1.New perspectives on the neuro-immune mechanisms of itch in allergic conjunctivitis

Allergic conjunctivitis is a common ocular inflammatory disease, with intense itching being the most typical and distressing symptom for patients. In recent years, with the in-depth study of the interaction between the nervous and immune systems, significant progress has been made in understanding the mechanism of itching in allergic conjunctivitis. This review elaborates on the neurobiological basis of itching in allergic conjunctivitis, with a focus on the complex dialogue between immune cells and sensory neurons, particularly the core role of the IL-33-ST2-CGRP signaling axis in mediating itching. Additionally, this article introduces new findings in genetic susceptibility research, including the identification of susceptibility genes for allergic conjunctivitis through transcriptome-wide association studies. The sensory nervous system not only transmits itch signals but also actively participates in the formation of antigen channels related to conjunctival goblet cells, thereby regulating the local uptake of allergens and the initiation of the immune response. Moreover, targeted novel therapeutic strategies offer hope for patients with refractory allergic conjunctivitis. Exploring the molecular and cellular mechanisms of itching in allergic conjunctivitis will provide a theoretical basis for the development of more effective treatment methods.
2.Local abaloparatide administration promotes in situ alveolar bone augmentation via FAK-mediated periosteal osteogenesis.

Insufficient alveolar bone thickness increases the risk of periodontal dehiscence and fenestration, especially in orthodontic tooth movement. Abaloparatide (ABL), a synthetic analog of human PTHrP (1-34) and a clinical medication for treating osteoporosis, has recently demonstrated its potential in enhancing craniofacial bone formation. Herein, we show that intraoral submucosal injection of ABL, when combined with mechanical force, promotes in situ alveolar bone thickening. The newly formed bone is primarily located outside the original compact bone, implying its origin from the periosteum. RNA sequencing of the alveolar bone tissue revealed that the focal adhesion (FA) pathway potentially mediates this bioprocess. Local injection of ABL alone enhances cell proliferation, collagen synthesis, and phosphorylation of focal adhesion kinase (FAK) in the alveolar periosteum; when ABL is combined with mechanical force, the FAK expression is upregulated, in line with the accomplishment of the ossification. In vitro, ABL enhances proliferation, migration, and FAK phosphorylation in periosteal stem cells. Furthermore, the pro-osteogenic effects of ABL on alveolar bone are entirely blocked when FAK activity is inhibited by a specific inhibitor. In summary, abaloparatide combined with mechanical force promotes alveolar bone formation via FAK-mediated periosteal osteogenesis. Thus, we have introduced a promising therapeutic approach for drug-induced in situ alveolar bone augmentation, which may prevent or repair the detrimental periodontal dehiscence, holding significant potential in dentistry.

3.Effect of PTEN mRNA expression level in granulosa cells on follicular fluid hormone secretion in infertile patients with polycystic ovary syndrome

Objective:To detect the levels of sex hormones and insulin in follicular fluid(FF)and the expression level of phosphatase and tensin homolog deleted on chromosome ten(PTEN)in granulosa cells in the infertile patients with polycystic ovary syndrome(PCOS),and to preliminarily explain the correlations between the insulin level and the expression level of PTEN mRNA.Methods:Seventy infertile patients were selected as the subjects and divided into PCOS group and control group(tubal obstruction or infertility due to male factors)according to infertility factors.All patients received in vitro fertilization-embryo transfer(IVF-ET)treatment.FF and ovarian granulosa cells were collected on the day of ovulation.The expression levels of PTEN mRNA in ovarian granulosa cells of the patients in two groups were detected by real-time fluorescence quantitative PCR(RT-qPCR)method.The levels of sex hormone and insulin in FF were measured by electrochemiluminescence.The correlations of the PTEN mRNA expression level in ovarian granulosa cells and testosterone(T)in FF with the level of insulin in FF were analyzed by Spearman correlation analysis method.Results:There were no significant differences in age,infertility years,body mass index(BMI),basic sex hormone,total dose of gonadotropin(Gn)and days of ovulation induction in two groups(P>0.05).Compared with control group,the anti-Mullerian hormone(AMH)and antral follicle counting(AFC)of the patients in PCOS group were significantly increased(P<0.05).The RT-qPCR results showed that the PTEN mRNA expression level in ovarian granulosa cells of the patients in the PCOS group was higher than that in control group(P<0.001).The electrochemiluminescence results showed that the levels of T and insulin in FF of the patients in PCOS group were higher than those in control group(P<0.05),whereas the estrogen and progesterone levels were lower than those in control group(P<0.05).The Spearman correlation analysis showed that that T level in FF was positively correlated with the insulin level of the patients in PCOS group(r=0.577,P<0.001),and the PTEN mRNA expression level in ovarian granulosa cells was positively correlated with the insulin levels in FF(r=0.616,P<0.001);in control group,there was no correlation between T level and insulin level in FF(r=0.266,P=0.123),and there was no correlation between the expression level of PTEN mRNA in granulosa cells and the insulin level in FF in control group(r=-0.214,P=0.216).Conclusion:The high expression of PTEN in granulosa cells of the infertile patients with PCOS may be related to the local hyperinsulin level in the ovary,and PTEN participates in the occurrence and development of PCOS.
4.Clinical observation on the effect of RIC-PTCY on allogeneic hematopoietic stem cell transplantation in patients with hematologic malignancies

