1.Analysis of patch test in 186 patients with rosacea

Objective:To analyze patch test results in patients with rosacea, investigate the prevalence of contact sensitization, and identify common allergens, to provide relevant evidence for the diagnosis, treatment, and preventive interventions for rosacea.Methods:A cross-sectional retrospective survey was conducted to analyze the positive contact allergy rate and allergen distribution in 186 rosacea patients who underwent the 60-item China Standard Series Patch Test (CB-1000) at the Department of Dermatology, Peking University First Hospital, from January 2023 to September 2024.Results:Among the 186 patients, 159(85.5%) showed positive reactions in patch tests. The top three allergens with the highest positivity rates were: metal salts, preservatives and fragrances. The top 10 individual allergens with the highest positivity rates were: nickelsulfate hexahydrate 46.8%(87/186), cobaltchloride hexahydrate 29.0%(54/186), hydroperoxides of linalool 15.1%(28/186), carba mix 14.0%(26/186), propolis 9.7%(18/186), fragrance mix Ⅰ 8.6%(16/186), methyl dibromo glutaronitrile 8.1%(15/186), Peru balsam 7.5%(14/186), gold sodium thiosulfate dihydrate 7.5%(14/186), and hydroperoxides of limonene 7.5%(14/186). The overall positivity rate for cosmetics-related allergens was 56.5% (105/186), with the top five allergens being: hydroperoxide of linalool 15.1%(28/186), propolis 9.7%(18/186), fragrance mix Ⅰ 8.6%(16/186), methyl dibromo glutaronitrile 8.1%(15/186), and Peru balsam 7.5%(14/186).Conclusion:The overall positivity rate for patch test allergens in rosacea patients is high. The most common allergen types are metal salts, preservatives, and fragrance agents. Cosmetics-related allergens also show a high positivity rate. Rosacea patients should avoid exposure to these allergens.
2.Analysis of the current status of skin contact allergy of wearable devices

In recent years, the wearable device market has been developing rapidly. Wearable devices with personalized health management and chronic disease monitoring are widely used in daily life by virtue of their powerful performance and convenience. However, with the popularization of these devices, skin adverse reactions caused by prolonged wearable wear are gradually increasing, among which allergic contact dermatitis (ACD) is the most common. Common allergens such as acrylates, methacrylates, rosin, chromium and nickel are widely present in adhesives and device components and are the main causes of ACD. Understanding the presence of potential sensitizers in wearable devices can help in diagnostic patch testing in users experiencing skin reactions caused by the device. Future innovations in wearable device materials will focus on the adoption of safer bonding technologies and biocompatible materials to reduce the risk of allergy. The introduction of new materials brings both development opportunities and challenges. Educating users on proper device usage, identifying specific allergens, selecting hypoallergenic materials, and optimizing device maintenance are important for reducing the risk of ACD.
3.Acupuncture at "four pharyngeal points" combined with Changma Xifeng tablets for simple vocal tics with liver hyperactivity disturbed wind in children: a randomized controlled trial.

OBJECTIVE:
To observe the effect of acupuncture at "four pharyngeal points" on simple vocal tics with liver hyperactivity disturbed wind in children.
METHODS:
Sixty children with simple vocal tics of liver hyperactivity disturbed wind were randomly divided into an observation group (30 cases, 1 case dropped out) and a control group (30 cases). The control group was given Changma Xifeng tablets orally, 3 times a day, while the observation group was treated with acupuncture at "four pharyngeal points" on the basis of the treatment in the control group, 15-20 min a time, once daily for 7 days, with a 3-day break. Both groups were treated for 3 months. The TCM syndrome score and Yale global tic severity scale (YGTSS) score of the two groups were observed before treatment and after 1, 2, 3 months of treatment, the disappearance time of simple vocal tics was recorded, and the therapeutic efficacy was evaluated after treatment.
RESULTS:
After 1, 2, 3 months of treatment, the TCM syndrome scores and YGTSS scores of the two groups were decreased compared with those before treatment (P<0.01, P<0.05), and the scores of the observation group were lower than those in the control group (P<0.05, P<0.01). The disappearance time of simple vocal tics in the observation group was earlier than that in the control group (P<0.05). The effective rate of the observation group was 93.1% (27/29), which was higher than 73.3% (22/30) in the control group (P<0.05).
CONCLUSION
Acupuncture at "four pharyngeal points" could improve symptoms in children with simple vocal tics of liver hyperactivity disturbed wind, and shorten the disappearance time of simple vocal tics.

