Acetaminophen (APAP) is the primary cause of drug-induced acute liver failure. Ovarian tumor deubiquitinase 6A (OTUD6A), a recently discovered deubiquitinase of the OTU family, has been primarily studied in tumor contexts. However, its role in APAP-induced liver injury (AILI) remains unclear. Therefore, this study aimed to investigate the involvement of OTUD6A in the pathogenesis of AILI. Our findings demonstrated a substantial upregulation of OTUD6A in both the liver tissue and isolated hepatocytes of mice following APAP stimulation. OTUD6A knockout exacerbated APAP-induced inflammation, hepatocyte necrosis, and liver injury, whereas OTUD6A overexpression alleviated these pathologies. Mechanistically, OTUD6A directly interacted with the enhancer of zeste homolog 2 (EZH2) and selectively removed K48-linked polyubiquitin chains from EZH2, enhancing its stability. This resulted in increased protein levels of EZH2 and H3K27me3, as well as reduced endoplasmic reticulum (ER) stress and cell death in hepatocytes. Collectively, our research uncovers a novel role for OTUD6A in mitigating APAP-induced liver injury by promoting EZH2 stabilization.
2.Current status of construction and monitoring of healthcare-associated in-fection system in Guangxi area

Objective To investigate the current status of construction and monitoring of healthcare-associated in-fection(HAI)system in maternal and child health care institutions in Guangxi Zhuang Autonomous Region,and provide reference for improving the quality of HAI management.Methods After discussed by members of expert group of HAI Prevention and Control Professional Committee of Guangxi Maternal and Child Health Care Associa-tion,the"Questionnaire on the construction and monitoring of HAI system in maternal and child health care institu-tions in Guangxi Zhuang Autonomous Region"was designed.The questionnaires were distributed to all maternal and child health care institutions in the region via"Questionnaire Star",and a WeChat group was established to ad-dress any questions in questionnaires.Results A total of 104 maternal and child health care institutions were in-cluded in the study,including 6 primary,74 secondary,and 24 tertiary level institutions.All maternal and child health care institutions have established HAI management departments,95.19%(n=99)of institutions held HAI management committee meeting at least once every six months.Among 201 HAI management professionals,most were female(90.55%),with nursing specialty(72.64%),with a bachelor's degree(66.17%),and aged 31-50 years old(73.13%).Only 30.77%of the maternal and child health care institutions had HAI monitoring system.Seventy institutions(67.30%)have carried out the special action of improving the pathogen detection rate before antimicrobial therapy for hospitalized patients,out of which 55(78.57%)were led by the HAI management depart-ment.Ninety-seven(92.27%)maternal and child health care institutions conducted surgery-related monitoring,with 50.96%of institutions relying on manual monitoring;only 30.77%of institutions have carried out the special action of"Strengthening perioperative infection prevention and control to ensure surgical quality and safety".Conclusion HAI management system in maternal and child health care institutions in Guangxi has been basically es-tablished and can carry out monitoring work,but personnel compostion is limited and talent reserve is insufficient.The information construction of HAI monitoring is underdeveloped,with a lagged implement of special action.Ad-ministrative support is needed to enhance infection prevention and control at the grassroots level.All levels of ma-ternal and child health care institutions should communicate and exchange work experience.Hospitals should in-crease their support for the construction of HAI information technology,improve level of HAI monitoring as well as prevention and control capabilities.
3.Artificial intelligence-assisted compressed sensing cardiac MR cine imaging for assessing biventricular function and left ventricular myocardial strain

Objective To explore the value of artificial intelligence-assisted compressed sensing(ACS)-MR cine imaging(Cine)for assessing biventricular function and left ventricular myocardial strain compared with conventional cardiac Cine and united compressed sensing(uCS)-Cine.Methods A total of 30 subjects who underwent conventional Cine,uCS-Cine and ACS-Cine were prospectively enrolled.Based on a 5-point scale,subjective scoring of image quality was performed and compared among 3 sequences.Biventricular function parameters and myocardial strain parameters of the left ventricle,including left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejection fraction(LVEF),left ventricular end-diastolic mass(LVEDM),right ventricular end-diastolic volume(RVEDV),right ventricular end-systolic volume(RVESV),right ventricular ejection fraction(RVEF),left ventricular global radial strain(LVGRS),left ventricular global circumferential strain(LVGCS)and left ventricular global longitudinal strain(LVGLS)were measured and compared among sequences.Results The subjective image quality scores of all 3 sequences were≥3,of ACS-Cine and conventional Cine were not significantly different(adjusted P=0.306)but both slightly higher than that of uCS-Cine(both adjusted P<0.05).LVEDV,LVEF and LVEDM based on ACS-Cine and uCS-Cine were not significantly different(all adjusted P>0.05),but all lower than those of conventional Cine(all adjusted P<0.05).No significant difference of LVESV was found between each two sequences(all adjusted P>0.05).RVEDV based on ACS-Cine was lower than that based on conventional Cine,RVESV based on ACS-Cine was higher than that based on uCS-Cine,and RVEF based on ACS-Cine was lower than that based on uCS-Cine and conventional Cine(all adjusted P<0.05),while no significant difference of right ventricular functional parameters was observed between uCS-Cine and conventional Cine(all adjusted P>0.05).Both ACS-Cine and uCS-Cine had lower LVGRS and LVGCS than conventional Cine(all adjusted P<0.05),with no significant difference was found between uCS-Cine and ACS-Cine(both adjusted P>0.05).No significant difference of LVGLS was observed among 3 sequences(P>0.05).Conclusion ACS-Cine could be used as appropriate supplement to conventional Cine for clinical assessment of biventricular function and left ventricular myocardial strain.
4.Risk factors for postoperative complications in patients undergoing implant-based breast reconstruction after mastectomy

