1.Survey of post-discharge exercise behavior and analysis of factors influencing exercise intensity in patients undergoing lung surgery

Objective To investigate the post-discharge exercise behavior and factors influencing moderate to vigorous intensity physical activity (MVPA) in patients undergoing lung surgery. Methods A total of 2874 patients from the large prospective, observational perioperative lung symptom study cohort (CN-PRO-Lung 3) in the Department of Thoracic Surgery at Sichuan Cancer Hospital between April 7, 2021, and January 31, 2024, were selected as the survey subjects. A survey was conducted using the Investigation of Exercise Behavior after Lung Surgery questionnaire and the International Physical Activity Questionnaire-Short Form (IPAQ-SF) among patients who underwent lung surgery. Binary logistic regression was used to analyze the factors influencing patients’ engagement in MVPA. Results A total of 702 patients were surveyed, including 252 males and 450 females, with an average age of (52.4±10.2) years. Patients with lung cancer accounted for 85.9%. Only 36.0% of the patients had regular exercise habits, while 42.3% did not engage in any physical activity. The three main barriers for postoperative exercise were physical discomfort (pain, coughing, shortness of breath, etc, 54.7%), lack of professional guidance (41.7%), and concerns about the surgical wound (28.9%). The proportions of patients engaging in vigorous, moderate, and low-intensity physical activity were 5.7%, 28.2%, and 66.1%, respectively. Multivariate analysis showed that patients with a personal annual income ≥50000 yuan (OR=1.52, 95%CI 1.01-2.29, P=0.044), high school education or above (OR=1.92, 95%CI 1.33-2.76, P<0.001), and lobectomy (OR=1.44, 95%CI 1.02-2.03, P=0.037) engaged in more MVPA. Conclusion Patients undergoing lung surgery have inadequate physical activity after discharge, particularly lacking in MVPA. Patients with higher income, higher educational levels, and lobectomy are more frequently engaged in MVPA. Measures such as symptom control, providing exercise guidance, and enhancing education on wound care may potentially improve the inadequate physical activity in lung surgery patients after discharge.
2.Application of AI versus Mimics software for three-dimensional reconstruction in thoracoscopic anatomic segmentectomy: A retrospective cohort study

Objective To analyze the application effects of artificial intelligence (AI) software and Mimics software in preoperative three-dimensional (3D) reconstruction for thoracoscopic anatomical pulmonary segmentectomy. Methods A retrospective analysis was conducted on patients who underwent thoracoscopic pulmonary segmentectomy at the Second People's Hospital of Huai'an from October 2019 to March 2024. Patients who underwent AI 3D reconstruction were included in the AI group, those who underwent Mimics 3D reconstruction were included in the Mimics group, and those who did not undergo 3D reconstruction were included in the control group. Perioperative related indicators of each group were compared. Results A total of 168 patients were included, including 73 males and 95 females, aged 25-81 (61.61±10.55) years. There were 79 patients in the AI group, 53 patients in the Mimics group, and 36 patients in the control group. There were no statistical differences in gender, age, smoking history, nodule size, number of lymph node dissection groups, postoperative pathological results, or postoperative complications among the three groups (P>0.05). There were statistical differences in operation time (P<0.001), extubation time (P<0.001), drainage volume (P<0.001), bleeding volume (P<0.001), and postoperative hospital stay (P=0.001) among the three groups. There were no statistical differences in operation time, extubation time, bleeding volume, or postoperative hospital stay between the AI group and the Mimics group (P>0.05). There was no statistical difference in drainage volume between the AI group and the control group (P=0.494), while there were statistical differences in operation time, drainage tube retention time, bleeding volume, and postoperative hospital stay (P<0.05). Conclusion For patients requiring thoracoscopic anatomical pulmonary segmentectomy, preoperative 3D reconstruction and preoperative planning based on 3D images can shorten the operation time, postoperative extubation time and hospital stay, and reduce intraoperative bleeding and postoperative drainage volume compared with reading CT images only. The use of AI software for 3D reconstruction is not inferior to Mimics manual 3D reconstruction in terms of surgical guidance and postoperative recovery, which can reduce the workload of clinicians and is worth promoting.