4.Value of preoperative abdominal CT-based scoring system for predicting difficulty in laparoscopic cholecystectomy

Objective:To explore the value of a scoring system based on preoperative abdominal computed tomography (CT) for predicting the difficulty of laparoscopic cholecystectomy (LC).Methods:A retrospective analysis was conducted on 105 patients diagnosed with gallstones or cholecystitis who underwent LC at Beijing Jishuitan Hospital, Capital Medical University from January 2021 and February 2022. Based on surgical video reviews, patients were divided into the easy group ( n=58) and the difficult group ( n=47) according to the intraoperative Parkland Grading Scale (PGS), with PGS grades 1-2 assigned to the easy group, and PGS grades 3-5 assigned to the difficult group. The normally distributed measurement data were expressed as mean±standard deviation ( ± s), and compared using independent samples t-test; the non-normally distributed measurement data were expressed as median (interquartile range) [ M ( Q1, Q3)], and compared using the rank-sum test. The count data were expressed as the number of cases and percentage, and compared using the Chi-square test or Fisher exact probability method. Univariate analysis and cut-off value determination: for continuous CT variables, univariate Logistic regression and stepwise regression analyses (with surgical difficulty grouping as the dependent variable) were performed to identify the optimal combination of predictive variables and establish a scoring system. For each significantly associated continuous variable or important CT image feature from a clinical perspective, receiver operating characteristic (ROC) curve analysis was used to evaluate its predictive performance for difficult surgery. The area under the curve (AUC) was calculated, and the optimal cut-off value was determined using the Youden index to maximize the sum of sensitivity and specificity. The categorical CT image features were scored according to their original groups. The Kappa consistency test was used to assess the strength of agreement between the preoperative abdominal CT score (grouped by the optimal cut-off) and PGS grades (easy/difficult). Decision curve analysis (DCA) was employed to validate the predictive performance of the model. Results:Stepwise Logistic regression identified seven key imaging features as the optimal predictive variables for constructing the preoperative abdominal CT scoring system: maximum gallbladder cross-sectional diameter, maximum gallbladder cross-sectional width, gallbladder wall thickness, common bile duct diameter, pericholecystic fat stranding, periductal fat stranding, and impacted cystic duct stones. Each case was scored after assigning scores based on the optimal cut-off values. The total score of the preoperative abdominal CT scoring system was ≥3 points predicted difficult LC with an AUC of 0.745 (95% CI: 0.650-0.839), sensitivity of 66.0%, and specificity of 75.9%. DCA confirmed the model′s reliable predictive performance, and the preoperative abdominal CT scoring system showed good agreement with PGS grades ( Kappa value was 0.420, P<0.001). Conclusions:The preoperative abdominal CT scoring system based on pericholecystic imaging features can effectively predict the difficulty of LC with good discriminative ability. It provides a quantitative tool for preoperative assessment, surgical scheduling, and ambulatory surgery management.
5.Application progress of machine learning in study on cold and hot properties of Chinese materia medica

The scientific interpretation of the theory of medicinal properties of TCM is a research hotspot in the modernization of TCM. It is of great value to clarify the property and degree of cold and heat in Chinese materia medica for guiding clinical precise medication. In recent years, the research on the cold and heat properties of Chinese materia medica has been carried out at the animal, cell and molecular levels. Based on the objective material basis of medicinal properties, from the perspective of biological effects such as thermodynamics and multiomics; with the help of infrared thermal imaging and other technologies for analysis; forming a variety of research models such as "property-structure relationship". Related research has developed from a single material component or index to a new model that tends to integrate multi-source information and multi-dimensional data. However, how to deal with the problems of large sample size, strong redundancy, high heterogeneity, and how to integrate multi-dimensional information are still research difficulties. With its powerful computing and learning ability, machine learning can show good discrimination and prediction ability in the study of cold and hot properties of Chinese materia medica, and play an important role in the study of cold and hot properties of Chinese materia medica. At present, the most widely used algorithms are linear discriminant analysis, Logistic discriminant analysis, support vector machine, decision tree, random forest and so on. The data dimension of the existing research needs to be enriched, the algorithm has room for further optimization, and a more detailed discriminant model of cold and hot properties of Chinese materia medica needs to be established.