Objective:To evaluate the efficacy and safety of reduced intensity conditioning (RIC) combined with posttransplant high-dose cyclophosphamide (PTCY) in the treatment of high-risk and relapsed/refractory malignant hematological diseases through allogeneic hematopoietic stem cell transplantation (allo-HSCT).Method:A retrospective analysis was conducted on clinical data of 42 recipients with high-risk or relapsed/refractory malignant hematological diseases who underwent allo-HSCT using the RIC-PTCY regimen at the First Affiliated Hospital of Soochow University from December 2014 to August 2023. Data were collected on engraftment rate, incidence of graft-versus-host disease (GVHD), infections, relapse, and survival outcomes.Result:The median age of the 42 recipients was 52.5 years (range, 20-75 years). Among them, 23 were classified as high-risk and 19 as relapsed/refractory. Three recipients experienced graft failure, and one recipient died of infection early after transplantation. The remaining 38 recipients achieved complete donor chimerism at a median of 21 days (range, 9-52 days). The median time to neutrophil and platelet engraftment was 14 days (range, 9-27 days) and 15 days (range, 7-51 days), respectively. The cumulative incidence (CI) of grade I-II and grade III-IV acute GVHD (aGVHD) at 100 days post-transplantation was 23.7% and 18.4%, respectively. The 2-year CI of chronic GVHD (cGVHD) was 30.1%, with no moderate-to-severe cGVHD observed. The CI of pulmonary and bloodstream infections was 45.2%. The CI of infections with cytomegalovirus (CMV), Epstein-Barr virus (EBV), and BK virus (BKV) was 21.1%, 2.9%, and 7.9%, respectively. The CI of grade I-II hemorrhagic cystitis was 15.8%. The median follow-up duration was 11.2 months (range, 0.4-106.9 months). At the last follow-up, the overall survival (OS) and disease-free survival (DFS) rates were 40.7% and 38.5%, respectively. The cumulative incidence of relapse and non-relapse mortality (NRM) was 44.9% and 19.5%, respectively.Conclusion:The RIC-PTCY regimen may improve early post-transplant outcomes and broaden donor availability. recipients with high-risk or relapsed/refractory malignant hematologic diseases may benefit from this transplantation strategy.
5.A Subgroup Investigation and Genetic Analysis of Blood Donors in Zhongshan,Guangdong Province,and Research on Preservation Related Blood Group Antigen and Antibody

Objective To investigate the distribution of A subgroups in the A and AB blood type populations among voluntary blood donors in Zhongshan,study the antigen-antibody characteristics of A subgroups,establish a local A subgroup database,and support the development of precision medicine.Methods ABO subgroup screening was performed using the microplate method.Specimens negative for monoclonal anti-A1 reactivity underwent sequencing of exons 1~7 of the ABO gene to confirm genotypes.Cryopreservation and thawing of glycerolized subgroup red blood cells(RBCs),as well as preservation efficacy of concentrated human-derived antibodies with preservatives,were studied.Results Among 1 212 blood donor specimens,28 subgroup specimens were identified,with a prevalence of 1.54%(15/971)in blood type A and 5.39%(13/241)in blood type AB.Sequencing of 10 specimens revealed 7 ABO genotypes:ABO*A2.01/O.01.02(2 cases),ABO*A1.02/B.01(3 cases),ABO*BA.02/O.01.02,ABO*AW.31.02-05/A2.05,ABO*A2.05/B.01,ABO*A2new/O.01.01,and ABO*A1.02/O.01.01(1 case each).Additionally,one rare allele mutation(c.700C>G)and one novel allele mutation(c.203G>T)(GenBank accession number:PQ152337)were identified.Human anti-A1 antibodies with a titer of 8 were successfully concentrated.Optimal preservation conditions included 0.1%preservative concentration and cryopreserved subgroup RBCs stored at 4℃for 3 days post-thaw.Conclusion The predominant A subgroups in Zhongshan donors are A2 and B(A).A preliminary database for A2 and A2B subgroups is established,along with the discovery of a novel ABO allele mutation.Cryopreservation with glycerol,PEG antibody concentration,and ProClin 300 preservative demonstrate effective applications in preserving ABO blood group antigens and antibodies.
6.A model for quantification technology of fetal right ventricular size and function and its application in the diagnosis of hypoplastic right heart syndrome