3.Methacrylated dermal extracellular matrix hydrogel promotes repair of abdominal wall defects

BACKGROUND:Synthetic polymers,such as polypropylene and polyester,used for the treatment of abdominal wall defects not only lack biodegradability and bioactivity but also fail to meet the demands of complex and irregular wounds.Therefore,finding bioactive materials with low immunogenicity and good histocompatibility has become a hot spot in the repair of abdominal wall defects. OBJECTIVE:To prepare methacryloyl modified dermal extracellular matrix hydrogel and explore its potential application in abdominal wall defect. METHODS:(1)The porcine dermis was acellular with 0.25%trypsin and 1%Triton X-100 in turn to obtain the dermal extracellular matrix.After pepsin digestion and methacrylic anhydride modification,the methacrylated dermal extracellular matrix hydrogel was formed by photocrosslinking.The microscopic morphology of the hydrogel was observed by scanning electron microscope,and its rheological properties,swelling properties and other physical and chemical properties were tested.(2)Mice fibroblasts(L929)were inoculated into methacrylated dermal extracellular matrix hydrogel to detect the cell compatibility.(3)Totally 12 SD rats were randomly divided into two groups(n=6)to create abdominal wall defect model with peritoneum preserved.The defect site of the polypropylene group was filled with polypropylene material,and the hydrogel group was filled with methacrylated dermal extracellular matrix hydrogel.The wound skin of both groups was covered with polypropylene material.The wound healing was observed and histological analysis was carried out. RESULTS AND CONCLUSION:(1)Enzymatic hydrolysis had a good decellularization effect on porcine dermis after decellularization,and the original glycosaminoglycans and collagen were well retained.Scanning electron microscope observation revealed that the dermal extracellular matrix hydrogel presented loose and porous structure.The aperture was between 70 and 120 μm.The swelling ratio was(16.88±3.24)%and the water absorption was(94.24±1.11)%.The rheological property test showed that the methacrylated dermal extracellular matrix hydrogel was stable and had shear thinning characteristics,with injectability.(2)CCK-8 assay and live/dead staining showed that methacrylated dermal extracellular matrix hydrogel had good cell compatibility.(3)The results of animal experiments showed that the skin wound healing rate of the experimental group was higher than that of the control group at 7,10,and 14 days after operation(P<0.05).Hematoxylin-eosin and Masson staining of skin and muscle tissue exhibited that compared with the polypropylene group,the skin wound epithelialization,hair follicle formation,collagen fiber arrangement,and neovascularization were better in the hydrogel group 14 days after surgery.The skin wound new tissue structure was similar to the normal tissue at 28 days after surgery,and scar hyperplasia was less.A small amount of muscle regeneration was observed on day 28 after operation.(4)The results show that the methacrylated dermal extracellular matrix hydrogel can promote wound skin healing and muscle tissue regeneration in rats with abdominal wall defect.
4.Mechanism of Ferroptosis in Regulating Chronic Heart Failure and Traditional Chinese Medicine Prevention and Treatment Based on Qi Deficiency and Stagnation: A Review

Chronic heart failure (CHF) is the final stage of cardiovascular diseases. It is a complex syndrome, with dyspnea and edema as the main clinical manifestations, and it is characterized by complex disease conditions, difficult cure, and high mortality. Ferroptosis, a new type of programmed cell death, is different from other types of programmed cell death. Ferroptosis is iron-dependent, accompanied by lipid peroxide accumulation and mitochondrial shrinkage, becoming a hot research topic. Studies have confirmed that ferroptosis plays a key role in the occurrence and development of CHF. The regulation of ferroptosis may become a potential target for the treatment of CHF in the future. The theory of Qi deficiency and stagnation refers to the pathological state of original Qi deficiency and abnormal transportation and distribution of Qi, blood, and body fluid, which has guiding significance for revealing the pathogenesis evolution of some chronic diseases. We believe that Qi deficiency and stagnation is a summary of the pathogenesis of ferroptosis in CHF. Deficiency of Qi (heart Qi) is the root cause of CHF, and stagnation (phlegm turbidity and blood stasis) is the branch of this disease. The two influence each other in a vicious circle to promote the development of this disease. Traditional Chinese medicine (TCM) plays an important role in the treatment of CHF, improving the prognosis and quality of life of CHF patients. This paper explores the correlation between the theory of Qi deficiency and stagnation and the mechanism of ferroptosis in CHF. Furthermore, this paper reviews the mechanism of Chinese medicines and compound prescriptions in preventing and treating CHF by regulating ferroptosis according to the principles of replenishing Qi and dredging to remove stagnation, aiming to provide new ideas and methods for the treatment of CHF with TCM.
