1.Predictive value of combined detection of serum vWF,MCP-1,and GDF-15 for postpartum hemorrhage in patients with pernicious placenta previa

Objective To investigate the predictive value of the combined detection of serum von Willebrand factor(vWF),monocyte chemotactic protein-1(MCP-1),and growth differentiation factor-15(GDF-15)for postpartum hemorrhage in patients with pernicious placenta previa(PPP).Methods One hundred and twelve patients with PPP admitted to our hospital between January 2021 and January 2024 were selected as the study group.They were further divided into a postpartum hemorrhage group and a non-postpartum hemorrhage group and 112 pregnant women with normal placental position during the same period were selected as the control group.ELISA was used to detect serum vWF,MCP-1,and GDF-15 levels.Results Serum vWF,MCP-1,and GDF-15 levels were significantly higher in the study group than in the control group(P<0.05).Serum vWF,MCP-1,and GDF-15 levels were significantly higher in the postpartum hemorrhage group than in the non-postpartum hemorrhage group(P<0.05).Logistic regression analysis identified vWF,MCP-1,and GDF-15 levels as factors influencing postpartum hemorrhage for women with PPP(P<0.05).The combination of serum vWF,MCP-1,and GDF-15 predicted postpartum hemorrhage in women with PPP better than either factor alone(P<0.05).Conclusion Combined detection of serum vWF,MCP-1,and GDF-15 levels has predictive value for postpartum hemorrhage in women with PPP.
2.Mediating effects of self-management and medication adherence between type D personality and viral load in HIV-infected individuals

Objective:To explore the mediating effects of self-management and medication adherence between type D personality and viral load in HIV-infected individuals.Methods:HIV patients who were newly diagnosed and undergoing antiretroviral therapy (ART) at the Heilongjiang AIDS Treatment Center of the Fourth Hospital of Harbin Medical University from October 2023 to March 2024 were enrolled by convenience sampling. Data were collected using the Type D Personality Scale-14 (DS-14), HIV Self-Management Scale (HIVSMS), and 8-item Morisky Medication Adherence Scale (MMAS-8). HIV-1 viral load was also measured. Pearson correlation analysis explored the relationships among type D personality, self-management, medication adherence, and viral load. The SPSS Process V4.0 macro was used to test the mediating effects.Results:A total of 318 questionnaires were distributed and 313 valid responses were collected (effective response rate: 98.43%). The mean scores for the 313 HIV-infected individuals were as follows: DS-14 (33.39±4.98), HIVSMS (29.84±4.41), and MMAS-8 (6.11±1.29). The mean viral load was (1 803.19±870.81) copies/ml. Pearson correlation analysis showed that type D personality was negatively correlated with self-management and medication adherence ( r=-0.307, -0.208, respectively; P<0.01) and positively correlated with viral load ( r=0.213; P<0.01). Medication adherence was negatively correlated with viral load ( r=-0.232; P<0.01), while self-management was positively correlated with medication adherence ( r=0.353; P<0.01). The mediation analysis revealed that self-management and medication adherence played a chain mediating role between type D personality and viral load, with a mediation effect value of 0.016, accounting for 8.08% of the total effect. Conclusions:Type D personality directly influences the viral load of HIV-infected individuals and indirectly affects it through the mediating effects of self-management and medication adherence.

Objective:To develop a treatment-related quality of life scale for Chinese patients with multiple myeloma (MM) and to test its reliability and validity.Methods:The initial scale was constructed through a literature search, Delphi expert correspondence, and cognitive testing. This study conducted a preliminary survey of 379 patients with MM and a formal survey of 865 patients from the hematology departments of 155 hospitals nationwide from February 2024 to March 2024. The final scale was obtained after conducting item analysis and reliability and validity tests on the initial scale.Results:The constructed scale contains 36 items covering six domains: physiological, psychological, social, treatment side effects, general health, and others. In the preliminary survey, the Cronbach’s alpha coefficient of each item ranged from 0.597 to 0.939, and the test-retest reliability was 0.747 ( P<0.001). Exploratory factor analysis extracted eight common factors with a cumulative variance contribution of 60.058%. In the formal survey, the Cronbach’s alpha coefficient of each item ranged from 0.484 to 0.930, and the test-retest reliability was 0.835 ( P<0.001). Confirmatory factor analysis revealed a comparative fit index of 0.750, a root-mean-square error of approximation of 0.090, and a root-mean-square residual of 0.067. Conclusion:The treatment-related quality of life scale for Chinese patients with MM designed in this study exhibited good reliability and validity, reflecting the impact of treatment on the quality of life of patients. This scale can provide a reference to clinicians for assessing the disease status of patients.
4.Intravascular large B-cell lymphoma of the nasal turbinate presenting with hemophagocytic lymphohistiocytosis: a case report and literature review

