Objective To explore the clinical features and prognostic factors of adult hemophagocytic syndrome(HPS).Methods The clinical data of 68 newly diagnosed patients with HPS who were hospitalized from December 2018 to December 2023 were retrospectively analyzed, and clinical features, treatment and prognostic factors of patients were analyzed. Results The median age of 68 patients was 49(17-75)years old,the most common cause was infection,40 cases(58.82%),and the most common clinical manifestation was fever,63 cases(92.6%).Of the 68 patients,12(17.65%)patients died and the median survival time was 32 months.Cox univariate analysis showed that the prognosis of adult HPS was affected by SF,epstein-barr virus(EBV)infection, platelet count(<20× 109/L),age over 60 years old, activated partial thromboplastin over 60s and other factors(P<0.05).Multivariate Cox analysis showed that EBV infection and platelet count(<20×109/L)were independent prognostic factors of adult HPS(P<0.05).Conclusion The clinical manifestations of adult HPS are not typical, and the prognosis of adult HPS is affected by many factors.EBV infection and platelet count(<20× 109/L)are independent risk factors for death.

Objective:To investigate the effect of arthroscopically-assisted open reduction and internal fixation of intra-articular distal radius fracture. Methods:A retrospective cohort study was made on clinical data of 44 patients with distal radial intraarticular fracture admitted to Jing′an District Central Hospital, Fudan University between June 2017 and August 2020. There were 13 males and 31 females, at age of 35-85years [(62.5±12.9)years]. According to AO/OTA fracture classification system, there were 7 patients with type B and 37 with type C. Open reduction and internal fixation with volar plate was used in all patients, among which 22 were operated on using arthroscopy assistance (arthroscopy group) and 22 were operated on with traditional intraoperative fluoroscopy (fluoroscopy group). The operation time in both groups and triangular fibrocartilage complex (TFCC) injury and fracture displacement in arthroscopy group were recorded. Patient-rated wrist evaluation (PRWE) score, disabilities of the arm, shoulder and hand (DASH) questionnaire and range of wrist motion were compared between the two groups at 12 months after operation. The incidence of complications was observed. Results:All patients were followed up for 12-15 months [(13.3±1.1)months]. The operation time in arthroscopy group was (104.0±40.5)minutes, longer than (71.3±32.1)minutes in fluoroscopy group ( P<0.05). In arthroscopy group, 14 patients (64%) with TFCC injury were diagnosed intraoperatively, with the fracture displacement gap and step for 0.8 (0.3, 0.8)mm and 1.0 (0.3, 1.5)mm under arthroscopic vision, which were reduced to 0.3 (0.0, 0.5)mm and 0.5 (0.0, 0.5)mm after arthroscopically-assisted reduction (all P<0.05). The PRWE score in arthroscopy group was (9.8±4.9)points at 12 months after operation, lower than (13.4±5.8)points in fluoroscopy group ( P<0.05). The DASH questionnaire in arthroscopy group was (9.0±5.0)points at 12 months after operation, lower than (13.0±6.1)points in fluoroscopy group ( P<0.05). The dorsal extension and posterior rotation of the wrist in arthroscopy group were (73.8±8.9)° and (82.5±8.0)°, higher than (65.8±14.2)° and (76.3±10.4)° in fluoroscopy group (all P<0.05). There were no postoperative complications such as loosened or broken screws, vascular nerve damage, incision infection or traumatic arthritis in both groups. Conclusion:Arthroscopic-assisted open reduction and internal fixation of intra-articular distal radius fracture can increase the accuracy of joint surface reduction, improve postoperative wrist function and confirm the diagnosis of TFCC injury during operation.

Objective To express the N protein of middle east respiratory syndrome coronavirus ( MERS-CoV) in prokaryotic expression system and evaluate the immunogenicity of the purified recombinant N protein. Methods The gene encoding N protein of MERS-CoV was synthesized and cloned into the vector pET30a to construct the recombinant expression plasmid pET30a-MERS-CoV-N.The transformed E.coli BL21 ( DE3) strains carrying expression plasmid were induced by IPTG to express N protein. The expressed protein was purified by using affinity chromatography.SDS-PAGE and Western blot assays were used to iden-tify the expressed N protein and evaluate its immunogenicity. Results The recombinant N protein was suc-cessfully expressed in soluble form with the size of 46×103 .The results of Western blot assay showed that the recombinant N protein could specifically react with rabbit serum samples positive for antibodies against N protein B-cell epitope peptide and mouse anti-His tag antibodies. No positive band against N protein was found when primary antibody was used with thirty adult serum samples from Beijing in 2008.Conclusion N protein of MERS-CoV was successfully expressed in prokaryotic expression system. The successful expres-sion of N protein laid the foundation for immunological diagnosis of N protein of MERS-CoV and researches on its immunogenicity.

Objective To make the gene mapping of one Chinese benign familial infantile convulsion (BFIC )pedigree on chromosome 19q12 13 1 Methods Five microsatellite DNA markers (D19S49, D19S250, D19S414, D19S416, D19S245) were chosen to make haplotype and linkage analysis of this BFIC family Results Among the 3 markers D19S49,D19S416,D19S245, the maximum LODs was located on D19S416 When the recombinant rate was 0 3, the maximum LOD score was 0 52; when the recombinant rate was 0, the LOD score was ∞; when the recombinant rate was 0 1, the LOD score was less than 0 The D19S414 and D19S250 could not provide information Conclusion BFIC gene of the family was not linked between D19S49 and D19S245, which suggested that there was heterogeneity in BFIC