5.The value of creeping fat CT imaging combined with serum optimization indicators in the diagnosis of Crohn's disease

Objective To explore the diagnostic value of CT imaging of creeping fat(CF)combined with serum optimization indicators for active Crohn's disease(CD).Methods The data from 68 cases underwent small intestine CT examinations(43 cases in the active CD group and 25 cases in the control group)were collected.CF imaging(non-contrast CT value,enhanced ΔCT value,submucosal fat deposition,and extent of intestinal wrapping)and serum optimization indicators[including neutrophil-to-lymphocyte ratio(NLR),C-reactive protein-to-albumin ratio(CAR),platelet-to-lymphocyte ratio(PLR),platelet-to-albumin ratio(PAR),neutrophil percentage-to-albumin ratio(NPAR),systemic immune-inflammation index(SII),lymphocyte-to-monocyte ratio(LMR),and albumin-to-globulin ratio(AGR)]were evaluated.The correlation between these idictors and CD activity were evaluated.Logistic regression and receiver operating characteristic(ROC)curve analysis were used to assess the diagnostic value of individual and combined indicators for active CD.Results Significant differences were observed in CF imaging and serum optimization indicators between the mild to moderate active CD groups and the control group(P<0.05).The area under the curve(AUC)for non-contrast CT value,arterial phase ΔCT value,venous phase ΔCT value,CF score,NLR,CAR,PLR,PAR,NPAR,SII and combined indicators in predicting moderate active CD were 0.914,0.856,0.923,0.703,0.902,0.880,0.858,0.717,0.802,0.875 and 0.972,respectively.Among these,venous phase ΔCT value and SII exhibited the highest sensitivity(100%),while non-contrast CT value and NPAR demonstrated the highest specificity(87.5%).Venous phase ΔCT value and CAR were identified as significant predictors of moderate active CD(P<0.05).Additionally,serum optimization indicators were significantly correlated with CF imaging(P<0.01).Conclusion CF imaging and serum optimization indicators serve as potential non-invasive markers for diagnosing active CD,with higher diagnostic value achieved through combined assessments.Venous phase ΔCT value and CAR are promising predictors for moderate active CD.
