Objective:To analyze the predictive factors associated with late-onset sepsis(LOS) in very low birth weight infants,and to develop a risk prediction score scale applicable to these infants three days postnatal.This will provide valuable insights for early diagnosis and timely intervention.Methods:Very low birth weight infants admitted to the Children's Hospital of Fudan University from January 1,2022,to June 30,2024,were selected as research subjects.These infants were categorized into two groups:the LOS group and the non-LOS group,based on whether they developed LOS.LASSO regression analysis,alongside univariate and multivariate regression analyses,was employed to identify predictive factors for LOS in this population.A Logistic model was constructed using the optimal combination of predictive variables,and a risk assessment scale was subsequently developed.The prediction performance of the model was evaluated using the Hosmer-Lemeshow chi-square test and the receiver operating characteristic curve.Results:A total of 444 cases of very low birth weight infants were included,of which 185 had LOS and 259 did not.After screening the variables,seven independent factors were included into the model:birth weight,gestational age,tracheal intubation,abnormal skin color,abdominal distension,elevated C-reactive protein levels,and right hand perfusion index.A predictive scoring scale was developed based on the regression coefficients of each variable,with corresponding risk scores assigned as follows:1,4,3,2,1,1,and 2; a score of ≥3.5 indicated high-risk groups.The Hosmer-Lemeshow test results demonstrated that χ2 = 7.602( P = 0.473).The area under the receiver operating characteristic curve was 0.792 ( P<0.001),with a sensitivity of 73.5% and specificity of 71.0%. Conclusion:The risk score scale developed in this study exhibits significant predictive capability,providing valuable insights for clinical medical personnel to assess the risk of LOS in very low birth weight infants during the early postnatal period.

Objective:Exploring the role and clinical value of tRNA methyltransferase 61A(TRMT61A)expression in head and neck squam-ous cell carcinoma(HNSCC).Methods:This study analyzed the expression levels of TRMT61A across various cancer tissues and specifically in HNSCC tissues compared to normal head and neck tissues using the Cancer Genome Atlas(TCGA)database.The expression level and pro-gnostic characteristics of TRMT61A in patients with HNSCC were investigated.Furthermore,the proliferation and colony-formation ability of an HNSCC cell line following TRMT61A knockdown was assessed using Western blot,cell counting kit-8(CCK-8),and colony-formation assays.Results:TRMT61A was highly expressed in HNSCC tissues,and its elevated expression significantly correlated with reduced overall survival(OS),progression-free survival(PFS),and disease-specific survival(DSS)in patients.In vitro experiments demonstrated that knocking down TRMT61A suppressed proliferation and the colony-forming ability of HNSCC cells.Conclusions:TRMT61A plays an important role in tumor progression in HNSCC.Targeting TRMT61A expression is a promising approach to affect tumor cell proliferation in HNSCC and is expected to become an effective prognostic indicator for patients with HNSCC.Thus,TRMT61A is a potential therapeutic target for the treatment of HNSCC.

