After continuous development and improvement, lung transplantation has become the preferred means to treat a variety of benign end-stage lung diseases. However, the field of lung transplantation still faces many challenges, including shortage of donor resources, preservation and maintenance of donor lungs, and postoperative complications. Lung tissue samples removed after lung transplantation are excellent clinical resources for the study of benign end-stage lung disease and perioperative complications of lung transplantation. However, at present, the collection, storage and utilization of tissue samples after lung transplantation are limited to a single study, and unified technical specifications have not been formed. Based on the construction plan of the biobank for lung transplantation in the First Affiliated Hospital of Xi'an Jiaotong University, this study reviewed the practical experience in the collection, storage and utilization of lung transplant tissue samples in the aspects of ethical review, staffing, collection process, storage method, quality control and efficient utilization, in order to provide references for lung transplant related research.

The observation group also scored higher than the control group in clinical judgment, organizational effectiveness, overall performance, and total score in the Mini-Clinical Evaluation Exercise, with statistically significant differences ( P<0.01). Additionally, the observation group reported higher satisfaction with the teaching plan and methods compared to the control group ( P<0.05) . Conclusions:Clinical teaching for spinal surgery nursing interns based on the ADDIE instructional design model can improve their specialized practical skills and overall clinical competence. The interns also expressed a high level of acceptance for this teaching design model.
5.Research on the extraterritorial regulatory experience of human germline genome editing
Chinese Medical Ethics 2024;37(10):1146-1153
Researching the extraterritorial regulatory experience of human germline genome editing (HGGE) has important reference significance for improving China's genome editing governance framework.Taking the United Kingdom (UK) and the United States (US) as examples, they already have relatively mature practices in terms of regulatory agencies, laws and regulations, and multi-group participation. The UK incorporates germline genome editing into the regulatory scope of assisted reproduction through the Human Fertilization and Embryology Authority (HFEA),while the US is mainly responsible for regulation by the Food and Drug Administration (FDA) and the National Institutes of Health (NIH).The experience of both countries shows that the regulatory framework for HGGE requires a regulatory agency with a clear division of responsibilities,as well as classified management and timely adjustment and revision of outdated laws and regulations to cope with development and ethical challenges of technology.Additionally, the authors believe that drawing on the consultation of various international and national organizations and increasing public participation is crucial to establishing correct values. It is recommended that China should clarify the regulatory responsibilities of institutions, formulate prove effective provisions of legal and regulatory, and promote multi-party dialogue and public participation.

Astronauts are affected by the space environment, and there are health problems in many systems of the body, which seriously affect the on-orbit work efficiency and the health recovery after landing.Although measures have been taken, there are still deficiencies.Acupuncture can regulate the body in a multi-target,multi-level and multi-system way to protect the health of astronauts, which is an effective measure to protect the health problems of astronauts. But there is no acupoint group database for aerospace medical problems, which limits the application of acupuncture in space. The aim of this paper is to solve the possible aerospace medical problems of astronauts by acupuncture.Firstly, the acupuncture indications in aerospace medicine are determined through literature research, expert consultation and expert consensus, and then the evidence collection, evidence evaluation and evidence aggregation are carried out through the"stratified evidence evaluation method"for ancient literature, modern literature and expert experience. Finally, acupoint prescriptions were recommended based on the comprehensive score of acupoints, and acupuncture prescriptions for all acupuncture indications in aerospace medicine were integrated to form an acupoint group database,so as to contribute more Chinese wisdom and solutions to solve aerospace medicine problems.

The clinical data of a case of neonatal bacterial meningitis complicated with posterior fossa subdural empyema in the West China Second University Hospital in December 2019 were retrospectively analyzed. The 3-day-old male newborn was admitted for jaundice with decreased intake for 1 day.Examinations on admission showed increased C-reactive protein. The count of karyocytes in cerebrospinal fluid (CSF) significantly increased, which were mainly neutrophils, and pyocytes could be found. The protein content in CSF increased, while that of glucose decreased. Both CSF culture and blood culture detected the presence of Escherichia coli.Enhanced magnetic resonance imaging (MRI) of the head indicated large abnormal signals in bilateral occipital extra-cerebellar spaces.T1-weighted images presented mixed low and high signals, and T2-weighted images presented high signals, and marginal enhancement was observed after enhancement. After 6 weeks of antibiotic treatment using Meropenem combined with Ceftazidime, the CSF index of the newborn patient returned to normal, and the subdural empyema of the posterior fossae subsided.Bacterial meningitis complicated with subdural empyema of posterior fossa is a rare and critical disease of the central nervous system. It is easily misdiagnosed due to the atypical clinical manifestations and early imaging features. The disease requires an adequate course of anti-infective treatment.Surgical removal of the empyema should be performed if the anti-infective treatment is unresponsive.
8.Clinical value of preoperative biliary drainage for the outcomes of pancreaticoduodenectomy in the patients with jaundice caused by malignant obstruction hi the low bile duct
Clinical Medicine of China 2011;27(7):747-750
Objective To investigate the effects of preoperative biliary drainage ( PBD ) on the morbidity and mortality of pancreaticoduodenectomy in patients with malignant obstructive jaundice in the lower bile duct. Methods Clinical data of 74 cases undergoing pancreaticoduodenectomy from Jan. 2000 to Dec. 2005 with preoperative total bilirubin level over 85 μmol/L were collected and retrospectively analyzed. Comparison was performed between patients receiving PBD and those not. Before surgery. The parameters sincluding perioperative situation, in-hospital death rate, post-operative complications and were calculated to evaluate the influence of pre-operative biliary drainage on the outcomes. Univariate analysis and Logistic analysis were used to identify the risk factors for post-operative complications. Results Forty (40/74) cases received PTCD procedure. The total bilirubin was significantly reduced from (338. 10±88. 38 )μwnol/L to ( 228. 50±82.24) μ,mol/L in PTCD population and was significantly lower than ( 328. 60±93. 02) μmol/L of the non-PTCD group. There was no significance between the two groups in terms of total complications and individual complication Logistic regression analysis showed that preoperative TB over 340 μmol/L and blood loss over 600 ml were important risk factors for post-operative complications. Conclusion High pre-operative total bilirubin over 340 μmol/L increases the risk of post-operative complications in the patients with malignant jaundice. Preoperative biliary drainage is useful to reduce the total bilirubin and improve the hepatic function of the patients. Prolonged pre-operative biliary drainage could be considered in the patients with high preoperative bilirubin, poor nutritional condition and impaired coagulation to enhance the tolerance of surgery.

Forty women with polycystic ovary syndrome( PCOS) were enrolled. Basal plasma and urine cortisol, pituitary hormones, homeostasis model assessment for insulin resistance, and plasma cortisol in 0. 25 mg dexamethasone inhibition test were determined. In over-weight or obese PCOS patients, cortisol levels before and after inhibition test were increased (P<0. 05 or P<0. 01 ). The results suggest that the feed-back regulation of glucocorticoids to hypothalamas-pituitary is impaired and the change in hypothalamic-pituitary-adrenal axis is associated with insulin resistance.