6.Study on the application of dynamic transfer model-based training transfer intervention in standardized training of newly recruited nurses

Objective:To explore the effect of training transfer intervention based on dynamic transfer model (DTM) on standardized training for newly recruited nurses, and to provide reference for the establishment of standardized training transfer intervention strategies for newly recruited nurses.Methods:The quasi-experimental method was used in this study.The convenience sampling method was used to select the newly recruited nurses of Zhengzhou Central Hospital Affiliated to Zhengzhou University as the research subjects.According to the entry time, the newly recruited nurses in July 2020 were selected as the control group ( n=50), and the training and effect evaluation was carried out according to the hospital regulations and "training syllabus for newly recruited nurses (trial)". The newly recruited nurses in July 2021 were taken as the experimental group ( n=60), and on the basis of the control group, the experimental group implemented the DTM training transfer intervention program for training. The scores of personal training transfer effect, general self-efficacy and organizational training transfer atmosphere were compared between the two groups immediately, 3 months and 6 months after training. Results:There were 11 males and 39 females in the control group; aged (24.80 ± 2.11) years old.And there were 14 males and 46 females in the experimental group;aged (24.40 ± 1.75) years old.After training, the total score of transfer effect of individual training in experimental group was (17.73 ± 1.56), (19.47 ± 1.92) and (22.30 ± 1.49) immediately after training, at 3 months and 6 months after training, and (15.84 ± 1.57), (15.98 ± 1.55) and (16.08 ± 1.55) in the control group respectively. The differences were statistically significant ( t=-6.32, -10.36 and -21.42, all P<0.05). Before and after the two groups of training, there were statistically significant differences among groups, within groups, and interactions ( F=137.10, 241.64 and 170.17, all P<0.05). After training, the general self-efficacy scores of the experimental group were (27.10 ± 1.81), (28.03 ± 1.85) and (29.05 ± 1.70) immediately after training, at 3 months and 6 months, respectively, which were higher than (26.28 ± 2.42), (26.36 ± 2.86) and (26.90 ± 2.87) of the control group. The differences were statistically significant ( t=-2.03, -3.95 and -4.87, all P<0.05). Before and after the two groups of training, there were statistically significant differences among groups, within groups and interactions ( F=7.64, 71.26 and 30.25, all P<0.05). After training, the total organizational training transfer atmosphere scores of the experimental group were (39.35 ± 1.96), (50.38 ± 3.17) and (51.47 ± 3.28) immediately after training, respectively, higher than (38.08 ± 2.63), (38.48 ± 2.36) and (38.66 ± 2.27) of the control group. The differences were statistically significant ( t=-2.89, -21.94 and -23.35, all P<0.05), and before and after the two groups of training, there were statistically significant differences among groups, within groups, and interactions ( F=419.75, 261.56 and 207.99, all P<0.05). Conclusions:DTM-based training transfer intervention can improve the effect of personal training transfer, enhance the sense of self-efficacy and create a positive atmosphere of organizational training transfer, and effectively guide new nurses to apply the learning content to clinical practice after standardized training, and improve the effect of training transfer.
7.Epigenetic modification of Igf2/H19 imprinting control region regulates PGC-1α/PI3K/AKT2 pathway in a rat model of intrauterine growth restriction

Background::Intrauterine growth restriction (IUGR) is associated with adverse metabolic outcomes during adulthood. Histone modifications and changes in DNA methylation-affected genes are important for fetal development. This study aimed to investigate the epigenetic mechanisms in IUGR.Methods::IUGR models were established in Sprague–Dawley rats using a maternal nutritional restriction approach during pregnancy. The abundance of insulin-like growth factor 2 (IGF2), phosphoinositide 3-kinase (PI3K), AKT serine/threonine kinase 2 (AKT2), and peroxisome proliferators-activated receptor gamma coactivator 1 alpha (PGC-1α) was examined by real-time polymerase chain reaction (RT-PCR) and Western blotting analysis. Chromatin immunoprecipitation RT-PCR was employed to analyze histone modification in CCCTC-binding factor (CTCF) 1–4 binding sites of the Igf2/H19 imprinting control region (ICR). The methylation states of CTCF1–4 binding sites were studied by pyrosequencing. Results::The IUGR models were constructed successfully. Igf2 mRNA abundance in the placenta, fetal liver, and newborn liver was decreased in the IUGR group ( P <0.01). Meanwhile, as compared with the control group, the expression levels of AKT2, PI3K, and PGC-1α were lower in newborn and 8-week-old livers in the IUGR group ( P <0.05). In addition, knocking down Igf2 reduced the protein expression levels of AKT2-phosphorylation and PGC-1α ( P <0.05). In CTCF binding sites 1-4 of the Igf2/ H19 ICR, acetylated histones H3 (AcH3) enrichment was significantly lower in CTCF1-3 in newborn and 8-week-old IUGR rats. Histone H3 tri-methylated lysine 4 (H3K4me3) enrichment was significantly lower in the CTCF1–4 of newborn and 8-week-old IUGR groups ( P <0.01). H3K9me2 enrichment was significantly higher in the IUGR group ( P <0.01). The CpG dinucleotide methylation levels of CTCF1 and CTCF3, but not those of CTCF2 and CTCF4 binding sites in IUGR rat fetal, 4-week old, and 8-week-old livers decreased significantly ( P <0.05). Conclusion::The methylation status and histone modification in the Igf2/H19 ICR are related to growth and lipid metabolism via the PGC-1α/PI3K/AKT2 pathway in IUGR rats.
