1.Nailfold video capillaroscopy in the Raynaud′s phenomenon associated with connective tissue disease: a meta-analysis

Objective:Nailfold video capillaroscopy (NVC) was used to evaluate the microvascular changes in patients with connective tissue diseases (CTD)-related Raynaud′s phenomenon and explore its potential application value in understanding the pathogenesis, diagnosis, and treatment of CTD.Methods:The literature on NVC of connective tissue disease related patients published by CNKI, China biomedical literature database, Weipu Database,Wanfang Database, PubMed, Embase, Web of Science and Cochrane were searched, and the publication cases of time was up until May 2024. Quality assessment of the included studies was performed using the National Institutes of Health observational cohort and cross-sectional study quality assessment tools. Begg′s test and Egger′s test were analyzed with random effects meta-analysis.Results:A total of 12 studies were included, including 495 cases of Raynaud′s phenomenon of Connective tissue diseases, and 716 no-Raynaud′s phenomenon of Connective tissue diseases. The abnormal rate of nail microcirculation in CTDs-RP group was higher than that in CTDs-nonRP group [ OR value(95% CI)=2.20 (1.55, 3.12), P<0.001], Subgroup analysis showed that the proportion of scleroderma pattern in the CTDs-RP group was higher than that in the control group [ OR value (95% CI)=10.88 (2.34, 50.52), P=0.002], Telangiectasia rate in the CTDs-RP group was higher than that in the control group [ OR value (95% CI)=5.12 (3.40, 7.70), P<0.001]. The SSc pattern was higher in patients with CTD-RP than in the control group [ OR (95% CI)=2.47 (1.60, 3.79), P<0.001], The rate of capillary malformation in the CTD-RP group was higher than that in the control group [ OR value (95% CI)=2.39 (1.36, 4.19), P=0.002], The capillary density was mild decreased in the CTD-RP group [SMD value (95% CI)=-0.27 (-0.66, 0.13), P=0.185]. Conclusion:The proportion of patients with nail-fold abnormality is higher in CTD-RP group was higher than in the CTD-noRP group, and the microcirculation injury is more severe in CTD-RP group.
2.Exploration of Optimizing the Allocation Path of Personnel Funds in One Hospital with Multiple Districts

Traditional funding allocation methods often only consider working hours or income ratios,making it difficult to fully measure the quality and technical complexity of medical work.Therefore,it first analyzed the difficulties in managing the cost of personnel expenses in public hospitals.Then,the limitations of two traditional allocation methods,namely the labor based econometric allocation model and the profit oriented allocation model,were analyzed.Subsequently,a work achievement sharing method based on performance bonuses in one hospital with multiple districts was proposed,which comprehensively integrates multidimensional factors such as workload,technical difficulty,surgical category,and personnel titles.The results indicate that for the allocation of personnel and hospitalization expenses,the econometric allocation model has the highest allocation ratio,which is 54.35%,Next is the profit oriented allocation model,with an allocation ratio of 30.64%.The proposed performance-based bonus based work achievement sharing method has the lowest sharing ratio of 20.55%and better reflects the quality and labor value of doctors'work.The research aims to provide reference for optimizing the cost accounting system and improving operational efficiency of public hospitals.

5.Mechanism of disordered subcellular localization of TFE3 fusion protein in TFE3 translocation renal cell carcinoma

【Objective】 To investigate the effects of the loss of exon 1 of TFE3 on nuclear localization of chimeric TFE3 protein in TFE3 translocation renal cell carcinoma (TFE3 tRCC). 【Methods】 The localization of TFE3 protein in TFE3 tRCC and clear cell renal cell carcinoma (ccRCC) were detected with immunochemistry. The exon retention of TFE3 gene in TFE3 tRCC was analyzed in databases and literatures. The plasmids containing TFE3 full-length and different-length of TFE3 exons which were constructed to pCDH-MCS-EGFP-Puro were transfected into HEK293T using Lipo FiterTM. The localization of EGFP protein in HEK293T cells were detected with confocal microscopy. The localization of TFE3 protein and truncated TFE3 protein were detected with Western blotting. The mRNA expression of the downstream genes of TFE3 protein were detected with q-PCR. 【Results】 Strong nuclear signal of TFE3 protein was observed in TFE3 tRCC, whereas TFE3 protein in ccRCC was mainly localized in cytoplasm. The results of fluorescence imaging and Western blotting showed that TFE3 full-length protein was expressed both in nucleus and cytoplasm, and the expression of truncated TFE3 protein was mainly localized in nucleus. The q-PCR analysis demonstrated that the deletion of exon 1 in TFE3 gene led to a higher transcriptional level of targeted genes of TFE3 protein. 【Conclusion】 The loss of exon 1 in TFE3 played a critical role in preventing TFE3 protein from entering the nucleus. In TFE3 tRCC, the loss of exon 1 in TFE3 gene leads to the nuclear localization of TFE3 fusion protein and activation of its downstream target genes. This mechanism promises to uncover the occurrence and development of TFE3 tRCC.
6.Excision and clinical application of ALTPF based on lateral branch of the lateral femoral circumflex artery descending branch

