1.Discussion on Research Paradigm and Layout Based on Disease (Psoriasis) Specifically Responding to Traditional Chinese Medicine

To implement the Opinions of the State Council of the Central Committee of the Communist Party of China on Promoting the Inheritance and Innovative Development of Traditional Chinese Medicine, delve into the diseases specifically responding to traditional Chinese medicine (TCM), and serve the building of advantageous specialties, clinical talent cultivation, national research layout, and academic innovation, the Chinese Association of Chinese Medicine sponsored the salons in multiple fields to discuss the diseases specifically responding to TCM. On August 26, 2023, the 25th salon on the diseases specifically responding to TCM was held in Shanghai. In view of the advantages of TCM and integrated traditional Chinese and Western medicine in the diagnosis and treatment of psoriasis, Chinese and western medicine experts and interdisciplinary researchers carried out extensive and in-depth discussions and formed specific suggestions and consensus on the diagnosis and treatment of psoriasis with TCM and integrated traditional Chinese and Western medicine. However, there was still a lack of detailed research path. Under the guidance of the Chinese Association of Chinese Medicine, this paper analyzes the problems in the diagnosis and treatment of psoriasis from the occurrence and development of psoriasis. According to the advantages and characteristics of TCM and integrated traditional Chinese and Western medicine in the diagnosis and treatment of psoriasis, this paper puts forward the main points of research layout for psoriasis from the three aspects of psoriasis diagnosis and treatment, comorbidity prevention, and chronic disease management as follows: ① optimization of the syndrome differentiation system for psoriasis, ② optimization of assessment indicators for psoriasis, ③ recurrence mechanisms of psoriasis, ④ construction and research of TCM prevention and treatment of psoriasis recurrence, ⑤ construction of the comorbidity spectrum and TCM theoretical system of psoriasis, ⑥ comorbidity mechanisms of psoriasis. Furthermore, this article proposed the research layout and directions, expected goals and values, and funding priorities. Therefore, on the basis of the series of salons about psoriasis as a disease specifically responding to TCM, this article puts forward the research paradigm of psoriasis, aiming to facilitate the high-quality development of TCM and provide reference for the national research layout, the research and development of new TCM preparations, the selection of research topics, and the formulation of guidelines and consensus.
2.Exploration on the Pharmacodynamic Material Basis of Bawei Chenxiang Powder in Treating Coronary Heart Disease Based on Network Pharmacology and Experimental Verification

Objective To investigate the pharmacodynamic material basis of Bawei Chenxiang Powder in the treatment of coronary heart disease.Methods The action targets of 16 blood-entry components of Bawei Chenxiang Powder were retrieved by TCMSP,PharmMapper and SwissTargetPrediction,and the disease targets were found by GeneCards and OMIM databases,which screened the intersection targets of the components and the disease;a component-target network was constructed using Cytoscape 3.9.1 software and key components were screened;the protein-protein interaction network was obtained by using STRING database and core targets were screened;GO and KEGG pathway enrichment analysis of intersecting targets was performed using DAVID database;molecular docking between key components and core targets was performed;molecular dynamics simulations were performed using Amber 20 software;UPLC-Q-Exactive hybrid quadrupole-Orbitrap mass spectrometry was used to quantify the key components in rat plasma;the hypoxia/reoxygenation model of H9c2 cells was constructed to investigate the effects of the key components on the survival rate and LDH of H9c2 cells.Results Totally 514 targets of the 16 blood-entry components of Bawei Chenxiang Powder,1716 targets of coronary heart disease,and 192 intersecting targets were retrieved,among which the key components were quercetin,kaempferol,naringenin,dehydrodiisoeugenol,and the core targets were ALB,IL6,TNF,GAPDH,AKT1,IL1B and TP53;the KEGG results showed that the mechanism of Bawei Chenxiang Powder for treating coronary heart disease may be related to PI3K-Akt signaling pathway,MAPK signaling pathway,HIF-1 signaling pathway,IL-17 signaling pathway,etc.;molecular docking showed that the key components had low binding energy with the core targets;molecular dynamics simulation results showed that the binding energy of TNF and quercetin was tight and stable,with a binding energy of-20.263 3 kcal/mol;the concentrations of the key components in plasma were quercetin(15.65±1.76)ng/mL,kaempferol(10.58±0.96)ng/mL,naringenin(3.07±0.69)ng/mL,and dehydrodiisoeugenol(4.90±1.18)ng/mL;the results of cellular experiments showed that the above four components could significantly increase the survival rate of H9c2 cells under hypoxia/reoxygenation conditions and reduce the LDH activity in the cell supernatant.Conclusion Quercetin,kaempferol,naringenin,dehydrodiisoeugenol and other components in Bawei Chenxiang Powder may be the pharmacodynamic material basis for treating coronary heart disease.
3.Development and reliability and validity testing of the questionnaire on rotavirus vaccination behavioral and social drivers