Intravascular large B cell lymphoma (IVLBCL) is a rare, aggressive subtype of diffuse large B cell lymphoma, with nasal turbinate involvement being uncommon. We report a 51-year-old woman with a 1-month history of fever of unknown origin. Laboratory findings showed cytopenia, hypertriglyceridemia, elevated ferritin, increased soluble CD25, and bone marrow hemophagocytosis. No infectious cause was identified. PET-CT revealed abnormal 18F-fluorodeoxyglucose (FDG) uptake in the nasal turbinates. Turbinate biopsy revealed tumor cells localized predominantly within vascular lumens, positive for CD20, BCL6, PAX5, and MUM1, with a Ki-67 index >60%, confirming a diagnosis of IVLBCL with hemophagocytic lymphohistiocytosis (HLH). The patient received one cycle of the DEP regimen (liposomal doxorubicin, etoposide, and methylprednisolone) for HLH, followed by five cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) and consolidation with auto-HSCT, achieving sustained complete remission. IVLBCL outcomes are heterogeneous; early diagnosis and prompt treatment improve survival, and R-CHOP plus auto-HSCT may be an effective strategy.

7.Chest contrast-enhanced CT combined with artificial intelligence iterative reconstruction for bronchial artery imaging

Objective To investigate the value of chest contrast-enhanced CT(C-CECT)combined with artificial intelligence iterative reconstruction(AIIR)for bronchial artery(BA)imaging.Methods Seventy patients who underwent C-CECT were prospectively enrolled.The images were reconstructed with AIIR(AIIR group)and hybrid iterative reconstruction(HIR,HIR group),respectively.The overall image quality,the traceability of BA,the sharpness of BA and the diagnostic confidence of abnormalities of BA were subjectively graded using a 5-point scale by two radiologists,respectively.The subjective scores and inter-observer agreement were compared between groups.The noise(SD)in reconstructed images of thoracic aorta,pulmonary trunk,BA and spinal erectors,the contrast-to-noise ratio(CNR)of the above 3 arteries relative to spinal erectors,and the diameters of BA at the origin,bifurcation and pulmonary hilum were compared between groups.Results The scores of the overall image quality,the traceability of BA,the sharpness of BA and the diagnostic confidence of abnormalities of BA were all significantly higher in AIIR group than those in HIR group(all P<0.001),all with good inter-observer agreement(Kappa=0.46-0.73).SD of the aorta,pulmonary artery trunk,BA and erector spinal muscle in AIIR group were lower than those in HIR group,while CNR of above 3 arteries were higher than those in HIR group(all P<0.05).No significant difference of the diameter of BA at each position was found between groups(all P>0.05),while the consistency of measurement of AIIR group was higher than that of HIR group(intra-class correlation coefficient:0.89-0.94 vs.0.63-0.78).Conclusion C-CECT combined with AIIR could significantly improve imaging quality and diagnostic confidence of BA.
8.Genotypic and phenotypic analysis of glucose-6-phosphate dehydrogenase in blood donors and recipients in Dongguan region and its clinical applications