Objective:To investigate the removal of ALTPF pedicled with the lateral branch of the descending branch of the lateral femoral circumflex artery and its application in wound repair.Methods:From January, 2019 to March, 2021, 32 cases of limb wounds were repaired with ALTPF pedicled with the lateral branch of the descending branch of the lateral femoral circumflex artery, including 25 males and 7 females. The age ranged from 21 to 63 years, with an average of 34 years. Injury mechanism: there were 12 cases of traffic accident injury, 8 cases of machine injury, 6 cases of heavy object crushing injury and 6 cases of chronic infectious wound. The area of soft tissue defect was 8.2 cm × 6.3 cm-18.6 cm × 11.2 cm. There were 25 cases of direct suture and 7 cases of free skin grafting. All patients who needed flap repair underwent CTA and high-frequency CDU before operation to judge the approximate direction of the lateral branch of the descending branch of the lateral femoral circumflex artery and the position of the perforating branch. All patients were followed-up regularly for 3 - 13 months, with an average of 11 months.Results:All patients who underwent free flap surgery had no vascular crisis and the flap survived smoothly. At the last follow-up, the appearance of the flap was beautiful without bloating and there were no complications in the donor area.Conclusion:Compared with the traditional ALTPF, the perforator flap of the lateral branch of the descending branch of the anterolateral femoral artery is relatively simple, which can replace the traditional ALTPF to a certain extent, and is worthy of popularization and application in clinic.
7.Application of fibular flap with partial continuous periosteum and cortex in hip preservation surgery for femoral head necrosis

Objective:To investigate the short-term clinical effect of using fibular flap with preserving the continuity of fibula in hip preservation surgery for femoral head necrosis.Methods:From September, 2017 to November, 2020, 13 cases of femoral head necrosis were repaired with fibular flap. The fibular flaps were cut with an improved method for preserving the continuity of the fibular cortex, and the donor sites were sutured directly. The fibuls were inserted into the femoral heads with single or double segment folding support. Autogenous iliac crest combined with platelet-rich plasma(PRP) was used for impaction of bone grafting in femoral head, and the fibular flaps were anastomosed with 1 artery and 2 veins. All follow-up data were obtained, including bone union by X-ray and CT as well as the functional recovery of the hip joint and donor site. Statistical analysis was performed. P<0.05 was considered statistically significant. Results:The followed-up time ranged from 6 to 23 months. The fibular bones were significantly thicker and the incisions healed well at the donor sites. There was neither abnormal sensation in toes, dorsal foot, and lateral of the leg, nor significant influence on foot function. The hip joint activities were normal. The outcome was proved to be remarkable according to the Harris score(from 58.9±10.6 points before surgery to 81.7±10.6 points after surgery), the difference was statistically significant ( P<0.05) . Conclusion:The method of the improved fibular flap in hip preservation surgery is beneficial to the repair and reconstruction of the necrotic femoral head since the donor area is less traumatic, and a satisfactory clinical effect can be obtained.
8.Successful replantation of severed double amputation of distal segment of right index finger in a 4-year-old child in COVID-19

To report a 4-year-old boy with severed right index finger amputations in 2 segments. There were severe contusion on the 2 amputated sections of finger. According to the prevention and control requirement for the novel coronavirus disease(COVID-19), the patient was firstly checked to exclude the COVID-19. Then the replantation surgery was successfully carried out under the strict protective measures. The replanted index finger survived well at 2 weeks after surgery.
9.Clinical application of superficial iliac artery perforator flap based on the concept of "Donor site under swimming wear"

Objective:To explore the application of the superficial iliac perforator flap (SCIP) based on the concept of "Donor site under swimming wear" in the repair of wounds of extremities.Methods:From June, 2018 to March, 2019, a total of 20 patients with traumatic soft tissue defects of the extremities were treated. All defects were repaired with free SCIP, including 16 males and 4 females. Location of defects: 10 in the hand, 4 in the forearm, 4 in the shank, and 2 in the foot. Wound areas were 7.3 cm×5.6 cm-16.3 cm×7.7 cm. Patients received routine CTA and high-frequency CDU examinations before surgery to assist in identifying the location of the perforating vessels and the running of the superficial and deep branches of the superficial iliac circumflex artery (SCIA). Combined intraoperative light transmission experiments were used to determine the dominant branch. The superficial branch of SCIA was used as the dominant branch in 18 cases, and the flap was cut in the superficial layer of Scarpa's fascia. The deep branch of SCIA was used as the dominant branch in 2 cases and the flap was cut in the superficial layer of the deep fascia. The donor site was directly sutured. Regular postoperative follow-ups were conducted without lost.Results:All flaps had no vascular crisis after surgery and survived successfully. All patients were followed-up regularly for 8 to 16 months, with an average of 12 months. At the last followed-up, the appearance and texture of the flaps in the recipient area were good, and roughly similar to the healthy tissues around the affected limb. The function of the affected limb recovered well. There was only a linear scar in the donor area.Conclusion:The SCIP based on the concept of "Donor site under swimming wear" is soft in texture, moderate in thickness, and concealed in the supply area. The patient can directly hide the scars of the donor areas after wearing the swimming trunks, which meets the concept of minimizing the damage to the donor area. It deserves to be promoted.
10.Landscape of emerging and re-emerging infectious diseases in China: impact of ecology, climate, and behavior.

For the past several decades, the infectious disease profile in China has been shifting with rapid developments in social and economic aspects, environment, quality of food, water, housing, and public health infrastructure. Notably, 5 notifiable infectious diseases have been almost eradicated, and the incidence of 18 additional notifiable infectious diseases has been significantly reduced. Unexpectedly, the incidence of over 10 notifiable infectious diseases, including HIV, brucellosis, syphilis, and dengue fever, has been increasing. Nevertheless, frequent infectious disease outbreaks/events have been reported almost every year, and imported infectious diseases have increased since 2015. New pathogens and over 100 new genotypes or serotypes of known pathogens have been identified. Some infectious diseases seem to be exacerbated by various factors, including rapid urbanization, large numbers of migrant workers, changes in climate, ecology, and policies, such as returning farmland to forests. This review summarizes the current experiences and lessons from China in managing emerging and re-emerging infectious diseases, especially the effects of ecology, climate, and behavior, which should have merits in helping other countries to control and prevent infectious diseases.