8.Trends and Age-Period-Cohort Model Analysis of Inci-dence and Mortality of Hepatitis B Virus-Related Liver Cancer Globally and in China from 1992 to 2021

[Purpose]To analyze the changing trends of the disease burden of liver cancer related to hepatitis B virus(HBV)globally and in China from 1992 to 2021.[Methods]Based on the Global Burden of Disease database in 2021,indicators such as the age-standardized incidence rate and mortality rate of HBV-related liver cancer globally and in China from 1992 to 2021 were collected.The Joinpoint regression model was used to analyze the changing trends of epidemiologi-cal characteristics,and the age-period-cohort model was adopted to analyze the impacts of age,period,and cohort factors on the incidence and mortality risks of HBV-related liver cancer globally and in China.[Results]From 1992 to 2021,the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer globally generally showed a trend of decreasing.The average annual percentage changes were-0.31%and-0.61%,respectively,and all the down-ward trends were statistically significant(both P<0.05).During the same period,the average annu-al percentage changes of the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer in China were-0.45%and-0.90%,respectively,and all the downward trends were statistically significant(both P<0.05).The results of the age-period-cohort model anal-ysis showed that from 1992 to 2021,the annual net drift rates of the incidence of HBV-related liver cancer globally and in China were-0.71%(95%CI:-0.84%~-0.57%)and-0.73%(95%CI:-1.01%~-0.44%),respectively.The annual net drift rates of the mortality were-1.15%(95%CI:-1.28%~-1.02%)and-1.42%(95%CI:-1.69%~-1.14%),respectively,all showing an over-all decline.The age effect showed that the risk of HBV-related liver cancer incidence in both the global and Chinese populations began to increase after 30 years of age,peaking in the 70~74 age group,while the risk of mortality surged after 40 years of age and peaked in the population aged 80 and above.The period effect indicated that the incidence and mortality risks were the highest from 1997 to 2001 and the lowest from 2017 to 2021.The cohort effect revealed that the inci-dence and mortality risks gradually decreased in populations born after 1962,with the 2007-2011 birth cohort having the lowest risks.The results of the Wald x2 test showed that there were statistically significant differences in the changing trends of the age,period,and cohort effects on the incidence and mortality of HBV-related liver cancer globally and in China(all P<0.05).[Con-clusion]From 1992 to 2021,the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer showed a downward trend both globally and in China.The disease burden of HBV-related liver cancer in China was higher than the global level.The age-period-cohort model has revealed the historical changes in the incidence and mortality of HBV-related liver cancer.The current and future situation of the disease burden of HBV-related liver cancer is not optimistic.It is recommended to implement precise stratified interventions for populations of different ages,periods,and birth cohorts,and actively transform the prevention,treatment,and management strategies for HBV-related liver cancer.