Objective To preliminarily analyze the genotypic and phenotypic characteristics of glucose-6-phosphate dehydrogenase(G6PD)in blood donors and recipients in Dongguan region,as well as the impact of these characteristics on the efficacy of blood transfusion therapy.Methods A total of 351 pairs of blood sam-ples from donors and recipients were collected from the Tenth Affiliated Hospital of Southern Medical Uni-versity/Dongguan People's Hospital between May and November in 2023.These samples were tested for G6PD genotype,G6PD enzyme activity,erythrocyte osmotic fragility,and blood routine parameters.Addition-ally,hemoglobin levels before and after blood transfusion were collected from recipients for statistical analy-sis.Results The carrier rates of G6PD mutant genes among donors and recipients in Dongguan region were 6.84%and 5.83%,respectively.Six mutation sites were identified,with c.1388G>A,c.1376G>T,and c.95A>G accounting for 84.09%of all mutations.A negative correlation was observed between G6PD activi-ty and blood storage duration in wild-type donors.The G6PD activity significantly decreased when blood from healthy donors was transfused into recipients through standard procedures.Recipients carrying c.1388G>A,c.1376G>T,c.95A>G,c.871G>A,c.1024C>T mutation sites showed no statistically significant difference in hemoglobin increment after receiving blood from donors carrying c.1388G>A,c.1376G>T,c.95A>G,c.871G>A,c.1376G>T/c.1360C>T,or c.1388G>A/c.95A>G mutation sites.In the internal medicine re-strictive transfusion group,recipients who received blood with enzyme activity≥1300 U/L demonstrated a significantly greater increase in hemoglobin levels compared to those transfused with blood with enzyme activ-ity<1 300 U/L(P=0.042).Conclusion The carrier rates of G6PD mutant genes among donors and recipi-ents in Dongguan region are both above 5.00%,indicating a relatively high carriage rate.The three primary mutation sites,c.1388G>A,c.1376G>T,and c.95A>G,account for over 80.00%of all mutations.No ad-verse effects on treatment efficacy are observed in recipients carrying G6PD mutant genes after receiving blood from donors with G6PD mutant genes.However,in restrictive transfusion practices within internal medicine,recipients receiving blood with low G6PD enzyme activity demonstrate a reduced hemoglobin increase per unit compared to those receiving blood with normal G6PD enzyme activity.
9.Study on Correlation Between Traditional Chinese Medicine Syndrome Elements and Risk Factors in Patients with Type Ⅱ Cardio-renal Syndrome

Objective To investigate the correlation between traditional Chinese medicine(TCM)syndrome elements and risk factors in patients with type Ⅱ cardio-renal syndrome(CRS).Methods A retrospective analysis was conducted on the medical records of 116 patients with type Ⅱ CRS.The distribution of TCM syndrome elements was analyzed,and the correlation between the syndrome elements and risk factors of gender,age,smoking history,alcohol consumption history,coronary heart disease,hypertension,diabetes,and anemia were evaluated.Results(1)The analysis of syndrome elements of 116 patients with type Ⅱ CRS showed that the main disease-location elements were heart(96 cases,82.76%),lung(89 cases,76.72%),kidney(81 cases,69.83%),spleen(71 cases,61.21%),and liver(25 cases,21.56%);the main disease-nature elements were qi deficiency(113 cases,97.41%),blood deficiency(96 cases,82.76%),yang deficiency(95 cases,81.89%),phlegm(94 cases,81.03%),yin deficiency(90 cases,77.59%),dampness(61 cases,52.59%),water retention(55 cases,47.41%),and blood stasis(47 cases,40.52%).(2)The analysis of age groups of 116 patients with type Ⅱ CRS showed that the majority of the patients were middle-aged and elderly individuals over 60 years old,and the age groups covered<60 years old(4 cases,3.45%),61-70 years old(10 cases,8.62%),71-80 years old(28 cases,24.14%),81-90 years old(64 cases,55.17%),and>90 years old(10 cases,8.62%).(3)The analysis of gender of 116 patients with type Ⅱ CRS showed that 46 cases(39.66%)were male,and 70 cases(60.34%)were female,the female outnumbering the male.(4)The exploration of correlation between TCM syndrome elements and risk factors with Logistic regression analysis showed that blood deficiency was positively correlated with alcohol consumption,hypertension,and anemia(P<0.05 or P<0.01),yang deficiency was positively correlated with anemia(P<0.05),yin deficiency was positively correlated with smoking and diabetes(P<0.05),dampness was positively correlated with smoking and anemia(P<0.01),water retention was positively correlated with gender and protein abnormalities(P<0.05 or P<0.01),and blood stasis was positively correlated with hypertension(P<0.05).Conclusion The illness of type Ⅱ CRS is located in the heart,involving the five organs,and is closely related to the lungs and kidneys.The general pathogenesis of type Ⅱ CRS is characterized by being deficiency of qi and yang in the root cause,and having the symptom manifestations of phlegm and blood stasis.In type Ⅱ CRS patients,men are more likely to suffer water retention,smokers are more likely to suffer yin deficiency and dampness,drinkers are more likely to suffer blood deficiency,individuals with hypertension are more likely to suffer blood deficiency and blood stasis,individuals with diabetes are more likely to suffer yin deficiency,individuals with anemia are more likely to suffer blood deficiency,yang deficiency and dampness,and individuals with protein abnormalities are more likely to suffer water retention.
