derived from patient-specific induced pluripotent stem cells contain gene mutations that are involved in cardiac dysplasia.Therefore,it can be used as a new tool to study the potential mechanisms of inherited heart disease.Up to now,induced pluripotent stem cells-derived cardiomyocytes have been widely used to study the molecular mechanisms of various genetic heart diseases,such as cardiac electrophysiological diseases,cardiomyopathy,and some syndromic inherited heart diseases.
5.Research progress on atrial functional mitral regurgitation

Atrial functional mitral regurgitation (AFMR) is mitral regurgitation in patients with atrial fibrillation (AF), whose left atrium (LA) is enlarged, the left ventricle is not enlarged or only slightly enlarged, the left ventricular ejection fraction is preserved, and the mitral valve itself has no apparent lesion. At present, the etiology, pathophysiology and mechanism of this disease have not been completely clear yet. Existing studies have found that the causes of AFMR mainly include AF, enlargement of LA and mitral annulus, destruction of mitral annular shape, inability of mitral valve remodeling to compensate for mitral annular expansion, and hamstringing of the posterior mitral leaflet by atriogenic tethering. AFMR is demonstrated to be associated with an increased risk of mortality and readmission due to heart failure. Therefore, it serves as a primary therapeutic target for patients with heart failure and AF. However, the optimal treatment of AFMR still remains controversial. Therefore, this article will mainly expound the current definition, etiology, pathophysiological mechanism, treatment, and prognosis of AFMR.
6.Real-world research of vonoprazan and amoxicillin dual therapy for Helicobacter pylori eradication

OBJECTIVE To explore the efficacy, safety and economics of a dual therapy consisting of conventional dose of vonoprazan combined with conventional dose of amoxicillin in patients with primary treatment of Helicobacter pylori (HP) infection. METHODS Using a prospective cohort study, the patients diagnosed with HP infection and receiving initial treatment in Chengdu Xinhua Hospital from July 2021 to July 2022 were collected according to inclusion and exclusion criteria. The patients were given vonoprazan/amoxicillin dual therapy (i.e. VA group, Vonoprazan fumarate tablets 20 mg, once a day+Amoxicillin capsules 1.0 g, twice a day, 14 days) and bismuth-containing quadruple therapy (i.e. LJAF group, Rabeprazole sodium enteric- coated tablets 20 mg, twice a day+Colloidal bismuth pectin capsules 200 mg, twice a day+Amoxicillin capsules 1.0 g, twice a day+ Furazolidone tablets 100 mg, twice a day, for 14 days) according to the patient’s medication willingness. Four weeks after the end of the treatment, HP eradication rates of the two groups were compared by using intention-to-treat (ITT), modified intention-to- treat (MITT) and per-protocol (PP) analysis. The occurrence of adverse drug reactions (ADR) was recorded, and an economic evaluation was performed for them. RESULTS Among the 58 patients in VA group, 55 completed the trial, 2 were lost to follow- up and one withdrew due to rash; among the 62 patients in LJAF group, 57 completed the trial, 3 were lost to follow-up and 2 withdrew due to rash. Results of ITT, MITT and PP analysis showed that HP eradication rates of VA group were 86.2%, 89.3% and 90.9%, and those of LJAF group were 87.1%,91.5% and 94.7%, respectively; there was no statistical significance among different groups (P>0.05). The incidences of ADR in VA group and LJAF group were 6.9% and 14.5%,which were not significantly different (P>0.05). The result of cost minimization analysis showed that the treatment cost of VA group was 340.9 yuan, which was lower than 373.5 yuan of LJAF group. CONCLUSIONS In patients with primary treatment of HP infection, the efficacy and safety of dual therapy of conventional dose of vonoprazan combined with conventional dose of amoxicillin is equivalent to the bismuth-containing quadruplex therapy with low cost.
7.Chinese Medicine Intervention on Deep Vein Thrombosis-related Signaling Pathways After Total Knee Arthroplasty: A Review

Total knee arthroplasty, as a common treatment option for advanced knee osteoarthritis, can alleviate the clinical symptoms of patients. Deep vein thrombosis of the lower extremity is the most common complication of total knee arthroplasty. Previous studies have shown that the occurrence of deep vein thrombosis after total knee arthroplasty is mostly related to the overexpression of inflammatory factors in vivo. Nuclear transcription factor-κB, Toll-like receptor 4, phosphatidylinositol 3-kinase/protein kinase B, tumor necrosis factor, and nuclear transcription factor E2-related factor 2 are typical signaling pathways related to inflammation. Regulating the expression of the signaling pathways can intervene the formation of inflammatory factors. Inhibiting the formation of inflammatory factors can help suppress the activation of platelets, thereby blocking thrombosis. According to previous research, Chinese medicine monomers, Chinese medicine extract, and compound Chinese medicine prescriptions all directly or indirectly inhibit the expression of inflammatory factors by regulating the above signaling pathways, thereby suppressing the occurrence of deep vein thrombosis after total knee arthroplasty. Therefore, Chinese medicine can reduce postoperative complications and promote postoperative recovery of patients at low cost with small side effects. This article summarizes the research on Chinese medicine intervention on deep vein thrombosis-related signaling pathways after total knee arthroplasty, which is expected to lay a basis for the in-depth study and clinical application of Chinese medicine in deep vein thrombosis.
8.Clinical study of self-made Xiaogan Tiaopi Kaiwei Decoction combined with conventional western medicine therapy in the treatment of infantile anorexia of spleen-stomach weakness