Background and Aims:Implant-based breast reconstruction is one of the most common reconstructive approaches after mastectomy for breast cancer.However,the incidence of postoperative complications remains significant,and the associated risk factors have not been fully elucidated.This study aimed to analyze the incidence of surgical complications following implant-based breast reconstruction in patients undergoing mastectomy at our center and to identify potential risk factors.Methods:A retrospective analysis was conducted on patients who underwent mastectomy and implant-based breast reconstruction at Sun Yat-sen Memorial Hospital between May 2004 and August 2022.Patients were grouped according to the presence or absence of postoperative surgical complications,and multivariate Logistic regression was used to identify independent risk factors.Results:A total of 545 patients with 602 reconstructed breasts were included.Surgical complications occurred in 13.6%(82/602)of the cases,including infection,wound dehiscence/poor healing,flap or nipple-areola necrosis,and implant leakage/rupture.Multivariate analysis revealed that nipple-areola complex resection(OR=1.934,95%CI=1.056-3.542,P=0.033),postoperative radiotherapy(OR=2.483,95%CI=1.527-4.036,P<0.001),implant volume≥300 mL(OR=1.663,95%CI=1.025-2.696,P=0.039),and surgeon experience with fewer than 10 cases(OR=1.804,95%CI=1.092-2.979,P=0.021)were all independent risk factors for complications.Conclusion:NAC resection,radiotherapy,large implant volume,and limited surgical experience are important independent risk factors for postoperative surgical complications following implant-based breast reconstruction.Thorough preoperative evaluation and appropriate surgical planning are essential to minimize risks.
5.An outbreak of neonatal Pseudomonas aeruginosa infection caused by wa-ter contamination

Objective To analyze the causes and intervention measure efficacy of an outbreak of neonatal Pseudo-monas aeruginosa(P.aeruginosa)infection in a medical institution.Methods The epidemiological investigation was conducted on 5 neonates who had P.aeruginosa infection in the neonatal intensive care unit(NICU)of this medical institution from May 26 to June 10,2024.Specimens were taken from potentially contaminated environ-ments to infer the cause of P.aeruginosa transmission in the NICU,infection control measures were also taken,and the efficacy of the control measures was continuously monitored.Results Among the 5 infected neonates,3 had lower respiratory tract infection and 2 had lower respiratory tract combined with bloodstream infection.Seven strains of P.aeruginosa were detected with a consistent antimicrobial resistance spectrum.Infected neonates were in the same ward and had spatial and temporal clustering.Through environmental hygiene monitoring,P.aerugi-nosa with consistent antimicrobial resistance spectrum of neonatal infection was isolated from the NICU handwashing sink and water-related environment(suction connection pipe,bedside isolation gowns).Infection was controlled af-ter improving sink disinfection methods and enhancing contact isolation measures,and no further P.aeruginosa was detected in the environment.Conclusion The outbreak of neonatal HAI in the NICU is inferred to be caused by contamination of ward handwashing sink by P.aeruginosa,then further contaminated the suction connection tube,thereby leading to the transmission of neonatal infection through suction procedures.Healthcare-associated infection surveillance and control professionals need to fully understand the characteristics of outbreak pathogens and take tar-geted measures to curb the transmission of neonatal infection.

10.Expert Consensus on Clinical Diseases Responding Specifically to Traditional Chinese Medicine: Pulmonary Nodules

In recent years, the incidence of pulmonary nodules has kept rising. To give full play to the advantages of traditional Chinese medicine (TCM) in the treatment of pulmonary nodules and identify the breakthrough points of integrating TCM with Western medicine, the China Association of Chinese Medicine organized medical experts in TCM and western medicine to carry out in-depth discussion regarding this disease. The discussion encompassed the modern medical advances, TCM theories of etiology and pathogenesis, the role and advantages of TCM in the whole course management of pulmonary nodules, contents and methods of research on pulmonary nodules, and science popularization work, aiming to provide a reference for clinical practice and scientific research. After discussion, the experts concluded that the occurrence of pulmonary nodules was rooted in the deficiency of the lung and spleen and triggered by phlegm dampness, blood stasis, and Qi stagnation. TCM can treat pulmonary nodules by controlling and reducing nodules, improving physical constitution, ameliorating multi-system nodular diseases, reducing anxiety and avoiding excessive diagnosis and treatment, and serving as an alternative for patients who are unwilling or unfit for surgical treatment. At present, the optimal diagnosis and treatment strategy for pulmonary nodules has not been formed, which needs to be further studied from multiple perspectives such as clinical epidemiology, biology, and evidence-based medicine. The primary task of current research is to find out the advantages, effective prescriptions, and target populations and determine the effective outcomes of TCM in the treatment of pulmonary nodules. At the same time, basic research should be carried out to explore the etiology and biological behaviors of pulmonary nodules. The expert consensus on the diagnosis and treatment of pulmonary nodules with integrated TCM and Western medicine needs to be continuously revised to guide clinicians to conduct standardized, scientific, and accurate effective diagnosis and treatment.
