Objective To investigate the predictive value of systemic immune-inflammation index(SII)combined with albumin(ALB)in the severity of community-acquired pneumonia(CAP)in elderly patients. Methods A retrospective analysis was conducted on 184 elderly patients with CAP in the Respiratory and E-mergency Medicine departments of the hospital from January 2023 to May 2024.According to the 2016 Chi-nese Adult CAP Diagnosis and Treatment Guidelines, patients were divided into severe CAP group(n=76)and non-severe CAP group(n=108).Basic information of patients was collected, routine blood tests and blood biochemical parameters were collected within 24 hours after admission, and the differences in SII,SIRI,hyper-sensitive CRP,and ALB were compared between the two groups. The ROC curve was applied to calculate the area under the curve and analyze the value of SII,SIRI,hypersensitive CRP,and ALB in predicting severe CAP.Results In terms of inflammation indicators,SII,SIRI,hs-CRP,and neutrophil count in the severe CAP group were higher than those in the non-severe CAP group(P<0.05),while lymphocyte count was lower than that in the non-severe CAP group(P<0.05).In terms of blood biochemistry, serum creatinine and BUN in the severe CAP group were higher than those in the non-severe CAP group, while ALB was lower than that in the non-severe CAP group(P<0.05).Binary logistic regression analysis showed that SII and ALB were in-dependent influencing factors for the severity of CAP in elderly patients(P<0.05).The ROC curve showed that the AUC of SII and ALB for predicting severe CAP in the elderly was 0.863 and 0.906,respectively,and the combined AUC was 0.937(0.904,0.970),with a sensitivity of 0.842 and a specificity of 0.880.Conclusion SII combined with ALB can serve as a predictor for early disease severity in elderly patients with CAP.

ABSRACT The development of hepatocellular car-cinoma(HCC)is closely related to the formation of tumour blood vessels.VEGF-mediated angiogenesis is a major driver of the immune escape response in tumours.VEGF binds to vascular endothelial growth factor receptor2(VEGFR2)on endothelial cells, promoting endothelial cell proliferation and migration, inducing vascular changes in HCC,and thus promote the growth of hepatocellular carcino-ma cells.Anti-VEGF and its receptor-targeted mo-lecular drugs are currently effective new treat-ments for HCC.Monoclonal antibodies against VEGF and small-molecule tyrosine kinase inhibitors targeting VEGF have been shown to block its angio-genic activity, alleviate the inhibitory effect of the tumour microenvironment, and ultimately achieve tumour regression. This article provides a review of the research progress of VEGF/VEGFR inhibitors in HCC treatment.

Hypersensitivity pneumonitis (HP) is an immune-mediated interstitial lung disease (ILD). It is a disease with highly heterogeneous clinical manifestations, severity and outcomes, which are associated with individual sensitivity, as well as property, dosage, duration and frequency of exposure to the antigens. The 2020 adult HP guideline reclassifies it and describes its radiographic features in detail.HP often occurs in adults, also affects the pediatric population and is one of the most common ILDs in children. The most common factors causing HP in children are avian and fungal antigens in the home environment. The diagnosis of HP is based on clear antigens, typical symptoms and characteristic radiological manifestations. The serum-specific IgG antibody, bronchoalveolar lavage fluid, and pulmonary function tests can help diagnose HP clearly, and lung histopathology is required for children whose diagnosis cannot be confirmed.Early diagnosis and adequate avoidance of antigen exposure are the keys to its treatment and prognosis.

OBJECTIVE To observe the auxiliary analgesic effect of electroacupuncture at Hegu and Neiguan combined with in-domethacin suppository in patients undergoing transvaginal ultrasound-guided oocyte retrieval(TUGOR and its effect on the outcome of in vitro fertilization(IVF.METHODS 64 IVF-ET patients undergoing TUGOR were randomly divided into a treatment group and a control group,32 cases in each group. One case dropped out of the treatment group during the treatment. The control group was given indomethacin suppository rectal administration, and the treatment group was given electroacupuncture at Hegu and Neiguan in addition to the treatment of the control group. The patients'tenderness threshold,VAS score, pain grade score, respiratory rate, and pulse rate were evaluated before and after TUGOR operation. The number of oocytes obtained, the rate of two pronuclei(2PN,embryo utiliza-tion rate, and high-quality embryo rate were evaluated after TUGOR operation. The adverse reactions of the two groups were monitored during and after operation.RESULTS After TUGOR operation, the VAS score and pain grade of the treatment group were signifi-cantly lower than those of the control group(P<0.01;the tenderness threshold of the two groups was significantly reduced after opera-tion(P<0.05,P<0.01,and the treatment group was better than the control group(P<0.05;the incidence of nausea during opera-tion,abdominal distension and nausea 48 h after operation in the treatment group were better than those in the control group(P<0.05;the high-quality embryo rate in the treatment group was better than that in the control group(P<0.05.CONCLUSION Electroacupuncture at Hegu and Neiguan combined with indomethacin suppository can effectively assist in analgesia, and can reduce the incidence of adverse reactions during and after TUGOR to varying degrees, and may have certain advantages in improving the rate of high-quality embryos.