9.Indications for prenatal diagnosis using copy number variation-sequencing and detection of abnormalities: a retrospective analysis of 17 994 cases

Objective:To investigate the indications for prenatal diagnosis using copy number variation-sequencing (CNV-seq) and the abnormalities detected by the method.Methods:This retrospective analysis involved 17 994 singleton pregnant women who underwent prenatal CNV-seq at the First Affiliated Hospital of Zhengzhou University from January 2019 to December 2022. These cases were divided into five groups based on the following indications for CNV-seq: abnormal fetal ultrasound findings, high-risk results indicated by non-invasive prenatal testing (NIPT) or Down's syndrome serological screening (Down's screening), adverse pregnancy history, and advanced maternal age. The proportions of cases with the indications for prenatal CNV-seq, the detection rates of abnormalities (numerical abnormalities of chromosomes, pathogenic/likely pathogenic CNV in structural abnormalities) in the five groups, and the distribution of these abnormalities were analyzed. Statistical analysis was performed using Chi-square test. Results:Among the 17 994 pregnant women, the women with abnormal fetal ultrasound findings, high-risk NIPT results, high-risk Down's screening results, adverse pregnancy history, and advanced maternal age accounted for 32.65% (5 875/17 994), 11.90% (2 142/17 994), 31.62% (5 690/17 994), 11.70% (2 105/17 994), and 12.13% (2 182/17 994), respectively. The detection rates of abnormalities in the five groups were 10.60% (623/5 875), 34.64% (742/2 142), 4.69% (267/5 690), 2.99% (63/2 105), and 3.67% (80/2 182), respectively. The overall detection rate of abnormalities was 9.86% (1 775/17 994). The cases with numerical abnormalities of chromosomes accounted for 68.79% (1 221/1 775), trisomy 21 was predominant (49.30%, 602/1 221). Chromosomal structural abnormalities were detected in 31.21% (554/1 775) of the cases with abnormalities, with 57.76% (320/554) harboring pathogenic CNVs and 42.24% (234/554) harboring likely pathogenic CNVs. The detection rate of chromosomal numerical abnormalities was higher than that of structural abnormalities in the abnormal fetal ultrasound group, NIPT high-risk group, and advanced maternal age group [6.81% (400/5 875) vs. 3.80% (223/5 875), χ2=53.10; 27.96% (599/2 142) vs. 6.68% (143/2 142), χ2=338.40; 2.43% (53/2 182) vs. 1.24% (27/2 182), χ2=8.61; all P<0.01]. A total of 416 microdeletions and 255 microduplications were detected in the 554 cases. The top three regions with the highest frequencies in microdeletions were Xp22.31 (12.74%, 53/416), 22q11.21 (7.93%, 33/416), and 17q12 (5.77%, 24/416); in microduplications, they were 22q11.21 (14.90%, 38/255), 17q12 (3.53%, 9/255), and 7q11.23 (3.53%, 9/255). Conclusions:Abnormal fetal ultrasound findings accounted for the highest proportion of prenatal diagnostic indications. The overall detection rate of abnormalities by CNV-seq is relatively high, especially in those with high-risk NIPT results as an indication for prenatal diagnosis. Among the chromosomal structural abnormalities detected in this study, the frequencies of Xp22.31 microdeletion and 22q11.21 microduplication are higher.

OBJECTIVE To explore the risk factors for the recurrent asthma in children with bronchopneumonia un-der 6 years of age in Fuyang City.METHODS A total of 368 children with bronchopneumonia who were less than 6 years old and treated in Fuyan Cancer Hospital from Jan.2021 to Jan.2023 were recruited as the research sub-jects and were divided into the asthma group with 130 cases and non-asthma group with 238 cases according to the status of recurrent asthma.The characteristics of seasonal distribution of the bronchopneumonia were analyzed.The predictive factors were screened out by the least absolute shrinkage and selection operator(LASSO)regression analysis,and multivariate logistic regression analysis was performed for the risk factors for the recur-rent asthma in children.RESULTS There was significant difference in the number of cases among the seasons(P<0.05),and it was most prevalent in winter.Three non-zero predictive factors were screened out by LASSO regres-sion analysis.The result of multivariate logistic regression analysis showed that family history of asthma,contact with dust or pollen and weather change were risk factors for the recurrent asthma in children with bronchopneu-monia(P<0.05).CONCLUSIONS The change of temperature makes the probability of bronchopneumonia display a remarkable feature of seasonal distribution,which is highly prevalent in cold winter and less prevalent in summer.The family history of asthma,contact with dust or pollen and weather change are the major factors for the recurrent asthma in children with bronchopneumonia.It is of great significance to take targeted prevention and control measures based on the above risk factors so as to reduce the risk of recurrent asthma in children with bron-chopneumonia.