Objective:To establish a normal data model of fetal right ventricular size and function using echocardiography,and to explore the clinical value of quantitative assessment of right ventricular size and function in the diagnosis of congenital heart diseases.Methods:(1)A simple random sampling method was employed to collect 1 004 pregnant women with normal singleton pregnancies at 24 to 32 +6 weeks of gestation who underwent fetal cardiac ultrasound examinations at the First Affiliated Hospital of Air Force Medical University from January 2021 to December 2023. Two-dimensional and M-mode echocardiography were used to measure the right ventricular end-diastolic diameter(RVEDD),right ventricular end-diastolic area(RVEDA),tricuspid annular plane systolic excursion(TAPSE)during systole,and the right ventricular fractional area change(RVFAC)was calculated. The correlations between the above parameters and ultrasound gestational age(USGA)were analyzed. Moreover,percentile growth curves for each parameter were plotted. With the above parameters as dependent variables and the USGA as the independent variable,a Z-score model was established through regression analysis.(2)A stratified sampling method was adopted to select 30 fetuses diagnosed with hypoplastic right heart syndrome(HRHS)and 30 fetuses diagnosed with pulmonary stenosis(PS)during the same period as the case group. The model was verified,and the morphological and functional characteristics of the right ventricle were analyzed. Results:The data of RVEDD,RVEDA,TAPSE,and RVFAC in normal fetuses showed a skewed distribution. Each parameter showed good linear correlations with USGA( r=0.836,0.834,0.846,0.242;all P<0.001). The constructed percentile curves for each parameter indicated that RVEDD,RVEDA and TAPSE increased significantly with the growth of USGA,while RVFAC showed a slow downward trend. All parameters in the HRHS group and TAPSE and RVFAC in the PS group deviated significantly from the normal reference range(all P<0.001). Conclusions:By analyzing RVEDD,RVEDA,TAPSE and RVFAC of normal fetuses,the percentile and Z-score normal reference value models of multiple parameters of fetal right ventricular size and function have been established,providing corresponding standards for quantitative analysis.
7.Hemolysis rates of three red blood cell components at the end of storage: a 5-year retrospective study

Objective: To evaluate the suitability of the existing hemolysis rate standards for locally processed red blood cell components by retrospectively analyzing 5-year hemolysis rate data at the end of storage. Methods: A total of 720 blood samples of three types of red blood cell components from our blood station from January 2019 to December 2023 were collected. Parameters included hemoglobin concentration (Hb), hematocrit (Hct), and free hemoglobin concentration (fHb). Hemolysis rate were taken as the control standard of 0.8% in accordance with the national standard. The hemolysis rates were compared against the national standard threshold of 0.8% (GB18469-2012), and annual trends of the detection parameters were observed. Results: The hemolysis rates (x-+s,%) of leukocyte-depleted whole blood at the end of storage were (0.038±0.023 8) in 2019, (0.049±0.039 5) in 2020, (0.043±0.040 7) in 2021, (0.049±0.030 7) in 2022, and (0.058±0.054 8) in 2023, respectively; The hemolysis rates (x-+s" />,%) of leukocyte-depleted suspended red blood cells at the end of storage were (0.093±0.050 2) in 2019, (0.086±0.049 5) in 2020, (0.123±0.072 3) in 2021, (0.122±0.052 1) in 2022, and (0.106±0.058 6) in 2023, respectively; The hemolysis rates (x-+s,%) of washed red blood cells at the end of storage were (0.127±0.038 2) in 2019, (0.150±0.066 5) in 2020, (0.121±0.052 2) in 2021, (0.124±0.038 9) in 2022, and (0.128±0.044 3) in 2023, respectively. Conclusion: Hemolysis rates at the end of blood storage of three red blood cell components were significantly lower than the limits specified in Quality Requirements for Whole Blood and Components (GB18469-2012), as well as standards from the EU, AABB and the United States. The results demonstrate excellent product quality control. A regional internal control standard of <0.2% is proposed for hemolysis rates at the end of storage.
8.Efficacy of selective laser trabeculoplasty in the treatment of primary open angle glaucoma

AIM: To assess the effectiveness of selective laser trabeculoplasty(SLT)in the treatment of primary open angle glaucoma(POAG).METHODS:Totally 24 patients(33 eyes)with POAG who had poor control of intraocular pressure(IOP)from June 2022 to December 2023 were included in this retrospective study, and all of them were treated with SLT. Furthermore, the IOP, mean deviation(MD), mean sensitivity(MS), and the thickness and vascular density(VD)of optic disc nerve layer were compared after treatment. RESULTS: There were significant reducion in IOP at 1, 3, 6, 12 mo after treatment(all P<0.001). There were no statistically significant differences in MD and MS before treatment and at 6 and 12 mo after treatment(all P>0.05), and there were no statistical significant differences in the thickness and VD of optic disc(all P>0.05).CONCLUSION: SLT can significantly reduce IOP in POAG patients in the short term, without damaging the visual field and optic disc nerves, while the effectiveness of SLT decreases over time.