Objective:To construct a questionnaire to measure parents′ behavioral and social drivers (BeSD) in administering rotavirus vaccine to their children, and to test the reliability and validity of the questionnaire.Methods:A questionnaire was constructed to investigate the BeSD of rotavirus vaccination among parents based on the WHO BeSD questionnaire. A survey was conducted among parents of children aged 0-3 years old in vaccination clinics from 15 community health service centers in Nanjing City of Jiangsu Province, Yuhuan and Kaihua Counties of Zhejiang Province, Anqing City of Anhui Province, and Yangpu District of Shanghai City from April to May in 2024. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to analyze the reliability (internal consistency reliability, combination reliability and split half reliability) and validity (structural validity, convergence validity and discriminative validity) of the questionnaire.Results:A total of 850 questionnaires were collected, of which 614 were valid, with an effective response rate of 72.20%. The EFA resulted in the refinement of the rotavirus vaccine BeSD questionnaire from 37 to 35 items (including 24 core items), and suggested a five-factor structure including the perceptions of vaccine, confidence in vaccine, social process of vaccination, motivation for vaccination, and practical problems of vaccination. The cumulative variance contribution rate reached 56.27%. The CFA confirmed an ideal five-factor model fit (GFI=0.82,CFI=0.86,PNFI=0.67,RMSEA=0.07). The AVE of each dimension was greater than 0.50. The AVE square root of each dimension of the questionnaire was greater than its correlation coefficient. The Cronbach′s α coefficient of the total questionnaire was 0.79.Conclusion:The developed BeSD questionnaire of rotavirus vaccine has good reliability and validity, and can be used to measure parents′ behavioral and social factors in administering rotavirus vaccine to their children.
4.Predictive value of nutritional risk related indicators in the prognosis of elderly esophageal squamous cell carcinoma patients undergoing radical radiotherapy

Objective:To investigate the predictive value of nutritional risk related indicators in the prognosis of elderly esophageal squamous cell carcinoma (ESCC) patients undergoing radical radiotherapy.Methods:A retrospective case series study was conducted. The clinical data of ESCC patients aged ≥ 70 years who received radical radiotherapy in Shanxi Province Cancer Hospital from January 2015 to December 2019 were retrospectively analyzed. According to the radiotherapy planning system, the maximum transverse diameter of gross tumor volume of primary tumors (GTVt) and GTVt volume in the esophagus were calculated. The nutritional risk related indicators of ESCC patients before and after radiotherapy were calculated and recorded, including body mass index (BMI), geriatric nutritional risk index (GNRI), and neutrophil-to-lymphocyte ratio (NLR); Kaplan-Meier method was used for survival analysis, and log- rank test was performed; Cox proportional risk model was used to analyze the factors influencing the prognosis of patients. The overall survival (OS), progression-free survival (PFS), short-term efficacy, and adverse reactions of patients with different clinicopathological characteristics were compared.Results:The last follow-up time was 30 December, 2023. The 1-year, 3-year, and 5-year OS rates of 161 elderly patients with ESCC were 79.5%, 33.9%, and 16.1% respectively, with a median OS time of 25.8 months (95% CI: 20.11-31.49 months); the 1-year, 3-year, and 5-year PFS rates were 65.8%, 28.9%, and 14.8%, respectively, with a median PFS time of 20.0 months (95% CI: 16.31-23.69 months). There were statistically significant differences in the median OS time of patients with different age, lesion contrast length, maximum transverse diameter of GTVt, GTVt volume, short-term efficacy, BMI before radiotherapy, BMI after radiotherapy, GNRI after radiotherapy, NLR before radiotherapy, and NLR after radiotherapy (all P < 0.05); there were statistically significant differences in the median PFS time of ESCC patients with different age, maximum transverse diameter of GTVt, GTVt volume, short-term efficacy, BMI before radiotherapy, BMI after radiotherapy, GNRI after radiotherapy, and NLR before radiotherapy (all P <0.05). The results of multivariate Cox regression analysis showed that the maximum transverse diameter of GTVt was an independent influencing factor of the patients' OS ( P < 0.05); GTVt volume, short-term efficacy, GNRI after radiotherapy, and NLR before radiotherapy were independent influencing factors for OS and PFS of the patients (all P < 0.05). Among 161 elderly patients with ESCC, 45 achieved complete remission (CR), 111 achieved partial remission (PR), and 5 achieved stable disease (SD). There were statistically significant differences in the proportions of patients with different maximum transverse diameter of GTVt, GTVt volume, BMI after radiotherapy, GNRI after radiotherapy, and NLR before radiotherapy reaching CR and PR+SD (all P < 0.05). Among the 161 patients, grade 0, 1, 2, and 3 radiation-induced esophagitis (RE) occurred in 59, 54, 42, and 6 cases, respectively during treatment and within 3 months after treatment; among them, ≥ grade 2 RE occurred in 48 cases (29.8%); grade 0, 1, 2, and 3 radiation pneumonitis (RP) occurred in 95, 38, 25, and 3 cases, respectively; among them, ≥ grade 2 RP occurred in 28 cases (17.4%). Conclusions:Nutritional risk related indexes GNRI and NLR may be predictive indicators for the prognosis of elderly ESCC patients undergoing radical radiotherapy.
5.Survey on the current status of medical escort services in Shanghai based on supply and demand sides