Objective:To investigate the efficacy and safety of endoscopic ligation and injection sclerotherapy for internal hemorrhoids.Methods:From April 2020 to October 2023, 106 patients with grade Ⅰ to Ⅲ internal hemorrhoids who underwent endoscopic ligation or injection sclerotherapy in the First Affiliated Hospital with Nanjing Medical University (Jiangsu Province People's Hospital) were consecutively enrolled in the retrospective cohort study. Patients were divided into the ligation group ( n=70) and the injection sclerotherapy group ( n=36) according to the endoscopic treatment methods. The effective rate, hospital stay, postoperative pain, the incidence of complications, patient satisfaction and the recurrence rate of the two groups were assessed. Results:The effective rate was 95.7% (67/70) and 100.0% (36/36) respectively in the ligation group and the injection sclerotherapy group without significant difference ( P=0.549). The hospital stay was 6.0 (4.0, 7.3) days and 3.0 (2.0, 4.0) days respectively in the ligation group and the injection sclerotherapy group with significant difference ( Z=4.305, P<0.001). The incidence of 24-hour postoperative pain was 58.6% (41/70) and 11.1% (4/36) in the two groups respectively and the difference was statistically significant ( χ2=20.020, P<0.001). There were no serious postoperative adverse events such as anal stenosis or thrombotic external hemorrhoids in the two groups. Only the incidence of anal distension was significantly different between the ligation group and the injection sclerotherapy group [38.6% (27/70) VS 5.6% (2/36), χ2=11.431, P<0.001]. The patient satisfaction rate of the two groups showed no significant difference [87.1% (61/70) VS 86.1% (31/36), P=1.000]. There was 1 patient and 15 patients lost to follow-up respectively in the ligation group and the injection sclerotherapy group, and the recurrence rate of the two groups showed no significant difference [14.5% (10/69) VS 33.3% (7/21), P=0.064]. Subgroup analysis of grade Ⅰ-Ⅱ internal hemorrhoids: there were 43 patients and 29 patients in the ligation group and the injection sclerotherapy group, respectively. No significant difference was found between the two groups in terms of the effective rate, incidence of postoperative hemorrhage, incidence of uroschesis, or patient satisfaction rate ( P>0.05). The incidences of postoperative pain [65.1% (28/43) VS 10.3% (3/29), χ 2=19.016, P<0.001] and anal distension [41.9% (18/43) VS 3.4% (1/29), χ 2=11.252, P<0.001] of the ligation group were significantly higher than those of the injection sclerotherapy group. Forty-three patients in the ligation group and 17 patients in the injection sclerotherapy group completed follow-up, and the recurrence rate was 11.6% (5/43) and 23.5% (4/17), respectively, without significant difference ( P=0.256). Subgroup analysis of grade Ⅲ internal hemorrhoids: there were 27 patients in the ligation group and 7 patients in the injection sclerotherapy group, and 26 and 4 patients completed follow-up, with a recurrence rate of 19.2% (5/26) and 75.0% (3/4), respectively. Conclusion:Endoscopic ligation and injection sclerotherapy are both safe and effective for grade Ⅰ-Ⅲ internal hemorrhoids. Injection sclerotherapy demonstrates shorter hospital stays and lower incidences of postoperative pain and anal distension. For grade Ⅰ-Ⅱ internal hemorrhoids, injection sclerotherapy is recommended due to its efficacy and fewer complications. Conversely, for grade Ⅲ internal hemorrhoids, ligation is suggested to reduce recurrence rates.

BACKGROUND/AIMS: The functional variant (rs56109847) in the 3'-untranslated regions (3'-UTR) of the serotonin receptor 3E (HTR3E) gene is associated with female diarrhea predominant irritable bowel syndrome (IBS-D) in British populations. However, the relationship of the polymorphism both to HTR3E expression in the intestine and to the occurrence of Chinese functional gastrointestinal disorders has yet to be examined. METHODS: Polymerase chain reaction amplification and restriction fragment length polymorphism analyses were employed to detect polymorphisms among Chinese Han women, particularly 107 patients with IBS-D, 99 patients with functional dyspepsia (FD), 115 patients with mixed IBS and 69 patients with IBS-D + FD. We also assessed microRNA-510 (miR-510) and HTR3E expression in human colonic mucosal tissues with immunohistochemistry and other methods. Dual-luciferase reporter assays were conducted to examine the binding ability of miR-510 and HTR3E 3'-UTR. RESULTS: Genotyping data showed the variant rs56109847 was significantly associated with IBS-D, but not with FD, mixed-IBS, or FD + IBS-D. HTR3E was abundantly expressed around the colonic mucosal glands but less expressed in the stroma. miR-510 expression decreased, whereas HTR3E expression increased in the colonic mucosal tissue of patients with IBS-D compared with those in controls. HTR3E expression was significantly higher in patients with the GA genotype than that in patients with the GG genotype. The single-nucleotide polymorphisms disrupted the binding site of miR-510 and significantly upregulated luciferase expression in HEK293 and HT-29 cells. CONCLUSIONS: The single-nucleotide polymorphisms rs56109847 led to reduced microRNA binding and overexpression of the target gene in intestinal cells, thereby increasing IBS-D risk in the Chinese Han population. The decreased expression of miR-510 might contribute to IBS-D.