8.Long non-coding RNA AW112010 improves insulin resistance in adipocytes of aging mice through the miR-204/POU2F2 axis

Objective:To investigate whether long non-coding RNA(lncRNA) AW112010 can improve insulin resistance in aging adipocytes through the miR-204/POU2F2 signaling pathway.Methods:In vivo experiment: C57BL/6 mice were divided into young control group(4 months old) and aging model group(18 months old) based on body weight. The expression levels of AW112010, miR-204-5p, POU2F2, aging related indicators(p16, p21), and insulin signaling pathway genes [insulin receptor(INSR), insulin receptor substrate 1(IRS1), phosphatidylinositol kinase(PI3K), protein kinase B(AKT)] in epididymal adipose tissue were detected using real-time fluorescence quantitative PCR(RT-qPCR) and Western blotting. In vitro experiment: Using adriamycin(ADR) to induce 3T3-L1 aging adipocyte model, β-gal staining was used to observe cellular senescence, and miR-204 inhibitor and miR-204 mimic small interfering RNA were successfully constructed and transfected into 3T3-L1 adipocytes. Results:RT-qPCR and Western blot results showed that compared with the young group, the expression of AW112010 in the adipose tissue of aging mice was increased, while the expression of miR-204-5p was decreased. The expressions of POU2F2, p16, and p21 in the adipose tissue of aging mice were increased, while the expressions of INSR, IRS1, PI3K, GLUT4 mRNA and protein were decreased. The β-gal stainging results showed that the number of 3T3-L1 senescent adipocytes induced by ADR was significantly increased, and the expression levels of AW112010, POU2F2, p16, and p21 in ADR-induced senescent adipocytes were increased compared with the control group, while the expression levels of miR-204-5p, INSR, IRS1, PI3K, GLUT4 were decreased, and remaining glucose in the culture medium was increased. Compared with control, overexpression of miR-204 resulted in decreased expressions of aging indicators p16, p21, and target gene POU2F2 while the expressions of INSR and GLUT4 were increased.Conclusion:Upregulation of lncRNA AW112010 in adipocytes of aging mice may induce insulin resistance by targeting miR-204-5p/POU2F2/IRS1.

Postoperative pain seriously affects the recovery process of patients, resulting in prolonged hospital stay and increased care costs. Appropriate application of patient-controlled analgesia devices can effectively relieve perioperative acute pain. In 1994 patient-controlled analgesia began to be used in Peking Union Medical College Hospital, and the Acute Pain Service Working Group was established in 2004. With the cooperation of anesthesiologists and specialist nurses, the group jointly has implemented the whole process and standardized management based on patient-controlled analgesia, and constantly improved and innovated working methods, laying a solid foundation for the development of postoperative pain management. This paper systematically reviews and summarizes the work from the aspects of clinical focus, nursing management experience, promotion and dissemination of pain treatment concepts, and development of acute pain service model under the new situation, with the hope of providing valuable reference for comprehensively strengthening pain management in the process of diagnosis and treatment, and enhancing patients' satisfaction with perioperative analgesia services.
10.Cell softness reveals tumorigenic potential via ITGB8/AKT/glycolysis signaling in a mice model of orthotopic bladder cancer

Background::Bladder cancer, characterized by a high potential of tumor recurrence, has high lifelong monitoring and treatment costs. To date, tumor cells with intrinsic softness have been identified to function as cancer stem cells in several cancer types. Nonetheless, the existence of soft tumor cells in bladder tumors remains elusive. Thus, our study aimed to develop a microbarrier microfluidic chip to efficiently isolate deformable tumor cells from distinct types of bladder cancer cells.Methods::The stiffness of bladder cancer cells was determined by atomic force microscopy (AFM). The modified microfluidic chip was utilized to separate soft cells, and the 3D Matrigel culture system was to maintain the softness of tumor cells. Expression patterns of integrin β8 (ITGB8), protein kinase B (AKT), and mammalian target of rapamycin (mTOR) were determined by Western blotting. Double immunostaining was conducted to examine the interaction between F-actin and tripartite motif containing 59 (TRIM59). The stem-cell-like characteristics of soft cells were explored by colony formation assay and in vivo studies upon xenografted tumor models. Results::Using our newly designed microfluidic approach, we identified a small fraction of soft tumor cells in bladder cancer cells. More importantly, the existence of soft tumor cells was confirmed in clinical human bladder cancer specimens, in which the number of soft tumor cells was associated with tumor relapse. Furthermore, we demonstrated that the biomechanical stimuli arising from 3D Matrigel activated the F-actin/ITGB8/TRIM59/AKT/mTOR/glycolysis pathways to enhance the softness and tumorigenic capacity of tumor cells. Simultaneously, we detected a remarkable up-regulation in ITGB8, TRIM59, and phospho-AKT in clinical bladder recurrent tumors compared with their non-recurrent counterparts.Conclusions::The ITGB8/TRIM59/AKT/mTOR/glycolysis axis plays a crucial role in modulating tumor softness and stemness. Meanwhile, the soft tumor cells become more sensitive to chemotherapy after stiffening, that offers new insights for hampering tumor progression and recurrence.