Objective:To analyze the current situation and core issues of medical escort services in Shanghai through a two-side (supply and demand) survey, so as to provide references for formulating targeted strategies for the standardized management of this industry.Methods:From June to August 2024, a questionnaire survey on awareness of escort services was conducted among patients at 16 tertiary public hospitals in Shanghai. The survey covered patients′ basic information, their needs, concerns, and expectations regarding escort services. Concurrently, a specialized questionnaire was administered to trainees enrolled in Shanghai Open University′s " elderly care escort service provider" program. This survey covered the companions′ basic profiles, types of service recipients, relevant qualifications, training programs, and standardized service procedures. Data were organized and analyzed by using SPSS 26.0 software.Results:The survey on awareness of patient escort services yielded 855 valid responses, achieving a 92.13% response rate. Among these 855 patients, the primary demand for escort services was queue management for examinations (515 cases, 71.47%), followed by assistance with registration/payment/medication pickup (528 cases, 61.74%). 587 patients (68.66%) expressed willingness to use the service. The main concerns included lack of professional expertise among attendants (538 cases, 62.92%), trust issues (494 cases, 57.78%), and fee concerns (456 cases, 53.33%). Expectations for service supervision primarily focused on service quality and safety (618 cases, 72.28%) and pricing standards (609 cases, 71.23%). The specialized survey on medical escort services yielded 194 valid questionnaires, achieving a 100.00% response rate. Among the 194 medical escort service providers, 82.5% (160 individuals) were full-time practitioners, 31.96% (62 individuals) held relevant qualification certificates, and 72.68% (144 individuals) had participated in related training programs. Regarding standardized services, 74.74% (145 individuals) of medical escorts obtained patient information prior to service delivery, 56.70% (110 individuals) signed agreements with users, and 67.01% (130 individuals) maintained records of their medical escort services.Conclusions:Shanghai′s medical escort services had achieved some success, yet faced challenges including high patient demand coupled with low trust, insufficient certification and training for escorts, and an urgent need to improve service standardization. It is recommended that medical institutions further strengthen their primary role in managing escort services. By enhancing top-level design, establishing standardized training and certification mechanisms, implementing comprehensive quality monitoring and credit evaluation systems, and other measures, they could elevate the professional competence of practitioners, reinforce patient safety safeguards, and promote the high-quality, standardized development of escort services.
6.Clinical Diseases Responding Specially to TCM Treatment: Psoriasis