Objective To observe the expression of discs large hom olog 4 (DLG4) protein in hippocam-pus, am ygdala and frontal cortex of rats and evaluate postsynaptic density in heroin dependence. Meth-ods The rat heroin dependent m odel was established by increasing intraperitoneal injection of heroin. DLG4 proteins in hippocam pus, am ygdala and frontal cortex of heroin dependent 9, 18, 36 days rats w ere detected with im munohistochem ical staining and com pared with that in the control group. Results DLG4 proteins in hippocam pus, am ygdala and frontal cortex w ere gradually reduced with extension of heroin dependent tim e. Conclusion Heroin dependence can affect postsynaptic density of hippocam pus, am ygdala and frontal cortex. The changes becom e m ore apparent with extension of heroin dependence tim e.

Objective To investigate the role and mechanism of simvastatin on colonic fibrosis in rats with 2,4,6-trinitrobenzene sulphonic acid (TNBS) induced colitis.Methods Forty-eight healthy male Sprague-Dawley rats were evenly divided into six groups:control group,TNBS group,simvastatin treated group Ⅰ,group Ⅱ (from zero to 21 days after modeling,simvastatin 5 mg/kg or simvastatin 20 mg/kg treated),group Ⅲ and group Ⅳ (from seven to 21 days after modeling,simvastatin 5 mg/kg or 20 mg/kg treated).Body weight and disease activity index (DAI) of the rats were inspected,and general colon,histological injury and fibrosis were scored.The expressions of collagen types Ⅰ and connective tissue growth factor (CTGF) at mRNA level were detected by reverse transcription-polymerase chain reaction (RT-PCR).The expressions of collagen types Ⅰ,CTGF and phosphorylation of myosin phosphatase target subunit-1 (p-MYPT-1) at protein level were determined by Western blotting.The data were analyzed by one-way ANOVA.Results Compared with control group,the colon length shortened,while colon weight,DAI score,general colon score,histological injury and fibrosis score significantly increased in TNBS group.And the expressions of collagen types Ⅰ also obviously increased.After intervention of simvastatin,both the colon length and weight of rats were improved.The DAI score,general score,histological injury and fibrosis score were lower than those of TNBS group.The expressions of collagen types Ⅰ,CTGF and p-MYPT-1 (group Ⅰ:0.68±0.22 ; group Ⅱ:0.59 ± 0.27 ; group Ⅲ:0.71 ± 0.20 ; group Ⅳ:0.59± 0.25) in colonic tissue were all lower than those of TNBS group (F=5.169,P＜0.05).There were no statistical significance among four groups (al1 P＞0.05).Conclusion Simvastatin can effectively prevent TNBS-induced rat colitis from colonic fibrosis,the mechanism may be related with Rho-kinase inhibition and down-regulation of CTGF over-expression.

Objectives:To discuss and analyse the causes of local recurrence after Dixon operation.　Methods:Retrospective analysis was made on 72 cases after resection of rectal cancer by Dixon operation in our department from 1995 to 1999.　Results:The local recurrence rate after Dixon operation was 12.5%(9 cases),the recurrence time was 3～26 months,and 16.2 months in an average after the operation.Seven cases of recurrence were within 2 years.The recurrence location occurred at the anastomotic stoma (6 cases),pelvic cavity (2 cases) and the perinum (1 cases) respectively.Based on Dukes classification, it showed one case of phase A, three cases of phase B and five cases of phase C.According to pathological classification, there were one case of papillary adenocarcinoma,five cases of rubiformadenocarcinoma and three cases of mucoid adenocarcinoma.A length from the lower margin of the tumors to the distal resection site,seven cases were within 3 cm,and two cases were beyond 3 cm.　Conclusions:The causes for local recurrence after operation were related to Duke classification,pathological types,length from the lower margin of the tumors to the distal resection site,lymphadenectomy and operation on the tumor itself.
