1.Organizational Agility in Public Hospitals:Concepts,Core Capabilities,and Development Pathways

In the current era filled with change and challenges,public hospitals,as a crucial part of the national healthcare system,urgently need to enhance their agility to swiftly respond to the ever-changing environment.Current it outlines the origins and connotations of organizational agility,argues for the necessity of organizational agility in public hospitals,and proposes the core competencies required to maintain organizational agility in public hospitals,namely:patient-centeredness,environmental sensitivity,proactive planning,organizational integration capability,flexibility,rapid response,iterative regulation,and continuous learning.Furthermore,it attempts to establish a cultivation pathway for organizational agility in public hospitals,encompassing multiple dimensions such as organizational culture,agile leadership,communication systems,organizational structure,the embedding of new productivity,management systems,partner management,training systems,performance evaluation,and compliance management.
2.Risk factors and prognostic analysis of immune checkpoint inhibitor-related pneumonitis in patients with advanced lung cancer

Objective To analyze the risk factors and prognosis of patients with advanced lung cancer complicated with immune checkpoint inhibitor-related pneumonitis(CIP).Methods A total of 220 patients with advanced lung cancer who were treated at Ganzhou Cancer Hospital from January 2022 to October 2024 were selected as the research subjects.The patients were divided into non-CIP group(n=179)and CIP group(n=41)based on whether they had concurrent CIP.Compare the clinical characteristics of two groups of patients and analyze the related influencing factors of patients with advanced lung cancer complicated with CIP.Results There were statistically significant differences in age,gender,underlying pulmonary diseases,smoking history,history of thoracic radiotherapy,treatment plans,serum albumin(Alb),platelet to lymphocyte ratio(PLR),neutrophil to lymphocyte ratio(NLR),and absolute lymphocyte count(ALC)between two groups of patients(P<0.05).Multivariate Logistic regression analysis showed that age,underlying pulmonary diseases,treatment plan,Alb,PLR,NLR,and ALC were all influencing factors for patients with advanced lung cancer complicated with CIP(P<0.05).The 90-day cumulative survival rate of patients in CIP group was significantly lower than that in non-CIP group(58.54% vs.77.09%,x2=5.918,P=0.015).With the increase of CIP classification,the 90-day cumulative survival rate of patients showed a downward trend(P<0.05).Conclusion The incidence of CIP in patients with advanced lung cancer is relatively high.Age,underlying pulmonary diseases,treatment plan,and levels of Alb,PLR,NLR,and ALC are all influencing factors.CIP can have adverse effects on the prognosis of patients.
3.Promoting and hindering factors of physical activity in children with cancer during chemotherapy: a Meta-synthesis of qualitative studies

Objective:To systematically analyze the promoting and hindering factors of physical activity during chemotherapy for children with cancer by Meta-synthesis method, so as to provide a basis for the subsequent formulation of scientific and standardized physical activity strategies.Methods:Databases of PubMed, Web of Science, Cochrane Library, Embase, PsycINFO, CINAHL, China National Knowledge Infrastructure, Wanfang Database, VIP database and China Biomedical Literature Database were retrieved on qualitative research about the experiences of physical activity in children with cancer during chemotherapy and the caregivers′ perceptions of the factors influencing physical activity in children with cancer. The retrieval period is from the establishment of the databases to January 31, 2024. The quality of the literature was evaluated according to Joanna Briggs Institute Evidence Based Healthcare Center Critical for qualitative studies in Australia. The pooled integration method was used to Meta-synthesis the research results such as research topic, implicit meaning and classification.Results:A total of 7 studies were included, and 26 research results were obtained through Meta-synthesis, similar research results were classified into 7 new categories, and finally 2 integrated results were formed:the hindering factors of physical activity in children with cancer during chemotherapy; the promoting factors of physical activity in children with cancer during chemotherapy.Conclusions:During hospitalization for chemotherapy in children with cancer, health care professionals should pay attention to the positive impact of physical activity on children, strive to overcome the obstacles to physical activity, and formulate a scientific and personalized physical activity strategy in combination with each child′s unique condition and personal needs.

7.A Model of Factors Influencing the Career Plateau of Hospital Administrators based on A Grounded Theo-ry Study

Objective To explore the influencing factors of the career plateau among hospital administrators,to provide scientific reference for enhancing the quality of hospital management and operation.Methods The interview data were analyzed by grounded theoretical research methods.It conducted semi-structured interviews with 22 tar-get objects and conducted qualitative research on the influencing factors of hospital administrators'career plateau with the help of NVivo 11.0 software.Results After a three-level coding,114 initial concepts,30 subcategories,10 main categories,and 3 core categories were summarized.A theoretical model was developed,encompassing three dimensions:aggravating factors,dissipating factors,and convertible factors.Conclusion The career plateau of hospi-tal administrators is affected by a complex interplay of aggravating,dissipating and convertible factors.Hospitals need to provide regular vocational training to cultivate organizational loyalty for their employees.Hospital administra-tors should engage in proactive career planning and skill diversification.and the government should coordinate the es-tablishment of links between hospitals and the external system to promote the openness and connection of the healthcare system to the outside world.
8.Impacts of CircASPH Targeting miR-28-5p/IGF-1R Axis on Proliferation,Migra-tion,and Invasion of Ovarian Granulosa Cells in Polycystic Ovary Syndrome

Objective:To explore the impacts of cyclic RNA asparagine hydroxylase(CircASPH)targeting the miR-28-5p/insulin-like growth factor 1 receptor(IGF-1R)axis on the proliferation,migration,and invasion of ovari-an granulosa cells in polycystic ovary syndrome(PCOS).Methods:Human ovarian granulosa cells KGN and COV434 were used as research subjects,the targeting relationship among CircASPH,miR-28-5p,and IGF-1R was confirmed through dual luciferase reporter gene experiments and pull down experiments.KGN and COV434 cells were grouped into si-NC group,si-ASPH group,si-ASPH+anti NC group,and si-ASPH+anti miR-28-5p group.The mRNA expression levels of CircASPH,miR-28-5p,and IGF-1R mRNA were detected by qRT-PCR,cell proliferation,migration,and invasion were detected by MTT assay,Edu staining,and transwell cell assay,respec-tively;and Western blot was applied to detect the expression of proliferating cell nuclear antigen(PCNA),matrix metalloproteinase-2(MMP-2),vimentin,N-cadherin,E-cadherin,and IGF-1R proteins.Results:Bioinformatics a-nalysis and dual luciferase reporter gene experiments showed that CircASPH,IGF-1R and miR-28-5p had targe-ted binding sites.Compared with si-NC group,the expression level of CircASPH,OD490 value,Edu positive cell rate,cell migration and invasion number,MMP-2,vimentin and N-cadherin in si-ASPH group were decreased,while the expression level of miR-28-5p and E-cadherin protein were increased,and the differences were statisti-cally significant(P<0.05).Compared with the si-ASPH+anti-NC group,the expression level of miR-28-5p and E-cadherin in the si-ASPH+anti-miR-28-5p group were decreased,and the OD490 value,Edu positive cell rate,cell migration and invasion number,MMP-2,vimentin and N-cadherin were increased,the differences were statisti-cally significant(P<0.05).Conclusions:In KGN and COV434 cells,inhibiting the expression of CircASPH can inhibit the proliferation,migration,invasion,and epithelial-mesenchymal transition of ovarian granulosa cells by reg-ulating the miR-28-5p/IGF-1R axis,which may become a new target for the treatment of PCOS.