Psoriasis is a common chronic inflammatory systemic disease in dermatology. Its high prevalence, recurrence rate, and numerous comorbidities impose a significant physical and mental burden on patients. With the continuous advancement of modern medicine, the emergence of biological agents has improved clinical efficacy, making it possible to overcome psoriasis, in addition to classical treatments. However, in clinical practice, adverse reactions, drug resistance, recurrence rates, and immune drift cannot be ignored. Traditional Chinese medicine (TCM) has a history of thousands of years in treating psoriasis, demonstrating good efficacy, high safety, and a low recurrence rate, but a standardized management system is lacking. Therefore, the 25th Clinical Diseases Responding Specially to TCM Treatment Series (Psoriasis) Youth Salon, hosted by the Chinese Association of Chinese Medicine and organized by the Youth Committee of the Chinese Association of Chinese Medicine, invited 29 experts and scholars from TCM, Western medicine, and interdisciplinary fields to actively discuss the "Advantages, Challenges, and Clinical Transformation of TCM and Western Medicine in the Diagnosis and Treatment of Psoriasis". The experts at the meeting concluded that the advantages of TCM in the treatment of psoriasis are as follows. Firstly, in the TCM-led treatment plan, TCM's understanding of psoriasis follows the principle of combining the differentiation of disease and syndrome. This approach distinguishes the basic contradiction from the current main contradiction and enables a clear grasp of the dynamic process of psoriasis development. Based on the system of syndrome differentiation and treatment, TCM intervention is applied to address the current main contradiction, and the optimal TCM treatment plan is formulated by combining internal and external treatments. Adhering to the principle of "what is visible outside must be addressed inside", TCM can prevent and treat psoriasis comorbidities early by differentiating syndrome types. Secondly, in the integrated TCM and Western medicine treatment plan, the combination of both methods not only enhances efficacy but also reduces the adverse reactions of immunosuppressants and biological agents, lowering the recurrence rate. This conference provides a reference for the diagnosis and treatment of psoriasis using TCM and integrated TCM and Western medicine, opening up new ideas for clinical and basic research and guiding future research directions.
7.Safe and effective concentration of ropivacaine in interscalene brachial plexus block for children aged 6 to 10 years:a sequential allocation study

Objective To determine the median effective concentration(EC50)of ropivacaine at the volume of 0.5 mL/kg for ultrasound-guided interscalene brachial plexus block in children aged 6 to 10 years.Methods A prospective dose-finding trial was conducted based a sequential Dixon up-and-down allocation.We recruited children aged 6 to 10 years who were scheduled for unilateral surgery on upper extremity regions in our hospital from April to December 2022.All of them were subjected to general intravenous anaesthesia combined with ultrasound-guided interscalene brachial plexus block.The ropivacaine volume for each patient was 0.5 mL/kg.The concentration of 0.2%for the first patient,subsequent concentrations were adjusted based on the block effect of previous patient,increase or decrease by 0.02%.The trial was stopped when there were 7 turning points.Isotonic regression and Bootstrap were used to calculate the values of EC50 and 95%effective concentration(EC95),along with their 95%confidence intervals(CI).General data,incidence of adverse events,and score of postoperative pain were recorded.Results A total of 26 children aged 6 to 10 years were included.The EC50 of ultrasound-guided interscalene brachial plexus block was determined to be 0.091%(95%CI:0.077%~0.105%),and the EC95 was estimated to be 0.117%(95%CI:0.110%~0.118%).Successful blockade was achieved in 16 cases(61.5%),while 10 cases(38.5%)failed.No statistical differences existed between successful and failed cases regarding sex,age,body weight,surgical site,surgical laterality,operative time,or anesthesia duration.None of the patients experienced adverse events such as pneumothorax,vascular injury,or Horner's syndrome,and the score of postoperative pain were all<6 by modified Children's Hospital of Eastern Ontario Pain Scale.Conclusion In children aged 6 to 10 years,the EC50 of ropivacaine is 0.091%at a volume of 0.5 mL/kg for ultrasound-guided interscalene brachial plexus block,and this low dose of regional anesthetic can reduce the risks such as systemic toxicity and direct neurotoxicity.
8.A Meta-analysis of Quality of Life Level and the Corresponding Influencing Factors in Infertile Patients

Objective To systematically evaluate the current status of quality of life in infertile patients and its influencing factors.Methods Computer search of PubMed,Cochrane Library,Web of Science,Embase,CBM,Knowledge Network,Wanfang,and Wipu,with a search timeframe from the establishment of the library to December 2023,and meta-analysis using CMA 3.3 software.Results A total of 33 literatures were included in the current status of quality of life,which showed that the level of quality of life was 60.841[95%CI(58.029,63.654),P<0.001]for the FertiQoL scale and 67.630[95%CI(59.541,75.718),P<0.001]for the SF-36 scale.A total of 45 papers were included in the literature on factors influencing quality of life,and the results showed that personal trait factors of young age(r=0.181,P=0.006)and history of pregnancy(r=0.258,P<0.001)were lowly positively correlated with quality of life,and that long years of infertility(r=-0.267,P<0.001),a history of treatment failure(r=-0.292,P<0.001),and long duration of treatment(r=-0.213,P<0.001)had a low negative association with quality of life.Behavioural factors such as anxiety(r=-0.547,P<0.001),depression(r=-0.556,P<0.001),and stigma(r=-0.530,P<0.001)were highly negatively correlated with the quality of life;high fertility stress(r=-0.333,P=0.010)was moderately negatively correlated with the quality of life;and good psychological resilience(r=0.383,P<0.001)and high self-esteem(r=0.343,P=0.001)were moderately positively associated with quality of life.Interpersonal network factors of good relationship with husband(r=0.337,P<0.001)were moderately positively correlated with quality of life,and having a reproductive history(r=0.124,P<0.001)was lowly positively correlated with quality of life.Good occupation(r=0.149,P=0.002)was low positively associated with quality of life among the factors of work and living conditions;high annual household income(r=0.307,P<0.001)was moderately positively associated with quality of life.Having health insurance in the policy environment(r=0.131,P<0.001)was low positively associated with quality of life.Conclusion The level of quality of life of infertile patients is impaired to some extent and there are many influencing factors,so healthcare professionals should comprehensively assess the quality of life of patients and adopt targeted and personalised interventions to improve the quality of life of patients.
9.Optimization of feeding and breeding methods for interferon-γknockout mice