Objective:To evaluate the efficacy of self-made Xiaogan Tiaopi Kaiwei Decoction combined with conventional western medicine therapy in the treatment of infantile anorexia of spleen-stomach weakness.Methods:Randomized controlled trial. A total of 100 children with anorexia of spleen-stomach weakness ,who were treated in the First Clinical Medical College of Shaanxi University of Chinese Medicine between April 2020 and September 2021, were selected as the observation subjects in a prospective cohort study, and they were divided into two groups by the random number table method, with 50 in each group. The control group was given routine treatment, and the Traditional Chinese Medicine (TCM) group was given self-made Xiaogan Tiaopi Kaiwei Decoction on the basis of the control group. The TCM syndromes were scored before and after treatment. The levels of trace elements such as iron, zinc and calcium were detected by atomic spectrometer, the absorptivity of D-xylose was measured by colorimetry. The adverse events during treatment were recorded. After 6 months of follow-up, the height, weight and BMI were measured and recorded, and the clinical efficacy was assessed.Results:The total response rate was 96.0% (48/50) in the TCM group and that in the control group was 84.0% (42/50) ( χ2=4.00, P=0.046). The scores of decreased appetite, anorexia, sallow complexion, abdominal fullness and distention and shortness of breath and laziness to speak and total score in the TCM group after treatment were significantly lower than those in the control group ( t=12.47, 13.42, 14.19, 16.39, 9.15, 17.72, P<0.01). The levels of blood trace elements such as iron [(414.58 ± 57.52) mg/L vs. (350.85 ± 53.33) mg/L, t=5.75 ], zinc [(8.26 ± 1.55) mg/L vs. (7.64 ± 1.37) mg/L, t=2.12 ] and calcium [(77.26 ± 15.30) mg/L vs. (71.05±14.26) mg/L, t=2.10] and urine D-xylose absorption rate [(31.76±5.28) % vs. (27.97 ± 4.61) %, t=3.82 ] in the TCM group were significantly higher than those in the control group ( P<0.01 or P<0.05). During treatment, there was no serious adverse reaction in the TCM group and 1 case of nausea in the control group. There was no statistical significance in the incidence rate of adverse reactions between the two groups ( χ2=1.01, P=0.315). After follow-up, the weight in the TCM group was significantly higher than that of the control group ( t=2.17, P=0.032). Conclusion:Self-made Xiaogan Tiaopi Kaiwei Decoction combined with conventional western medicine therapy can relieve the clinical symptoms, promote the absorption of trace elements and improvement of spleen-stomach function, and enhance the long-term efficacy in the treatment of children with anorexia of spleen-stomach weakness, and it has no adverse reactions.

Objective:To analyze the MRI findings of tendinopathy of the common extensor tendon (CET) and common flexor tendon (CFT) and to explore its application for injury classification.Methods:Sixty-two patients with elbow tendinopathy of CET and/or CFT who were examined in the Radiology Department of Beijing Jishuitan Hospital from August 2019 to April 2022 were enrolled as the case group, and 20 healthy subjects (40 elbows) were recruited as the control group. The clinical data of patients and MRI findings of elbow joint of all subjects were reviewed. The MRI of tendinopathy of CFT and CET were read by 2 senior radiologists independently, and the consistency was tested by the κ test. The MRI manifestations for injury classification were discussed. Results:The κ values of CFT, CET, medial collateral ligament complex and lateral collateral ligament complex were 0.645, 0.657, 0.615 and 0.653 respectively. MRI clearly showed the anatomical structure of CFT, CET and surrounding soft tissue. The CFT and CET of the elbow of 20 normal volunteers (40 sides) in the control group showed thin line or banded low signal in each MRI sequence, and the muscle fibers were continuous. In 27 patients with classification Ⅰ injury of CFT and/or CET of elbow joint, MRI showed thickening or thinning of tendons and increased signal intensity on fat suppression T2-weighted imaging. In 27 cases of classification Ⅱ injury, MRI showed that the fluid signal involved 20% to 80% of the tendon thickness. In 8 cases of classification Ⅲ injury, MRI showed that the fluid signal involved more than 80% of the tendon thickness. Conclusions:Tendinopathy of CFT and CET of elbow joint has its characteristic MRI manifestations, indicating that MRI can be used for the diagnosis of tendinopathy. In addition, MRI can also be used for the injury classification.
