1.Preoperative prediction of lymphovascular invasion in breast cancer based on multimodal radiomics model combining MRI and digital mammography

Objective To investigate the value of multimodal model integrating digital mammography(MG)and MRI radiomics features for preoperative prediction of lymphovascular invasion(LVI)status in breast cancer.Methods The clinical and imaging data from 336 patients with pathologically confirmed invasive breast cancer were retrospectively analyzed and randomly divided into a training group(235 cases)and a test group(101 cases)according to the ratio of 7∶3.Feature dimensionality reduction was carried out by Pearson correlation analysis followed by least absolute shrinkage and selection operator(LASSO)regression.Radiomics models were constructed based on MG craniocaudal(CC),dynamic contrast enhancement(DCE),T2 WI,and integrated MRI sequences;a multimodal model was further developed by incorporating clinical high-risk factors.The predictive efficiency of each model was evaluated by plotting receiver operating characteristic(ROC)curve.Results The ROC curve analysis showed that the multimodal model performed the best predictive efficiency,with area under the curve(AUC)of 0.989 and 0.861,accuracy of 0.949 and 0.782,sensitivity of 0.923 and 0.828,and specificity of 0.962 and 0.764 in the training group and test group respectively.Conclusion The multimodal model,integrating MG and MRI radiomics features,show optimal performance and can be served as a preoperative prediction of LVI status in breast cancer.
2.Relationship between individual arsenic exposure and hypertension of Tibetan residents in Nyingchi City, Tibet Autonomous Region

Objective:To investigate the relationship between individual arsenic exposure and hypertension of Tibetan residents in Nyingchi City, Tibet Autonomous Region (referred to as Tibet).Methods:From June to August 2023, Tibetan residents aged 18 to 78 years who had resided in Bomi County and Chayu County of Nyingchi City, Tibet for at least 10 years were selected as the study subjects by cluster sampling. Questionnaire survey, physical examination, and biochemical index testing were conducted on the study subjects. Meanwhile, hair samples were collected to determine the arsenic content. Binary logistic regression was used to analyze the influencing factors of hypertension.Results:A total of 447 Tibetan residents were included, with a hair arsenic content of (1.24 ± 0.02) mg/kg, including 155 hypertensive patients (34.68%). There were statistically significant differences in age, blood oxygen saturation, neck circumference, waist hip ratio, hemoglobin, hair arsenic, blood glucose, uric acid, and smoking status between the hypertension and normal blood pressure groups ( P < 0.05). The results of binary logistic regression analysis showed that age ( OR = 1.05, 95% CI: 1.03 - 1.07), uric acid ( OR = 1.00, 95% CI: 1.00 - 1.01), hair arsenic ( OR = 1.75, 95% CI: 1.07 - 2.84), and smoking ( OR = 2.22, 95% CI: 1.05 - 4.69) were risk factors for hypertension. Conclusion:The hair arsenic content is a risk factor for hypertension of Tibetan residents in Nyingchi City, Tibet.

5.Relationship between mean platelet volume/lymphocyte ratio and left atrial thrombosis in elderly patients with nonvalvular atrial fibrillation

Objective To investigate the relationship between mean platelet volume/lymphocyte ratio(MPVLR)and left atrial thrombosis in elderly patients with non-valvular atrial fibrillation(NVAF).Methods A total of 178 elderly patients with NVAF admitted to the hospital from January 2019 to December 2022 were enrolled in the study.The patients were divided into thrombosis group(28 cases)and non-throm-bosis group(150 cases)according to the left atrial thrombosis judged by using esophageal echocardiography(TEE).The white blood cell count(WBC),red blood cell count(RBC),lymphocyte count,lymphocyte pro-portion,platelet count(PLT)and mean platelet volume(MPV)were detected by automatic blood cell analy-zer,and MPVLR was calculated.The liver and kidney function indicators and blood lipid indicators were detec-ted by automatic biochemical analyzer.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of MPV,lymphocyte ratio and MPVLR for left atrial thrombosis in NVAF patients.Multiva-riate Logistic regression was used to analyze the influencing factors of left atrial thrombosis in elderly NVAF patients.Results MPV,lymphocyte proportion and MPVLR in the thrombosis group were higher than those in the non-thrombosis group,and the differences were statistically significant(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of MPV,lymphocyte ratio and MPVLR for predicting left atrial thrombosis in NVAF patients were 0.821(95%CI:0.764-0.882),0.771(95%CI:0.714-0.842)and 0.901(95%CI:0.861-0.949).respectively.The course of disease in the thrombosis group was longer than that in the non-thrombosis group,the proportion of patients with chronic heart failure,the proportion of patients with stroke,CH A2DS2-VASc score,LAEF,LAD,LVEDV,MPVLR,serum uric acid,MPV,lymphocyte proportion and MPVLR were higher than those in the non-thrombosis group,and LVEF was lower than that in the non-thrombosis group,the differences were statistically significant(P<0.05).Multivariate Logistic regression a-nalysis showed that disease duration ≥1.93 years(OR=3.050,95%CI:1.928-4.824),chronic heart failure(OR=3.333,95%CI:1.808-6.144),MPVLR≥3.10(OR=3.873,95%CI:1.734-8.650)were independ-ent risk factors for left atrial thrombosis in elderly NVAF patients(P<0.05).Conclusion The increase of MPVLR is associated with left atrial thrombosis in elderly patients with NVAF,and it can be used as a an in-dicator to predict left atrial thrombosis in patients with NVAF.
6.A scoping review of application of blood flow restriction training in rehabilitation of patients undergoing anterior cruciate ligament reconstruction

Objective:To review the scope of research on the application of blood flow restriction training in the rehabilitation of patients undergoing anterior cruciate ligament (ACL) reconstruction.Methods:According to the research method of scoping review, computer searches were conducted on China National Knowledge Infrastructure, Wanfang Database, China Biology Medicine disc, VIP, Cochrane Library, PubMed, Embase, with the search period was from eatablishment of databases to December 31, 2022. The included literatures were screened, summarized and analyzed.Results:A total of 16 literatures were included, all of which were randomized controlled trials. The basic content of blood flow restriction training intervention included 8 aspects, such as intervention time, training intensity, training amount, training mode, frequency, interval time, training cycle and blood flow restriction pressure. The effect evaluation mainly involved two kinds of indexes, namely safety indicators and efficacy indicators.Conclusions:Blood flow restriction training can effectively enhance muscle strength and improve knee joint function in patients undergoing ACL reconstruction. Future studies should focus on exploring the best intervention strategy, formulating standardized and unified evaluation criteria and providing the best blood flow restriction training program for patients undergoing ACL reconstruction.