Objective The purpose of the study is to breed homozygous interferon-γ knockout(IFN-γ-/-)mice and optimize the breeding strategies to achieve continuous and stable reproduction of IFN-γ-/-mice,which could be used as an ideal animal model for fundamental research.Methods Initially,heterozygous IFN-γ knockout(IFN-γ+/-)C57BL/6J mice were used as the parental generation for breeding.Subsequently,3 breeding strategies were employed using the offspring:(1)female heterozygotes mated with male heterozygotes;(2)male homozygotes mated with female heterozygotes;(3)female homozygotes mated with male homozygotes.The number and survival rate of IFN-γ-/-mice were compared across the three breeding strategies to determine the optimal breeding strategy.Under the optimal strategy,the effects of female mating age and diet type on the reproductive performance of IFN-γ-/-mice were further evaluated.Data from the first three litters of 60 IFN-γ-/-female mice,including litter size,number of weaning survivors,and weaning survival rate,were recorded and analyzed.In addition,the effects of dietary supplementation of pregnant mice and environmental optimization measures,such as the provision of shelters,were evaluated.Results Under conditions where the nutritional needs of pregnant mice were adequately met by supplementation with egg yolk and sunflower seeds,mating of female and male IFN-γ-/-mice result ed in a litter size of five to eight IFN-γ-/-mice,demonstrating higher efficiency compared to other breeding strategies.In addition,diet type and mating age significantly influenced female reproductive performance.When 7~9 weeks old female IFN-γ-/-mice were mated to male IFN-γ-/-mice and fed a high-protein breeding diet,litter size(6.9±1.7),weaning survival number(6.5%±2.0%)and weaning survival rate(93.2%±17.8%)were higher than those under other conditions.In addition,providing shelters to prevent fighting between breeding pairs further improved reproductive outcomes.Conclusions By adopting an optimized breeding strategy,combined with a high-protein diet,nutritional supplementation,and standardized mating age management,the breeding efficiency and stability of IFN-γ-/-mice can be significantly improved.This provides a reliable animal model for related research.
10.Clinical significance of detection of interleukin-6,C reactive protein,and eutrophic-to-lymphocyte ratio alone and in combination in the prognosis assessment of severe pneumonia

Objective To investigate the clinical application value of detection of interleukin-6(IL-6),C reactive protein(CRP),and neutrophic-to-lymphocyte ratio(NLR)alone and in combination in the prognosis assessment of severe pneumonia(SP).Methods A total of 81 patients with SP admitted from December 2022 to December 2023 were selected as the research subjects.According to the discharge outcomes of the patients,they were divided into the survival group(n=62)and the death group(n=19).The general clinical data at admission and laboratory parameters of the two groups were collected and compared.The receiver operating characteristic(ROC)curve was used to analyze the value of serum IL-6,CRP and NLR detection in the prognosis assessment of SP.Results At admission,the pulse of the death group was higher than that of the survival group,while the systolic blood pressure(SBP)was lower than that of the survival group(P<0.05,P<0.01).The incidence of chest tightness in the death group at admission was significant higher than that in the survival group(P<0.05).Compared with the survival group,the number of lymphocyte counts and albumin levels in the death group were lower,while the percentage of neutrophils,NLR and procalcitonin were higher than those in the survival group(P<0.05,P<0.01).ROC curve analysis showed that the area under the curve(AUC)for predicting poor prognosis of SP by the combined detection of IL-6,CRP and NLR was 0.75,which was higher than that predicted by each indicator alone.Conclusion Compared with the detection of IL-6,CRP and NLR alone,the combined detection of the three has a more significant value in predicting the risk of death in patients with SP.It suggests that clinicians should promptly,comprehensively and dynamically monitor the above infection indicators during diagnosis and treatment,and take prompt and effective intervention measures to reduce the mortality rate of patients.
